It is our pleasure to welcome you to the 2015 Empire Ball, the annual social and fundraising event of the season for New York City’s real estate industry. It has been 32 years since the inception of this incredible fundraising effort, and from the beginning, the funds raised have supported research dedicated to unlocking a cure for diabetes. To date, thanks to your generosity and dedication, the Empire Ball has raised more than $35 million – an outstanding achievement! On this evening, we would like to congratulate and extend our sincere gratitude to all of the honorees, chairs, and committee members. Your loyal support and focus has helped make this year’s event a great success. As is the tradition, earlier this year, several honorees and committee members traveled to the Diabetes Research Institute (DRI) in Miami to get a firsthand look at our research. The group learned about the progress being made on our DRI BioHub, a bioengineered mini organ that mimics the native pancreas.

In fact, we have some exciting progress to report to all of you! The first patient in the DRI’s FDA-approved BioHub pilot clinical trial is now free from insulin injections! The results have been remarkable, and we have all of you to thank. By supporting this special event, you are providing the necessary resources for the scientists at the DRI to continue this life-changing work toward a cure. Undoubtedly, your colleagues who have visited the DRI could share a whole new perspective with everyone here tonight, underscoring the meaning behind this very special event. On behalf of the Diabetes Research Institute and Foundation, we want to thank everyone for joining us in making diabetes history. Enjoy your evening.
Your support over these many years has allowed this milestone to happen. Collectively, we have raised over $35 million and counting towards our next milestone; finding a definitive cure. This year I am proud to take part in honoring Carl R. Kuehner III, Avi Itzikowitz and William Sotomayor; all three with different connections to the mission, as you will hear this evening. They will be joining a long and very distinguished list of former honorees, many of whom have maintained an active participation with our committees--something not very common these days. I am personally humbled to be receiving the lifetime Achievement Award and the only thing that could make me happier is a time when we can say we have unlocked the cure for type 1 diabetes. This year has been a very special year in many ways, as we head into the next generation of the Empire Ball. led by Stephen Rizzo and James Halpin, the "Steering Committee" has taken on this task of moving us into this next phase of fundraising and we hope we can count on your continued generous support. As always we all very much appreciate your participation and wish you a wonderful holiday season ahead.

CARL R. KUEHNER III Chairman and Chief Executive Officer Building and land Technology Carl R. Kuehner is Chairman & CEO of Building and land Technology (BlT), a second-generation privately held real estate private equity, development and property management firm. Founded in 1982, BlT is vertically integrated and has invested, developed, owned and managed over five million square feet of commercial space and over 10,000 residential units. Based in Stamford, CT, BlT is one of the largest office landlords in Connecticut, and its office holdings include the corporate headquarters of Gen Re, GE Commercial Finance, Diageo, GE Real Estate, Xerox, GE Energy, Design Within Reach, Kayak Software, AON/Hewitt, Starwood Hotels and Resorts, and Structured Portfolio Management. Mr. Kuehner has spearheaded the development of Harbor Point, a transformative mixed-use development in Stamford, CT. One of the largest redevelopments on the eastern seaboard, Harbor Point encompasses an entire neighborhood, with a full mile of waterfront, more than 4,000 residential units, and several million square feet of commercial development. Harbor Point offers an abundance of restaurant and retail opportunities for residents and office tenants, and features approximately 11 acres of public open space in two parks, two waterfront promenades, and numerous marina slips. With Mr. Kuehner’s vision and leadership, the former Jersey City Medical Center has been redeveloped into more than 1,100 luxury residential units. Now known as The Beacon, it is one of the largest historic renovations in the country, with over a million square feet that has been expertly restored to recapture the dramatic architectural details and historic grandeur of its Art Deco-era origin. The Beacon features panoramic views of the Manhattan skyline, liberty State Park, and Freedom Tower. In addition to serving as Chairman & CEO of BlT, Mr. Kuehner is active in a number of philanthropic organizations. He was appointed by the U.S. Secretary of the Interior to serve as a member of the Board of Directors of the National Fish & Wildlife Foundation (NFWF). Mr. Kuehner has served in various leadership positions for the Foundation, including Chairman of the Board. He currently serves as the Chairman of the foundation’s Gulf Environmental Benefit Fund Committee, which is responsible for directing a total of $2.544 billion to the National Fish and Wildlife Foundation (NFWF) to fund projects benefiting the natural resources of the Gulf Coast that were impacted by the 2010 Deepwater Horizon explosion and oil spill. He is a Founder and Board Member of Bankwell Financial Group Inc. (NASDAQ: BWFG). In a recent report, the U.S. Treasury Department ranked Bankwell Financial Group first in the nation for growth in qualified small business lending in its Small Business lending Fund. Mr. Kuehner is a member of the World Presidents’ Organization (WPO), having served in a number of leadership positions, including Chairman, of the Young Presidents’ Organization (YPO), Fairchester Chapter. Mr. Kuehner is also very active in the family-founded charity, “Hope for Haiti,” which supports hospitals, schools and orphanages working towards sustainability for the Haitian people. Mr. Kuehner has also served on various corporate and philanthropic boards including The National Association of Homebuilders and The Waterside School.

Mr. Sotomayor is a Regional Director for Ware Malcomb’s New York City Office. Ware Malcomb is an award winning, international design firm providing architecture, planning, interior design, graphic design and civil engineering services for corporate and commercial real estate clients. Established in 1972, the firm has seventeen offices throughout the United States, Toronto (Canada), Panama, and Mexico.
Avi Itzikowitz joined ATCO Properties and Management as Director of Operations in March 2003. As Director of Operations, Mr. Itzikowitz is responsible for the day to day operations of the ATCO portfolio both in the New York Metropolitan area as well as several properties in other markets. He is an accomplished, results-oriented operations manager with a strong record of building value in commercial, residential, retail and industrial properties through increased cash flow and satisfied tenants. With over 20 years of experience in property management his responsibilities include lease administration, contract negotiation, team building/mentorship, tenant improvement/construction, formation of operating and capital budgets and project management.
Mr. Sotomayor graduated from the New York Institute of Technology with a Bachelor of Architecture. He is a founding member of the Knights of York, a New York City-based nonprofit organization comprised of business leaders in the construction, real estate and technology industries. The Knights of York is devoted to contributing services and donations to help children in need. William is also current Board Member of the National Hispanic Business Group and served as past Chairman, the group consist of prominent Hispanic entrepreneurs with a vision to create an organization entrusted with developing opportunities for Hispanic Businesses. In 1995, Mr. Sotomayor established TSC Design, an architectural design firm in Manhattan. He conceptualized TSC with the belief that providing excellence in service to clients, no matter how large or small, is the surest way to succeed and build successful relationships. He expanded the firm to New Jersey, opening an office in 2005, and grew TSC to a 120-person operation. TSC provided architectural and interior design services to corporate, retail, publishing, media and law firm clients nationally and internationally clients include Goldman Sachs, Citi, TIAA/CREF, JP Morgan Chase, Morgan Stanley, Weil Gotshal & Manges, ABC Disney, CBS, AON, Tiffany & Co, DKNY, Polo Ralph lauren and many others. Bill later sold and merged his company with an architectural design firm in New York in 2011. Mr. Sotomayor joined the Ware Malcomb team in 2014, seeking to combine his 30 years of regional knowledge and expertise with Ware Malcomb’s expanding presence on the East Coast. Mr. Sotomayor is responsible for managing the newest Ware Malcomb office in New York City and will also oversee business development and client relationships.
Some of his key contributions during his tenure at ATCO have been assisting in the conversion of a 25 acre industrial site into a mixed use commercial, retail park complex. Oversee effort to achieve both Energy Star rating and lEED Silver EB certification for two key properties in the ATCO portfolio. Initiate the installation of 8,335 square foot green roof as well as lead the effort to install two 50KW solar systems in New York and a 310 KW solar system in New Jersey. Additionally Avi, has planned and implemented several elevator modernization projects in both commercial and residential properties in the ATCO portfolio as well several local law #11 façade projects including the restoration of an historic façade in one property and a complete façade strip and rebuild in another. Prior to joining ATCO, Mr. Itzikowitz was employed by vornado Realty Trust where he oversaw and managed all facets of a total renovation of landmarked 1.2M sq. Ft commercial office building as well as implementing a new building security system and elevator modernization at a property situated above Penn Station. After a distinguished career spanning more than 40 years as an officer and Chief Operating Officer of ATCO Properties and Management, a vertically integrated Real Estate company that delivers the highest level of professional services including, ownership, development, management, brokerage, investment, and interior design and construction, Mr. DiCapua retired. Shortly after his retirement, multiple companies approached Mr. DiCapua to serve on their Boards of Directors. One of those companies was CodeGreen Solutions, for which he accepted the position of Chairman. CodeGreen helps real estate and business owners develop and achieve their energy reduction and sustainability goals with an expertise that lies at the cross section of energy engineering, financial incentives and technology. His involvement and leadership in many real estate associations has kept him at the forefront of the industry and he has been well recognized at the highest levels in the real estate industry through his many awards, including Recipient of the Real Estate Management Man of the Year Award, presented by The Real Estate Board of New York, in recognition of “his dedication, competence, professionalism, civic responsibility and contribution to the betterment of the community and its citizenry.” During his long and well-respected career, Mr. DiCapua has served on the Boards and held offices in numerous professional organizations, including the Board of Governors of the Real Estate Board of New York, as vice Chairman and Director of the N.Y. Building Congress, as a Director on the Realty Advisory Board on labor Relations, and as the President of Building Owners and Managers Assoc. of N.Y. He has also been the recipient of the John M. Griffin Community Service Award, the Kenneth R. Gerrity Humanitarian Award, The George A. Fox Public Service Award, The Esther Montanez leadership Award and the Distinguished Service Award presented by the Real Estate Division of the Diabetes Research Institute, Empire Ball. Mr. DiCapua was the founding Chairman of the laGuardia Community College Foundation and serves on the Board of Trustees of Cabrini of Westchester.
Michael Kay is the Yankees play-by-play announcer for the YES Network, who also hosts the network’s CenterStage series and its various programming specials. Kay won a 2015 New York Emmy Award as the best play-by-play announcer for his work on YES’ Yankees telecasts. In total, Kay has earned 30 Emmy nominations and eight Emmy wins while at YES. Kay has also won two CableFAX Program Awards in 2011 and 2013 for his hosting duties on CenterStage. The Michael Kay Show, a sports talk show heard weekdays on ESPN Radio 98.7 FM in New York which Kay co-hosts with Don leGreca, has been simulcast live weekday afternoons on YES since February 2014. The influential Radio Ink magazine, which covers the radio industry, named Kay the second-most influential local sports talk show host in America in 2012. In addition to his co-hosting The Michael Kay Show, Kay also handled play-by-play duties for the ESPN Radio Network during the 2013 American league Divisional Series (Tigers-A’s) and the 2008 National league Divisional Series (Phillies-Brewers), and has been a frequent contributor to ESPN’s Emmy Awardwinning Sports Reporters television show airing on Sunday mornings. Prior to joining the YES Network in 2002, Kay worked at the MSG Network since 1989 as a Yankees reporter. In 1992, he added Knicks locker room reporter to his responsibilities, and continued in that role until the 1998-99 season. Kay was awarded the Dick Young Award for Excellence in Sports Media by the New York Pro Baseball Scouts in 1995. In 1998, he was on the MSG Network team that won a New York Emmy for "Outstanding live Sports Coverage – Series." In 1996 and 1997 he was a member of the MSG Network team that won New York Emmys for "Outstanding live Sports Coverage - Single Program" for Dwight Gooden's no-hitter and "The Battle for New York: Yankees vs. Mets." He was also a part of the Yankees/MSG Network production team that was nominated for New York Emmy Awards for six consecutive years. Shortly after graduating from Fordham University in 1982 with a B.A. in Communications, Kay worked at SportsPhone and as the public address announcer for the New York Pro Summer Basketball league. In 1982, Kay landed the job as general assignment writer for the New York Post. Two years later, he began covering college basketball (1984-85) and then spent two years as the writer covering the New Jersey Nets. In 1987, he moved to baseball at the Post, serving as the Yankees beat reporter. While he was in that position, he got his first television job with MSG Network as host of the Hot Stove League segment of MSG's SportsNight. In 1989, Kay moved to the New York Daily News, where he was the Yankees beat reporter before taking the job as a Yankees broadcaster on WABC Radio in 1992. With that jump, Kay became the first newspaper reporter in any sport to make the move into the broadcast booth full-time, performing both play-by-play and analysis. Kay stayed on the radio for 10 years, broadcasting the Yankees’ four World Series championships during that time, before moving to the television side to become the lead play-by-play man for YES in 2002. Kay has been involved in the Baseball Assistance Team (B.A.T.), a charitable organization whose mission is to provide assistance to members of the baseball community. He co-hosted the 2013 B.A.T. fundraising dinner in New York. Kay also was master of ceremonies at the 2013 Thurman Munson Awards fundraising dinner in New York. In 2005, he was honored in his home borough of the Bronx, having an honorary street sign in his name erected on the Grand Concourse. Kay is active with the Alzheimer’s Association in memory of his mother, Rose, who passed away from the disease in 2006. He has also teamed up with Yankees manager Joe Girardi and Girardi’s Catch 25 Foundation to organize annual charity dinners to raise money for Alzheimer's research. Kay and his wife Jodi Applegate have a daughter, Caledonia Rose Kay, born in January 2013, and a son, Charles Applegate Kay, born in November 2014.

FACTS ABOUT DIABETES Diabetes is increasing at an alarming rate in the United States. According to the CDC’s (Centers for Disease Control) National Diabetes Statistics Report for 2014, cases of diabetes have increased almost 13% over the last four years (29.1 million in 2012 vs. 25.8 million in 2010). Below is a summary of the latest diabetes statistics included in the CDC’s report. You can view the full report at http://www.cdc.gov/diabetes/pubs/statsreport14/national-diabetes-report-web.pdf. how many People have Diabetes? • 29.1 million people, or 9.3% of the U.S. population, have diabetes, including 8.1 million people who have diabetes but have not yet been diagnosed (All ages, 2012). • Diabetes impacts all social, economic, and ethnic backgrounds. • Type 1 diabetes accounts for about 5% of all diagnosed cases of diabetes, affecting approximately 1.5 million people.
new cases of Diabetes in adults and children • Among people 20 years or older, 1.7 million were newly-diagnosed with diabetes (2012) – approximately 4,657 new cases of diabetes are diagnosed each day. • About 208,000 people younger than 20 years of age have been diagnosed with diabetes (type 1 or type 2). This represents 0.25% of all people in this age group (2012). • The incidence of type 1 diabetes is significantly higher in people under age 20. According to a 2008-2009 study of 23,525 youths with diabetes, 78% were newly-diagnosed with type 1 diabetes vs. 22% who were newly-diagnosed with type 2 diabetes.*
Incidence of Diabetes complications • Diabetes can affect many parts of the body and is associated with serious complications, such as heart disease, stroke, blindness, kidney failure, and lower-limb amputation, among other conditions. • heart Disease, heart attack and stroke • In 2003-2006, after adjusting for population age differences, cardiovascular disease death rates were about 1.7 times higher among adults aged 18 years or older with diagnosed diabetes than among adults without diagnosed diabetes. • In 2010, after adjusting for population age differences, hospitalization rates for heart attack were 1.8 times higher among adults aged 20 years or older with diagnosed diabetes than among adults without diagnosed diabetes. • In 2010, after adjusting for population age differences, hospitalization rates for stroke were 1.5 times higher among adults with diagnosed diabetes aged 20 years or older compared to those without diagnosed diabetes. • blindness and Eye Problems • In 2005-2008, of adults with diabetes aged 40 years or older, 4.2 million (28.5%) people had diabetic retinopathy, damage to the small blood vessels in the retina that may result in loss of vision. • In 2005-2008, of adults with diabetes aged 40 years or older, 655,000 (4.4%) had advanced diabetes retinopathy that could lead to severe vision loss. • Kidney Disease • Diabetes was listed as the primary cause of kidney failure in 44% of all new cases in 2011. • In 2011, 49,677 people of all ages began treatment for kidney failure due to diabetes; and a total of 228,924 people of all ages with kidney failure were living on chronic dialysis or with a kidney transplant.
• amputations • In 2010, about 73,000 non-traumatic lower-limb amputations were performed in adults aged 20 years or older with diagnosed diabetes. • About 60% of non-traumatic lower-limb amputations among people aged 20 years or older occur in people with diagnosed diabetes. • hypoglycemia and hyperglycemic crisis • In 2011, about 282,000 emergency room visits for adults aged 18 years or older had hypoglycemia (low blood sugar levels) as the first-listed diagnosis and diabetes as another diagnosis. • In 2011, about 175,000 emergency room visits for people of all ages had hyperglycemic (high blood sugar levels) crisis as the first-listed diagnosis.
Diabetes is a Deadly Disease • Diabetes was the seventh leading cause of death in the United States in 2010 based on the 69,071 death certificates in which diabetes was listed as the underlying cause of death. In 2010, diabetes was mentioned as a cause of death in a total of 234,051 death certificates. • Diabetes may be underreported as a cause of death. Studies have found that only about 35% to 40% of people with diabetes who died had diabetes listed anywhere on the death certificate and about 10% to 15% had it listed as the underlying cause of death.
The cost of Diabetes • In 2012, the total cost of diabetes in the U.S. was $245 billion. • Of that, $176 billion accounted for direct medical costs. After adjusting for age and sex differences, average medical expenditures among people with diagnosed diabetes were 2.3 times higher than people without diabetes. • $69 billion was attributed to indirect costs (disability, work loss, premature death).
more about Diabetes • Diabetes is caused by the body’s inability to create or effectively use its own insulin, which is produced by islet cells found in the pancreas. Insulin helps regulate blood sugar (glucose) levels – providing energy to body cells and tissues. • Without insulin, the body’s cells would be starved, causing dehydration and destruction of body tissue. • People with type 1 diabetes must have insulin delivered by injection or a pump to survive. • Many people with type 2 diabetes can control their blood glucose by following a healthy meal plan and a program of regular physical activity, losing excess weight, and taking medications. Medications for each individual with diabetes will often change during the course of the disease. Insulin also is commonly used to control blood glucose in people with type 2 diabetes.
