Extreme Conference is going to change your world once more. In 2010 we were focused on “getting ourselves ready” and “being involved in our community”. This year we want to go further. We’re going back to basics… with discipleship. Jesus tells us very clearly to go and make disciples, so this is our ﬁrst priority. We believe that discipleship is a vital key to on-going transformation and revolution in our world. We exist for our communities, our friends and families and for those who are to come - for those who don’t know Jesus. This year at conference we are privileged to host guest ministry who have great inﬂuence, great humility and most importantly a great passion for Jesus and empowering the church. We’d love to host you at Extreme Conference and champion you in becoming the person God sees - the person to change this world for His Glory. GST/RECEIPTS: The registration price does not include GST as this conference is considered a ‘religious service’. Should you require a tax invoice for your records, please photocopy this registration form prior to returning to us. REFUNDS/TRANSFERS: No refunds will be paid. However you may transfer your registration to another person. All transfers must be made to Extreme Conference in writing and will incur an administration fee of $2.50 per change. CHILD MINDING: Child Minding is available for children aged 2-10 years. The parent or guardian of children aged between 2–10 years must be full-time delegates and are in attendance whilst child is being minded. INDEMNITY: I hereby agree to indemnify, release and hold harmless Extreme Conference, its staff, volunteers and agents against any and all claims arising from, or in connection with, any injury, accident, misfortune, damage or loss that may occur to myself/my child and/or my/their property, equipment or personal effects while present at the Event and within the Event premises, including any injury that my child may cause to another person. CUSTODY: I hereby agree to inform the Child Minding Check-in Team of any custody issues that are in effect.
