Direct care

New model focuses on old-fashioned doctor - patient relationship

Two years ago Dr. Rob Rosborough was fed up with his career and looking for a change.

“It was either do something different altogether in medicine or open a taco truck,” he laughed.

That’s when Rosborough stumbled upon a radically different practice design which eliminates the need for insurance and Medicare billing while also giving doctors more quality time with patients. Known as Direct Primary Care (DPC), it allows patients to pay the bulk of their medical costs up front, cutting down on administrative costs and allowing practitioners to see fewer patients.

“It was becoming such a grind and I didn’t feel very satisfied,” he said. “I was getting incredibly frustrated with the insurance model of medicine. That relationship was being challenged; too many clicky boxes, too many computers.”

No insurance is billed and care does not count toward an insurance deductible. However it is recommended that patients have a catastrophic care insurance plan as backup.

Rosborough explained. “It’s $60 or $120 for families.

Kids are no charge. Everything’s included. The only thing we charge extra for are medications and we pay pennies on the dollar.”

DPC also creates a closer relationship between doctor and patient because the patient has one primary care physician whom they have unlimited access to.

“Everybody gets a half-hour appointment. New patients get an hour,” Rosborough said. “They have my personal number.”

Although Rosborough’s new clinic, Township Health DPC, has only been open a few weeks, he has already

noticed a change in the relationship he has with the patients who followed him.

“The cool thing about this model is I’ve had time to talk to someone I’ve known over a decade and found out something new,” he said.

Township Health DPC is in a small space in downtown Silverton with a staff consisting of office manager Janet Daffron and medical assistant Sara Quijano, both of whom have worked with Rosborough for 20 years.

“I thought it was a good idea, a good concept,” Quijano said. “The hours are better for my life. I had been working really long hours.”

The hours, as listed on the front door, are “catered to the patient needs.” Rosborough explained that he is available to patients at any time and even makes house calls on occasion for those who cannot make it into the office.

“I think the biggest misconception is when you open up your access people will bug you all the time,” he said, adding he rarely gets calls on weekends and almost never at night, He feels that open door policy is important.

“It’s how do you take care of people and how do they have access to you?” he said.

That openness is a theme across his entire practice including the in-house pharmacy, laboratory tests and x-rays.

“It’s completely transparent pricing. I have a book that has all the costs,” he said. “There are no prior authorizations. You don’t have to get approved through the insurance. It’s completely outside the insurance base.”

Because Rosborough pays cash for outside testing upfront, he is often able to save his patients money and by working with a wholesaler he stocks his pharmacy with prescription and over the counter drugs as well as vitamins and supplements for much cheaper than usually priced elsewhere.

For many of Rosborough’s patients who fill multiple

prescriptions every month this has been a huge savings.

“I have a patient who came in and filled seven medications for $36,” he said. “He’s seeing me for free because of the (reduced cost of the) medicine and he’s getting more personal care and time with me.”

Typical of most DPC clinics, Rosborough said about 10 percent of his previous clientele, around 200 patients, followed him to the new office. That number has been increasing, on average, by about 10 per week.

“A lot of people think, ‘OK, this is some kind of snake oil medicine,’” he laughed. “We’re so ingrained that you have to use insurance. You absolutely don’t.”

He does caution, however, that although Township Health DPC offers many services including setting of simple fractures, laceration care, vasectomies, circumcisions and wart removal, it is a good idea to have insurance to cover emergencies and catastrophic health issues.

Overall Rosborough said he is very pleased with his new practice and is enjoying getting to know his patients on a deeper level.

Scotts Mills may have a small population of only 355, but it’s home to Rollin Sandwiches, a family-run eatery with big flavor.

Owner D.Anne Evans, along with her husband, Jim, have been serving up homemade hot and cold sandwiches and other treats since July 2015. Operating from inside of their 1969 seven-foot-wide, 18-foot long Prowler trailer, the Evans love serving their community. Since May 5, a new adjoining indoor seating area opened up – thanks to repurposing the former Scotts Mills City Hall building, which was being used for storage. D.Anne quickly sprang into action and approached the city council in January and got the ball rolling. The close-knit community banded together and the necessary building repairs commenced. In all, the project “took four and a half weeks. They just slammed it out. Everybody was so gracious,” she said.

Married for 28 years, D.Anne, 47, and Jim, 58, have two sons, Jubal, 27, and Ethan, 18. Jubal is married and has a three-year-old son. Ethan graduated from Silverton High School last month.

D.Anne’s given first and middle name is Dorothy Antonell, but she shortened it and uses a period instead of an accent. She and her family also chose a creative, and symbolic, name for their business.

“We were kicking around names and Ethan said ‘Well, we’re on wheels so we should just call it Rollin Sandwiches without the g,’” D.Anne explained. She went on to say that after they agreed upon the name and had it officially registered, “it was funny because my husband and his brothers owned a painting company for

503-400-4827 years and they were always ‘rollin’ walls’ as they would always say. It just fit so perfectly.”

Taught to cook by her mother starting at the tender age of four, D.Anne has honed her culinary skills well. She and Scotts Mills Mayor Paul Brakeman worked together for a while at the Glockenspiel Restaurant in Mount Angel. He trained her as his sous chef for 2 1/2 years. It is no surprise that Hell’s Kitchen is one of her favorite TV shows.

When the couple have time off together, they enjoy training and riding their horses, gardening, hiking and reading. Because owning their own business takes up so much energy, even on days off D.Anne admits it’s a challenge to totally relax. They are always thinking ahead.

Future plans include seating on the back deck area, growing their own fruit and vegetable garden onsite and updating their menu to welcome the new seasons. Savory soups will be added to their Fall offerings.

D.Anne hopes to eventually provide allinclusive weekend dinner specials including an entrée, soup, salad and a beverage. She’s thinking about making “comfort foods” like macaroni and cheese, and meatloaf with mashed potatoes and gravy. She’s envisioning “made from scratch desserts” too.

The latest in the series of the historic podium plaques is currently being installed in Old Mill Park (behind the Visitors Center). Architect and project guru Victor Madge says the podium will be a great place to sit on the bench and reminisce on the quieter times when the Fischer Flour Mill sat prominently along Silver Creek.

At long last the logs from the Abbey’s own tree farm have been cut for the construction of the Benedictine Brewery and tasting room. Watch for an old-fashioned Mount Angel community “barn raising” sometime early this fall.

Beware of aimlessly wandering zombies...

The Man was recently dragged to the big city, kicking and screaming, to a place I’d never been before, an odd, foreign land called I K E A.... which as best as I could tell, is a place that serves Swedish meatballs with a side of home furnishings. Now, if you know De Männen, you’ll know I prefer to be out in the woods or on a mountain somewhere, I don’t do crowds. Sooo, the first indication I may have slipped into the Third Dimension of Hell was the shuttle bus from the “off site parking” and the fact that the striped shirted minions hand you a map in a kind of a grim faced “take this, you’re going to need it” kind of

way.... never mind that the map did not resemble the store layout.... at all.

If you’ve spent any time in a Las Vegas casino this will start to look familiar, the business model of “if we keep the customers lost, confused, and unable to find a way out, and in this case with merchandise inexplicably labeled Skorva, Axag and Nattjasmin, they will spend more money out of sheer desperation. At one point, I asked one of the minions about the location of a particular item and he, sensing my hopelessness, leaned over, whispered “follow me” and took us through a maze of shortcuts and swinging doors, past hoards of mumbling, aimlessly wandering zombies who likely had been lost for days if not weeks. Then, just when you’re hoping the pings from your cell phone will lead search and rescue teams to your location, you come upon a full on restaurant, where a faint “Bork, bork, bork” could be heard coming from the kitchen... and windows so you can see outside.....

You can’t get there of course, but it’s apparently a tease to give you hope of returning to your life on “the outside.”

The sustenance does, however, prove to be critical as you realize that you are, in fact, only half way through the maze of Vitsippa, Hampdan and Mossflox which leads you at last to the warehouse which in size and scope, resembles the final scene in Raiders of the Lost Ark where the ark is wheeled into an impossibly large cavern of antiquities. It is here that you stand in yet another line to load up your huge cardboard box that doesn’t look at all like what you thought you purchased..... but at this point, you just want to go home and figure it’s best to avoid eye contact lest a problem be discovered and you’ll have to start the whole process over..... Nope, I don’t do crowds, I like Our Towns much better.... bork, bork, bork...

Terri Rae Goodwin, 57, passed away in her home in Mount Angel June 23.

She was born to Roger and Shirley Goodwin in Corvallis; the eldest of three children. Terri graduated from Cascade High School, class of 1978. She enjoyed watching The Young and the Restless, Judge Judy and Ellen. She loved her family, friends and pets. Trevor, her 9-year-old chocolate lab, was her pride and joy as was her black cat, Snoopy.

Terri, a kind and loving person, always willing to help her neighbors, was preceded in death by her mother, Shirley.

She is survived by her dad, Roger, and step-mom, Karen, of Dallas; her life partner of 24 years, Gary Rizzotto, of Mount Angel; her son, Shane Goodwin and his wife Jessica of Aumsville and their children Zach and Zoey; her sisters, Laurie of Grants Pass and Renee of Salem; her step-brothers, Matt, Mitch and Mike Ficek of Salem/Albany; her nieces Cassy of Grants Pass and Sydney of Salem; and her close friends, Angie and Scott. A private Celebration of Life will take place at a later date. Assisting the family with services is Unger Funeral Chapel of Mount Angel.

McKenzie advises having friends and family come early for the weekend and stay after Monday to avoid some of the crowds at airports, train stations and on the road.

“We look forward welcoming visitors and helping them create lifetime memories of the natural world – terrestrial and celestial,” McMahon said. “We anticipate a great number of visitors, and there is a good possibility for congested roads and already full campsites.”

McMahon asks campers to stay on trails and not create new campsites or fire rings.

“August is a time of high fire danger, so be prepared to camp using gas stoves as fires may likely be prohibited at this time of year,” he said. “Bring a small shovel to bury your human waste and toilet paper.

If you have gloves and an extra trash bag, please consider taking out something another visitor may have left behind.

“Give yourselves plenty of time to experience and savor this special weekend with your neighbors, friends and family,” McKenzie advised.

The key, the folks in the know said, is to plan ahead.

“Don’t wait till the last minute for groceries or fuel,” Fossholm said.

“Be patient and calm. If you don’t have to travel stay home and off the roads so you don’t add to the problems or traffic congestion.”

Businesses are gearing up to handle the crowds of people that will be coming from all around the world, McKenzie said.

“We’re routinely hearing about groups coming from England, Germany and Japan to spend time with us that weekend,” she said. “Think about how you want to welcome visitors. Eclipse guides can help people know what’s going on and can help visitors find fun things to do in the woods, on the water, and in our small towns. A bottle of water, a nice piece of fruit, or a homemade cookie go a long way toward ensuring happiness and good street credibility for our communities.”

“Plus, no matter how busy you are with visiting friends and family or your business, take time to view the eclipse on Monday morning,” McKenzie said.

“Make sure that if anyone watches the solar eclipse (they) wear the approved safety glasses as we don’t want anyone permanently damaging their eyes,” Fossholm said.

“This is a special event for Oregon, and we are smack dab on the path of totality.

Those who chase eclipses around the world speak with awe about what the experience is like.,” McKenzie said. “The world won’t end if you pause for a few minutes to enjoy yourself!”

New deal City eyes Eugene Field site What do you think of a new Silverton Police Station and potentially a city hall on the Eugene Field School site on North Water Street?

The Silverton City Council is seeking public input and the question is urgent ­– there’s a July 17 vote on the line.

On July 7 the council found its bid for the site had moved from second position to first after a developer withdrew his offer to the district. That made the issue pressing.

“The offer that the council has made is representative of the uniqueness of this property, it’s importance to our community, and much-stated citizen opinion that a civic center stay in the downtown if possible.

“Many citizens have expressed concern with abatement of known issues related to the existing structure and those potential issues have been considered... We would still have a due diligence period during which we would complete a number of assessments on the property as part of the offer prior to the sale becoming final.”

“The terms of our offer require us to take action at our first scheduled council meeting,” Silverton Mayor Kyle Palmer explained in his call for immediate public input.

“We are adding an item to the July 17 meeting agenda that relates to approval of a purchase and sale agreement between the city and the Silver Falls School District.

“A few weeks after the decision was made to not pursue our offer on the former Square Deal Lumber property, the council began looking at the Eugene Field site and directed the city manager to begin negotiating the details of an offer on the property,” Palmer continued.

Real property negotiations are exempt from Oregon Public Meeting Law and discussed only in closed executive session

until the negotiation reaches the approval stage, he said, adding all votes must be taken in open session.

Opener set

New Foxes football coach faces Sandy in Sept. 1 debut

New Silverton High football coach Josh Craig and the Foxes will open the 2017 season at home at McGinnis Field on Friday, Sept. 1 at 7 p.m. against visiting Sandy.

Craig replaced John Mannion, who led Silverton to a 57-20 record in his seven seasons at the helm. Mannion is starting a new program at Mountainside High in Beaverton.

The Foxes visit Redmond on Sept., 8. Silverton also faced Sandy and Redmond to prep for last year’s league season, downing the Pioneers 20-13 and outlasting the Panthers 41-35. Redmond, however, took the rematch 35-27 in the OSAA Class 5A playoffs.

Silverton opens Mid-Willamette Conference play Sept. 15 at home vs.

Bright future Silverton was established in 1885. The University of Cambridge is just slightly older, by almost seven centuries. The former is a charming little Oregon town with 9,500 people and 163-year history.

The other is one of the world’s top four universities, situated in Cambridge, England with more than 20,000 undergraduates and graduate students.

One has a veritable “Who’s Who” on their alumni list, including Charles Darwin, Jane Goodall, Sir Isaac Newton, John Harvard (yes the Cambridge graduate who later founded another pretty wellknown school that bears his name), Dian Fossey, and Stephen Hawking. The other has a graduate named Alex Kraemer, a Silverton High alum (Class of 2011) who may be destined to join the list of famous Cambridge grads.

Kraemer, 24, graduated from Lewis and Clark College in 2015 and the University of Cambridge in 2017 on a fully-funded Davies-Jackson Scholarship. He holds an honors degree in history and a minor in English from the Portland college, and a second Bachelor of Arts from the prestigious English university. It’s a threeyear program Kraemer finished in two.

For graduate school he is contemplating either public policy or international affairs.

Career-wise, Kraemer says, “I’m thinking government, politics, or journalism, with a smattering of additional writing in each case. My overall hope would be to contribute positively to the world, as saccharine as that might sound; I

feel it would be an abdication of moral responsibility on my part if I didn’t try to do something to help however I could.”

His parents, Mary and Dan Kraemer, are quite happy. Mary said of her oldest son, “I think it was a great honor for Alex to go to Cambridge. Not many people get the opportunity, and he should be pleased that he got to go. I didn’t expect Alex would end up doing something like that, but I always knew he would do some incredible things. We are very proud.”

About his experiences in England, Kraemer holds many dear.

“I have a special place in my heart for going to this bar, the Pickerel, with my friends a few times a week, drinking a few pints and chain-smoking. Perhaps a simple ritual, but a cherished one nonetheless.”

One can only imagine the banter among international students as they regale each other with stories about their countries, the British Monarchy, the U.S. election, and the future of the world.
