Mary Bird Perkins Cancer Center (MBP) strives to provide the highest quality radiation therapy and compassionate support to all patients and their families. Upon completion of treatment, patients at MBP locations are given a quality of care questionnaire in which their identity remains anonymous. Results are sent to an outside independent research consultant who compares the results to those of other similar community cancer centers across the country.

Recently, the Mary Bird Perkins Cancer Center location in Hammond received a perfect score for patients “likely to recommend MBP to family and friends.” In addition, the Hammond facility excelled in the amount of time nurses spent with patients, overall quality of care and also received a perfect score in promptness of doctors returning phone calls.

With a brief look over my shoulder, I mention September and Hurricane Gustav for one reason only. Before, during and after the storm, Center staff worked around the clock doing everything possible to communicate with patients at all Mary Bird Perkins locations to ensure continued treatment and to keep patients as informed as possible. I wish to take this opportunity to extend my deepest thanks to each and every staff member for continuing to make the critical needs of cancer patients a priority. You make MBP the special place it is.

TomoTherapy Technique Developed at MBP Procedure Offers Improved Cosmetic Outcomes for Postmastectomy Patients

F

For the past two years, Mary Bird Perkins Cancer Center has been using TomoTherapy for postmastectomy chest wall irradiation. The technique, developed at Mary Bird Perkins, is the result of research funded by TomoTherapy and performed by MBP’s medical physics group in collaboration with the radiation oncology staff. This technique offers the advantage of a highly uniform dose to the chest wall, which translates into excellent cosmetic outcomes for the patient.

“I’ve been extremely pleased with this treatment delivery method for several reasons. We are now able to confidently include ‘risk areas’ in our treatment planning that are either compromised or even excluded when using nontomotherapy techniques.”

Renee Levine, M.D. Radiation Oncologist, MBP “This was a challenge in the past for us when treating women who had their reconstruction prior to radiation treatment. It is now a non-issue. Also, TomoTherapy has allowed us to vastly improve the cosmetic outcomes of those women who choose reconstruction after radiation treatment, which is the most common way the treatments are sequenced. This improvement cosmetically has even benefited reconstruction techniques themselves,” stated Levine, one of the primary users of the technique.

Mary Bird Perkins Cancer Center radiation oncologists and medical physicists, world experts in treatment of superficial cancer using electron beams, recognized shortly after beginning treatment with TomoTherapy in 2005 that TomoTherapy offered an attractive alternative for postmastectomy chest wall irradiation. Mary Bird Perkins’ research has helped radiation oncologists to understand the best way to use TomoTherapy to irradiate the chest wall and other superficial disease.

MBP radiation oncologist Greg Henkelmann, M.D. acknowledged, “TomoTherapy allows for a more uniform radiation dose distribution with some superficial cancers and eliminates the dose inhomogeneity problems that can be associated with abutting radiation fields. TomoTherapy has also proven to be very useful for rare, but complex total scalp irradiation and is excellent for head and neck and prostate cancers.”

The goal of chest wall treatments is to irradiate cancer cells that remain in the chest wall and regional lymph nodes after a mastectomy while giving minimal dose to the healthy tissue, particularly nearby lungs, heart and opposite breast.

Traditionally, conventional accelerators have provided an arsenal of techniques – tangentially opposed x-ray beams, en-face electron beams or a combination of the two. TomoTherapy is now added to that arsenal. The TomoTherapy technique, in addition to its unique irradiation technology, utilizes a 1-cm bolus (plastic shield) conformed to the chest wall. The bolus helps ensure that the patient’s chest wall remains in the radiation field, even with small respiratory motion.

This technique would not be possible without medical physics researchers working with the radiation oncology staff. For his MS

thesis research, LSU medical physics graduate student Michael Ashenafi (2006), under the supervision of former staff medical physicist Dr. Robert Boyd and radiation oncologist Dr. Kenneth Lo, conducted a study comparing TomoTherapy with electron beam treatment plans. Results of that study showed that treatment plans for TomoTherapy provided both a more uniform dose to the chest wall with equally high cancer control probability and an equally low dose and low probability of normal tissue complications of the lung and heart. Dr. Lo concluded that these results were clinically impressive and would make TomoTherapy a more optimal choice of treatment for some patients, outweighing the slightly increased risks that result from a greater patient volume of low dose levels by TomoTherapy.

Medical physicists Drs. Dennis Cheek, John Gibbons, Kenneth Hogstrom and Isaac Rosen have reported on the accuracy of the TomoTherapy dose calculation used for the treatment planning system, particularly important for superficial tissues. Results of this study and an earlier one by LSU medical physics graduate student Koren Smith (Dr. Gibbons, supervisory professor) were recently published in February and August 2008 issues of the prestigious journal Medical Physics. Dr. Hogstrom stated, “These results showed the TomoTherapy dose calculation was sufficiently accurate for clinical use. Although results showed small inaccuracies at depths less than 10 mm, this dose falls in the patient bolus and has no patient impact.”

Chief of Clinical Physics Dr. John Gibbons added, “Common with new techniques like this, the clinical medical physics group closely monitors treatments by placing small thermoluminescent dosimeters on the patient’s skin surface to record daily treated dose. Results of these in-vivo measurements are part of our patient quality assurance practice.”

Results of these data along with patient motion data (4D treatment planning) are being analyzed by graduate student Shima Ito (Dr. Brent Parker, supervisory professor). Dr. Parker stated, “Results of the patient data are quite impressive, and results of similar data measured in a plastic, water filled human-like phantom show the importance of our clinical practice to minimize the air gap between the bolus and skin surface.”

“This is a prime example of how the research capabilities of the MBP-LSU medical physics partnership can ultimately affect the way cancer is treated. Publication of the results of the TomoTherapy research conducted at Mary Bird Perkins will benefit postmastectomy patients around the world by helping other TomoTherapy users better understand how to apply this new technique.”

What is a Medical Physicist?

The medical physicist is the member of the radiation oncology team who is responsible for the technical aspects of radiological imaging and radiation therapy of cancer patients. The medical physicist’s duties include selecting and implementing new equipment and techniques, planning and delivering complex treatments such as radiosurgery and providing continuous quality control of patient treatments.

The medical physicist also manages radioactive material and ensures patient safety. A major pathway to becoming a medical physicist requires an M.S. or Ph.D. in medical physics and two years of residency or on-the-job clinical training, after which certification by the American Board of Radiology in therapeutic radiologic physics can be achieved.

Established in 2004, the MBP-LSU Medical Physics program has made impressive strides, including luring Dr. Kenneth Hogstrom, a renowned heavyweight in the field. Hogstrom left a 25-year career at The University of Texas M. D. Anderson Cancer Center in Houston to help grow the program. He holds the Dr. Charles Smith Chair of Medical Physics at LSU and serves as academic program director. He is also chief of physics at Mary Bird Perkins.

Currently, there are about 6,000 medical physicists in the U.S., and most, like Neck, work as clinicians. The American Association of Physicists in Medicine, AAPM, projects a need for 300 new medical physicists per year nationally, which is about double the capacity of established programs. That shortage will only intensify as Baby Boomers age, says Hogstrom.

Thus, the intention of the MBP-LSU program is to produce well-trained, marketable medical physicists who can both work in South Louisiana and move on to top jobs nationwide. The average pay in 2007 was $164,000, according to AAPM.

Next on the horizon is a PhD program, which Hogstrom believes will allow MBP-LSU to produce more research, the key, he says in positioning it to be competitive. Students in Hogstrom’s program published seven research papers in respected academic journals over the last year.

In his short tenure, Hogstrom has crafted a strong master’s degree curriculum, attracted top-caliber students, built an infrastructure and gained accreditation from the Commission on Accreditation of Medical Physics Education Programs.

“If you don’t have students doing research, you don’t have time day-to-day to push the abilities of the program,” he says.

During the last academic cycle, more than 70 students from around the country applied for five openings.

“I’m a firm believer in focusing on quality, not quantity. If you attract the best people, you’ll grow the right way.”

As a well-known field leader and the former medical physics chair at UT-M. D. Anderson, Hogstrom is often asked what it will take to propel the Mary Bird Perkins-LSU program to the top ranks of national cancer centers.

“Becoming like M. D. Anderson might be 50 years out,” he says, “but if we keep doing what we’re doing and we move into research, we’ll be on the right track.”

Mary Bird Perkins Cancer Center and LSU recently co-hosted a seminar titled “Dedicated Breast CT Imaging System” in recognition of Breast Cancer Awareness Month. An internationally-known expert in the field, Dr. John M. Boone, professor and Vice Chair (Research) of Radiology at the University of California Davis Medical Center was the featured speaker. Boone presented his research to students and faculty pertaining to the use of computer tomography – or CT – imaging systems as part of a comprehensive diagnostic breast examination from pre-diagnosis risk assessment to post diagnosis virtual biopsy and therapy.

MBP Adds Two M. D. Anderson Graduates to Medical Physics Research Staff

M

Mary Bird Perkins Cancer Center welcomes Drs. Jonas Fontenot and Mich Price to its Medical Physics team. Drs. Fontenot and Price, who both joined MBP in August 2008, were appointed as academic medical physicists and will be splitting their time between clinical and academic medical physics activities.

Dr. Jonas Fontenot is a native of Crowley, LA. He received his B.S. in Physics from the University of Louisiana-Lafayette in 2002 and his M.S. and Ph.D. in Medical Physics from The University of Texas M. D. Anderson Cancer Center (MDACC) in 2005 and 2008, respectively. For his M.S. thesis, Fontenot studied proton dose calculations for eye treatments, and for his Ph.D. dissertation, he compared the estimate of cancer risks for proton therapy with those for IMRT, both under proton therapy expert Dr. Wayne Newhauser. Fontenot’s research efforts have contributed to 13 publications in scientific journals. He received first place in the Young Investigators Symposium at the fall 2007 meeting of the Southwest Chapter of the American Association of Physicists in Medicine for his Ph.D. research.

Dr. Mich Price is a native of Philadelphia, PA, where he received his B.S. in Physics from Temple University in 2000. He received his M.S. and Ph.D. in Medical Physics from MDACC in 2004 and 2008, respectively. For his M.S. thesis, Price studied electron dosimetry for the PBRA treatment planning dose algorithm under MBP Chief of Physics Dr. Kenneth Hogstrom, who at the time held the P.H. and Fay Etta Robinson Distinguished Professorship in Cancer Research at MDACC.

For his Ph.D. dissertation, Price studied the imaging and dosimetric capabilities of a novel, MR/CT-suitable, anatomically adaptive, shielded HDR/PDR intracavitary brachytherapy applicator for the treatment of cervical cancer under brachytherapy physics expert, Firas Mourtada. Dr. Price’s research efforts have contributed to six publications in scientific journals. He received second place in this year’s John R. Cameron Young Investigator Competition at the annual meeting of the American Association of Physicists in Medicine in Houston, TX for his Ph.D. research.

The addition of Fontenot and Price brings the total number of medical physicists on the Mary Bird Perkins team to eleven.

one of only six centers selected by TomoTherapy to evaluate the software that ensures safe, accurate radiation treatment for cancer patients. Gibbons’ presentation also discussed dose reconstruction, the process of evaluating TomoTherapy’s CT system to determine the daily dosage of radiation of each patient’s treatment.

Gibbons presented alongside nearly 230 other exhibitors participating in the annual meeting. An estimated 12,000 oncologists of all disciplines gathered throughout the week for the event. ASTRO, founded in 1958, is the largest radiation oncology society in the world.

Throughout the month of September, National Prostate Cancer Awareness Month, the Cancer Program of Our Lady of the Lake and Mary Bird Perkins offered free screenings to the public with the goal of reaching as many men as possible and educating them about the importance of early detection. Prostate cancer is the most frequently diagnosed cancer in men according to the American Cancer Society.

The resolution recognized and expressed appreciation for Holden’s continued support of MBP and its early detection initiatives. First of MBP’s $20 Million Investment Arrives in Gonzales

The first of Mary Bird Perkins Cancer Center’s (MBP) $20 million dollar investment in Elekta digital linear accelerator systems recently arrived at the new MBP facility in Gonzales. “St. Elizabeth Hospital is very excited to partner with Mary Bird Perkins Cancer Center to bring radiation therapy services to the people of Ascension Parish and surrounding areas,” said Dee LeJeune, president and CEO of St. Elizabeth Hospital. “The new facility not only brings state-of-the-art equipment for the treatment of cancer to our area, it will provide highly qualified physicians and staff to care for members of our community who are in need in a

MBP Volunteers Raising Funds To Extinguish Cancer

Mary Bird Perkins’ fundraising volunteers gathered at Chairman Bill O’Quin’s home to kick off the 2008 Annual Giving Campaign. Sixty volunteers climbed aboard one of several fire engines present for the evening symbolizing the theme of this year’s campaign, Extinguish Cancer. Along with staff members from the Center’s Development Office, the volunteers will meet with individuals and companies throughout the community to share Mary Bird Perkins’ story and raise financial support for the community outreach, education, early detection, survivorship and research programs the Center provides.

Dynamite Duo Teams Up To Extinguish Cancer

E

Extinguish Cancer is the theme of this year’s volunteersupported Annual Giving Campaign, and MBP has a mother/daughter team helping to lead the way.

When she became a volunteer last year, Mary Kadair Crump quickly recruited her daughter, Elizabeth McKnight. Crump, owner of Kadair Advertising Mary Kadair Crump and and a Baton Rouge native, is a Elizabeth McKnight member of the committee responsible for steering the efforts of the Campaign along with several other volunteer members.

“For the longest time Mary Bird Perkins was just a building on Essen Lane that I drove by,” said Crump, who first became aware of the need for community cancer treatment when both her aunt and father’s business partner’s wife and daughter were diagnosed with cancer. “That’s when it became more than just a building on a corner.”

Her 24-year-old daughter, McKnight, is one of the campaign’s youngest volunteers. “I feel it’s my responsibility to be a part of something that benefits those in our community. Part of growing as a person is doing something to better the lives of others, and I see this as a great opportunity to do that.”

“I am a passionate person and to see the compassion and care that MBP has for its patients is important to me,” added Crump. “Knowing that we all have access to state-of-the-art radiation treatment, quality cancer care and early detection services is why I want to partner with MBP to make a difference.”

“I’ve come full circle again this year,” said Crump. “I am very excited about the sense of accomplishment all our volunteers will experience and what we’ll be giving back to the community.”

To thank Hammond and Northshore donors for their loyal support and interest in Mary Bird Perkins Cancer Center, a reception hosted by owners Rob and Pam Mingo was held at Heiner Brau Brewery in Covington on October 2. Approximately 40 guests strolled through the microbrewery co-owned and operated by Henryk and Angela Orlik sampling beer custom brewed by master German brewer Henryk Orlik.

“To have an organization like Mary Bird Perkins Cancer Center in our community is something to be proud of,” said guest Cindy Harlan, vice chair, MBP Greater Northshore Business & Community Council. “No one in the community is denied radiation treatment due to inability to pay, and their Early Bird screenings focus on cancer early detection for the medically underserved. Dow’s public affairs leader, Stacey Chiasson, who helps obtain the annual gift, is more than just a financial supporter.

She serves as the chair of the Corporate Division of the Annual Giving Campaign. At a recent Corporate Division meeting, she shared why Dow is so supportive of the Mary Bird Perkins mission.

Walk/Run Benefiting MBP New Event for Oktoberfest

O

Oktoberfest is Covington’s version of the renowned Bavarian event celebrated annually in Munich, Germany. Held in October, a new activity was added to the list of festivities that make up the fun-filled weekend. The first annual Runner’s Choice Walk/Run benefiting Mary Bird Perkins Cancer Center attracted over 230 participants and raised over $3,000.

“We had a great time volunteering at the race and want to thank the Oktoberfest organization for letting us be a part of such a wonderful event,” said Sherri Blackwell, one of the MBP Covington employees who helped with the race. “It’s a joy to be part of such a giving community.”

Mandeville Seafood Association To Make Yearly Donations to MBP in Memory of Russell Nash

T

The Greater Mandeville Seafood Festival, an organization run entirely by volunteers, has given approximately $1 million to support many local charities, foundations and public service agencies in the Greater Mandeville area since its inception 29 years ago. Due to the efforts of Russell Nash, five years ago Mary Bird Perkins Cancer Center became one of the Festival’s top beneficiaries.

“Russ and I were at a St. Tammany West Chamber meeting and met a representative from Mary Bird Perkins,” explained Bill Dobson, president of the Greater Mandeville Seafood Association, Inc. “She invited us to take a tour of the Covington Center. We were very impressed and what stuck with us was Mary Bird Perkins’ commitment to serving all patients, regardless of ability to pay.”

Nash went back to the Seafood Association board and asked that Mary Bird Perkins be given a $20,000 donation from the Festival that year. Shortly after, one of the board members received a very serious cancer diagnosis. “She was treated at Mary Bird Perkins and she sang its praises during and after treatment,” said Dobson. “That truly affected the Board of Directors.” Again, the Center received a generous donation of $20,000.

In October 2005, Nash himself was diagnosed with cancer and was given a very grim prognosis. “When it came time for radiation, he never thought of going anywhere else but Bill Dobson and Jocelyn Nash Mary Bird Perkins,” said his widow, Joycelyn. “I thank God we were able to stay so close to home.”

“Mary Bird Perkins has given us more satisfaction in return than any organization,” said Dobson. “Not just by the Center’s thank you letters but by the benefit Mary Bird Perkins provides to our community. To have that kind of service right in our backyard is something we want to see continue.”

At its last board meeting, the Greater Mandeville Seafood Association decided to make this year’s donation to Mary Bird Perkins, and all future donations, in memory of Russell Nash.

Adameks Select Survivorship Life Insurance as Estate Planning Tool Mary Bird Perkins Named Among Beneficiaries Tom Adamek’s original roots are in Rochester, NY. His wife, Lisa’s are in Pittsburgh, PA. Their paths crossed when their fathers’ respective jobs in the petrochemical industry brought them to Baton Rouge. Tom’s family ended up staying and putting down new roots. Lisa’s family moved on, but she chose to stay to attend LSU. The two met in 1996 after graduating from LSU. Lisa was practicing law, and Tom was employed with Premier Venture Capital.

For a young couple, one might wonder how estate planning – particularly the choice of a second-to-die or survivorship life insurance policy – was something on their radar. “It evolved over time,” said Tom. “I had become involved with Mary Bird Perkins through a friend of mine, Blanchard Sanchez, who recruited me for the Annual Giving Campaign to help raise funds for the Center.”

“Since Lisa’s mom passed away of cancer at an early age and my sister was successfully treated at Mary Bird Perkins for non-Hodgkin’s lymphoma, it was a good fit for us. A few years back, Lisa and I began giving our own donation as part of the annual giving campaign and have done so ever since.

Based on our participation in planned giving, Mary Bird Perkins Cancer Center Foundation’s board chairman, Lee Griffin, asked me to consider serving on the Foundation’s Development Committee. I accepted and have learned much about various estate planning tools with philanthropic components that piqued my interest.”

Tom and Lisa purchased a type of life insurance on two people (usually married) that pays proceeds to the beneficiaries only after the last surviving spouse dies. The principal purpose of a second-to-die policy is to pay estate taxes that would otherwise become a burden on the heirs, reducing the value of the estate passed on. Thus, second-todie insurance is an estate planning tool to which philanthropic components can be neatly added. In addition to the Adameks’ children, Mary Bird Perkins is one of their policy’s beneficiaries.

“It would be my hope that people our age would utilize the many resources available to educate themselves about planned giving,” said Lisa. Her husband agreed, adding that over the years they have reduced the plethora of organizations they used to support in order to give more sizeable gifts to the ones upon which they truly want to have an impact.

Preparing for a First Visit with a Professional Advisor: If you are about to visit a professional advisor for the first time, members of the council agree, your first visit with a financial advisor is not unlike your first visit to a new doctor, when a nurse begins by asking you about the nature of your ailment, your past illnesses, your current medications and so on. Similarly, before you meet with a financial advisor, it helps to be prepared. Think about your short-term and long-term goals and be ready to answer some personal questions regarding your finances and your investment strategies.
