How Laser Therapy Can Keep Your Skin Looking Young The field of laser skin therapy has been evolving over the past few decades. One of the key innovators in the field, Dr. Dieter Manstein, M.D., Ph.D., explains the benefits of laser treatments. Dr. Manstein, the president of the American Society for Laser Medicine and Surgery, is responsible for some of the greatest innovations in laser skin treatment, from cosmetic improvements to wound and scar healing. He co-invented fractional photothermolysis, which made laser therapy a leading treatment for aging or scarred skin. As far as lessening the effects of aging, Manstein said the trend now is to get ahead of the curve. “These days, due to the new treatment opportunities, some patients are much more proactive and delay the aging process with laser treatment,” he said. Further, with scarring treatment, “I strongly feel that burn scar patients should at least know about this option and specifically inquire their physicians about it.” Manstein hopes patients start to feel empowered to seek the best treatment options available. ncontinence, both fecal and urinary, is a major concern for patients and caregivers. Aside from the embarrassment and isolation often associated with soiling, the skin in (primarily) the but-

tock, inner thigh, and perineal areas is at risk for damage, which can be painful and lead to other problems. IAD is a form of irritant dermatitis or inflammation that develops from chronic exposure to urine or liquid stool. IAD is one of four types of moisture-associated skin damage that has been reported. The skin is a multilayered, self-renewing organ that deserves clinical attention to preserve its barrier protection and other important functions. It is perhaps an obvious strategy that care providers should consider methods

to eliminate or reduce episodes of incontinence by using behavioral and practical strategies such as timed voiding, use of toileting aids, and wearing of garments that are easier to remove for toileting, such as those with elastic waistbands. Meeting with a pro People experiencing incontinence, IAD, or who are having difficulty in tailoring a treatment or management care regimen to their individual situation may benefit from a consultation with a healthcare professional specializing in this area. Since those suffering with incontinence often minimize or fail to report their condition to their provider, delays in referral to those who might make a difference are common. Certified wound ostomy and continence nurses are valuable resources in the area of incontinence, IAD treatments, and the use of devices and bodyworn absorptive products. Patients and care providers alike should consider them as allies. n

A Positive Approach to Switching Negative Pressure Wound Treatment Systems SPONSORED

When a hospital system with nearly 3,000 nurses switched negative pressure wound treatment systems, ensuring a smooth transition started with having a plan. Negative pressure wound therapy (NPWT) applies a vacuum to wounds; this draws out fluid and increases blood flow, aiding the healing process. For a hospital, the choice of NPWT system is a serious one; Medicare expenditures related to wound care run into the billions, and NPWT expenditures account for a large portion of a hospital’s associated wound care

costs. The complexity of managing these systems often discourages any attempts to change, even when newer systems present clear advantages in terms of cost and patient experience. “It’s like, you don’t change a batter’s stance when he has a great average,” says Michelle ‘Mickey’ Monte MSN, RN, a clinical nurse educator at Buffalo General Medical Center, part of Kaleida Health. “Change is scary. When you multitask like nurses do, you don’t really have time to learn new stuff.” Choosing a new NPWT system Monte knows better than most of the challenges involved. She recently orchestrated a transition of the NPWT system at Kaleida, a system with over 1,400 beds. Monte

recalls the impetus to make the change began with a plastic surgeon, Dr. Danielle Dauria, who was exposed to different NPWT devices at her wound clinics. “She was using NPWT systems (or wound vacs) from different companies,” says Monte. “She suggested we pull in a couple alternative NPWT devices and have the nurses and doctors evaluate which one is easier to use.” When the dust settled, the clear winner was Medela Healthcare, a global manufacturer of healthcare products. “We weighed the pros and cons of each manufacturer, and Medela had more pros than cons,” Monte says. “It’s easier to use, has fewer notification noises, intelligently designed, and it’s lighter.” In general, the pump is

very quiet as it operates. This is important, as loud pumps disrupt sleep and act as stressors. Having a low-profile, quiet option is a key priority. Choosing a new system was just the first step, however. Now Monte, in her role as clinical nurse educator, had to oversee a transition involving nearly 3,000 nurses across multiple sites. Organized and methodical Monte decided to begin with the largest hospital in the system — Buffalo General Medical Center. Step one was to create awareness by posting short videos to their internal website, explaining the product and how it was used. “We assigned it to every nurse in the whole system, and we gave them one month to watch. Then they had to print out a certificate

and attest to watching it. This way I knew they were prepared to come to a hands-on class.” The hands-on training classes were assisted by Carol Davis, RN, BSN, CWOCN, a clinical sales specialist from Medela, who embedded herself in the process. “Carol’s amazing,” says Monte. “She listened, and she made herself available. We covered the weekends, the day shifts, and the night shifts. A class every hour — we trained 686 nurses in two weeks.” When one site began the hands-on classes, the next site would begin the web video training. This staggered approach kept everything organized while Monte ensured compliance. “I sent out daily reminders — send your staff!” 100 percent success Monte says the changeover was a complete success. “It went over well — a lot of people say they like the Medela NPWT device a lot more. Everything was positive.” She also notes significant savings for Kaleida Health as a result of the change. For anyone faced with leading a similar NPWT changeover, Monte has some simple advice. “First, make everyone aware of how important wound and skin care is. The more publicity you can manage, the better. Next, be open to different products — ease people in with a trial. And finally, be organized. Leading a holistic life can only be partially successful if you live in separation or duality. That is a life of opposites. These opposites could be moods, like feeling good one day and bad the next, or physical contrasts between feeling sick or well. Even the division of mind and body imposes a line of separation that doesn’t exist in nature. Duality, then, consists of mental constructs that humans have created.

Living in a busy, fast-paced world, how do you keep yourself centered and grounded on a daily basis? You do it by noticing whenever you feel distracted, overburdened, rushed, pressured, or stressed. That’s the first step. The second step is to act as soon as you notice any of these states — don’t postpone and put off doing something. Third comes the actual process of re-centering, which involves finding a quiet place where you can be alone and undisturbed, taking some deep breaths, and with eyes closed, placing your attention in the area of the heart until you feel centered again. What are some tips you can offer for someone to begin to incorporate more holistic practices into their everyday life? Nothing is more important than reuniting the artificial separations we’ve imposed through mental constructs that divide body from mind, self from other, inner from outer. There is only one reality, and therefore only one life. This life consists of consciousness changing into different modes of knower and known. n

Why Board Certification Matters When Seeking Someone to Care for Your Skin Do you turn to social media and digital influencers for skincare tips? Do you know if the training your provider received is enough qualification to perform the treatment you’re considering? Unfortunately, there is a widespread lack of knowledge when it comes to “who’s wearing the white coat.” A dermatologist is a physician who has completed 12 years of rigorous training, including 12,000 patient care hours, and is certified by the American Board of Dermatology. Unlike aestheticians, dermatologists are trained in the full anatomy and physiology of the face and body, to truly know how all the aesthetic treatments and procedures work together to create natural, beautiful, and high-quality results. Dermatologists perform scientific research to improve current treatments and invent new ones. They work with policymakers at the state and federal levels to make sure skin treatments are both effective and safe.

Fact checking According to a 2018 study by the American Society for Dermatologic Surgery (ASDS), nearly 70 percent of consumers are considering a cosmetic procedure, including wrinkle-relaxing injections, fillers, laser hair removal, body sculpting, and tumescent liposuction. If you’re in that group, remember that a cosmetic treatment is a medical procedure. There are real risks of complications and serious problems when procedures are done by practitioners who do not have appropriate training and expertise. It’s crucial that you verify your provider’s credentials to ensure they are qualified to advise you on your skin’s health, and perform dermatology-related procedures. You’re so outspoken about the importance of quality skincare. What advice do you have for someone struggling to find the right routine? My advice would be to not give up! I tried so many things before I found my perfect routine. What works for one person may not work for the next, so it’s truly trial and error. Your best bet is to start off with products that are mild and natural. Do your research first and see a dermatologist. How has prioritizing your skin care helped your self-image? I used to have horrible breakouts. I would pick on my face and sleep in makeup. My skin routine was trash. I used to be so insecure about my face that I wouldn’t leave the house. I was afraid that everyone who was looking at me was staring at my bumps and scars. It took me years to realize that my skin was a direct reflection of how I was caring for myself. My diet sucked, I wasn’t exercising, and I had a very negative outlook on life and myself. Once I realized this, I started to really practice self care and everything changed, including my skin. n

What is melanoma? Melanoma is the deadliest form of skin cancer, which starts when a particular cell in the body becomes damaged and begins to grow in an uncontrolled and dangerous way. This “rogue” cell multiplies quickly and invades surrounding tissues, and can spread to other parts of the body. Melanoma rates continue to rise because of a lack of daily preventative measures, most of which are pretty easy to do. Seek shade from 10 a.m. to 4 p.m. (the sun’s peak hours), wear sun-protective clothing, and don’t forget to apply (and reapply!) sunscreen every two hours. The good news is that, if caught at the earliest stage, melanoma is almost always curable. If you’ve been diagnosed, your treatment choices might include surgical removal, immunotherapy, targeted therapy, chemotherapy, and/or radiation. IMPACT Melanoma

Melanoma, the most serious form of skin cancer, is on the rise, having dramatically seen increasing diagnoses over recent decades. According to a recent study by the American Cancer Society, approximately 96,480 new melanomas will be diagnosed in 2019. The overall incidence of melanoma continues to rise, doubling since the 1980s. More than 5.4 million cases of non-melanoma skin cancer were treated in over 3.3 million people last year. How do we reverse the trend?

Dermatologists are using phototherapy to treat patients with psoriasis, vitiligo, and other photoresponsive diseases. Now patients have the additional option to treat themselves in their own homes.

hototherapy is a treatment that uses ultraviolet light to treat a variety of skin disorders, including vitiligo, psoriasis, and eczema. David Swanson, the CEO of Daavlin, a leading manufacturer of phototherapeutic products, has long championed the benefits of

phototherapy. For psoriasis, continued treatment can effectively clear the skin. “Recent studies show that 70 percent of patients cleared in an average of 58 days when treating three times a week with phototherapy,” Swanson said. Dermatologists most commonly prescribe Narrow Band UVB phototherapy, which treats the skin

with a small portion of the ultraviolet spectrum, enhancing the therapeutic nature of the light and reducing side effects. “Narrow Band UVB phototherapy is considered a first-line treatment for these conditions,” Swanson said. “It’s safe enough for pregnant women, children, and the immunocompromised to use.” A reasonable option Phototherapy may seem costly at first but it provides an affordable treatment option over time. “In-office phototherapy treatments or the onetime cost of a home unit are reasonably priced, and covered by Medicare and most insurance companies,” Swanson said. “Other treatments, such as biological drug therapies, can cost more than the price of the average car.” “The treatments are working,” said a phototherapy patient from Reno, Nevada. “I am only in Week 4 and the plaquing is less and my feet don’t look as red. “Usually heat really flares things up, and I was in Sacramento for three days this week and the temps were up to 106.
