A new electrosurgical knife, the SB Knife Monopolar ESD Knife, is now available is the U.S. through an exclusive distribution agreement between Olympus and Sumitomo Bakelite Co. This scissors-type device, named for the shape of the pincers of a stag beetle (SB), is used in Endoscopic Submucosal Dissection (ESD) to remove early gastric cancers that have not entered the muscle layer. The minimally invasive endoscopic procedure allows the physician to perform en-bloc resection of larger lesions (defined as specimen removal as a single piece) and can help the patient avoid open surgery.

Thus far in 2017, healthcare has been at the forefront of the national discussion. Even during the tumultuous passage of the Affordable Care Act, seven years ago, there wasn’t as much debate about the repercussions of national healthcare policy as there has been in the effort to “repeal and replace” it.

Back then, the particulars regarding the ACA and its legislative journey were often framed as a consideration of political winners and losers. The most consistent questions were centered on the speculative benefits for the politicians who supported or opposed it.

But those who work at hospitals, clinics, or other treatment centers are well aware that healthcare legislation has far more significant repercussions than polling bumps or dips.

They understand how shifts in insurance availability or the protections in place — for patients and providers — play out in people’s day-to-day lives.

And as much as political figures might want to keep the debate in the realm of the safely abstract, plenty of citizens are stepping forward with personal stories about how Washington choices alter the basic availability of treatment.

There seems to be a more pronounced interest in determining the impact of healthcare legislation on the citizenry. How will proposed alterations affect individuals? Will the changes improve lives or add challenges?

An even greater transformation is in the substantive attempts to examine how new federal policies might shift the fiscal footing of medical facilities, especially in more remote locales.

“All these rural hospitals are operating on thin margins,” Alan Morgan, CEO of the National Rural Health

Association, told Kaiser Health News. “The removal of any income source or coverage, or expansion of bad debt, is going to create significant financial hardship.”

Because rural hospitals are more likely to serve a lower-income population, ACA provisions that help those individuals get coverage naturally grow patient volume. That influx of new healthcare consumers doesn’t address every budgetary concern, but losing those freshly insured patients could be devastating.

This concern hasn’t gone unremarked upon. The same is true of all the other potential repercussions that come with tugging on the overlapping and interconnected beams of the healthcare infrastructure.

It’s all being talked about, and the debate is being largely conducted openly, honestly, and with intellectual rigor. The talking points are being knocked aside by clear-eyed commentary on realistic outcomes, whether from engaged citizens in the offices of their elected officials or industry representatives acknowledging the gaps in coverage that still exist — and could worsen if handled carelessly.

The more people — politicians and average citizens — who become acquainted with the rough waters healthcare providers sail on, the more likely we are to arrive at productive fixes. That makes it imperative for healthcare professionals to continue adding their own perspectives. More than anyone else, they see the big picture. They can see what’s wrong and right, and they know how to offer a diagnosis.

Much as the topic might seem well-worn, it’s likely that the complex conversation is still in the earliest stages. Now is the time to join it.

Purging Pathogens Healthcare facilities need to employ proactive and effective methods to combat C. diff infections

When most people think of hospitals, they imagine a safe, clean environment where patients can comfortably receive treatments in an effort to become healthy.

In reality, hospitals can pose potential risks to both patients and staff, through the threat of infections caused by pathogens such as MRSA, E. coli, and Clostridium difficile (C. diff). In fact, C. diff is the most common microbial cause of healthcare-associated infections in U.S. hospitals and costs up to $4.8 billion each year in excess healthcare costs.

Despite a universal commitment to addressing C. diff infections within health facilities, prevention and containment pose numerous challenges as the spores can survive for months on a large variety of surfaces. Fortunately, proactive measures — including the use of surface disinfectants — can be taken by healthcare facility managers to mitigate the risk and promote the well-being of both patients and staff.

Product Considerations For Surface Disinfectants When choosing an effective disinfectant to use, it’s important that healthcare facilities and hospitals consider the following factors: 1. Determine whether the product effectively addresses the facil-

ity’s organism(s) of concern.

For example, to mitigate C. diff spores, facilities should use an EPA-registered antimicrobial product effective against C. diff spores, as opposed to a general-purpose disinfectant. Standard EPA-registered hospital disinfectants are not effective against C. diff spores. The EPA maintains a listing of appropriate C. diff sporicidal products under “List K”.

2. Consider the dwell time — the amount of time that a surface must remain wet to allow the chemical to be in contact with the organism in order to kill it — before choosing a product.

It’s important to note that not allowing for the appropriate dwell time will decrease efficiency and reduce or negate the efficacy claims as directed on each product label. To not use an EPA-registered disinfectant per its specific label directions is a violation of federal law. Ensure the disinfectant is used safely and correctly by individuals, and that it is applied onto and/or with the variety of surfaces and equipment for which the disinfectant has been approved.

3. As cost is an important consideration when making purchase decisions across the entire healthcare industry, consider choosing a product that is highly versatile and provides an extended shelf life.

Bleach and peracetic acid in ready-to-use form can be expensive, and many people have aversions to the smell. In addition, the products can damage surfaces and cause discoloration.

Additional Considerations When Addressing HealthcareAssociated Infections In an effort to mitigate healthcare-associated infections, it’s imperative that facility managers take a proactive approach to their cleaning routine to minimize risk, reduce hazardous exposure, and address potential issues. to product choice, facility managers should consider several factors when designing a cleaning plan.

First, training environmental services staff is a key factor in preventing the spread of infection. A proper training program will allow facility managers to teach and train their staff the proper way to clean and remove the organisms, while keeping safety and efficiency paramount.

In addition to training, it’s beneficial to develop a solid partnership with the healthcare facilities’ infection control department. By working collaboratively with the hospital’s infection control staff, both parties can ensure they are effectively working to reduce infection rates without duplicating efforts.

Facility managers should also implement a comprehensive surveillance and reporting plan to measure surface cleanliness and identify where and when infections are occurring in a building. Developing and executing a strong infection control strategy will help promote and ensure the most effective disinfection practices across the facility.

Most importantly, an effective plan can help identify potential trends and threats before they happen, as well as provide confirmation that surfaces and rooms are being cleaned appropriately.

In recent years, C. diff prevention has become a nationwide priority with new national reduction targets scheduled for implementation by 2020, according to the National Action Plan to Prevent Health Care-Associated Infections: Road Map to Elimination. As a result, it is critical for healthcare facilities to have a thorough plan in place that addresses potential infections both in the short-term — through the use of an effective surface disinfectant — and in the future — through planning, surveillance, and reporting.

Added Dimensions EchoPixel is changing the way surgeons look at imaging data by bringing scans into the 3D world

Plenty of companies are trying to determine how to best leverage virtual reality and similar technologies to modernize healthcare. EchoPixel is somewhat unique in that it began on that quest with an outsider perspective.

“EchoPixel is my second company,” explains Sergio Aguirre, MSc, the founder and CTO of EchoPixel.

“My previous company focused on visualization systems for geographic information systems and oil exploration. But I was always curious about getting into the medical space. Once I discovered radiology, I decided to start EchoPixel.”

In Aguirre’s view, the healthcare community wasn’t taking full advantage of the data they already had on hand. Although imaging technology gathers information on anatomy that is best examined and understood in its complete form, most physicians look at scans in a 2D format.

Aguirre determined there was a better methodology, allowing the gathered data to be presented in 3D, more accurately replicating what surgeons encounter in a procedure.

But Aguirre also understood he’d benefit from the insights of the healthcare professionals who’d be reliant on the tools he developed.

“We engage with clinicians and try to understand the clinical problems to great depths and try to understand how our technology can alleviate those problems,” Aguirre says. “Then we combine

by the experience of Aguirre and the EchoPixel team. They’ve found that healthcare professionals have a strong grasp of all the different possibilities, but pulling all the components together into a new, complete solution eludes them.

That’s where EchoPixel comes in.

“A lot of people think — and this is one of the things that doctors who are tech-savvy stumble upon — is that if only they had this hologram, they‘d be able to see everything; that’s not necessarily true,” Aguirre says. “You can incur data overload very quickly. CTs and MRI have an abundance of data. So, you need the right interaction tools to get to part of the data that you want.”

The EchoPixel software is designed to maximize the existing data to generate a 3D image that can be viewed above a flat panel monitor set horizontally or at an incline.

"Imagine lines coming out of each corner of a display that creates a cube on top of the flat panel and within that space you can load data from a PACS server of whatever imagining repository the hospital has: CT, MRI, or ultrasound image data," says Aguirre.

Once the 3D image is in front of the physician, they can use a stylus to interact with it.

“You can move around it and see around it,” notes Aguirre. “You can use a stylus to reach into that open 3D space and create cross-sections or measurements

This approach allows surgeons to examine the area they’ll be operating on before the patient goes under anethesia. It can be precise enough — perfectly duplicating dimensions, size, and placement — that surgeons can extensively practice and develop muscle memory for the procedure before the patient is wheeled into the OR. As an example, Aguirre recounts how the system might be used for a complicated heart surgery.

“Let’s say that you have a pulmonary valve replacement or they're doing some pulmonary artery defect correction or even septal defects,” he says. “Instead of doing full sternotomy, they might want to do a partial sternotomy. So, what they do is they bring the data they have and put the patient in the same orientation and position they will have on the operating table.

They use our stylus to define and edit the data of how that partial sternotomy is going to look like

once they have the patient opened up.”

Surgeons can check that all the instruments will fit comfortably and anticipate other potential issues.

Once the 3D image is in front of the physician, they can use a stylus to interact with it.

They can also use the EchoPixel tool to make sure the entire surgical team has a complete understanding of the plan before the procedure gets underway.

For especially complicated procedures, surgical teams can even pause in the midst of their work to consult the EchoPixel imaging as confirmation they’re still on the right clinical path.

Aguirre believes that all surgeons should incorporate strategic use of medical imaging directly into their work. Too many hospitals, he notes, still use a structure that’s reliant upon radiologists analyzing imaging data and composing summaries for surgeons.

He also recognizes the inherent disconnect between most current imaging technologies and the needs of surgeons. He suggests EchoPixel is especially well suited to bridge that divide.

“The other reason these different surgical specialties are not necessarily utilizing medical images is because it's not anatomically meaningful to them,” Aguirre says. “They were not trained to look at 2D images. We talk to surgeons about imaging and they say, ‘I never open a patient and see a 2D view which makes it very difficult to understand what exactly I should expect in the OR.’” EchoPixel doesn’t just add a dimension; it provides a recognizable view of the patient. For surgeons, that is no small distinction.

Compatible Combat In the fight against infections, it’s important to keep surface material compatability in mind

This crucial factor is surface material compatibility, or the ability of material to be exposed to a disinfectant’s chemistry without exhibiting reduced functionality as a result.

Compatibility ensures that a materi-

al’s chemical resistance aligns with a disinfectant’s particular chemical profile, allowing a healthcare facility’s surfaces and devices to be properly cleaned and maintained while maximizing the surface material’s performance over time.

Problems with compatibility can result in issues that have a profound impact on devices, the facility, and the patient.

Incompatibility can lead to cracks where bacteria can hide. The article states that inadvertent perioperative hypothermia affects an estimated 70 percent of surgical patients and is associated with adverse clinical outcomes.

These results indicate that there is still a significant need for ongoing interventions to educate and promote best practices on hypothermia prevention. Titan Medical is one of the manufacturers working hard on bringing forward a new option for the OR. They are currently in the pre-commercial phase with their SPORT Surgical System, but they promise it’s on the fastest possible track.

To learn more, Surgical Products interviewed David McNally, president and CEO of Titan Medical.

What’s different about the Titan Medical SPORT Surgical System?

Our system is based on a fundamental, multi-articulating arm technology that allows us to deploy snake-like arms — along with a camera — through a single incision that is expected to be 25 mm in diameter. We are focused on single-incision surgery in the abdominal area in single- and

multi-quadrant surgeries. The system consists of a surgeon workstation and patient cart, and the patient cart has a single boom — unlike some multiport systems — with a single arm that then suspends the camera insertion tube, through which the multi-articulating arm, end effectors, and the camera are deployed into the body.

I’d say in terms of differentiating features of our system, they begin with the multi-articulating arms with disposable end effectors that allow the surgeon to experience first-use quality during each surgical procedure. The mobile patient cart can be easily moved from operating room to operating room as well as within hospital facilities. Our surgeon workstation is designed for upright positioning of the surgeon in a very comfortable manner. It is adjustable for outstanding ergonomics and comfort for the surgeon. And the 3D, high definition visualization is delivered with a flat panel display, which provides the opportunity for 3D viewing by not only the surgeon but also fellows and residents who can see exactly what the surgeon is seeing.

Will it also be more economically feasible for those facilities?

For smaller hospitals, footprint and mobility are issues, but cost is a major concern, too.

We’re mindful of price point with respect to the capital equipment — which further manifests itself in annual service costs after the first year of warranty — as well as the consumables, those being the disposable and reusable components of the system that drive per procedure costs. We are carefully studying the market and pricing, and we believe that we can position ourselves to provide savings versus the entrenched systems in the marketplace today.

Have you built surgeon feedback into the process of developing the system?

We have engaged surgeons from the beginning in providing us with feedback with respect to the surgeon workstation as well as the patient cart.

Where is Titan Medical going in the process of bringing this to market?

With 30 years of experience in medical device commercialization, I was brought in to take the company's clinically-inspired technology and apply a disciplined approach to product development. Based on the experts I brought in since the beginning of the year — including an experienced vice president of research and development and an experienced vice president of quality and regulatory affairs — we believe that we can both complete product development and navigate the regulatory processes for commercialization sometime in 2019 for a limited commercial launch that year in both the U.S. and Europe. Those are our best projections, of course assuming that — based on the experience of our team — we can successfully navigate processes with known pathways established by those that are already in the market.

Do those manufacturers who’ve already made it to market with robotic surgery systems clear the way for you somewhat in the regulatory process?

Yes. We have great respect for Intuitive Surgical and what Intuitive has accomplished in market penetration and procedural depth with the da Vinci. We are also excited to see other competitors, such as TransEnterix, that have entered the marketplace, and we believe that helps to further validate the presence of robotics today and its potential for growth in the future.

That trend is especially true in surgical units where grueling hours, complex cases, and bureaucratic hassles are the norm.
