Dear Readers,

As the new editor of Metro East Healthy Cells Magazine, I did not know what to write about in my first editorial. Then my fourteen-year-old son who is an aspiring pediatric surgeon asked me if he could make the introductions and explain why he wishes to take on the endeavor of many more years of school and this editorial. So let me introduce Xavier Ludwig:

eroes come in all shapes and sizes. When someone my age is asked about what a hero is: Spiderman, Ironman, or the Hulk usually come to mind. There are some that are more down to earth and think that policemen and firemen constitute a true hero, and they do. To me, a hero is someone that sacrifices just little parts of themselves to do good for others. My parents could be considered heroes, for they sacrifice so much to make my life better than the one they had. But when I think of the word “hero,” in its true definition, only one name comes to mind: Justen Barrow. In 2007, when my father and I first moved back to his hometown in Illinois from Tennessee I thought the whole world had turned upside down. I moved from a major city to a small town of only 400 people, most of which, I found out later, were related to me. But the one friend, the one person that made Irvington feel like home was Justen Barrow. He was a few years older, but went out of his way to greet me and to make sure that I knew everyone in school and to ensure everyone that the new kid was under his protection and guidance. Justen was different from anyone I had ever known, or ever will. He was born different. To look at him initially would bring sorrow or pity, but Justen would never allow that. He always had a smile on his face and a kind word for others. Justen brightened up any room that he walked into with his infectious laugh and go-get-em attitude. He made you thank God for placing someone like Justen in your life, and also question God for taking him away from us. At the too young age of 16, Justen passed on Jan. 6, 2012. His last year was spent in and out of the hospital fighting complications from a rare form of cancer and leukemia. But through all of that, he remained Justen: a die-hard sports fan, an incredible upbeat friend, and a hero to all that knew him. A group was formed in Irvington while Justen was battling his illness, Justen’s Army. There were fundraisers to help with his medical expenses, and classes to make people aware of the how they could not only help Justen, but the millions of kids around the world that suffered the same disease that eventually took Justen from us. One of the most important things that I learned is that any one of us have the possibility to be a hero like Justen. We have the capability of saving or enhancing dozens of lives by saying seven little words to our parents: “I want to be an organ donor.” I know this is something that kids our age should not have to think about. No one likes to think about what could happen, but just know that it could. And what better legacy to have, than to help save others like Justen. We are too young to make a decision like this ourselves, but we can inform our parents now that if anything does happen to us we would like the chance to help others. It is as simple as saying to them, “I want to be an organ donor.” With the holiday season upon us, we are taught that it is a time of giving. We can give the cards to our classmates, we can give a bracelet to our girlfriend, we can make breakfast for our mom, but we can also give the gift of life to at least eight other people by saying to our parents, “I want to be an organ donor.” I often question why God takes a true angel from our lives, like he did with Justen. But I also think that everything He does is for a purpose. Maybe, just maybe, it took God taking Justen from us so that I could write this column and become a Pediatrician. If just a small percentage of those who read this column say those seven easy words, and actually have a conversation with their parents, thousands of lives could be saved and Justen’s Army marches on. “I want to be an organ donor.” Below are websites that you and your parents can visit to learn more about becoming an organ donor: LifeGoesOn.com: Illinois’ organ donor registry. Do You Suffer from Postprandial Somnolence? By Patrick Miller

C

ases of postprandial somnolence in the United States can reach epidemic levels in late November. Perhaps you have seen the telltale symptoms in a loved one — a vacant expression, heavy eyelids, extreme armchair drowsiness, and a tendency to unfasten their trousers. Commonly known as “food coma,” postprandial somnolence traditionally strikes hardest after Thanksgiving dinner. Its effects can be as minor as drifting off during a football game, or as devastating as neglecting to wash, help wash, or even offer to help wash the postmeal dishes. Don’t feel guilty; it is not your fault. You have a medical condition. Although there is no vaccination or cure (other than a long nap on the couch), with a little knowledge and effort, it can be avoided. Many Americans have wrongly blamed this phenomenon on turkeys. The reasoning goes something like this: I get sleepy after Thanksgiving dinner, Thanksgiving dinner includes a generous amount of turkey, turkey meat contains tryptophan, tryptophan is converted into serotonin, serotonin is converted into melatonin, melatonin induces sleep; therefore, Page 10 — Healthy Cells Magazine — Metro East Area ­— November 2013

turkeys are the culprit of my food-induced lethargy. As logical as this explanation sounds, it is ultimately false because of one very important fact. Turkey is not exceptionally rich in tryptophan. It certainly contains a healthy amount of it, but not substantially more (and sometimes less) than beef, pork, chicken, tuna, soybeans, and many other protein-rich foods. Since this is the case, turkeys are innocent, and skimping on portions will not reduce the after-dinner fatigue. Even attempts at replacing turkey with ham or tofu are ineffective at keeping postprandial somnolence from rearing its slothful head. So what causes a food coma if not turkey? This is the truly confusing part because the ultimate answer is... tryptophan. It is true tryptophan is converted into serotonin and eventually melatonin by the human body, and it is the melatonin that induces the sleepiness. But, you may be thinking, how can tryptophan be causing postprandial somnolence if eating tryptophan doesn’t cause postprandial somnolence? That is a complicated question, which can only be answered by understanding how the body processes this amino acid.


Tryptophan is an amino acid. Amino acids perform myriad jobs throughout the body, but this article will focus on a single task — passing through the blood brain barrier. The blood brain barrier protects the brain from unwanted substances in the blood. Not just anything can get to the brain, but amino acids are in the long list of essential nutrients that can. They enter it as people enter a subway. They stand in line and pass through turnstiles (in this case, capillaries) one at a time. For the purposes of this analogy, only the essential amino acids will be used. The blood brain barrier and bio-chemical reactions in the brain are extraordinarily complex subjects that do not need to be understood to grasp how Thanksgiving dinner makes you sleepy. So, for this analogy only, please pretend there are only nine amino acids vying to cross the blood brain barrier. Generally, the different amino acids pass the barrier at the same rate as their blood levels — meaning, if you have a lot of tryptophan in your blood, your brain is going to get a lot of tryptophan. Again, this makes turkey look like the bad guy. Turkey might be a good source of tryptophan, but it is significantly richer in the other eight essential amino acids. For example, a four-ounce portion of turkey contains 217 mg of tryptophan and over 1,500 mg of leucine, another essential amino acid. So, despite eating a food high in tryptophan, the level of tryptophan in the blood compared to other amino acids is not increased, and, in turn, the amount entering the brain remains too little to trigger an episode of postprandial somnolence. Returning to the subway turnstile analogy, imagine the amino acids as different groups of people. So long as the ratio of people in each group remains the same, they will enter, one at a time, at the same rate

no matter how many total people there are. The only thing that can disrupt this regulated entry is if the ratio of people suddenly changes. Now pretend three of those groups of people are firefighters, police officers, and emergency medical technicians. Someone yells, “Fire!” Those three groups will immediately run off to do their jobs, making much more room for the remaining groups who know nothing about dealing with the emergency. Their rate of entry might double or triple with the removal of the other groups. This is essentially what happens to increase the level of tryptophan entering the brain. In this scenario, the firefighters, police officers, and EMTs are leucine, isoleucine, and valine — also known as the branched-chain amino acids. These guys do a lot of work with muscles, and one of their triggers (the one who yells, “Fire!”) to do this job is insulin. Insulin is released when a person eats carbohydrates, and the more carbs the more insulin. So, if someone eats a meal loaded with stuffing, mashed potatoes, and cranberry sauce, they will produce a lot of insulin. The insulin will alert the branched-chain amino acids to forget about getting into the brain and try to get into muscles instead. This greatly increases the proportion of tryptophan in the blood, thus increasing the rate it is taken through the blood brain barrier. The increased levels of tryptophan are converted into serotonin, then melatonin, and then... postprandial somnolence. In the end, it is (hopefully) clear to see that turkey is blameless, and carbohydrates are truly the guilty party in causing postprandial somnolence. This year, maybe consider enjoying the turkey and taking it easy on the carbohydrates if you want to remain sharp-minded and alert after Thanksgiving dinner. But then, on the other hand... are naps really all that bad?

inety percent of Americans at some point in their life will experience mechanical low back pain. In fact, back pain is endemic in society. Most people have an episode where it is somewhat debilitating, and they often end up either out of work or having to take medicine, physical therapy, etc. Due to the large number of people who develop back pain, this is a significant drain on the medical dollar. Many treatment options have been used in alleviating lower back pain such as therapy, anti-inflammatory medication, stretching, and time. Some options are a little more aggressive such as chiropractic care and even surgery. For most people, back pain is localized to the lower back area. It occasionally radiates down into the buttock. diate down the leg. If the pain does radiate down the leg at that time, the concern is that the patient not only has low back pain but may have a component of nerve root impingement as well. This makes the treatment options potentially much more difficult. Without nerve root impingement, the typical treatment for low back pain is a course of physical therapy, stretching, antiinflammatory medication, and time. Ninety-eight percent of the time the symptoms resolve, and the patient is returned to normal activity without the need for significant intervention. On rare occasions, the pain persists. For those people with continued pain or nerve root impingement, they may need more aggressive treatment. Treatments include epidural injections and nerve blocks and occasionally

Most people obviously would prefer to avoid this if at all possible. Typically the first step in low back pain treatment is to see your orthopedic surgeon and have an evaluation to make sure there are no neurologic or structural problems. Assuming there are none, typical treatment is instituted with anti-inflammatory medication and a short course of physical therapy. Generally a home exercise program is instituted. Therapy for longer than a couple of weeks probably is not necessary or helpful in these patients. If this treatment fails, then an MRI is typically obtained to rule out any structural or other problems. assage can be considered an art form. Schools teach the science of massage, but each individual therapist develops their own massage art. When thinking about music, one ponders that “a symphony must be like the world — it must contain everything,” said Gustav Mahler, composer and conductor. That sentiment applies to massage also; massage “must contain everything.” Each massage session is a new composition, tailored for the specific client. Where music uses volume, tone, rhythm, and instrumentation; massage uses pressure, quality of touch, strokes of different duration, rhythm, and instruments: thumbs, palms, fingers, handheld tools, or simply two hands. David Lauterstein wrote in The Deep Massage Book: ‘‘The human hand, acting in concert with the heart, mind, and spirit, is arguably the most sophisticated tool in the known physical universe. With its pressure and warmth, guided by intelligence, care, and inspiration, we can work with muscles and fascia, literally remodeling the human form and dramatically altering each and every human function.”

Dispelling the Myths of Hospice Submitted by Heartland Hospice

Myth: I thought that if my mother was under the care of hospice, she could no longer go to the hospital if she needed to. In the past year, she has been hospitalized several times because her pain got out of control. Truth: While hospice strives to manage pain and other uncomfortable symptoms outside of the hospital, a hospice patient always has the choice of whether or not go to the hospital. The Medicare Hospice Benefit covers short-term general inpatient care in the hospital when a patient’s symptoms can no longer be managed in another care setting.

Truth: Hospice provides care wherever the patient calls home, including, but not limited to, assisted living facilities, skilled nursing facilities, independent living facilities, and personal homes.

Myth: I always thought that “hospice” was a place, like that building that I passed on the way home from work. Truth: Hospice is a philosophy of care, not a place. Hospice care can be provided in any place the patient calls home, whether that is a personal home, a skilled nursing center, assisted living facility, or anywhere else.

Myth: I didn’t consider hospice care early enough because Dad was still getting blood transfusions regularly. We weren’t ready to stop because each time he would get one, it seemed to make him feel better for a few days and we just wanted him to feel like himself for as long as possible. Truth: The Medicare Hospice Benefit may cover chemotherapy, radiation, blood transfusions, or other treatments if those treatments are providing comfort for patients eligible to receive the benefit (life expectancy of 6 months or less if the illness runs its normal course).

Myth: I didn’t know that hospice could help take care of my dad in his assisted living apartment. He could have really benefited from the support from the hospice caregiving team. Page 18 — Healthy Cells Magazine — Metro East Area ­— November 2013

Myth: The doctor said I had to sign a Do Not Resuscitate (DNR) order for my wife in order for her to get hospice. I just couldn’t do that; it seemed so final. Truth: A patient can receive hospice without having signed a DNR. The hospice regulation actually says that hospices cannot discriminate against patients because of any advance directive choices.


Myth: I thought that hospice was only for patients with cancer. I didn’t realize that my husband who suffered from Alzheimer’s disease could have benefited from support from the hospice caregiving team too. Truth: More than 60 percent of hospice patients nationwide have diagnoses other than cancer. Many patients have other diagnoses or advanced stages of chronic diseases like pulmonary disease, Alzheimer’s disease, renal disease, HIV/AIDS, and cardiovascular or neuromuscular diseases. Myth: I was under the impression that I had to be available as the designated caregiver 24 hours a day in order for my mom to get hospice. Truth: While some hospices require caregivers prior to the patient being admitted, Heartland does not. We help coordinate community resources to keep the patient at home as long as possible and then, help the patient find an alternative location to receive care when care at home is no longer possible. Myth: M y dad lives at home. We weren’t considering hospice for Dad because we thought it was too expensive. Between the doctor’s visits and the cost of medications today, we have to save everything we can to make sure we can care for his ongoing needs. Truth: The Medicare and Medicaid Hospice Benefits cover services at 100 percent, so there is no cost to the patient. If patients have private insurance or managed care, Heartland assists in checking their benefit coverage to make sure patients and families understand any potential medical out-of-pocket costs.

Myth: I wanted to learn more about hospice, but thought I had to wait for the doctor to bring it up first so he could order it for my brother. Truth: Patients can choose for us to meet with them and discuss the benefit of hospice at any time. We cannot provide hands-on care to a patient without a physician’s order. We work with each patient’s physician to identify each patient’s individual needs. Myth: I was so scared to even say “hospice.” I thought that it meant I had to give up. Give up hope. Give up trying. Give up praying for a cure. Truth: In order to receive hospice, the patient must be eligible and have an understanding that hospice focuses on comfort care (palliative), not cure. But, patients and families do not have to be “ready to die” before getting the care they need and deserve. Hospice helps patients and families deal with what is happening to them on their own terms and in their own time frame. It is important to get hospice early so that the benefit can help patients prepare and get ready for what is happening; it is not required that patients be ready prior to receiving care. Claustrophobic with CPAP? By Dr. Anna Smith, Dentistry with TLC

leep apnea is a common condition and occurs when a person stops breathing during sleep. Most people are instructed by their physicians to use a CPAP to help them breathe. However, about half the people directed to use a CPAP are unable to tolerate the device. They stop using it within just one week. Many times these people feel hopeless. They are unaware of another more successful and comfortable option. In most cases, patients instructed to use CPAP therapy can instead use a comfortable and affordable oral appliance. This appliance looks like a retainer and is only worn while sleeping. Most patients who use the appliance report sleeping better, feeling more rested, snoring no longer, and getting up less frequently at night. Many lose weight, lower their blood pressure, and have their doctor take them off medications! Dawn Wakeford of Alton explains, “I felt restrained and smothered when I tried to use a CPAP. Now that I use an oral appliance, I can breathe normally while I sleep. I can even talk and answer the phone with it in my mouth. rying to cope with the loss of a loved one during the holiday season is not easy. The holiday season is a naturally stressful time of the year. Spending time with family, cooking, and gift giving can all be very rewarding. However, being surrounded by a lot of people, preparing food, shopping, and wrapping gifts require time and effort. The grieving process is a natural response to the loss of a significant relationship. During the grieving process and adjustment period following your loss, you can experience a wide range of emotional responses. At times, these emotional responses can be difficult to cope with and understand. In her 1969 book “On Death and Dying,” Elisabeth Kubler-Ross identified five stages of grief: denial, anger, bargaining, depression, and acceptance. There is no hard and fast solution to the grieving process. Listed below are some suggestions that may help to ease the stress and minimize the negative emotions people encounter during the holiday season. • Set boundaries on your expectations and other’s expectations. • Have a meeting or discussion with family and friends about holiday plans. • Rehearse how you will respond to difficult situations beforehand. • Observe the day in a way that has meaning for you. • Make the holiday an opportunity for healing, and a day for renewal of your physical, emotional, and spiritual needs. • Be flexible. • Re-create or change traditions. Start something new. • Bring special touches of your loved one to the holiday. • Focus on the concept of gifts and gift giving. Buy a gift for yourself as well as others. • What is the gift you were given by your loved one? • What did they teach you about life and living? • What positive qualities did they bring out in you? • What was their purpose for being in your life? • Have a plan. There are opportunities for grief support available through Hospice of Southern Illinois. innitus (“TIN-a-tus” or “Tin-EYE-tus”) refers to “ringing in the ears” when no other sound is present. Some individuals describe the tinnitus not as ringing but as hissing, roaring, pulsing, whooshing, chirping, whistling, or clicking. Tinnitus can occur in one or both ears and may be more noticeable in one ear versus the other. While not everyone experiences tinnitus, most individuals will experience brief periods of mild ringing at one time or another. For some, these sounds are constant and can be quite bothersome. Around one third of all adults have this more annoying type of tinnitus and about 10 to 15 percent of adults have prolonged tinnitus that may require medical evaluation. My patients frequently ask what causes the tinnitus to occur. While we have a good idea of where the problem originates anatomically, the exact cause of tinnitus is often unknown. There is one thing of which we are certain…Tinnitus is not imaginary. Just as fever or headache accompanies many different illnesses, tinnitus is a symptom common to many problems. If you have tinnitus, chances are the cause will remain a mystery. Possible causes of tinnitus might include: hearing loss, Ménière’s disease, loud noise exposure, migraines, head trauma, drugs or medications, anemia, hypertension, stress, cerumen build-up, certain types of tumors, caffeine, and smoking. Thus, tinnitus is a symptom of a problem. It is not a diagnosis. If you have tinnitus and have not been seen by a medical professional for this problem, you should have a medical examination with special attention given to conditions associated with tinnitus. You should also receive a comprehensive hearing evaluation by an audiologist to ascertain whether hearing loss might be the cause of your tinnitus. Hearing loss is the most commonly referenced cause of tinnitus. The hearing evaluation can help provide information about the cause and treatment options for you. There is presently no cure for tinnitus. The most effective treatment is to eliminate the underlying cause. As previously mentioned, tinnitus can be a symptom of a treatable medical condition. In some cases, the cause of tinnitus cannot be identified, or medical or surgical treatment is not an option. In these cases, the tinnitus can still be managed using a variety of other methods. Audiologists and otolaryngologists (ear, nose, and throat doctors or ENTs) routinely collaborate in identifying the cause of tinnitus and providing treatment and management. Some examples of tinnitus management options might include relaxation therapy, biofeedback, tinnitus maskers, sound machines, and habituation therapies. Consult your audiologist about what might be the most appropriate treatment for you. Remember, a treatment that is useful and successful for one person may not be appropriate for another. If you have a hearing loss, there is a good chance that a hearing aid will both relieve your tinnitus and help you to hear. An audiologist can assist with the selection, fitting, and purchase of the most appropriate hearing aids for you. Your audiologist will also help you learn how to get the best use out of your hearing aids.
