Hormones and Weight: Finding the Right Balance By Betsy Stander, APRN

A

re you struggling to lose weight even when you’re doing everything ‘right’? Are you suddenly gaining weight although your diet and exercise have remained the same? Your hormones could be to blame! Hormones are important to all body functions and play a significant role in your ability to lose weight and maintain a healthy weight. There are many hormonal imbalances that can occur in the body including, but not limited to: elevated insulin, elevated cortisol, elevated or low estrogen, low testosterone, low DHEA, and hypothyroidism. It is important to recognize possible imbalances that may be occurring in the body and correct them in order to reach your weight loss goals.

Insulin – the fat storage hormone

Insulin has an important job. It processes the sugar in the blood stream and carries it to your cells – it’s then used for energy or stored for later use in the form of fat. Elevated levels of insulin can be caused by excess consumption of carbohydrates. A diet high in carbohydrates, such as processed foods and sugary beverages, in combination with a low protein and low fiber intake, can lead to the storage of fat, weight gain and high insulin levels. Poor diet along with lack of exercise, alcohol consumption, stress and high blood pressure can lead to chronically high levels of insulin and the development of Type 2 diabetes.

Cortisol – the stress hormone

High levels of cortisol can be harmful to your health. Studies indicate that a high stress level can contribute to the accumulation of belly fat. Cortisol levels are higher in people who suffer from conditions such as anxiety, depression, PTSD, and excessive fatigue. Not only does elevated cortisol lead to belly fat, it also increases appetite and cravings. This is why people turn to www.kchealthandwellness.com

“comfort foods” when feeling significant amounts of stress! Other harmful effects of high cortisol include decreased bone density, memory loss and depression. What is the best way to combat high cortisol? Get some sleep! At least 7 hours a night. It is also important to eat a high protein breakfast and find an outlet for all that stress. Activities such as yoga, meditation or even taking a long walk can help decrease stress.

K a n s a s C i t y – M a r c h 2 0 1 6 Health & Wellness

Estrogen – the reproductive hormone

Both men and women naturally produce estrogen. In men, an excess of belly fat can cause testosterone to be converted to estrogen. As a man’s estrogen level rises, so does the fat accumulation. In addition, high levels of estrogen in men can contribute to prostate cancer and heart disease. In women, both excess estrogen and low estrogen contribute to weight gain and difficulty maintaining a healthy weight. Estrogen levels fluctuate as a woman goes through phases of her reproductive life. What can be done if you suspect an estrogen imbalance? Get your hormones tested. Testing can identify if an estrogen imbalance exists and what methods of treatment would be best. Also, it is important to eat a healthy diet including green tea, flax seed and chia seeds. All of these products can assist the body in processing extra estrogen.

Your thyroid gland

The thyroid gland is responsible for producing hormones that regulate the body’s metabolic rate, heart and digestive function, muscle control, brain development and bone maintenance. The thyroid gland can become overactive (hyperthyroidism) or underactive (hypothyroidism). Thyroid conditions can be quite dangerous and cause many unpleasant symptoms. For patients suffering from hypothyroidism, weight gain is a common side effect. Thyroid conditions can be safely treated with medication.

DHEA – the mother of all hormones

There is much talk these days about DHEA, but what exactly is it? DHEA is a precursor hormone secreted by the adrenal glands that can be converted into testosterone or estrogen in the body. DHEA levels typically hit their peak in our 20’s and then naturally decline as we age. DHEA is responsible for many important functions including the body’s ability to burn fat and keep it off. Low DHEA levels can be detected in hormone testing. Correcting an imbalance can improve libido, energy and weight loss.

Betsy Stander, APRN

Betsy received her Bachelor of Science in Nursing from Washington Adventist University located in Maryland and went on to complete her Masters of Science from University of Missouri in Kansas City. Betsy has worked in women's health care for over 15 years including labor and delivery and gynecological surgery. As a nurse practitioner for Mirabile M.D. Beauty, Health & Wellness, Betsy provides gynecological services, bioidentical hormone replacement therapy and medically supervised weight loss counseling (Medi-Weightloss®).

Testosterone – the sex hormone

Increased stress (high cortisol levels), abdominal fat and aging all contribute to low testosterone levels. Low levels of testosterone can lead to conditions such as depression, obesity, heart disease, loss of muscle tone, decreased stamina and decreased libido. Simple hormone testing can identify a low level of testosterone. When levels are corrected, it has been shown to decrease body fat and improve stamina, muscle mass and libido. A healthy diet in lean protein, as well as weightbearing exercise can help naturally increase testosterone levels.

7

Get your levels checked

Hormones play a significant role in your health, including weight loss and weight management. A simple blood test is the first step in understanding your hormonal health. At Mirabile M.D. Beauty, Health & Wellness, we are experts in the field of bio-identical hormone replacement therapy. We offer safe, natural and effective methods for both men and women. Smart Choices for Brain Injury Prevention

The Brain Injury Association of America cites the most common causes of brain injury as vehicle crashes, falls, sports injuries and violence. To prevent injury, MidAmerica Rehabilitation Hospital encourages its community to think ahead and make the following smart choices for brain injury prevention: • Wear a seat belt every time you drive or ride in a motor vehicle; • Buckle your child into a child safety seat, booster seat, or seat belt in the car; • Never drive or engage in physical activity while under the influence of alcohol or drugs; • Wear a helmet and make sure your children wear helmets during appropriate activities; • Use safe guards to avoid falls; • Check first the depth of the water when swimming or diving;

K a n s a s C i t y – M a r c h 2 0 1 6 Health & Wellness

• Make sure the surface on your child's playground is made of shock-absorbing material, such as hardwood, mulch and sand; and • Keep firearms stored unloaded in a locked cabinet or safe. Store bullets in a separate secured location

Awarded Disease-Specific Care Certification

MidAmerica Rehabilitation Hospital affirms that it has maintained the highest standards to address these conditions by achieving DiseaseSpecific Care Certification for its brain injury rehabilitation program through The Joint Commission. To help restore the brain’s function following an event that results in a brain injury, MidAmerica Rehabilitation Hospital engages individualized treatment programs, state-ofthe-art technology and dedicated rehabilitation teams to work with patients one-to-one for a welcomed return home. Daily physician* visits, ‘round-the-clock certified nursing care and experienced therapists guide patients in reaching their goals for a faster return home.

Feeling PULLED BETWEEN Your Parents and Your Kids? Continuing care at Tallgrass Creek may be the solution.

T

oday’s generation of 40 to 60 year olds find themselves in an unprecedented situation. Many members of this so-called “sandwich generation” are pulled between the needs of their children and their aging parents for whom they’re often providing full-time care.

While caring for Mom or Dad can be a reward, it can also take a heavy emotional toll. As a result of their “double duty,” more and more adult caregivers report increased feelings of stress, particularly as their loved one’s needs become more complex. One solution is to research the numerous care options available, including Tallgrass Creek. Located in Overland Park, this 65-acre senior living community offers independent living, plus dedicated assisted living, memory care, nursing care, and post-acute rehabilitation. Here, Mom or Dad enjoy a stimulating environment filled with friends and activities—all while receiving a higher level of care than you could provide at home.

Doing “double duty”

The Pew Research Center estimates that just over one in every eight Americans age 40 to 60 cares for a parent in addition to a child. While the average lifespan has increased, many

seniors still find themselves in need of complex and costly care. Meanwhile, a sluggish economy and changing cultural norms have seen more young adults returning home after college or in between careers. Members of the sandwich generation are working a double shift, providing emotional and financial support to their children as well as their parents—with few outlets for their own needs. Factor in the demands of our 24/7 work lives, and it’s easy to see why so many caregivers feel overwhelmed.

When to consider continuing care

Mom or Dad’s health can change in an instant, so it’s smart to research the care providers in your area ahead of time. This way, you’ll be ready should your loved one need to make a quick move. Often, however, Mom or Dad’s needs evolve over time. Eventually, you may not feel qualified to offer the level of support required. In other instances, the pressure and responsibility of caregiving can begin to affect your family, your relationships, and even your career. In any of these situations, it may be wise to consider a continuing care provider like Tallgrass Creek. www.kchealthandwellness.com

What to look for in a provider

Ultimately, you should feel confident in the team who will be providing care. At Tallgrass Creek, the dedicated team specializes in the unique needs of seniors. They take the time to get to know and care about your loved one on a personal level, providing one-on-one attention and support. Tallgrass Creek will even coordinate the details of Mom or Dad’s care, scheduling appointments with the doctors at the on-site medical center. You won’t have to spend another morning on-hold with Mom’s medical office, or take off work to drive Dad to his follow-up. While many continuing care providers claim to be experts, Tallgrass Creek has the experience to support those claims. The community is backed by Erickson Living®, a network of communities with more than 30 years of experience in senior living and health care.

Start your research today

By learning about your options today, you can save time and reduce your stress in the future. To find out more about assisted living, memory care, nursing care, or post-acute rehabilitation at Tallgrass Creek in Overland Park, call 913-752-9553 or visit EricksonLiving.com.

TIPS: yOU’RE CARING FOR yOUR PARENTS, BUT ARE yOU CARING ENOUGH FOR yOU? FOLLOW THESE 6 TIPS TO AvOID CAREGIvER BURNOUT: 1 Research your loved one’s condition. Learn as much as you can about their health concerns, medications, symptoms, and how they may change. This will help you stay informed and know what to expect. 2 Consider a few home modifications. Install grab bars, shower seats, and invest in easy grip utensils that can help your loved one remain independent and complete daily tasks. 3 Ask for help. Tell friends, family, and neighbors when you need help. Try not to criticize if they don’t care for your loved one exactly as you do—as long as your loved one’s needs are met. 4 Remember to take care of you. Eating well, exercising, and taking time to unwind are key to avoiding burnout. Familiarize yourself with the signs of depression, which can affect caregivers. 5 Don’t take it personally. Alzheimer’s, dementia, pain, and the stress of chronic conditions may cause your loved one to say hurtful things. Remember, it’s the illness talking, not your loved one. 6 Know the resources available to you. There are a wide variety of programs and services to help you manage the challenges of caregiving. Review the following helpful resources for just a few examples.

Helpful resources

• Eldercare locator. visit www.eldercare.gov to search for services in your local area. • Community resources. your local United Way and faith-based organizations can also help you find assistance. • Social workers. t is a frustrating feeling to work hard your whole life to support yourself and your family only to suffer a debilitating injury or illness that cuts your career short. It's even more frustrating when Social Security, the government safety-net that you have been contributing to for years, denies your claim to collect Social Security Disability benefits. At the Law Offices of Sharon J. Meyers, in Kansas City, Missouri, we understand how the frustration caused by not being able to go to work is compounded by the stress of facing mounting bills and not having sufficient income to pay them. Our first message is not to panic if your initial application for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) has been denied. Some 80 percent of firsttime applicants are denied benefits by the Social Security Administration. 0VS $PNQFOTBUJPO )JOHFT 6QPO :PVS 4VDDFTT We take all SSDI and SSI cases on a contingency fee basis, which means you do not pay attorney fees unless your claim is approved. It is common and important to appeal a denial of an initial SSDI or SSI claim. The key is to be persistent. And it helps to enlist the services of an experienced Social Security disability lawyer who understands the complex application process. 0VS FYQFSJFODFE MFHBM UFBN — and it truly is a team effort at the Law Offices of Sharon J. Nutrition for Older Persons Both the number and the proportion of older persons – defined as aged 60 and over – are growing in virtually all countries, and worldwide trends are likely to continue unabated.

Defining the Specific Nutritional Needs of Older Persons

Older persons are particularly vulnerable to malnutrition. Moreover, attempts to provide them with adequate nutrition encounter many practical problems. First, their nutritional requirements are not well defined. Since both lean body mass and basal metabolic rate decline with age, an older person’s energy requirement per kilogram of body weight is also reduced. The process of ageing also affects other nutrient needs. For example, while requirements for some nutrients may be reduced, some data suggest that requirements for other essential nutrients may in fact rise in later life. There is thus an urgent need to review current recommended daily nutrient allowances for this group. There is also an increasing demand worldwide for WHO (World Health Organization) guidelines which competent national authorities can use to address the nutritional needs of their growing elderly populations.

Malnutrition and Older Persons

Many of the diseases suffered by older persons are the result of dietary factors, some of which have been operating since infancy. These factors

are then compounded by changes that naturally occur with the ageing process. Dietary fat seems to be associated with cancer of the colon, pancreas and prostate. Atherogenic risk factors such as increased blood pressure, blood lipids and glucose intolerance, all of which are significantly affected by dietary factors, play a significant role in the development of coronary heart disease. Eating well is important at any age. But health issues and physical limitations sometimes make it difficult for seniors, the fastest-growing segment of the U.S. population, to get the nutrients they need for a balanced diet. Poor nutrition and malnutrition occur in 15 to 50 percent of the elderly population. But the symptoms of malnutrition (weight loss, disorientation, lightheadedness, lethargy and loss of appetite) can easily be mistaken for illness or disease. If you are a full- or part-time caretaker for an elderly parent or grandparent, there are plenty of steps you can take to help your loved ones maintain good nutrition as they age. The best ways to find out why your loved one isn't eating well are

“WHAT WE DO BEST IS CARE!” www.kchealthandwellness.com

to pay attention, look for clues and ask questions. Encourage him to talk openly and honestly, and reassure him that he is not a burden to you or anyone else.

Possible Causes of Poor Nutrition

Whether it’s because of physical limitations or financial hardship, many seniors don’t eat as well as they should. Arthritis can make cooking difficult, while certain medications can reduce appetite, making meals unappealing. A 1990 survey by Ross Laboratories found that 30 percent of seniors skip at least one meal a day, while another study found that 16 percent of seniors consume fewer than 1,000 calories a day, which is insufficient to maintain adequate nutrition. There are many reasons why a senior may skip a meal, from forgetfulness to financial burden, depression to dental problems, and loneliness to frailty.

Beyond the Flip-Flop Stylish Sandals That Are Actually Good For Your Feet By Lucas Wetzel, on behalf of Comfort Plus Shoes & Footcare

THE ExERCISE SANDAL The staff at Comfort Plus believes in the science behind the “exercise sandal.” Exercise sandals usually contain a raised toe crest and contoured sole to promote healthy feet. Sandal styles worn without a back-strap encourage the wearer to use their toes to naturally grip and flex while walking. Toe-gripping facilitates muscle growth and can be especially beneficial to anyone who might have been wearing tight, constrictive shoes during the workday or through the colder months. FREE PEDIGRAPH READING AND FooTBED CuSTomIzATIoN oFFER RIGHT FIT Many of these sandals feature a cork footbed, allowing the shoe to naturally take the shape of the wearer’s feet over time. The shoe professionals at Comfort Plus will use their inhouse technology as well, including a pedigraph reading and footbed customization, to make sure you get the right fit every time.

E

very year around spring break, I pick up a new pair of cheap flip-flops in anticipation of warmer temperatures. By the time the pool opens or the trip to the beach arrives, however, one of the straps has already broken or my plantar fasciitis has flared up. If the sandals do survive that long, most of the cushioning is worn out and I might as well be walking on concrete. Fortunately, when it comes to sandals, there is a better way. After visiting with the staff at Comfort Plus Shoes & Footcare in Leawood, I learned about half a dozen different companies that make high-quality sandals designed to provide tons of comfort and ample support, as well as style. These sandals give me the barefoot freedom I crave while also providing proper arch support.

www.kchealthandwellness.com

One popular brand is the Finnamic series by Finn Comfort, which are made in Germany. Finnamic sandals have a cork sole and often feature a backstrap, which can be better for tourists or anyone planning to do an extended amount of walking. The women’s Finnamic Sausalito, for example, features a soft cork footbed that promotes good posture and proper foot function while cradling and guiding the feet from heel to toe.

mEPHISTo

BIRkENSToCk

Mephisto, a brand of shoes made in France, also have a cork footbed that is heatmoldable to allow for greater adaptation to the wearer’s feet. Mephisto sandals feature a soft-air midsole that minimizes shock to the feet, creating greater protection, relief for joints and promoting better back health. These sandals come in many different designs and also include a midsole designed to enhance air circulation in the feet.

WoLkY

Wolky is a popular brand of sporty, funky shoes made in Holland. Wolky sandals come in a range of styles, from fashionable sandals with an elevated heel to those designed for walking. Most of them feature a back-strap and robust, contoured soles built for an active lifestyle.

17

NAoT

One of the most iconic sandal styles is the Arizona from Birkenstock. The Arizona features a classic two-strap look with no back-strap. The natural cork, heel cupping, and arch support make them a favorite for many. Birkenstocks are unique in that they have a flatter footbed and zero heel elevation, giving them a different feel than typical exercise sandals.

FITFLoP

FitFlop sandals combine the best of both worlds — the streamlined style, openness and lightness of a flip-flop with all the support, comfort and shape of fitness-promoting footwear. FitFlops have a wedge shape and feature a comfortable heel and an enhanced arch that reduces overpronation. FitFlop has made use of the latest technology to create pressure-reducing soles that have three times the density of other sandals, resulting in increased foot-to-midsole contact and a cushioned heel strike.

Last but not least, Naot sandals are a stylish, comfortable sandal made with natural cork and latex footbeds covered with suede, and Italian leather for the top sections. Naot has an interesting history, too, originating as a one-room workshop in 1942 and growing to become Israel’s top shoe manufacturer.

WALk A mILE IN A PAIR oF THESE! So when it comes to shopping for sandals this year, I recommend forgoing the cheap flip-flops and checking out one of these brands instead. Chances are, once you walk a mile in a pair of these sandals – whether it's along the beach on spring break or just through your living room – you’ll never look back. If any of these products sound like they might be beneficial to you, stop by Comfort Plus Shoes & Footcare to speak with one of their licensed foot professionals or call us at (913) 451-4494 to learn more. First things first – what is diabetes?

DIABETES is a condition that prevents the body from properly using energy from food. This condition occurs when the body can’t use glucose (a type of sugar) normally. Glucose is the main source of energy for the body’s cells. The levels of glucose in the blood are controlled by a hormone called insulin, which is made by the pancreas. Insulin helps glucose enter the cells. In diabetes, the pancreas does not make enough insulin (type 1 diabetes) or the body can’t respond normally to the insulin that is made (type 2 diabetes). This causes glucose levels in the blood to rise, leading to symptoms such as increased urination, extreme thirst, and unexplained weight loss.

What are risk factors for diabetes? • A family history of diabetes • High waist measurement • Man: waist >35.5 inches • Woman: waist >31.5 inches • Body mass index higher than 25 • Age > 45 years • High blood pressure

Simple tips make life easier

Life with diabetes is never easy, so we’re sharing five simple tips to make life with this perplexing illness easier or better in some practical way. These are non-medical tips; please consult your physician before changing your routine. 1. Starburst Instead of Glucose Tabs. As fast as glucose tablets, but more palatable, four Starburst candies contain 16 grams of carbohydrates – just like glucose. Make sure to take in a good source of protein such as peanut butter crackers to help stabilize blood sugar afterwards and keep it from becoming too low again. 2. my Fitness Pal. This free app is amazing for counting carbs. Their bar code scanning feature not only accurately tracks what you consume, but can provide precise nutrition information, thanks to the world’s largest nutrition and calorie database – over 5 million foods!

3. Soft Drink Cheat. If you’re not sure if a soft drink is regular or diet, test your blood sugar, then dip your finger in the drink and test again. It’s an easy way to catch a sugary soda before drinking it. 4. measure the Glasses in Your Cupboard. Learn which glasses or cups hold 3 or 4 oz only, so when your blood sugar is low, you can grab one and fill it with juice without worrying that you’re over-treating. 5. Relax. If you’ve recently been diagnosed with diabetes, seeing a high number can be alarming. Do not panic. Stressful situations can release hormones that will cause your blood sugar to go even higher. Simple relaxation exercises and other stress management techniques can help you gain more control over blood sugar levels, according to a study conducted at Duke University. Once you’ve pinpointed your stressors and notice which ones affect your blood sugar levels, you can find a way to relax.

Having a daily snapshot allows us to identify potentially dangerous trends early, and helps you manage your diabetes at home. To learn more about the compassionate care provided by village Home Health, please call their Kansas Office at 913-403-8343 or their Missouri Office at 816-524-1133.

Create a bedtime routine that prepares your “mind” for what is coming. Limit stimulates like computer and cell phone use the last 2 hours before bedtime. Leave those devices out of the bedroom after retiring. Unless you’re a physician on call 24 hours, it can wait until morning. Take a hot bath or shower, do some minor leg lifts after you’re in bed and clear your mind of worrisome issues. Think about things that happened awhile back that are pleasant like recalling a vacation or family event instead of thinking about what you need to do tomorrow or the next day. Maintain a tidy bedroom. Believe it or not, the National Sleep Foundation suggests that a clean bedroom environment can help encourage sounder sleep. Spritz the room or pillows with a lavender fragrance because that scent has been shown to help people fall asleep faster and wake up less frequently during the night. Look Into SnorIng ISSueS If you or your spouse complains about snoring issues, get it looked into. The Epworth Sleepiness Test is one method you can use to confirm your lack of restful sleep. Use the following scale to choose the most likely number in each situation.

EPWoRTH SLEEPINESS TEST • 0 – Would never doze off or fall asleep in this situation • 1 – Slight chance of dozing in this situation • 2 – Moderate chance of dozing in this situation • 3 – High chance of dozing or falling asleep in this situation Score your answers to these questions using the scale above. 1. Sitting and reading _______ 2. Watching television_______ 3. Sitting, inactive in a public place (theater, meeting, etc.)______ 4. As a passenger in a car for an hour without a break_______ 5. If your total score is 1-6, chances of Obstructive Sleep Apnea is less likely. If your total score is 7-8, you are average. If your score is 9 or higher, Obstructive Sleep Apnea is more likely. Make an appointment with your primary care doctor who may want to schedule a sleep study. Sleep apnea is a medical condition that needs treatment. Sleep apnea can cause a host of other conditions like high blood pressure and heart problems. Not all snoring is sleep apnea and not all persons with sleep apnea snore. oraL appLIance can Manage SLeep apnea and SnorIng dr. pribyl has a special interest in patients who wish to manage their sleep apnea and snoring with an oral appliance. This is a device made from impressions of your teeth that works by advancing your lower jaw slightly forward bringing the tongue with it. The tongue is what falls back and blocks your airway during sleep apnea. Seventyfive percent of patients who choose this method can see improvement the first night. Latest advances in Minimally Invasive gallbladder Surgery Just got Smaller!

23

r. Christopher daggett with advanced Surgical associates is among the first physicians in the Kansas City area to provide micro-laparoscopic gallbladder removal surgery (cholecystectomy).

recent advances in minimally invasive surgery aimed at diminishing incision size have led to the development of micro-laparoscopic surgical instruments. The increased level of complexity using such instruments requires a higher level of surgical skill compared to traditional laparoscopy.

having performed general Surgery in Eastern Jackson County and the kansas City Metro for more than 30 years, dr. daggett describes this procedure as a significant improvement for laparoscopic surgery. A new smaller micro-laparoscopic instrument is benefiting cholecystectomy patients and demonstrating great success. It requires a 50% smaller incision at the surgical opening versus traditional laparoscopic cholecystectomy or robotic surgeries. CareFusion, a leading global medical technology company, recently launched its new micro-laparoscopic line, with surgical instruments as small as 2.7 mm in diameter, half that of the standard 5 mm or 10 mm. hand instrumentation is made of a proprietary ceramic titanium alloy material to provide strength comparable to standard 5 mm laparoscopic instruments.

about the gallbladder

The gallbladder is a pear-shaped organ beneath the liver. It collects and concentrates a digestive liquid called bile that is produced by the liver. Bile is released from the gallbladder into the small intestine after eating, aiding in digestion. gallbladder problems are typically caused by the formation of gallstones, which are small hard masses made up of cholesterol and bile salts that form in the gallbladder or in the bile duct. These may show up in X-rays and other imaging tests. It's uncertain why some people form gallstones and there is no known means of prevention. These stones tend to block the flow of bile out of the gallbladder causing swelling, sharp abdominal pain, vomiting, indigestion and occasionally, fever. In the United States, there are 20-25 million newly diagnosed gallstone patients each year. however, most people with gallstones don't know they have them. It is quite common to have stones in the gallbladder that cause no symptoms. When they do affect your health, surgery to remove the gallbladder is usually recommended.

Less Scarring and discomfort

dr. daggett is trained in the newest techniques to treat gallbladder issues and relieve your pain. If you are a candidate for micro-laparoscopic surgery, smaller incisions provide the benefit of less patient discomfort and less scarring as they eliminate the need to close the incision site with sutures in some procedures. Better cosmetic results are achieved through incisions that do not require stitches, as scars are virtually invisible. In addition, candidates appropriate for this approach tend to have shorter hospital stays with less postoperative discomfort.

Micro-laparoscopy may not be appropriate for some patients who have had previous upper abdominal surgery causing dense scar tissue, or who have some pre-existing medical conditions such as obesity. a thorough medical evaluation by your personal physician, in consultation with a surgeon trained in micro-laparoscopy, can determine if this method of gallbladder removal is an appropriate procedure for you.

up-to-date technology

Using the most up-to-date technology, your treatment will be safe, convenient and tailored to your specific needs. Rest assured, Dr. Daggett will have you back to your normal activities as quickly as possible. To learn more or to schedule a consultation, please call advanced Surgical associates at 816-246-0800 or visit online at www.asakc.com. Christopher Daggett, D.O.

Dr. Daggett is a board certified general surgeon who was born and raised in Kansas City. He completed his residency at Lakeside Hospital in 1983 and immediately went into a solo general surgery practice in the Kansas City area. Dr. Daggett is Program Director of the General Surgery Residency program at Kansas City University of Medicine and Biosciences (KCUMB). Advanced Surgical Associates (ASA) is a leader in providing minimally invasive general surgery for residents in Eastern Jackson County and the Kansas City Metro. The laparoscopic techniques of Christopher Daggett, D.O., Adam Kramer, D.O. and Christie M. Brock, D.O. benefit patients by allowing for shorter recovery times and minimal scarring.

Traction Alopecia: How to Fix Those Edges! By Meena Singh, MD

A

s a dermatologist with a special interest in hair conditions, it consumes me how many women I see with permanent hair loss from our own styling practices!! One common form of hair loss is called traction alopecia.

Traction alopecia can be a permanent, sometimes scarring form of hair loss that typically occurs around the hairline and crown. You do not need to be a dermatologist to diagnose this. We have all seen it on girlfriends, family members, and, unfortunately, many prominent celebrities.

CAUSE

It is thought to be caused by hairstyling practices that pull hair tightly away from the scalp, such as braids, weaves, and locks. Similar to trauma that happens elsewhere on the skin, the body senses the insult, sends inflammatory cells to the site of trauma, and tries to heal it with a scarring process. This is exactly what occurs with traction alopecia. The tight pulling leads to inflammation around the hair follicle and if the inflammation remains untreated, fibrosis or scar tissue forms and the hair is permanently lost. This condition is most common in females of African descent, but can be seen in anyone with traumatizing hairstyling practices. For example, in India, many Sikh males that wear their hair in tight buns under their turbans and can also suffer from this problem. Ballerinas that pull their hair back tightly can experience this, as well.

PREVENTION AND TREATMENT

Luckily, there are ways to prevent traction alopecia and also ways to treat it. Most importantly, do not let allow the hair to be pulled too tight!!!! yes, we all love to see our friends flaunt a great weave down their backs, so I will never be one to restrict wearing these types of hairstyles. HOWEvER, I cannot stress the importance of being able to recognize signs of when the hair is being pulled too tight. This includes any pain, tenderness, burning or itching. These are signs of inflammation!!!

Secondly, see the previous paragraph‌ It’s that important. Many of my patients will tell me that they did not notice the hair loss until months to years after wearing braids or a weave. So it is very important to pick up on signs of inflammation as soon as possible, so that you have a possibility of being treated and holding onto the hair that you have. As far as treatment options are concerned, the most important first step is seeing a dermatologist as soon as you notice any problem. The longer you wait for an evaluation, the more time for scar to form and for the hair loss to become permanent. When you are seen, likely a biopsy will be performed to determine if there is any inflammation left to treat or if scar is already present. If there is inflammation present, treatment may include steroid injections to the scalp, topical steroids, or oral antibiotics. Even if there is scar present and the hair loss is felt to be permanent, hair transplantation is a fantastic option to help restore the hairline.

In summary, women love to experiment with a myriad of fabulous hair styles. Unfortunately, many of these styling practices can lead to permanent hair loss and deflated senses of self. Therefore, it is of the utmost importance to recognize when the hair is being traumatized to prevent traction alopecia from occurring. Changing the “SHAME AND BLAME” Game

By Rachel Lewis

S

hame and blame constantly surround us, but especially surround teenagers. Throughout history and literature, shame has been used as a tool to keep kids and society in line. Whether it’s Hester Prynne being made to wear a scarlet “A” in The Scarlet Letter, or the way that people attack one another online and through Twitter, shame has become something that is always present.

SHAmE vS. GuILT

We tend to think of shame as being very similar to guilt, but the subtle differences between the two are astounding. In their book, Facing Shame, Mason and Possum say that, “While guilt is a painful feeling of regret and responsibility for one’s actions, shame is a painful feeling about oneself as a person.” Brene Brown, PHD, LMSW, who gave a fantastic TED Talk on shame and is a shame and vulnerability researcher says that, “Shame is the intense painful feeling that we are unworthy of love and belonging.” The idea that shame has become such a naturalized part of society is disturbing to how we perceive ourselves but most especially to how we teach teenagers. Teens are still incredibly impressionable and are looking to find who they are and how they fit in. Proponents of the “shame and blame” method believe that shame works because of past precedent. Shame has helped change behaviors in the past so therefore we can utilize it as a tool now. Other proponents believe that since teens are still impressionable that verbal criticism and shame can curtail bad behaviors more effectively. Unfortunately, those shaming behaviors and verbal smack downs can have an incredibly negative effect on self-esteem and self-worth.

As stated above, shame is a negative emotion about oneself. Once those negative emotions are there, all shame will do is prove that those feelings are right. Shame increases the chances of distress and alienation which continues that same shame cycle. Shame, unlike guilt, takes those negative emotions about one’s actions and turns them inward.

Rachel Lewis

Rachel Lewis graduated with honors from the University of Kansas in just three and a half years in December, 2010, and has already written a book and started her second business. She has been interviewed by the New york Times, USA Today, Fox Business and has been cited in an article in Forbes on successful businesses. She has worked with students from middle school through college helping with goal setting, confidence building, study skills, and getting ready for "the real world." She launched Success Skills Weekly with her mother and brother to assist students with critical skills that are needed for success, but not being taught anywhere. Rachel is a member of the Junior League of Kansas City, and was selected as a Belle of the American Royal in 2011 and has been an active volunteer for the BOTAR and the American Royal organizations. She is also the Head Cheer Coach at her high school alma mater.

CHANGE THE GAmE

So how do we change the “shame and blame” game? By trying to be a more wholehearted and vulnerable person. We need to be the model for the behaviors that we want teens to emulate. When parents preach about how something is wrong but then turn around and praise stories about their high school years, teens see the hypocrisy. The things that we say and the way that we act matters. If we are shaming people through social media, then teens will learn that behavior is okay. We are the models they look to more so than anyone else.

As Gandhi says, “You must be

the change you want to see in the world,” and that applies double when we should be the change we want to see in our teens. www.kchealthandwellness.com
