APPLICATION PACKET FOR LEAD CERTIFICATE COVER LETTER May 6, 2011 Dear UFMCC Lay Leader: Thank you for choosing to submit your application for UFMCC’s L.E.A.D. (Laity Empowered for Active Discipleship) program, offered by the Office of Formation and Leadership Development. The purpose of this letter is to familiarize you with the application process. The application process consists of three distinct components: • • •
Five part application Leadership Key Questions (4) Recommendation form
The application includes the following five sections: • • • • •
Personal information Education and professional training Professional development (last three years) Past/present church leadership/ministry positions Professional/personal references
Please complete each of these five sections thoroughly. The second component consists of four key leadership questions. Please answer each question. Your responses for all four questions should not exceed a total of four pages (double spaced, 12‐pt. font). The third part of the process requires you to secure two written recommendations. The one page recommendation form is included with the application. Please select at least one of the following individuals to complete the recommendation form: a member of MCC clergy, an MCC Elder, a Ministry Leader from your congregation, or an MCC/UFMC staff member. If you select one (rather than two individuals) from the above list, you may also choose to select an individual who knows you on a professional basis (a boss or colleague). This person should be able to describe your leadership experience/competencies.

Your application, key leadership question responses, and recommendation letters will be reviewed by the Program Admissions Committee. You will be informed of the Committee’s decision by July 15, 2011. If you are accepted into the L.E.A.D. program, you will be required to attend the L.E.A.D. retreat to be held August 18‐21, 2011 at the Day Spring Conference Center in Ellenton, FL. Cost of the retreat is $400 which includes three nights lodging and all meals. Upon your acceptance into the program more detailed information will be sent to you about the retreat schedule and travel options. The purpose of L.E.A.D. is to offer lay leaders of UFMCC an opportunity to deepen their spiritual growth, develop their existing skills, and furnish them with additional leadership tools so that they may be effective leaders in the church and the world. L.E.A.D. curriculum will engage a variety of methods to include self paced, instructor led, and seminar based training(s) to support each individual’s learning style. Persons will begin the program by attending an L.E.A.D. Retreat offered by the Office of Formation and Leadership Development in which they will be introduced to the program, basic principles of leadership development and in discussion with faculty will create an L.E.A.D. Development Plan that spells out their specific requirements for completion of the Certification program.
