INTRODUCING THE ORTHODONTIC TEAM TO SLEEP DISORDERED BREATHING IN CHILDREN

Sleep disordered breathing (SBD) is a spectrum of chronic disease conditions united by a reduced oxygen perfusion while sleeping. Even mild SBD is often associated with specific oral and craniofacial characteristics that orthodontists are uniquely qualified to identify. With an understanding of available clinical and radiographic tools, orthodontists can be highly accurate for screening SDB. Furthermore, with a proper understanding of the SDB etiology, orthodontists are important contributors to the multi-disciplinary treatment of select forms of SDB. This presentation will provide an overview of the etiology of sleep disordered breathing in children to establish orthodontic context for the in-depth presentations by a team of experts. Dr. Michael P. Major, DMD, MSc, FRCD(C) Director, Inter-disciplinary Airway Research Clinic (I-ARC) Assistant Clinical Professor, Orthodontics University of Alberta, Edmonton, Alberta Dr. Major is a certified specialist in Orthodontics and a Fellow of the Royal College of Dentists of Canada (FRCD-C). Dr Major graduated with his dentistry training from Boston University (DMD 2009). He completed a post-doctoral residency in dental management of medically complex patients at the University of British Columbia (GPR 2010), and a graduate residency specializing in Orthodontics at University of Alberta (MSc 2013). In addition to maintaining a private practice in Edmonton, Alberta, Dr. Major is an Assistant Clinical Professor at the University of Alberta’s School of Dentistry and the Director of the Inter-disciplinary Airway Research Clinic (I-ARC). Dr. Major’s research and clinical interests include: evidence-base practice, craniofacial esthetics, pediatric sleep disordered breathing, and skeletal orthopedics. He has published numerous peer-reviewed publications in a variety of orthodontically related topics, and is a committee member for the Alberta Dental Association and College developing guidelines for dental management of sleep disordered breathing.

SLEEP DISORDERED BREATHING – A TEAM SPORT

Obstructive sleep disordered breathing is not a distinct disease but rather a syndrome of upper airway dysfunction during sleep that can result from various different disorders. Although the main diagnostic tool is an overnight polysomnogram (overnight recordings of breathing and sleep), various different tools are used to make the diagnosis because of limited access to polysomnography. The importance of diagnosis in children relates to the associated morbidity, which can include: elevated blood pressure, daytime sleepiness, learning problems, behavioural problems and growth failure. Adenotonsillectomy has been the standard choice of treatment for children with obstructive sleep apnea but there is mounting evidence that this standard treatment is unpredictable in normalizing sleep and breathing patterns in affected children. As a result there is no consensus on the best method of managing pediatric obstructive sleep apnea. Given that genetic background and environmental and lifestyle factors may also play a role in the degree of upper airway impairment, and possibly associated morbidity over the long-term, obstructive sleep apnea may require a long-term interdisciplinary treatment approach addressing various factors involved in upper airway impairment, including: craniofacial structure, inflammation, upper airway tone and ventilator control for normalization of breathing during sleep. This session will highlight evidence for the role of managing children with obstructive sleep apnea in an interdisciplinary treatment team. Dr. Manisha Witmans, BcsMed, MD, FRCPC, FAAP, RPSGT, FAASM

Dr. Manisha Witmans is a Board Certified Pediatric Sleep Medicine Specialist who also specializes in Pediatric Pulmonology. Dr. Witmans began her studies at the University of Alberta and completed her medical training at the University of Saskatchewan, achieving her BScMed, MD with Distinction. Dr. Witmans completed her Pediatric Residency and Postdoctoral Fellowship in Pediatric Respirology, specializing in Sleep Medicine, at the Alberta Children’s Hospital in Calgary, Alberta, with an exchange during residency to Mater Children’s Hospital in Brisbane, Australia. Dr. Witmans continued her fellowship training in Pediatric Pulmonology and Sleep Medicine at the Children’s Hospital in Los Angeles, California, before returning back to the University of Alberta Hospital where she developed the pediatric sleep program at the renowned Stollery Children’s Hospital. Dr. Witmans has been integral in the development of the current international guidelines on the use of polysomnography in pediatric respiratory and non-respiratory sleep disorders. She holds a strong presence in the international sleep community, sitting on numerous committees nationally and internationally, working to address sleep problems in children. Dr. Witmans is an Associate Clinical Professor at the University of Alberta and an Adjunct Professor at the Thompson Rivers University in British Columbia. She remains involved with the Northern Alberta Pediatric Sleep Lab at the Stollery Children’s Hospital and continues to work as a specialist within the Stollery Children’s Hospital. Dr. Witmans is the Medical Director of Sound Sleep Solutions, a private and fully comprehensive adult and pediatric level one sleep laboratory in Calgary, Alberta. Sponsored by 8

THE CLINICAL CORRELATES OF UPPER AIRWAY DYSFUNCTION ASSOCIATED WITH MAXILLOMANDIBULAR DISPROPORTION FROM A PEDIATRIC OTOLARYNGOLOGIST PERSPECTIVE

The objectives of this talk will be to outline a pragmatic approach to nasal obstruction and associated conditions in children, discuss some surgical treatment options, and review the experience drawn from collaborative work. A reasoned approach for assessing children with nasal obstruction, other related symptoms, and associated sleep complaints will be presented. This will include an emphasis on endoscopic examination, the role of radiology, and other available tools. Ultimately, the impact of a multi-disciplinary assessment on the otolaryngological management of these children and the various solutions to nasal and pharyngeal pathology will be discussed. Dr. Hamdy El-Hakim, MB ChB, FRCS (Ed), FRCS (OR) Pediatric Otolaryngology, Walter MacKenzie Health Science Centre, Edmonton, Alberta Dr. El-Hakim graduated from Ain Shams University in Cairo (MB, ChB 1986), Egypt. His higher surgical training was completed in the United Kingdom (CCST in Otolaryngology 2001). He is a fellow of The Royal College of Surgeons of Edinburgh (FRCSEd 1994) and obtained also the Intercollegiate Board Examination in Otolaryngology (FRCS-ORL 1999). Dr. El-Hakim’s Fellowship training in Pediatric Otolaryngology was at The Hospital for Sick Children in Toronto from July 1999 -June 2001. He has been in his current post since June 2002. He is an Associate Professor at the Department of Surgery and has an adjunct appointment at the departments of Pediatrics and Dentistry at the University of Alberta. He is also the Pediatric Otolaryngology Fellowship Program Director, the Research Director for the Division of Otolaryngology at the University of Alberta. He is an Associate Editor for the Journal of Otolaryngology Head & Neck Surgery, an Editorial Board member for the International Journal of Pediatric Otorhinolaryngology. He has over 65 peer-reviewed publications to date. His research interests include sleep disordered breathing, swallowing disorders and laryngology and is the primary airway surgeon at the Stollery.

UPPER AIRWAY IMAGING

Imaging of the upper airway identifies anatomic sites of potential obstruction and enables objective measurements before and after therapy. 3D reconstruction of the upper airways provides a platform to apply endless quantitative, qualitative, or functional assessment. Such analyses can be insightful to identify patients at risk of sleep disordered breathing syndromes or disease staging, prediction or assessment of treatment outcomes. This however demands accurate methods of segmentation and meaningful analyses reflective of clinical changes. Imaging modalities, with main focus on cone beam CT, proper segmentation and tools of analyses of the upper airway are discussed. Dr. Noura Alsufyani, BDS, MSc, FRCDC, Diplomate of the ABOMR University of Alberta, Edmonton, AB Dr. Noura Alsufyani is a specialist in Oral and Maxillofacial Radiology. She received her Bachelor’s degree from King Saud University, Saudi Arabia. In 2006, she enrolled at the University of Toronto where she completed her Master’s degree and specialty training in Oral and Maxillofacial Radiology. Dr. Alsufyani is a fellow of the Royal College of Dentists of Canada in Oral and Maxillofacial Radiology and a diplomate of the American Academy of Oral and Maxillofacial Radiology. Currently, she is a clinical assistant professor at the University of Alberta and pursuing a PhD in Dentistry involving upper airway imaging.

Practicing in a Populated Area: It Can Be Done! Contrary to popular belief, it is possible to establish and to grow an orthodontic practice in a competitive area. Success can be measured using a variety of yardsticks; case starts, patient satisfaction and annual gross are but a few of the ways one evaluates one’s practice. Even in a well-serviced area, it is rare for an orthodontic practice not to survive but successful practices involve much more than mere survival. Each of the four panelists will give a 15 minute presentation on one aspect of their practice that they feel has been elemental to their success in a competitive area. Topics of discussion will centre on marketing, customer service, staffing and communication, all from the perspective of the profession in Canada. The panel will entertain a question and answer period following the presentation. Dr. Steven Budd Dr. Budd graduated with his DDS from UBC in 1994 and his MSc in orthodontics from the University of Toronto in 2005. He started his practice in Langley B.C. immediately upon graduation.

Dr. Austin Chen Dr. Chen graduated with his DDS from the University of Toronto in 2002 and his MSc in orthodontics from U of T in 2007. He started his practice in Vaughan Ontario in 2010.

Dr. Lorne Kamelchuk Dr. Kamelchuk graduated with his DDS from the University of Alberta in 1993, his MSc in Oral Biology from U of A in 1994 and his Diploma in Orthodontics from the University of Toronto in 1998. He started his practice in Calgary Alberta in 1999.

Dr. Bruno Vendittelli Dr. Vendittelli graduated with his DDS from the University of Toronto in 1994 and his Diploma in Orthodontics from U of T in 1999. He started his practice in Toronto Ontario in 2000.

As recent studies have highlighted the importance of the interdisciplinary management of sleep disordered breathing, the role orthodontists can play in the treatment of this disease has become better defined. The relationship between sleep disordered breathing and craniofacial growth and development, as well as the current evidence of managing pediatric sleep disorders with dentofacial orthopedics will be discussed. Dr. Benjamin Pliska, DDS, MS, FRCD(C) Assistant Professor, Faculty of Dentistry University of British Columbia Vancouver, BC Dr. Benjamin Pliska is an Assistant Professor of the University of British Columbia Faculty of Dentistry and maintains a private practice of orthodontics in Vancouver. Dr. Pliska is a graduate of the University of Western Ontario School of Dentistry, and obtained his Certificate in Orthodontics and Master’s Degree in Dentistry from the University of Minnesota. Dr. Pliska also serves as orthodontic consultant at BC Children’s Hospital and member of the Pediatric Research in Integrated Sleep Medicine (PRISM) clinical team. An active researcher, Dr. Pliska publishes and lectures in the areas of airway imaging and sleep medicine.

OBSTRUCTIVE SLEEP APNEA AND ORTHODONTICS IN ADULT PATIENTS

Oral appliances used for the treatment of adult Obstructive Sleep Apnea are regarded as a simple, silent, bed partner-friendly, reversible, tolerable and efficacious choice and impact a number of essential medical outcomes, including overnight sleep data, subjective and objective measurements of sleepiness, cardiovascular functions, neuropsychological behaviours and quality of life indices. Orthodontists, with their knowledge and training of functional appliances and established skills to evaluate jaw position, are ideally suited to provide oral appliance therapy in this field. Several factors contribute to their effectiveness – appliance design and titration, severity of the sleep apnea, amount of forward advancement, vertical opening of the mandible, positional apnea and Body Mass Index. Lowe, DMD, Dip Ortho, PhD, FRCD(C) Professor and Chair, Division of Orthodontics Faculty of Dentistry The University of British Columbia Vancouver, B.C. Dr. Alan Lowe is Professor and Chair in the Division of Orthodontics at the University of British Columbia and has maintained a private orthodontic specialty practice in Vancouver, Canada for 35 years. He has lectured extensively both nationally and internationally in the areas of neurophysiology and orthodontics with a particular emphasis on the etiology and treatment of snoring and Obstructive Sleep Apnea. His research activities have been funded by both provincial and federal governments and he has published over 120 papers, 150 abstracts and 15 book chapters. In addition, he has successfully filed Canadian, U.S. and international patents for each of three inventions. Dr. Lowe was one of the founding members of the American Academy of Dental Sleep Medicine and has served for many years as a member of their Board of Directors, a Research Advisor to the Board and Chair of their Award’s Committee.

Open bite can be brought by various etiologic factors. Each etiologic factor results in different types of open bites. Especially in this lecture, open bite pattern and facial morphology change related with TMD will be explained. Open bite can be treated by one of three mechanics; extrusive, intrusive or combined. These mechanics will be presented with cases. Step-by-step slides and clinical tips will make it easy to understand how to apply the mini-implants and/or anchor plates in open bite treatment. Based on the 30-year clinical experience in open-bite treatment and more than 15-year experience of intruding molars by using mini-implants, this lecture will provide 1) the most efficient and easiest method of intruding posterior teeth, 2) step-by-step procedure to treat open bite with cases, 3) stability of intruded molars, and 4) suggestions for long-term stability of outcomes. Dr. Tae-Woo Kim, DDS, MSD, PhD Seoul, Korea Dr. Tae-Woo Kim is Professor in Orthodontics, Seoul National University, where he completed his orthodontic specialty training in 1986. He was a visiting professor (1995-7) of University of Washington. He was an editorin-chief of Korean Journal of Orthodontics (KJO) for many years. He is now President of the Korean Association of Orthodontists. He works as a reviewer of KJO, AJODO and other famous international journals. His focused research fields are TMD, open bite, mini-implants and long-term stability.

INTRODUCING THE ORTHODONTIC TEAM TO SLEEP DISORDERED BREATHING IN CHILDREN

Sleep disordered breathing (SBD) is a spectrum of chronic disease conditions united by a reduced oxygen perfusion while sleeping. Even mild SBD is often associated with specific oral and craniofacial characteristics that orthodontists are uniquely qualified to identify. With an understanding of available clinical and radiographic tools, orthodontists can be highly accurate for screening SDB. Furthermore, with a proper understanding of the SDB etiology, orthodontists are important contributors to the multi-disciplinary treatment of select forms of SDB. This presentation will provide an overview of the etiology of sleep disordered breathing in children to establish orthodontic context for the in-depth presentations by a team of experts. Dr. Michael P. Major, DMD, MSc, FRCD(C) Director, Inter-disciplinary Airway Research Clinic (I-ARC) Assistant Clinical Professor, Orthodontics University of Alberta, Edmonton, Alberta Dr. Major is a certified specialist in Orthodontics and a Fellow of the Royal College of Dentists of Canada (FRCD-C). Dr Major graduated with his dentistry training from Boston University (DMD 2009). He completed a post-doctoral residency in dental management of medically complex patients at the University of British Columbia (GPR 2010), and a graduate residency specializing in Orthodontics at University of Alberta (MSc 2013). In addition to maintaining a private practice in Edmonton, Alberta, Dr. Major is an Assistant Clinical Professor at the University of Alberta’s School of Dentistry and the Director of the Inter-disciplinary Airway Research Clinic (I-ARC). Dr. Major’s research and clinical interests include: evidence-base practice, craniofacial esthetics, pediatric sleep disordered breathing, and skeletal orthopedics. SLEEP DISORDERED BREATHING – A TEAM SPORT

Obstructive sleep disordered breathing is not a distinct disease but rather a syndrome of upper airway dysfunction during sleep that can result from various different disorders. Although the main diagnostic tool is an overnight polysomnogram (overnight recordings of breathing and sleep), various different tools are used to make the diagnosis because of limited access to polysomnography. The importance of diagnosis in children relates to the associated morbidity, which can include: elevated blood pressure, daytime sleepiness, learning problems, behavioural problems and growth failure. Adenotonsillectomy has been the standard choice of treatment for children with obstructive sleep apnea but there is mounting evidence that this standard treatment is unpredictable in normalizing sleep and breathing patterns in affected children. As a result there is no consensus on the best method of managing pediatric obstructive sleep apnea. Given that genetic background and environmental and lifestyle factors may also play a role in the degree of upper airway impairment, and possibly associated morbidity over the long-term, obstructive sleep apnea may require a long-term interdisciplinary treatment approach addressing various factors involved in upper airway impairment, including: craniofacial structure, inflammation, upper airway tone and ventilator control for normalization of breathing during sleep. This session will highlight evidence for the role of managing children with obstructive sleep apnea in an interdisciplinary treatment team. Dr. Manisha Witmans, BcsMed, MD, FRCPC, FAAP, RPSGT, FAASM

Dr. Manisha Witmans is a Board Certified Pediatric Sleep Medicine Specialist who also specializes in Pediatric Pulmonology. Dr. Witmans began her studies at the University of Alberta and completed her medical training at the University of Saskatchewan, achieving her BScMed, MD with Distinction. Dr. Witmans completed her Pediatric Residency and Postdoctoral Fellowship in Pediatric Respirology, specializing in Sleep Medicine, at the Alberta Children’s Hospital in Calgary, Alberta, with an exchange during residency to Mater Children’s Hospital in Brisbane, Australia. Dr. Witmans continued her fellowship training in Pediatric Pulmonology and Sleep Medicine at the Children’s Hospital in Los Angeles, California, before returning back to the University of Alberta Hospital where she developed the pediatric sleep program at the renowned Stollery Children’s Hospital. Dr. Witmans has been integral in the development of the current international guidelines on the use of polysomnography in pediatric respiratory and non-respiratory sleep disorders. She holds a strong presence in the international sleep community, sitting on numerous committees nationally and internationally, working to address sleep problems in children. Dr. Witmans is an Associate Clinical Professor at the University of Alberta and an Adjunct Professor at the Thompson Rivers University in British Columbia. She remains involved with the Northern Alberta Pediatric Sleep Lab at the Stollery Children’s Hospital and continues to work as a specialist within the Stollery Children’s Hospital. The objectives of this talk will be to outline a pragmatic approach to nasal obstruction and associated conditions in children, discuss some surgical treatment options, and review the experience drawn from collaborative work. A reasoned approach for assessing children with nasal obstruction, other related symptoms, and associated sleep complaints will be presented. This will include an emphasis on endoscopic examination, the role of radiology, and other available tools. Ultimately, the impact of a multi-disciplinary assessment on the otolaryngological management of these children and the various solutions to nasal and pharyngeal pathology will be discussed. Dr. Hamdy El-Hakim, MB ChB, FRCS (Ed), FRCS (OR) Pediatric Otolaryngology, Walter MacKenzie Health Science Centre, Edmonton, Alberta Dr. El-Hakim graduated from Ain Shams University in Cairo (MB, ChB 1986), Egypt. His higher surgical training was completed in the United Kingdom (CCST in Otolaryngology 2001). He is a fellow of The Royal College of Surgeons of Edinburgh (FRCSEd 1994) and obtained also the Intercollegiate Board Examination in Otolaryngology (FRCS-ORL 1999). Dr. El-Hakim’s Fellowship training in Pediatric Otolaryngology was at The Hospital for Sick Children in Toronto from July 1999 -June 2001. He has been in his current post since June 2002. He is an Associate Professor at the Department of Surgery and has an adjunct appointment at the departments of Pediatrics and Dentistry at the University of Alberta. He is also the Pediatric Otolaryngology Fellowship Program Director, the Research Director for the Division of Otolaryngology at the University of Alberta. He is an Associate Editor for the Journal of Otolaryngology Head & Neck Surgery, an Editorial Board member for the International Journal of Pediatric Otorhinolaryngology. He has over 65 peer-reviewed publications to date. His research interests include sleep disordered breathing, swallowing disorders and laryngology and is the primary airway surgeon at the Stollery.

UPPER AIRWAY IMAGING

Imaging of the upper airway identifies anatomic sites of potential obstruction and enables objective measurements before and after therapy. 3D reconstruction of the upper airways provides a platform to apply endless quantitative, qualitative, or functional assessment. Such analyses can be insightful to identify patients at risk of sleep disordered breathing syndromes or disease staging, prediction or assessment of treatment outcomes. This however demands accurate methods of segmentation and meaningful analyses reflective of clinical changes. Imaging modalities, with main focus on cone beam CT, proper segmentation and tools of analyses of the upper airway are discussed. Dr. Noura Alsufyani, BDS, MSc, FRCDC, Diplomate of the ABOMR University of Alberta, Edmonton, AB Dr. Noura Alsufyani is a specialist in Oral and Maxillofacial Radiology. She received her Bachelor’s degree from King Saud University, Saudi Arabia. In 2006, she enrolled at the University of Toronto where she completed her Master’s degree and specialty training in Oral and Maxillofacial Radiology. Dr. Alsufyani is a fellow of the Royal College of Dentists of Canada in Oral and Maxillofacial Radiology and a diplomate of the American Academy of Oral and Maxillofacial Radiology. Currently, she is a clinical assistant professor at the University of Alberta and pursuing a PhD in Dentistry involving upper airway imaging.

Dr. Benjamin Pliska, DDS, MS, FRCD(C) Assistant Professor, Faculty of Dentistry University of British Columbia Vancouver, BC Dr. Benjamin Pliska is an Assistant Professor of the University of British Columbia Faculty of Dentistry and maintains a private practice of orthodontics in Vancouver. Dr. Pliska is a graduate of the University of Western Ontario School of Dentistry, and obtained his Certificate in Orthodontics and Master’s Degree in Dentistry from the University of Minnesota. Dr. Pliska also serves as orthodontic consultant at BC Children’s Hospital and member of the Pediatric Research in Integrated Sleep Medicine (PRISM) clinical team. His proudest achievement, though, besides the fact that he really was at Woodstock is his wonderful family: his wife, Phyllis Simon, owner of Vancouver Kidsbooks to whom he has been married for 44 years, his two fabulous sons Jonah and Tim and their wonderful partners, Aviva and Lindsay, and especially his gorgeous, talented, genius, amazingly athletic and utterly captivating grandchildren, 3-year-old-going-on-15 granddaughter, Masha, and 2-year-old grandson, Henry.
