Chronic Kidney Disease Management

The clinical team at Mont Marie works in partnership with Dr. Jonathan Slater to provide specialized care and services to patients with chronic kidney disease (CKD) and related diagnoses. Preserving kidney function and preventing further decline are the primary objectives of our CKD Management program. When caught early it is possible to slow, stop or even reverse CKD depending on the cause.

Our program aims to, reduce CKD Symptoms, decrease disability, increase quality of life and promote independence. Through treatment and education, our clinical team of professionals assists patients in a better understanding of their conditions, enabling them to enjoy a greater level of independence and improved quality of life.

Dr. Jonathan Slater, board certified in both Internal Medicine, Nephrology and Hypertemsion and an assistant professor of Medicine Tufts. Cardiopulmonary Rehabilitation

Our physician-led Cardiopulmonary Rehabilitation program, under the direction of a pulmonologist, together with our team of Respiratory Therapists is focused on helping patients achieve the most active life possible despite any cardiopulmonary diagnoses and physical limitations. We tailor our program for individuals with heart disease and pulmonary disease, with diagnoses such as CHF, COPD, post heart and lung transplants, emphysema, acute myocardial infarctions and acute respiratory failure.

Serving as a bridge between an acute stay and home environment, our program aims to reduce the need for hospitalization and promotes independence and overall quality of life.

Cardiac Rehab

Benefits of cardiac rehab:

• Healthy lifestyle changes- poor dietary choices and an inactive lifestyle can exacerbate heart conditions, but old habits are hard to break. About 31% of all global deaths are due to cardiovascular disease, which develops as a result of unhealthy weight, poor stress management, and smoking. Cardiac rehab teaches people how to control these risk factors and improve long-term heart health.

• Personalized exercise plans- patients who have gone through cardiac events are understandably anxious about exercise and other strenuous activities. One of the biggest benefits of cardiac rehab is having a personalized exercise plan and a dedicated coach who can ease your concerns and provide encouragement as you transition back to daily life at home. Physical activity not only helps your heart, but it strengthens your coordination, balance, and muscles.

• Ongoing assessments and analyzations- risk factors like blood pressure, blood sugar, and cholesterol need to be monitored after a heart attack. For example, too much cholesterol blocks arteries and can lead to a heart attack, but the perfect amount helps you digest food and make Vitamin D. Going through cardiac rehab teaches patients how to review each risk factor and interpret the results so that they have early warning signs of new or returning issues.

• Psychological and social support- cardiac events affect patients physically, emotionally, and socially in ways they never anticipated. Cardiac rehab provides patients with all elements needed to completely recover, including counseling for depression or other psychological concerns. Many programs also connect patients with similar conditions in order foster a community of recovery and support. When people are happy, they live longer (and healthier) lives.

• Improved access to healthcare professionals- the recovery process is one that too many patients undertake without the aid of doctors and other healthcare professionals. Participating in cardiac rehab keeps patients connected to doctors and other therapy providers who ensure that the patients have access to information and interventions as soon as possible. Pulmonary Care

Dr. Andrey Pavlov is board certified in pulmonary disease, critical care medicine and internal medicine. He has been successfully affiliated with multiple hospitals in the area for 10 years. He is a graduate of University of Connecticut medical residency program and University of Texas Southwestern Medical School. Dr Pavlov is currently affiliated with Holyoke Hospital and HMC Pulmonology Center.

Orthopedic Transition

Our specialized Orthopedic Transition Program is geared towards maximizing the recovery of individuals recovering from joint replacement, fractures, traumatic injury or spinal surgery. To get back on your feet following orthopedic surgery, a rigorous and targeted rehab regimen is key. We design an effective treatment plan in consultation with our therapists, doctors, your physician, and a board-certified physiatrist. The Urgent SNF Service at Marquis Health Consulting Services supports a global health care expectation for the right provision of care in the right place at the right time.Therefore, our solution driven service aims to provide an alternative option to individuals not meeting acute admission criteria, however requiring and/or benefiting from supervisory skilled rehabilitation services, skilled nursing oversight or stabilization.
