A re yo u extending your aesthetic practice? It is tremendously rewarding to be part of the aesthetic industry where we can make a phenomenal difference to the appearance and comfort of our clientele. Laser and IPL treatments have dominated the industry in the last two decades. Micro needling, hair removal and chemical peels, toxin fillers and the non-invasive removal of common, everyday skin irregularities and blemishes are now the focus of the day. Janine Thomson elaborates.
What do we mean by skin irregularities? Identifying sebaceous lesions are fairly easy, the common ones being milia, cholesterol deposits, acne pimples and cysts. Sometimes we come across these cauliflower or donutshaped lesions that have a centre that secretes a little sebum and hardened pockets of lipids that are impossible to extract. They are found on sun exposed areas on asphyxiated skin.
nose area. The Lamprobe is my first modality of choice for facial capillaries and spider naevi because I can deliver results in one treatment without any risk of blisters and burns. There are 4 different probes, so treatment with the Lamprobe is also very precise, so one can see immediate blanching of the blood as the capillary is thermocoagulated. There is no risk of scarring with this treatment as it differs from the old cautery methods, where the needles previously needed to be inserted into the papillary dermis to cauterize the capillary, and scarring or post inflammatory pigmentation could occur. The sensation with the Lamprobe is very tolerable even on eyelids.

Lesions in awkward areas like milia on the eyelids or clogged pores in the ears, those stubborn sebaceous cysts or acne pimples that reoccur in the same place every month, or those painful ingrown hairs on the face or in the groin area are easily and effectively treated with this unique combination of radio & high frequency current that electro-desiccates sebaceous lesions. Treatments with the Lamprobe take therapy to a whole new level.

CONVERTING YOUR PATIENTS
When it comes to starting any new business venture, there is always a direct relationship between the success or failure of a business and the amount of time, money and thought that is devoted to how the business will sell its products and services. This means that if you are adding Aesthetics’ to your Practices list of services, you had better start thinking about how to sell those new services to your existing patient / customer database.
I
f you are reading this and you are a Doctor, you probably don’t like what I have just said. You might hate the idea of selling. You may even think that as a highly educated Professional that selling is not for you, it is for the other people at your practice. When I talk to Doctors about converting their patients into Medical Aesthetic customers I usually hear objections like, “Vaughan I am different, I am a Doctor, my business is different, it’s a practice.” Doctors often tell me that they don’t want to have anything to do with sales and marketing, they just want to spend most of their focus on injecting, however, if there

isn’t a proper strategy to convert patients into customers, there won’t be anyone for the Doctor to inject! The truth is that a Practice, whether it is a Law Practice or a Doctors Practice, is actually just like any other business. And, just like any other business, a Practice must continually generate sales from customers or patients because, without sales, it is very hard to sustain an on-going business of any kind – including your Medical Aesthetic Practice. When I talk about sales and converting patients into customers, I am not saying that you as a Doctor must become a salesman who manipulates or pushes products and services onto unwilling people. But what I am saying is that as the Owner of your practice you have to be intimately involved with the design and creation of the sales process so that you understand your Practices unique marketing opportunities and obstacles. So, as a Doctor, how do you go about creating a process to convert your patients into customers? As a Marketer, I believe that the answer to this question is a lot simpler than most Doctors realize because every Doctor’s practice has two priceless marketing assets that most business owners would kill to have.

Marketing Asset # 1 - People trust you You are a highly trained professional who has spent hundreds of thousands of Rands studying to help, heal and care for people. Over the years you have built relationships with patients who have developed a deep trust in you and your skills. Do you understand what a tremendous asset this is when it comes to marketing your Aesthetic practice? Please make sure you use it. If you can be trusted to heal and help, surely your patients will trust you to help them look and feel beautiful too! Having said that people trust you as a Highly Trained Professional doesn’t mean that you customers will automatically beat a path to your door, you will still have to market yourself and your services. So when you market your Aesthetic practice, talk about your authority, expertise and trustful reliability.
Marketing Asset # 2 – You have a Goldmine in your backyard! I am talking about the fact that most Doctors who move into Medical Aesthetics already have an existing customer database. For new start-up’s, building a customer database is usually the most difficult, expensive and urgent business problem that needs to be solved. You however, don’t have this problem. I recently spoke to a Doctor who has a fairly new medical practice that has recently added Aesthetics to her list of offerings. I asked her, “How many existing customers do you have on your patient database?” The answer was more than 1500. Very few new business ventures have the privilege of having 1500 trusting ‘clients’ when they
get started. Having an existing customer database is also important for another reason. It helps you keep your marketing costs down because it is always easier and cheaper to convince an existing customer to buy from you a second time than it is to convince a new customer to buy from you for the first time. Step one in creating your strategy to convert patients into customers is to oversee the process of getting your staff to clean up your patient database. This means checking the accuracy of the customers contact details, age, marital status etc. Never forget that rule number 1 of marketing says that “the more information you have about a person the easier it is to get them to spend.” Next, identify prospects or people on your database who would be likely candidates to want the Aesthetic products and services your practice offers. The simple truth is that not everyone on your patient database will be interested in Botox or Peels. So go through your database and identify who you want to target. Remember, marketing is simply the “right message to the right people at the right time.”
Business is built on relationships and relationships are built on communication After you have updated your database and identified qualified prospects for your Aesthetic Services, you can now start crafting your message and communicating with your patients / customers. Remember to adhere to guidelines set out by the HPC.

You should always start with the basics, one on one communication when people visit your practice, monthly educational e-mails, newsletters, social media, your website, small workshops etc… But, when you start talking to your customers it is important to communicate the right message. Remember that customers buy services from your Aesthetic Practice because they; a. Have a problem that they want solved b. Want to feel good about themselves. So, for example, if you offer Botox, don’t sell Botox, sell the solution that Botox offers for the client and talk about how she will feel when she sees the visible results from her treatment. Here’s food for thought: None of your patients woke up this morning wanting to have a needle stuck in their face, they wanted youthful looks, more confidence, a better sex life etc… So sell this image instead of a picture of a woman with a needle in her face. CATCH ME IF YOU CAN By Chris Parker
Is this you: “I trust my staff, but how can I be sure that they are not stealing from me?”
I
n my seventeen years at ESP there has been one thing that has always been present in the businesses of our clients, and that is employee theft. In this month’s article I would like to look at this issue, what causes it and what possible steps can be taken in order to avoid it.
THEFT IS EVERYWHERE As some background research into this article I did the usual and Googled the topic. The top results were some recent events including a Flight Centre employee’s theft of $117 000 in New Zealand and a former employee of a Witchita bank’s theft of over $289 000 in the U.S.
Movies and books are often about robberies, whether banks, trains, casinos or whatever you can imagine. If it has value, you will find humans who want to steal it. Some examples of real events in history are movies like Catch Me If You Can (where I have borrowed this articles title from) with Leonardo Di Caprio playing the role of Frank Abagnale and the cheque and other fraud that he committed by posing as a pilot and various other people. You may also be familiar with the story of Nick Leeson who was responsible for the collapse of Englands oldest bank (Barings Bank) due to his illegal private trading using the banks funds in the nineties. In the news each day you will hear stories of theft, corruption and misappropriation of funds at every level and all walks of life.

“THE PROBLEM IS NOT NECESSARILY...
POVERTY” Whether its someone who is unemployed and homeless, a wealthy CEO, a small business owner, a business partner, an employee, a politician or even a president, theft and dishonesty is there. It also doesn’t matter what country you are from, whether its South Africa or Switzerland. The problem is not necessarily lack of education or prevalence of poverty as these are not always present in the circumstances that lead to theft. However, there is one thing that is always common whenever theft and dishonesty occur and that is people. It’s odd that humans don’t need to be taught how to steal, rather we need to be taught to not steal. No-one is beyond suspicion when it comes to being 100% honest, not even you. Therefore, when it comes to staff members stealing it is almost surprising when we are surprised by it.

IS SECURITY THE ANSWER? At my home in Cape Town we have a very low front wall and high back wall, which has electric fencing on it. We have talked for a long time about putting up a high wall in front so that we can enjoy more privacy in our front garden. However, we are led to believe that as soon as we do this we are making it easier for criminals to move around on our property more freely because a high wall will hide them from public view. I know someone who has electric fencing, 24 hour video surveillance, alarms with armed response and security beams in their garden that trigger an alarm if anyone tries to walk across their garden. Somehow, with all this security in place burglars still managed to get into their house very easily and rob them. It’s almost like burglars are more interested when you go to greater lengths to protect your belongings, as if you have more value to protect and they are always
looking for even a single weakness in your security that they can exploit. Does this mean that we should just leave our houses open and not take any security measures? Of course not, but it does mean that these things in and of themselves will not always protect us but they may often deter criminals looking for an easy target. Therefore, security in your business, whether in your computer software, the safe or your internet bank access are all critical but you cannot then sit back, relax and think that you are fully secure.
VIGILANCE When looking up safety and security tips for protecting yourself and your belongings you will see that most of the advice revolves around being vigilant and that actual security systems are really only deterrents or last resorts. There is more advice about our behavior and our awareness than there is about what security systems to install.
Below are some ways in which you can be vigilant in order to prevent employee theft:

PLAN TO STEAL FROM YOURSELF
Have you ever been locked out of your own home? If so, were you surprised at how easy it was for you to break in to your house? Looking at your home security from the point of view of a criminal changes how you think about your security and helps you to attend to any security vulnerabilities that may be present. In the same way if you were a dishonest employee how would you steal from your own business? Ensure that you pay special attention to any areas of your business that offer employees an opportunity to steal. Write down a list of all possible weaknesses and start looking for any patterns that look out of place. Obvious areas (or processes) are things like sales transactions paid by cash, gift vouchers, day end cash up procedures, stock control, manipulating of computerized transactions after the time and banking procedures.

ANALYSE YOUR DATA
If you’ve done the exercise of “planning to steal from yourself” as mentioned above then you will have some ideas of where employees are possibly stealing from you. It is your responsibility to analyse your data in order to detect possible theft. Looking for patterns of behavior is important. If you pick up something suspicious then let staff know that there seem to be some odd patterns and are they able to shed any light. This alone could deter theft as staff members then know that you are scrutinizing their work and will be less likely to try and steal from you.

ALTERNATE RESPONSIBILITIES
Try and avoid giving too much or sold responsibility to one person. Rather try and rotate tasks between employees so that it is more difficult to conceal the fact that they may be stealing. Also this will help to highlight different patterns between employees work as you analyse your data.

CONCLUSION (AND CATCH)
Theft has been with us from the beginning of time and will be with us until the end of time. As we invent ways of protecting our belongings we invent ways of getting what we want (which often includes stealing it). A combination of security measures and vigilance is the best way for you to limit the effects of theft on your business, whether from external criminals or internal employees. However, when considering theft in your own business and the idea that someone may be stealing from you, look in the mirror ask the same things of yourself. Are you hiding money from the tax man? Are you paying all your suppliers what you owe them? Are you paying your staff a fair salary? These things may seem less harmful and distasteful than putting your hands in the til but they still end up in someone receiving less than what is due to them, whether by law or in fairness and in conscience. USE VISIBLE SURVEILLANCE
Visible surveillance, like cameras, are one way to deter employees from stealing. Placing the camera near your point of sale computer system is preferable as it shows things like whether the employee actually entered the sale into the computer, whether they gave a receipt to the client or whether they put the cash into the cash draw.

Trends in Cosmetic Surgical Procedures Written by Dr Chetan Patel
L
ies, damn lies and then statistics. The last survey of Plastic Surgeons Worldwide was conducted in 2010 and just under 700 questionnaires were received back of the 20000 questionnaires sent out. So clearly exact statistics mean very little, but there are general trends that can be extrapolated. Outpatient surgical procedures are definitely on the increase. Non-surgical procedures, especially Botulinum toxin injections, followed by hyaluronic acid injections (fillers) are the leading procedures requested in regions worldwide. There are definite geographic patterns in the top cosmetic procedures performed, but breast augmentation and lipoplasty (liposuction) remain at the top of the surgical list. So what’s happening locally? Well albeit on a smaller scale, trends in South Africa do mirror the worldwide trends. Outpatient surgical procedures, i.e. those that can be done in the “chair” or

office as opposed to having an admission to hospital are gaining in popularity. Non-surgical facial and skin procedures remain top of the list when it comes to numbers of procedures performed, because clients are seeing visible results with relatively low down times. Not only is office based surgery becoming popular but day case surgery in smaller surgical centres is also on the increase, particularly in cosmetic surgery where privacy and attention are important considerations for clients, as opposed to being one of a procession of patients being pushed along on the conveyer of larger hospitals.
COMMONLY PERFORMED OFFICE BASED SURGERIES: 1. Lipoplasty Liposuction - of particular areas only. Remembering that liposuction is a CONTOURING procedure and not a
weight loss or “drop-a-dress-size” procedure. So although those clients who are close to their ideal weights are better candidates, it is the EXPECTATION of the result that must be discussed very closely with your surgeon. Outpatient liposuction is usually performed under sedation controlled by an anaesthetist whilst the amounts that can be safely liposuctioned should be limited. This is a decision that is best discussed at a consultation with a surgeon. Love handles, lower abdomen and inner knees and neck and chin areas are suited to outpatient liposuction. Common misconceptions regarding liposuction are that it is unsafe; any procedure and for that matter any endeavor has potential risks. It is important to discuss those risks with a qualified professional (i.e. Plastic Surgeon) so that you are well informed of your options. Get answers to the following: - recovery
options in terms of managing this sensitive area. A few points to note: cosmetic surgery of the genital area in females deals primarily with the external genitalia - i.e. labia, mons (pubic area), and associated skin of the outer vagina. The procedure itself neatens the labia minor (inner lips), with additions to the procedure being liposuction of the mons (pubis area) and fat grafting to the labia major (outer lips) in order to improve the aesthetics. Sedation and local anaesthesia control pain and allow this procedure to be comfortably performed in an outpatient setting. Importantly if there are any urinary or pelvic “tightness” issues then these are problems that need to be dealt with by a gynaecologist. “G- spot” enhancement does not form part of the surgery (this is more hype than actual reality), although increased sensitivity of the clitoris due to increased exposure the clitoris from labial neatening may occur.

2. Blepharoplasty (Eyelid Rejuvenation) Upper and Lower Eyelid Rejuvenation is ideally suited to being performed in the office or chair. Local anaesthetic with or without Specialist Anaesthetist controlled sedation will be administered for your comfort. Candidates are those who have excess skin in the upper eyelids and sagging skin or bags of the lower eyelids. Potential risks are bruising, scar issues with respect to visibility and healing time (approximately 5-7 days before makeup will cover effectively)
3. Labiaplasty (Outer Vaginal Rejuvenation) This has become a popular procedure as more clients are educated about the

4. Liposculpture (Fat Grafting especially Facial) This procedure entails the removal and processing of fat followed by immediate re-injection into sites where it will markedly influence the effects of aging. Commonly injected sites are facial areas around the eyes, mouth, in the cheeks, temples, brow; backs of the hands and outer labia in the genital area. Fat grafting can be performed almost anywhere in the body where augmentation is desired (breasts or buttocks); where there are contour defects and where skin rejuvenation is required. Its advantage is that it is permanent and the effects long lasting. It is performed under sedation and locally injected anaesthetic to provide maximum comfort for smaller areas and under general anaesthesia in an operating room for larger areas (breasts and buttocks).

5. Surgical Tattoo Removal Tattoo removal is commonly requested procedure. The only 100% guaranteed procedure to remove tattoos is to remove them! As simple as that sounds, that is exactly what surgical excision does. It removes the entire affected area by removing all of the skin and then achieving closure by carefully suturing the skin edges together. A combination of surgical excision and laser therapy is also available in tattoos that are not amenable to being cut out, i.e. those that are too large or those that are in anatomical areas where the resultant scar from the excision is not desirable. A common misconception is that the scar which may result will be worse than the tattoo. Scars take time to heal and most scars treated in the appropriate manner will be inconspicuous at 6 months. The excision of tattoos depending on the size can be done under local anaesthetic alone and rarely may require sedation. Scarring if problematic can be effectively treated and need not be a lifelong concern. It is important to consult a Plastic Surgeon to discuss what your options are.

A
cne has emerged as the most common problem faced by dermatologists and aesthetic practitioners in private practice throughout the world. Progresses made in the control of many other skin conditions have increased its relative significance. Modern-day living with its increased stresses and strains, decreased exposure to ultraviolet light as well as the use of androgenic medications and supplements, have increased the prevalence of the condition. Acne has also become more common in older age groups, with 1% of men and 4% of women being affected at the age of 50! Due to its potential devastating effect on the skin and the psyche, acne needs to be addressed seriously at the primary, secondary and tertiary levels of intervention. At the primary level, patients with strong family histories of acne need to be referred for effective treatment when they first develop comedones. Avoidance of acnegenic medication and supplements as well as comedogenic topical preparations is important. Females with a history of acne as teenagers should avoid androgenic contraceptives (e.g. Mirena) as they can trigger the return of acne in the twenties and thirties. Diet does not play a significant role in triggering acne, but a healthy lifestyle with good nutrition and regular exercise reduces physical and psychological stress and contributes towards a healthy skin. At the secondary level, choice of therapy is determined by the grade of acne (where grade 1 is non-inflammatory comedonal acne and grade 4 is nodulocystic acne), the gender of the patient, and various other factors. For example, a patient who scars easily or whose income is dependent on a flawless skin, will be offered more potent treatments at an earlier stage. As no treatment can guarantee a cure, the aim of therapy is to improve the aesthetic appeal of the skin and to prevent scarring, using treatment that is safe long term and ideally has no adverse effects.

TREATMENTS ATTEMPT TO COUNTER THE 4 ETIOPATHOGENIC FACTORS OF ACNE, NAMELY:
1. Increased sebum production 2. Ductal keratinocyte hyper proliferation causing duct blockage and hypoxia 3. Overgrowth of Propionibacterium acnes 4. The innate immune system’s intense peri follicular inflammatory cascade.
Topical Retinoids are most effective at preventing and treating comedones, but are irritant. Comedo expressio2,5n may contribute towards a quicker result. Topical benzoyl peroxide, antibiotics and azaleic acid control the Propionibacterium acnes and the inflammation. Cleansers should be used twice daily and should be “gentle”, as over cleansing and abrasive ( “scrub cleansers” ) tend to irritate the skin. Salicylic acid is anti-bacterial, antimicrobial, and antiseptic, which deep cleanses inside the follicle naturally. Salicylic acid naturally stimulates exfoliation and therefore preventing acne breakouts. Benzoyl Peroxide kills acne bacteria. Helps prevent congestion and acne. A water gel base product delivers benzoyl peroxide deep into the pores. Vitamin A normalises the skin and reduces the activity of the sebaceous glands (oil glands). Grades 2, 3 and 4 acne require oral therapy in addition to topicals. Antibiotics remain first choice, with tetracyclines and cotrimoxazole being the most effective. The problems with long-term use of antibiotics are many, such as the development of yeast infections, the lowering of the body’s natural defences and the development of drug-resistance to antibiotics. Antibiotics should not be seen as a long-term solution, and is not recommended for more than 3-6 months at a time. In all cases of acne, never press or pierce the spots yourself, as part of the staphylococcus will penetrate the blood stream and produce even more spots if done incorrectly. What’s more, an acne scar will appear on the skin, making the whole treatment longer to complete.

In females, especially those with menstrual problems, those needing contraceptives and those with hormonal abnormalities (e.g. poly cystic ovary syndrome), antiandrogenic therapy (e.g. cyproterone acetate) may be more appropriate. The most effective acne therapy is oral isotretinoin. It is nearly always effective at the correct dosage and a full course may result in a “cure”, but it has many potential adverse effects and requires monitoring of liver functions, cholesterol and triglycerides. The psychological effect of acne should never be underestimated and should be addressed in all cases. Referral to a clinical psychologist may be indicated. At the tertiary level, the treatment of acne scarring has improved tremendously over the last 20

years. Cryotherapy and dermabrasion have been replaced by other surgical techniques (e.g. subcision and punch excision), medium and deep peels, microneedling and fraxel laser resurfacing. Keloids may respond to intralesional steroids or excision and electron beam therapy. It is usually advisable to treat the scarring only once the acne is no longer active. In conclusion, acne should no longer be regarded as “a condition that teenagers should just live through”! Therapy has become more effective and more user-friendly and should be implemented as early as possible.

The aesthetic industry is continually flooded with a plethora of antiageing treatment technologies. Consequently, choices can be overwhelming when considering which treatment modality is best suited to a particular indication. There is also talk of combining a variety of treatments in order to get the best results. Karen Ellithorne investigates.
A
n obvious question to ask when considering aesthetic treatments is – at what age should one start? “Prevention is better than cure,” says Dr Minoka Nadesan from Novi Skin in Pretoria. “Prophylactic treatment has proved to be more successful than trying to repair skin that is already damaged.” She agrees that there are age-appropriate treatments, such as Fraxel and VBeam, but stresses that basic skin rejuvenation and ageing awareness should start at a young age. Therefore, educating customers about the effects of UV A and UV B on the skin is essential. In addition, promoting water consumption will ensure that the skin is hydrated and that cellular functioning is at optimum levels. “Good skin care is not age-dependent or age-restrictive,” continues Nadesan. “A good doctor can perform a skin analysis and work out an appropriate skin care plan to prevent ageing, or correct the effects of ageing.” By the time you are in your sixties, 80% of the aging that you see on your face is due to sun damage and only 20% is due to the passage of time, according to dermatologist, Dr Mariaan Duvenage. “So you should be very diligent regarding the use of sunscreen from a young age. However, due to the use of sunscreen,
we are depleting ourselves of vitamin D, as your skin forms vitamin D due to the exposure to UVB rays from the sun. It is advisable to have your vitamin D levels checked about twice a year, as low Vitamin D levels can have disastrous consequences for your health,” she emphasises. Plastic surgeon Dr Japie de Wet concurs. “You can never be too young to start with a good skin care regimen and healthy habits.” He explains that invasive treatments, especially ones with permanent effects, need to be carefully considered and appropriately timed. For instance, a hyaluronic acid dermal filler treatment in the early twenties will not do any harm. By contrast, an inappropriate fat graft to the lips might result in a lifetime of regrets. De Wet emphasises the fact that a trustworthy physician will strive to undertake a lifelong journey of appropriate interventions with the customer.
WHAT TREATMENTS, WHEN? During one’s twenties rejuvenation deals mostly with prevention rather than treatment. COMBINING TREATMENTS Dr Martina van der Mescht, an aesthetic practitioner on the West Rand, says: “No single cosmetic procedure can be expected to reverse all the unwanted signs of photo- or chronologicageing of the face. However, a growing body of scientific studies, coupled with recent clinical experience, shows that combining Botulinum toxin treatment with other techniques synergistically enhances, refines, and may even prolong, the effects that can be achieved with any single procedure.” The key signs of chronologic ageing of the face are either dynamic changes (eg. rhytids arising from hyper-functional muscular activity) or static changes, such as atrophy of underlying soft tissue, sagging and certain superficial wrinkles. Most patients seeking facial rejuvenation treatment have both dynamic and static signs of ageing. It is, therefore, a logical approach
to combine various treatment modalities to address these different needs, according to Van der Mescht She advises that whatever combination is ultimately chosen, the treatment should preferably result in a subtle, natural look, which leaves patients looking refreshed rather than changed. To achieve this requires both a careful assessment of each patient's particular facial anatomy in both dynamic and static positions, as well as a detailed discussion about their needs and expectations. Equally important is the physician's recommendation about which combination of treatments to use, and in which order they should be given. “Botulinum toxin treatment provides effective, reversible chemodenervation, resulting in the softening and relaxation of facial wrinkles. Its unique mechanism of action, simplicity, ease of use, safety and adjunctive use can be considered with every other treatment modality, and is mostly used by the 20 – 30 and 30 – 40 age groups.

Karen Ellithorne asks three experts some of the questions we have often wondered about but never had clarity on. We have all seen the expressionless faces and over-exaggerated lips of some celebrities in the media and wondered why anyone would want to do this to themselves. In reality we would all like to age gracefully and embrace our wrinkles but subconsciously we are aware of the fact that as we age we become invisible to society. Today people are living longer and having to work way past retirement age, so

maintaining our youthfulness has become an important factor. Dr Bryan Mendelson, the former president of the International Society of Aesthetic Plastic Surgery, says that pretty people not only get a head start in life, they also tend to have better relationships and earn more money than their less attractive peers. This explains why medical aesthetic treatment has become so popular and why this industry continues to grow dramatically every year. However, aesthetic treatments have always been controversial due to the publicity these treatments have received in the press when the results achieved on celebrities are neither favourable nor subtle.

Dr Renier van Aardt: Although it is true that muscles atrophy during long durations of not being used, they can certainly regenerate when treatments cease. In the case of receiving botulinum toxin for cosmetic use though, it actually works in the patient’s favour, by extending the intervals between treatments and thereby lowering the cost of maintaining the cosmetic results.

If someone has botulinum toxin continually long term, is there a danger that the muscles that the toxin paralyses may atrophy at some stage? Dr Malcolm Murdoch: No, botulinum toxin causes a temporary, dose-dependent and fully reversible paralysis of the injected muscles. During the time of paralysis there will be muscle atrophy, but full strength will be regained once the effects of botulinum toxin wear off. Repeated treatments before full recovery will result in sustained muscle atrophy (and this may indeed represent the desired effect). However, once such treatments are stopped, recovery of the treated muscles

Is it true that botulinum toxin treatments cause new lines to develop elsewhere as you end up using muscles that you would not normally use to compensate for the botulinum toxin-paralysed muscles? Murdoch: Yes, if some of the muscles in a ‘synergistic group’ have not been treated then increased neuronal input will cause these muscles to hypertrophy and may produce new lines. An example of a ‘synergistic group’ is the frown muscles, which consists of up to seven muscles [one procerus, two corrugator supercilli muscles, two depressor supercilli muscles and two orbicularis occuli muscles]. If only three of these muscles are treated, then when a patient tries to frown, supra-normal neurological activity will occur in the non-treated muscles causing their hypertrophy. Van Aardt: Areas that are typically treated, for instance the glabella, or depressors of the mouth corners, target a specific muscle complex, leaving none, or very minor ability to compensate
with adjunctive muscle activity. Often patients will notice other lines in the vicinity as the target areas become smoother, but it is usually only because they appear more obvious in contrast. Even if an area does become affected by compensatory movement, it is usually easily remedied by a touch-up treatment.

What is the history of toxins and how long have they been used safely in humans? Murdoch: The discovery that botulinum toxin is produced by strains of the bacteria, clostridium botulinum, was made in 1897 by Belgian microbiologist Emile van Ermengem. It was only during the 1960s and 1970s that therapeutic uses of the toxin were investigated. The first human trials were conducted in 1980 for the correction of strabismus (squint). In 1989, Botox, manufactured by Allergan, Inc. was licensed by the FDA (Federal Drug Administration), making it the first commercially available botulinum toxin. The use of Botox for cosmetic reasons was licensed by the FDA in 2002. Presently, more than 30 million patient treatments with Botox are performed every year.

What do doctors mean when they talk about off label use? Van Aardt: Off label use refers to using a prescription medication in a way that did not receive approval by the regulatory body, such as the FDA, either because no studies for a particular use were submitted, or because insufficient evidence was provided. Doctors may, in their professional discretion and judgement, use a medication in an off label manner, as long as it is disclosed to the patient and consent was given. Medical Aesthetics 2014/15 //

Additionally, since the botulinum toxin molecule is actually an enzyme, heating the tissue to temperatures above physiological normal may cause the enzyme to be denatured. I therefore advise my patients that pressure and heat in the treated area should be avoided for six up to 24 hours if possible.
5.
How long before the customer sees a result? Van Aardt: Results can become evident as soon as an hour after a treatment is done, but the effects usually peak about 10 days after. There is some variation between patients in this regard.

Do you have any specific home care advice post treatment? Murdoch: The after-care advice varies widely between practitioners. From a biological point of view, avoiding physical pressure in the area of injection is recommended, since the botulinum toxin molecules must undergo active transport from the extra-cellular environment into the nerve cell. Before this process is complete, the molecules may be moved into areas (by tissue pressure such as massage) where their effect is not intended. This may cause unwanted and adverse effects.

What is the correct treatment protocol for botulinum toxin? Van Aardt: To answer this, we need to understand the mechanism by which botulinum toxin works. A small part of the botulinum toxin molecule, known as the light chain, enters the end terminal of the nerve after the toxin molecule binds to a receptor on the cell. This light chain disrupts the mechanism by which the nerve sends the message to the muscle to move. It takes the nerve cell 91 days to regenerate the disrupted mechanism before movement will return. Therefore, in the initial phase of about 12 months, treatments should be spaced about three to four months apart to relax the muscle enough to weaken it. Thereafter, treatment intervals will gradually prolong because the muscles shrink and weaken and plateau at about six months for most patients. Some areas may not last as long due to differences in muscle shape and size. Treating more often than 91 days is not harmful, but it would be wasteful.

Can you have laser and RF treatments at the same time as having botulinum toxin to rejuvenate and firm the area? Murdoch: No. It is not advisable to combine laser or RF treatments with botulinum toixn because of the risk of denaturing the toxin molecule due to the heat generated by the device (see above). In addition, a more even and aesthetically better result from the laser or RF treatment may occur when the wrinkles are relaxed and the skin smoothed. Thus, most practitioners recommend treatment with botulinum toxin first and the laser of RF device after two weeks.

Is there anything specific to look for when purchasing a dermal filler? Murdoch: There are over 180 different HA (hyaluronic acid) fillers on the market, as well as numerous other filler materials. Because dermal fillers are registered as medical devices and not scheduled medications, less rigorous regulatory requirements need to be fulfilled before these products may be marketed. This means that a wide range of quality is presently available. For patients and practitioners alike, it is essential to choose a product that has proven clinical benefits, has undergone rigorous testing and has an extensively studied safety profile. One of the best ways to do this is to choose a product from a leading global brand, with an outstanding clinical heritage. Modern day fillers consist of hyaluronic acid. Please elaborate on this ingredient. Ursula Hunt: HA is a gel-like water holding molecule that is a space filler and cushioning agent. It cushions joints, nerves, hydrates skin and hair, and fills the eye. HA acts as a network that transfers essential nutrients from the bloodstream to skin cells. The presence of HA in epithelial tissue has been shown to promote keratinocyte proliferation and increase the presence of retinoic acid, causing skin hydration. Present in the extracellular matrix of basal keratinocytes, HA is critical to the structural integrity of the dermal collagen matrix. These benefits make hyaluronic acid a very effective topical humectant.

Hyaluronic acid helps to retain over a 1000 times its weight in water within the cells of skin, making it an excellent moisturiser. No other biological substance can retain as much water as HA, resulting in increased smoothness, softening and decreased wrinkles. Decreasing levels of HA are known to accompany the ageing process and it is estimated that by the time we reach our mid-40s, the synthesis of HA is roughly half that required by the body.

4 Strengthen your team ime spent together as a team is essential. Some of my clients organise an event (which they pay for) with the T entire team about once a month. They gather together for a fun experience such as a professional cooking lesson, drumming circle or dance class. This fosters terrific rapport between staff members of all levels.
5 Train from the top et your top three product retailers to train your entire team with their tips and tricks of the trade. G The same method could be utilised with those staff members who are particularly good at building strong and lasting relationships with their clients. Making an example of these staff members will also give your top achievers the recognition and limelight they deserve for doing their job well.
6 Conduct ‘stay’ interviews ather than conducting exit interviews, find out from your staff members why they stay and what R they enjoy about their position. This will give insight into what they value about your employment approach, thus allowing you to expand on whatever is working for you already.
7 Provide a clear path for advancement Employees will become bored or frustrated if they see no clear future for themselves at your company. A great employer will provide the best foundation for employees to grow within their careers. Communicate closely with employees who report to you to ensure that each individual is excited and challenged to perform.
