UNIVERSAL FLU VACCINE MOVES CLOSER Scientists have found a flu “super antibody” called FI6 that can fight all types of influenza A viruses that cause disease in humans and animals and say their discovery may be a turning point in the development of new flu treatments. Researchers from Britain and Switzerland used a new method aimed at beating “needle-in-a-haystack-type-odds” and managed to identify an antibody from a human patient which neutralizes both main groups of influenza A viruses. Although it is an early step, they said, it is an important one and in time may pave the way for the development of a universal flu vaccine. Vaccine makers currently have to change the formulations of their flu shots every year to make sure they protect against the strains of the virus circulating. This is a cumbersome process which takes time and money, so the goal is come up with a universal flu vaccine that could protect people from all flu strains for decades, or even for life. “As we saw with the 2009 pandemic, a comparatively mild strain of influenza can place a significant burden on emergency services. Having a universal treatment which can be given in emergency circumstances would be an invaluable asset,” said John Skehel of Britain’s National Institute for Medical Research, who worked on the study with colleagues from the privately-owned Swiss firm Humabs. Antonio Lanzavecchia, Humabs’ chief scientific officer and director of the Swiss Institute for Research in Biomedicine, said high rates of seasonal flu and the unpredictability of possible future pandemics underlined the need for better treatments that target all flu viruses. When someone is infected with the flu virus, their antibodies target the virus’ haemagglutinin protein, the researchers explained in their study, which was published last month in the journal Science. Because this protein evolves so rapidly, there are currently 16 different subtypes of influenza A, which form two main groups. Humans usually produce antibodies to a specific subtype, and new vaccines are made each year to match these strains. To make progress toward a universal shot that could be used every year, scientists need to identify the molecular signatures that prompt the development of broadly neutralizing antibodies. Previous research work has found antibodies that work in Group 1 influenza A viruses or against most Group 2 viruses, but not against both. This team developed a method using X-ray crystallography to test very large numbers of human plasma cells, to increase their odds of finding an antibody even if it was extremely rare. When they identified F16, they injected it into mice and ferrets and found that it protected the animals against infection by either a Group 1 or Group 2 influenza A virus. “As the first and only antibody which targets all known subtypes of the influenza A virus, FI6 represents an important new treatment option,” Lanzavecchia said in a statement. DIET REPORT HIGH SALT + LOW POTASSIUM = EARLY DEATH Put down the salt shakers. Eating too much salt and too little potassium can increase the risk of death, researchers said last month. The findings from a team at the Centers for Disease Control and Prevention are a counterpoint to a fiercely-debated study released earlier last month that found no evidence that making small cuts in salt intake lowers the risk of heart disease and premature death. “Salt is still bad for you,” said Dr. Thomas Farley, Health Commissioner for New York City, which is leading a campaign to reduce salt in restaurant and packaged foods by 25 percent over five years. Most health experts agree with Farley that consuming too much salt is not good for you and that cutting salt intake can reduce high blood pressure, which raises the risk of heart attack and stroke. Salt intake has been rising since the 1970s, with Americans consuming about twice the recommended daily limit.

The CDC study, published in the Archives of Internal Medicine, specifically focused on growing research that shows a diet high in salt and low in potassium is especially risky. For the study, researchers looked at the long-term effects of sodium and potassium intake as part of a 15-year study of more than 12,000 people.

The good news: potassium often counteracts the effects of salt By the end of the study period, 2,270 of the study participants had died; 825 of these deaths were from heart disease and 433 were from blood clots and strokes. They found that people who had a high salt intake and a low potassium intake were most at risk. Researchers said potassium often counteracts the effects of salt in the diet. “If sodium increases your high blood pressure, potassium decreases it. If sodium retains water, potassium helps you get rid of it,” they said. You can increase the levels of potassium in your diet by adding more servings of fresh fruits and vegetables, such as spinach, grapes, carrots, sweet potatoes, and low fat milk and yogurt.

CELL PHONE CANCER UPDATE MOBILE PHONE USE DOESN’T ALTER KIDS’ CANCER RISK, ACCORDING TO NEW STUDY BY SINEAD CAREW

C

hildren and adolescents who use mobile phones are at no bigger risk of developing brain cancer than nonusers, according to a study of patients aged 7 to 19.

The research, published in the Journal of the National Cancer Institute last month, addresses concerns that children may be more vulnerable to health risks from electromagnetic radiation of mobile phones. Children’s nervous systems are still developing, and there are fears that their smaller head circumferences could allow radiation to penetrate deeper into the brain. But the study -- the first to look specifically at children and the risk of cancer from cellphones -- found that brain tumor patients were no more likely to be regular phone users than control subjects who did not have cancer. “If mobile phone use would be a risk factor, you’d expect cancer patients to have a higher amount of usage,” said Professor Martin Roosli who conducted the study at the Swiss Tropical and Public Health Institute in Basel, Switzerland. Funders of the study included several groups such as the Swiss Research Foundation on Mobile Communication, which is partly supported by Swiss mobile operators. The funders were not involved in the study design or the collection, analysis, or interpretation of the data, according the authors. The World Health Organization (WHO) reignited interest in possible health risks from cellphones after it said in May that using a mobile phone might increase the risk of certain types of brain tumors. Roosli’s research, conducted between 2004 and 2008 in Norway, Denmark, Sweden and Switzerland, looked at phone use of 352 brain cancer patients and 646 control subjects.

About 55 percent of the cancer patients reported regular mobile phone use compared with 51 percent of the control subjects, according to the study. “What we found was that there was no (significant) difference in the amount of use,” Roosli told Reuters, adding that if there is a risk “it would be a really small risk.” But since the study involved face-to-face interviews, Roosli acknowledged that he could not be certain of the accuracy of the subjects’ recollection of past cellphone use. In a subset of the study, Roosli’s also examined information from mobile service providers about the length of the subjects’ cellular subscription when available. From operator data he found that the cancer risk had doubled for those people who had used phones from more than 3 years, but noted that this data was unreliable as more cancer patients had provided carrier records than control subjects. Roosli’s said phone company records were not always available as some people had changed their numbers and some operators were required by law to delete call records after six months. The study found no evidence of any increase in the risk of tumors in brain areas most exposed to cellphone radiation. Roosli said that future studies should examine longer term phone use among children. For example, he suggested collecting phone records from a bigger group to see who develops a tumor. “(This study) provides quite some evidence that use of less than 5 years does not increase the chance of a brain tumor, but naturally we don’t have a lot of long-term users,” he said.

But experts say for anyone who really wants to kick-start a fitness regimen, lose weight, or just change their sedentary ways, training for a 3 mile run may be the shortest distance from couch potato to easy runner. “It’s a better first step than just setting a goal to lose weight,”

Thanks in part to charity events, the little race has come to dominate the field. Some 4 million people nationwide crossed a 5K finish line in 2009, according to Running USA, a non-profit organization for the running industry. “Event organizers push it because of the accessibility of the distance. Marathons are not for everyone. The 5K is a lot easier to host,” Fitzgerald said, and a healthy adult can get in shape for a 5K within 10 weeks.

“Most people can walk a 5K right now,” he said. “Just try to get to a point where you can jog the whole way.”

Pete McCall, an exercise physiologist with the non-profit American Council on Exercise, said a top distance runner can knock out a 5K in 14 or 15 minutes. For beginners 30 minutes is a moderate and accessible target. But be prepared to walk before you run.

And there’s no glory in staring at a scale. “The magic of crossing the finish line is what hooks people,“ said San Diego based Fitzgerald. “It’s life changing.”

“Walk three-to-five minutes, then run three-to-five minutes, then gradually increase the amount of time you run,” McCall said. “It’s all about setting goals and giving your body time to adapt.” He said any realistic plan should also involve flexibility and at least three days per week of core-stabilizing exercises. To avoid overuse injuries, runners refrain from doing too much too quickly. McCall recommends that the novice invest in a shoe designed specifically for running.

ENVIRONMENT UPDATE SMALL FISH VITAL TO SEAS, LOWER CATCHES URGED Small fish play a big role in the oceans and catches should be cut sharply to safeguard marine food chains from plankton to blue whales, an international team of experts said last month. Rising human exploitation of little fish -- including anchovy, sardine, herring, mackerel and capelin -- had had far less attention in marine research compared to big commercial species such as cod, tuna, swordfish or salmon, they said. Over-fishing of small fish has “significant effects on other parts of the marine ecosystems,” said Tony Smith, the lead author of the study at the Commonwealth Scientific and Industrial Research Organization in Australia. He said that the findings, published in the journal Science, were the first comprehensive analysis of how catching small fish, as well as shrimp-like krill, can disrupt marine food chains and so affect human food supplies. Little fish play a pivotal role since they mainly eat tiny plankton and are in turn food for predators such as large fish, whales or seabirds. Small fish account for more than 30 percent of world fish production and are a key food source for many people in developing nations.

The scientists, who used computer models to study stocks of small fish off Peru, the California current, southern Africa, the North Sea and Australia, suggested that catches of should be cut sharply, perhaps backed up by no-fishing zones. They said some stocks were harmed even by a level of catches known as the “maximum sustainable yield” (MSY) of a stock.

With proper planning and precautions, the 7 in 10 Americans hitting the roads and skies this summer can help ensure a healthy vacation for their families. Here is some travel health advice, according to a recent report at www.webmd.com.

5. WASH YOUR HANDS

1. BE PROACTIVE “Think about health in advance of your trip,” advises Bradley Connor, MD, a travel health specialist at Weill Medical College of Cornell University and president of the International Society of Travel Medicine (ISTM). “If you have a chronic health problem, get a checkup before you leave,” he says. And find out as much as you can in advance about destination-specific health risks.

2. PACK A TRAVEL HEALTH KIT Expect the unexpected, warns Connor. He tells all his patients to pack a travel health kit, equipped with the following: . An anti-inflammatory drug such as ibuprofen or naproxen . Anti-diarrhea medication such as Imodium, available overthe-counter . Motion sickness medication -- also available over-thecounter . Band-Aids . Antiseptic cream . Drugs for stomach upset If you wear glasses, pack a spare pair. Mosquito or other bug repellents may also be advisable. And if you take any prescription drugs, be sure to bring along enough for the entire trip, he says. If you’re flying, always place the medication in your carry-on luggage in case your baggage is lost.

3. A SHOT IN THE ARM

“If there is one message we want to say over and over, it is, “Wash your hands,” says Isabelle Nuttall, MD, an infectious disease specialist at WHO headquarters in Geneva. Good hygiene is the first line of defense against any viral or bacterial ailment, be it the common cold or the potentially deadly severe acute respiratory syndrome. When should you wash? Before you prepare or eat food; treat a cut or wound; tend to someone who is sick, or insert or remove contact lenses. And, of course, you should wash after you go to the bathroom; handle uncooked foods, particularly raw meat, poultry or fish; change a diaper; blow your nose, cough or sneeze; handle garbage; tend to someone who is sick or injured, or handle an animal or animal waste.

6. DRINK & EAT SENSIBLY Travelers’ diarrhea throws a wrench into more vacations than any other disease, striking an estimated 10 million travelers each year, says David Shlim, MD, medical director of Jackson Hole Travel and Tropical Medicine in Wyoming. High-risk destinations: Mexico, Latin America, Africa, the Middle East, and Asia. To minimize risk, follow this standard travel health advice: . Drink only bottled or boiled water. . Eat well cooked, rather than raw or undercooked meat and seafood. . Avoid any foods or beverages purchased from street vendors or establishments with unhygienic conditions. . Avoid eating raw fruits and vegetables unless you peel them yourself. . Don’t put ice in your drinks.

About 30 million Americans go abroad each year, some 8 million of whom visit developing countries where the risk of tropical and infectious diseases is high, notes Connor. And many of them fail to follow basic travel health advice. “The good news is most of these diseases are preventable, but the bad news is most people fail to take steps to protect themselves... Ensure you have all the right vaccinations and medications before you leave,” he says.

4. CONSIDER A FLU SHOT Flu shots are now recommended for all people traveling to developing nations, in a group, or on a cruise, says David O. Freedman, MD, travel health specialist at the University of Alabama at Birmingham. 16

MOVE AROUND, DRINK PLENTY OF FLUIDS Sitting in a cramped position for long periods -- whether in an airplane, car, or a bus -- can lead to an increased risk of potentially deadly blood clots, warns Wolfgang Schobersberger, MD, professor of intensive care medicine at the University of Innsbruck in Austria. To minimize risk, drink plenty of non-alcoholic fluids and frequently move your legs, he says. Taking frequent, deep breaths can also

But these ingredients can be toxic, so be sure to follow the instructions carefully.

FDA: YOU CAN’T KEEP BREAST IMPLANTS FOR LIFE Women who get silicone breast implants are likely to need additional surgery within eight to 10 years to address complications such as rupture of the device, health regulators said last month. The Food and Drug Administration will work to revise safety labels for silicone breast implants after reviewing data from several long-term studies, which also showed that implants had a small link to a rare form of cancer. “The key point is that breast implants are not lifetime devices,” said Jeff Shuren, director of the FDA’s Center for Devices and Radiological Health. “The longer you have the implant, the more likely you are to have complications.” There were almost 400,000 breast enlargement or reconstruction procedures in the United States in 2010, according to the American Society of Plastic Surgeons. That includes silicone and saline implants. Post-approval studies showed up to 70 percent of all women who received reconstruction surgery, and up to 40 percent of those getting an enlargement procedure using silicone, needed another surgery within eight to 10 years. The FDA first approved silicone gel-filled breast implants in 2006 for Allergan’s Natrelle and Mentor Worldwide’s MemoryGel. Since then, the manufacturers and the agency have been conducting studies to review postoperation safety. The FDA said the most common complications were 18

hardening of the breast area around the implant, additional surgeries and rupture or deflation of the implant. Other local complications include implant wrinkling, asymmetry, scarring, pain, and infection at the incision site.

The longer you have the implant, the more likely you are to have complications The studies also found a small correlation between implants and anaplastic large-cell lymphoma (ALCL), a form of cancer that affects about 3,000 Americans a year. From 1997 to 2010, there were about 60 cases of ALCL reported for women worldwide out of about 5 million to 10 million women who had breast implants, Shuren said. “If there’s a true association between that cancer and implants, it’s very, very rare,” he said. However, the FDA said most long-term studies confirmed initial results, and that most risks of implants were well known. “Most women reported high levels of satisfaction with their body image and the shape, feel and size of their implants,” the FDA report said, adding that women should monitor their breasts for the rest of their lives to make sure there are no problems.

ALTERNATIVE REPORT COLD REMEDIES: YOU GOTTA BELIEVE! People who believe a cold remedy will work may indeed feel better sooner -- even if they don’t get the real treatment, according to a study. The findings are evidence that the so-called “placebo effect” is at work in recovery from the common cold, which famously has no cure, researchers wrote in the Annals of Family Medicine. So if the treatment that people believe in -- from chicken soup to Vitamin C -- does no harm, they might as well stick with it, they added. “These findings support the general idea that beliefs and feelings about treatments may be important and perhaps should be taken into consideration when making medical decisions,” wrote lead researcher Bruce Barrett, of the University of Wisconsin in Madison. The placebo effect refers to a phenomenon seen in clinical trials

More evidence that what people believe about their medicines matters when people given inactive, fake “treatments,” such as a sugar pill or saline -- show improvements. It has been observed in a range of conditions, including chronic pain, depression, inflammatory disorders and even cancer. Barrett and his colleagues randomly assigned 719 people with the beginnings of cold symptoms to one of four groups. In one group, people were given the herbal cold remedy Echinacea and knew they were taking it. Two other groups were given either Echinacea or a placebo, but participants did not know which they were taking. The fourth group received no pills of any kind. Overall, there were no significant differences among the groups when it came to the severity or duration of the participants’ symptoms, which lasted about a week in all cases. But then the researchers focused on the 120 people who, upon entering the study, gave high ratings to Echinacea’s effectiveness. In that group of Echinacea believers, those who were given pills -- Echinacea or placebo -- felt better faster. Placebo users recovered a full 2.5 days sooner than their no-pill counterparts, while Echinacea users were cold-free about 1.5 days sooner. “That’s actually a huge difference. No treatment out there has ever been shown to reduce the duration of colds,” Barrett said. He added that the findings show more evidence that “what people believe about their medicines matters.” 220 20 0
