Supported Living Facilities Community Workshop and Training Center’s Supported Living Program assists individuals with life skills training while living in an apartment/home setting. Support services are provided to all residents on an individual basis dependent on their needs and preferences. Some of the areas of support may include: financial management, meal preparation, benefits and access to the community.

• 18 years of age or older. • Must have a documented disability. • Should be able to access community resources. • Must meet HUD guidelines. • Commitment to maximizing independent living skills • Ability to take action for selfpreservation in the event of an emergency.

All staff members are trained in basic first aid, CPR and CPI (crisis intervention). Prior case management experience required. Staff is trained in Illinois mandated Rule 50. “The Supported Living program offers individuals with disabilities an opportunity to maximize their independence in a safe, secure environment. Family Support staff assist the residents with their individualized programs. Accessing the community to enhance their skills is promoted and coordinated by staff.”

has been a resident in a Supported Living apartment in Peoria for seven years. Over the past few years, she has made significant progress improving her quality of life. Facing some major health problems, Mary Ann took an aggressive approach to diet and exercise, with the assistance of her case manager, and now has managed to get her health problems under control. She has become her own payee for her Social Security benefits and works with staff to maintain financial stability by budgeting her own money. She also serves as vicepresident of CWTC’s self-advocacy group. Mary Ann has been successful in setting realistic goals and achieving them.
