DoD official: No changes to women’s essential health care Department of Defense WASHINGTON — The recent Supreme Court opinion in Dobbs v. Jackson Women’s Health Organization does not prohibit the Defense Department from continuing to provide essential women’s health care services to service members, dependents, other beneficiaries and DoD civilian employees, the undersecretary of defense for personnel and readiness said in a memo released June 28.

Gilbert R. Cisneros Jr. noted that under existing federal law, DoD may only perform or pay for abortions if the life of the mother would be endangered if the fetus were carried to term or if the pregnancy is the result of rape or incest – called “covered abortions.”

The recent Supreme Court decision does not prohibit DoD from continuing to perform these covered abortions, consistent with federal law, he wrote.

“There will be no interruption to this care,” Cisneros said.

“Health care providers will continue to follow existing departmental policy,” he said, adding that military medical treatment facilities will implement measures to ensure continued access to care.

Cisneros said it is the Justice Department’s longstanding position that the states generally may not impose criminal or civil liability on federal employees who

are performing their official duties in a manner authorized by federal law.

DoD will work with the Justice Department to ensure access to counsel for civilian employees and service members if needed and as appropriate, he wrote.

The Supreme Court’s decision also does not affect DoD’s leave policies, Cisneros said.

“Existing department policy authorizes active-duty service members to travel as necessary to receive abortion care,” he said. The travel may be government-funded, official travel for a covered abortion, or for all other cases, it may be undertaken as regular leave at the service member’s expense.

“Access to emergency or convalescent leave remains unchanged for all service members,” Cisneros noted.

DoD civilian employees may continue to use sick leave or other forms of leave as necessary to care for themselves or their family members, he said. Sick leave may also be used to cover travel that may be needed to obtain any type of medical treatment.

DoD, the military departments and the DoD Office of General Counsel will continue to review current DoD policies and procedures in light of evolving state laws to assess any impact they may have on DoD. Additional guidance will be issued as appropriate.

Biden Announces Changes in U.S. Force Posture in Europe Jim Garamone DOD NEWS

President Joe Biden announced moves that will enlarge the U.S. troop presence in Europe and strengthen the NATO alliance.

Biden made the announcement at the beginning of the NATO Summit in Madrid today following a welcome from NATO Secretary General Jens Stoltenberg.

The move belies efforts by Russian President Vladimir Putin to shatter the Trans-Atlantic alliance.

“NATO is strong, united, and the steps we’re taking during this summit are going to further augment our collective strength,” Biden said. “To that end, today, I’m announcing the United States will enhance our force posture in Europe to respond to the change [in the] security environment, as well as strengthening our collective security.”

The president’s announcement comes on the heels of a surge of 20,000 U.S. service members to Europe to defend alliance countries in the face of Putin’s unprovoked and unnecessary war on Ukraine. There are now more than 100,000 U.S. service members serving in Europe with many along the eastern front of the alliance.

Ride-alongs improve TACC flight manager, aircrew collaboration Travis Air Force Base Flight managers from the 618th Air Operations Center, or Tanker Airlift Control Center, at Scott Air Force Base, Illinois, are now able to participate in ‘ride-along’ flights on mobility aircraft to observe firsthand how pilots and aircrew conduct operations in the sky.

On June 1, the first ride-along took place on a regularly scheduled sortie, flying two flight managers in a C-17 Globemaster III from Travis AFB, California, to Joint Base Pearl Harbor-Hickam, Hawaii. They returned to Travis AFB with another C-17 aircrew June 5.

The program’s goal is to create a feedback loop and a shared perspective to help improve collaboration between flight managers and aircrew, while also streamlining operations and improving mission effectiveness.

Throughout a scheduled mission, volunteer flight managers will join aircrew for multiple flights, observing the crew in action and discussing any challenges or required updates to the flight plan and mission. Less experienced flight managers will be able to pair with managers who have previous aircrew experience to encourage further learning and communication.

ARLINGTON, Va. (AFNS) — Offering a report card on the state of transformation across the Air and Space Forces, Secretary of the Air Force Frank Kendall said June 24 that progress has been made but more is needed, and the pace of change must quicken to reshape and refocus the services to meet fast evolving global threats.

Appearing at a “fireside chat” sponsored by the Air Force Association, Kendall pointed to

Ukraine to dismiss doubts about the threats and dangers in the world today while also emphasizing that China poses an even more significant security challenge.

“We have had the lesson of Ukraine in the last few months that has demonstrated that aggression by major powers against lesser powers does occur,” Kendall said. “It can occur very quickly and very violently.

If we are to deter that we have to have the capabilities to defeat that kind of threat.”

While the world is focused

on Russia’s invasion of Ukraine, Kendall said that for the Air and Space Forces the bigger challenge is China and that reshaping the services to better address that reality is a major goal.

China, he said, “has figured out what we depend upon to project power. On that are a relatively small number of high value targets – aircraft carriers, forward bases and satellites as well as command and control nodes and logistics centers.

They have built a large arsenal

of conventional precision munitions . . . to attack those assets. . .

They’re doing that to intimidate us and make it difficult for us to project power.

“We have to reorient ourselves and develop a sense of urgency about out-thinking, outspending, out-smarting a very capable adversary,” Kendall said, offering a slight variation on a theme he has aggressively voiced since becoming the department’s highest ranking civilian leader nearly a year ago.

He also offered a blunt assessment.

“I don’t want to understate or undersell the capabilities the United States has,” Kendall said.

“We are a very formidable military power, and it would be a grave mistake for anybody, including China, to try and take the United States on. We have a lot of capability we can bring to bear and we have a very professional, very well-trained military.

Blood loss or “bleeding out” is the leading cause of preventable death on the battlefield, military health experts say.

So one of the best ways to save lives during combat operations is to provide blood products to forward deployed medics and corpsmen as soon as possible.

During the wars in Iraq and Afghanistan, getting the needed blood products to injured warfighters was typically not a major challenge when the U.S. military controlled the skies and maintained a nearby network of medical facilities.

“We were pretty reliant on medevac ‘dust off’ to deliver our blood,” said Air Force Col. (Dr.) Stacy Shackelford, chief of the Joint Trauma System.

However, in future conflicts against a “near-peer” adversary, Shackelford said, that could be far more difficult. Injured troops may have to remain at the frontlines for days while needing blood transfusions or other

major medical care.

The solution: Drones may become essential to combat medicine.

“I think it’s going to come down to drone delivery of blood by some type of unmanned vehicle that can fly in and drop off more blood or more bullets, whatever is needed,” Shackelford said.

Blood Resupply “The primary issue with blood resupply is that it has to be maintained at specific temperatures, as do a number of pharmaceuticals including certain pain medications, and antibiotics,” Meledeo said.

“The biggest technological hurdles right now are being able to maintain those temperatures inside those drone payloads very consistently, at a variety of altitudes, and a variety of different ambient conditions for potentially lengthy periods of time, without drawing too much power away from the system itself.”

The Marines Corps used drones for resupply during an exercise in Australia. Drones have also been used in Rwanda and Uganda to transport medical supplies to rural areas across mountain ranges and in bad weather, Meledeo said.

“I think that we’re going to get there much faster than we had initially anticipated,” he said.

Resupply by Drones “We think that drone resupply of blood and immediate-need medical products are really just around the corner,” said Dr. Adam Meledeo, a research scientist for coagulation and blood research at the U.S. Army Institute of Surgical Research, Fort Sam Houston, San Antonio, Texas.

“There are multiple off-theshelf solutions that are being considered,” and DHA is funding a number of other innovations to optimize the ability to provide drones in the battle space, Meledeo said.

problem,” Meledeo said.

“There’s also been some talk of outfitting some of our combat hardware drones that are already in use with alternative payloads that would be able to supply blood, medical supplies and really just about anything, such as MRE’s, ammunition, and water” to frontline medics or service members caring for wounded soldiers, he explained.

“I’m not suggesting we’re at any kind of grave risk right now.

I am concerned about these developments, however, and what they portend,” he said, referring to China’s actions to expand and modernize its own military.

Facing that reality, Kendall said for the U.S. to maintain the nation’s security and historical advantage, the Air and Space Forces must change and refocus.

The path to achieving that is embodied in his seven Operational Imperatives, the blueprint he developed for rapidly changing the hardware, policies and cultures of the Air and Space Forces to better position them to confront current and emerging threats.

The first of the seven

imperatives focuses on space. On that front, Kendall said, the Space Force “is developing a resilient force design to modernize and deliver new capabilities at operationally relevant speeds” in a once “benign” domain that is now a contested environment.

For the most part Kendall said he is pleased with the progress and expects additional advances to come as a result of the fiscal year 2023 budget. Maintaining progress and momentum, however, is not guaranteed.

“We have a reasonable balance. We are meeting the needs of combatant commanders today and we are making reasonable investments for our future. I was quite comfortable defending the budget that I presented but I also put a warning note on the table that FY 24 was going to require some hard choices.

At the same time, Kendall repeated one of his most common refrains – the services must move fast.

“Our most precious resource is time. ” he said.

While candidly describing the challenges the services are facing, he also pointed out examples where progress has been made.

He highlighted gains made in space by the U.S. Space Force and expressed general comfort with the always difficult effort to balance existing “capacity” and “capability.” That balance has been struck while keeping room in the budget to modernize to address threats that are likely to emerge decades in the future.

He noted that the F-35 Lightning II remains the cornerstone of the future fighter fleet and characterized the larger, departmentwide strategy as developing tools, practices and capabilities that, taken together, are sufficient to offset capabilities from China.

“I’m all about real capability and getting real capability in the hands of warfighters as quickly as possible,” he said.

At the same time, he

July 1, 2022 acknowledged that the Air Force today is working to close a capability gap and that being forced to carry older, less capable equipment is slowing progress at a time when time is short.

That reality is the reason that the Air and Space Forces must transform and fast, he said, adding it is the reason he continues to reinforce a sense of urgency.

He endorsed the military-wide push toward a new generation of joint operation known as Joint All Domain Command and Control but with a caveat. The Air Force’s contribution to the larger effort, known as the Advanced Battle Management System, is showing promise, Kendall said. However, the “deliverable” must be “identifying the tangible benefits we need to get into the hands of warfighters to make an operational impact.”

In all respects, he said, “We need to move fast but in the right direction.” That is especially true in the decision and development

surrounding the so-called Next Generation Air Dominance package. Unlike previous upgrades that focused heavily on state-ofthe-art fighters, this new version would include crewed and uncrewed aircraft, space and cyber assets and information technology that amplifies capabilities and dilemmas for adversaries.

“Our system-of-systems approach to the future of air dominance goes beyond buying the next aircraft to complement our other fighters or bombers in our inventory. It’s about building the full family of innovative platforms and systems,” he said.

The efforts, as embodied in his Operational Imperatives, include advances in readiness and how the force is based. One major effort is Agile Combat Employment, which relies on a network of smaller, more nimble and self-sustaining bases that are connected to traditional, large installations in a hub-and-spoke arrangement.

U.S. partner countries are examining some of these platforms that can evacuate a patient rapidly without risking other personnel in potentially contested airspace, he added.

“Part of it is marking the vehicles appropriately with standard medical nomenclature. That gives you the Geneva Convention protection. But, obviously, we go up against certain enemies that will not care about that at all,” he explained.

Artificial Intelligence Stealth technology continues to improve. When it comes to drones, “it may be just a matter of keeping the drones low to the ground, and that they’re being piloted by an artificial intelligence system,” Meledeo explained.

“So, hopefully, AI will be faster to react than a human would be. But even still, I think there are a lot of concerns about” the use of drones to extract wounded warfighters.

“The long-term goal…is to actually have some sort of robotics onboard these drones that would be doing medical care to the patient during transport,” he said.

DARPA AI Initiative A new Defense Advanced Research Projects Agency initiative called “The In the Moment Program” aims to ultimately give AI systems the same complex, rapid decision-making capabilities as military medical staff and trauma surgeons who are in the field of battle based on algorithms of care and decisionmaking capabilities.

One example is smart tourniquets that will be able to detect whether they need to be released. Other automatically guided solutions include IV placement or catheter placement, Meledeo said.

“At least on a rudimentary level, the community is already pursuing a lot of automated solutions or artificial intelligence-derived solutions for automation of different medical processes.”

This research is underway but there’s no timeline on this concept.

“Hopefully, we’ll be able to get some confident results from some of these different technologies that are going to be packaged together in this system and enable the drones to then not only resupply at the point of injury, but also actually take care of the transportation and the management of patients during that transportation.”

Ask the Doc: heat stroke vs. heat exhaustion – what’s the difference?

Dear Doc: During a recent run in the hot weather, I started to feel dizzy and got a headache.

Could this be heat exhaustion or heat stroke? Can you please explain the difference between the two so that my buddies and I can learn ways to prevent heat-related injuries in the hot weather?

Thank you, -Sgt. Dizzy Run Dear Sgt. Run: Whew! That sounds dangerous. With those symptoms, you probably experienced some sort of heat illness.

Fortunately, I found the perfect expert to talk to about this.

Army Col. (Dr.) Anthony Noya is the chief of primary care at Bayne-Jones Army Community Hospital, in Fort Polk South, Louisiana. Here’s what he had to say: “Heat-related illnesses refer to a spectrum of conditions that worsen as the body loses its ability to compensate for the

increased core temperature due to environmental heat or metabolic heat.

There are warning signs to be aware of and measures to take to prevent the progression of heatrelated illnesses.

Simply put, as with any illness, the result is an inability to function, which can be mild, moderate, or severe.

Risk factors are numerous – generally speaking, the main risk factors are: people who take certain medications that lead to fluid or electrolyte alterations, people at the extremes of age, and insufficient cooling, hydration, and fitness. There are also genetically predisposing conditions that can increase someone’s risk of heat illness.

Heat exhaustion occurs earlier in the spectrum of heat-related illnesses. If left unaddressed, heat exhaustion can rapidly progress to the medical emergency we call heat stroke. It can be difficult to differentiate the two without accurate diagnostic measurements.

TRAVIS AIR FORCE BASE — President Joe Biden proclaimed June to be Lesbian, Gay, Bisexual, Transgender, Queer and Intersex Pride Month for the United States. The Department of Defense recognizes and celebrates the contributions service members of the LBGTQ+ community bring to the fight.

U.S. Air Force Senior Airman Natalie Proctor, 22nd Airlift Wing C-5M Super Galaxy loadmaster, and a South Carolina native, sat down to discuss her life experience announcing she is gay in the South, her religious upbringing and what having a diverse group of Airmen can do to strengthen and make the mission more efficient.

How do you contribute to the mission at Travis AFB, Air Mobility Command and the USAF? Proctor: As a loadmaster I help provide unrivaled strategic airlift, safely and professionally – which supports AMC’s core mission of rapid global mobility. Being a loadmaster, I am responsible for loading that cargo safely, and making sure it gets to its destination in a timely manner.

How long have you been in the Air Force and what do you enjoy most about your career?

Proctor: I have been in the Air Force a little over three years now and with being aircrew, travel is by far my favorite. I get the privilege to see and experience different cultures from all over the world.

Where are you from and what life experience do you bring to the USAF? Proctor: I am from Saluda, South Carolina, originally, but spent a lot of time in Clemson after graduating from there. I have had jobs with a lot of supervising experience, worked with many partnerships over

Growing up in the South, do you feel like your lifestyle and the culture you bring to the USAF is any different than anyone else?

Proctor: Growing up in South Carolina, I learned how to work hard, I grew up in church and learned family values. I was raised Pentecostal and, even though I was still figuring out my sexuality at a young age, I never steered away from church. After college, I joined a church who accepted me for my sexuality and loved me unconditionally.

To this day, they are still my biggest supporters and will always

be family. I believe we all have a story when it comes to how we were raised or our stories on religion and what we bring to the table. That’s what makes the USAF amazing is the differences and how we can bring all of our

differences to the table.

Can you share a story where your experience and lifestyle has helped move the USAF mission forward?

Proctor: I wouldn’t say I have a specific experience, I am

professional at work, on social media and to the world. I have never hidden who I am nor will I ever. Hiding never helps move equality forward. Being who I am at work or on the road opens myself up for questions from other service members genuinely wanting to know how they can be better allies and Airmen in this growing world. I have never been the one to turn someone away from asking questions even if they are difficult or sensitive. I try to stay vulnerable because being vulnerable is a way to grow.

Why is it important the DoD & USAF recognize and celebrate Pride?

Proctor: Recognizing Pride is a part of recognizing all service members. Simply put, it’s not up for debate. Each one of us are willing to sacrifice our lives to serve this country and moving the mission forward. That includes the LGBTQ+ community.

What challenges do the LGBTQ+ community face in the military and how can an ally help with those challenges?

Proctor: It’s hard to speak for everyone about every challenge because I’ve been blessed with an amazing squadron, or you could say they are family, I have at the 22nd. I’ve never been treated differently for my age, sex or sexual orientation. Nonetheless, that’s something we can take as a positive and use as the standard. I’ve heard stories from friends, other Airmen in the community, who are not being treated in the same aspect as me. They are looked at as “less than” or questioned because of different lifestyle choices. But obtaining the standard of equality across the board is a step.

We no longer live in a “don’t ask, don’t tell” military, but we can’t stop just there. We should continue the fight for diversity, equity and inclusion within our force.

Thankfully, the majority of heat related illness is heat exhaustion or heat cramps. However, if left unaddressed, heat exhaustion can rapidly progress to heat injuries like non-cerebral end organ dysfunction or heat stroke, with severe elevations of core body temperature and nervous system, brain, and other organ dysfunction.

To avoid heat-related illness, it’s important to remove the person from the heat, measure his or her core body temperature (rectal is the preferred method) and initiate rapid cooling. If a rectal thermometer is not available, one reasonable approach is to cool the patient until they begin to shiver.

Cooling them off through coldwater immersion or applying ice packs to their neck, underarms, and lower abdominal or groin area is highly effective if suitable equipment is available.

In circumstances when ice water immersion is not feasible, particularly if the patient has complications requiring aggressive medical intervention, such as a compromised airway or active seizures, trained professionals should use alternative methods of cooling. The goal is to lower the core temperature to about 101 or 102 degrees.

Calling 911 will mobilize trained professionals to assist casualties of heat illness. They will determine if transporting the patient to a hospital for more intensive intervention and management is necessary.
