Physical Medicine & Rehabilitation
Under the supervision of experienced Physiatry Providers we aim to enhance and restore functional ability and quality of life to people with physical impairments or disabilities.
Our PM&R Physicians
• Treat patients of all ages
• Focus treatment on function
• Diagnose and treat pain as a result of an injury, illness or disabling condition
• Determine and lead a treatment / prevention plan
• Treat the whole person, not just the problem area
• Lead a team of medical professionals, which may include physical therapists, occupational therapists, and physician extenders to optimize patient care
• Work with other physicians, including primary care physicians, neurologists, orthopedic surgeons, cardiologist, pulmonologist and many others
Chronic Kidney Disease Management
Working in Partnership with Concerto Renal Services our Chronic Kidney Disease Management clinical team is trained to provide specialized care and services to patients with chronic kidney disease (CKD) and/or other related renal diagnoses.
Memory Care Services
Our 35- bed memory care neighborhood is specifically designed for those living with Alzheimer’s disease and other dementias. Residing in a secure memory care community allows residents to maintain their functional abilities with a heightened level of safety.
The staff in our memory care neighborhood are dedicated to bringing joy and supporting residents’ physical, emotional, cognitive, and social needs. Level of care plans are customized to help maintain independence and quality of life at each stage.

Our highly trained nursing staff provides 24-hour care with an emphasis on fostering our residents’ independence and encouraging family members and friends to be active participants in their loved one’s daily life.
