The talk essentially centered on the basic details pertaining to Insulin resistance, rudimentary concepts concerning Atherosclerosis and the basic mechanisms involved in the pathogenesis, viz., at the interface of insulin resistance and atherosclerosis. Understanding the molecular/biochemical defects associated with insulin resistance is beginning to signal the arrival of modern generation therapeutic targets for treatment of the metabolic syndrome , type 2 diabetes and associated cardiovascular abnormalities.
Dr. Vasudevan A. Raghavan spoke in detail about the Insulin resistance as a major factor in the pathogenesis of the metabolic syndrome and type 2 Diabetes mellitus, as also the vascular abnormalities with reference to atherosclerosis. India would soon emerge as the world leader in Insulin Resistance by 2030.Though the mechanisms responsible for insulin resistance remain to be exactly characterized, the defects largely point to insulin-stimulated glucose transport. A number of other metabolic abnormalities might also enhance fatty acid metabolites that are
associated with insulin resistance In cardiovascular tissues there are two pathways of insulin receptor signaling, one that is predominant in metabolic tissues and another being a growth factor-like pathway. These are implicated in atherosclerosis.

The chief guest then took charge of the show and brought forth the legal duties of a doctor and ethical issues, related to emergency services. He also cautioned us “any case can become medicolegal case” with no time limit to it and shared Do’s and Don’ts in handling any emergency cases from the time of receiving the case until discharge. The chief guest dealt the importance on consent, detail documentation and emphasized that still there exist the application “honesty is the best policy” in medical profession in regards to legal issues. He said emergency services constitute the most important service rendered by any hospital. The image of a hospital most often depends upon the image of the emergency –setup. Whereas therapeutic issues override the Medicolegal aspects, the latter can not be lost sight of, lest a peril overtakes! Injuries, poisoning cases, disasters, political/communal clashes, police firings, protests, anger against doctors, all focus attention in media and hence the image factor. Any case can turn into a Medicolegal case. However accidents, poisoning, sudden unexpected suspected illness leading to death, sexual assaults, homicide and suicide constitute routine Medicolegal cases. World Health Day is commemorated every year on 7 April, under the sponsorship of the World Health Organization (WHO). The World Health Day is held to mark WHO’s founding, and is seen as an opportunity by the organization to draw worldwide attention to a subject of major importance to global health each year. The theme of World Health Day 2011, marked on 7 April 2011, was “Antimicrobial resistance and its global spread” and focused on the need for governments and stakeholders to implement the policies and practices needed to prevent and counter the emergence of highly resistant microorganisms. When infections caused by resistant microorganisms fail to respond to standard treatments, including antibiotics and other antimicrobial medicines - also known as drug resistance - this may result in prolonged illness and greater risk of death. For World Health Day 2011, WHO called for intensified global commitment to safeguard antimicrobial medicines for future generations. Antimicrobial resistance is not a new problem but one that is becoming more dangerous; urgent and consolidated efforts are needed to avoid regressing to the pre-antibiotic era. Thus the Scientific
and Academic Forum of MGMC&RI , IGIDS AND KGNC in support with the Department of Community Medicine of MGMC&RI and the Department of Public Health Dentistry of IGIDS jointly organized a Health Awareness Programme on 7.04.11 at the MRD complex, MGMC&RI, to focus the need to the patients and the public about Antimicrobial resistance and its global spread. The programme was formally inaugurated by Prof. Muthurangan, Dean of Faculty, MGMC, Pro f.K.R.Thyagarajan, Medical Superintendent, Prof.Vijayalakshmi, Principal, IGIIDS and Prof. Kamalam, Principal, KGNC. The physical, social, emotional and intellectual needs are the motivating force behind human behavioral. She emphasized on physiological needs and non physiological needs. The physiological needs are water, air, essential nutrition, warmth, hygiene, elimination, rest, sleep, comfort. The non physiological needs and higher needs are, Safety and security need for the protection from environmental hazards, Need for love and affection, closeness, intimacy with family and friends for emotional maturity, Need for creativity and self esteem to become independent, Need for creativity and self actualization for ultimate use of individual abilities and Cognitive needs for knowledge, comprehension, understanding of new ideas and learning. The major aspects of child care should be emphasized through promotion of health, prevention of illness, maintenance of health and restoration of health.

After a brief audience interaction ,Dr. Kannan, Secretary of SAF proposed the vote of thanks. Dr. S.Uma Devi spoke on the topic “Antimicrobial Resistance & its Global Spread”. Antimicrobial Resistance (AMR) is a global issue
for the last few decades. AMR is resistance of a microorganism to an antimicrobial medicine to which it was previously sensitive. A high percentage of hospital-acquired infections are caused by methicillin-resistant Staphylococcus aureus (MRSA) and Vancomycin-Resistant Enterococci (VRE) globally. New resistance mechanisms, such as the beta-lactamase NDM-1, have emerged among several gram negative bacilli. About 440 000 new cases of multidrug-resistant tuberculosis (MDR-TB) emerge annually, causing at least 150 000 deaths. Most common facilitating factors of AMR are Inappropriate and irrational use of medicines. To overcome this problem good quality, regular supply and rational use of antimicrobials are essential. The first rule of antibiotics is try not to use them, and the second rule is try not to use too many of them. MGMCRI u SSSMCRI u IGIDS u KGNC

Dr.Mohamed Hanifah Assoc. Prof., Dept. of Internal Medicine, introduced the concept of “Rational Therapeutics”. He highlighted the fact that more people die due to medical errors, in particular prescription errors, than cancer. He emphasized that medical errors can be avoided if irrational use of drugs are avoided. He put forth the argument that irrational prescription is the net result of lack of information, faulty and inadequate training of medical graduates, lack of diagnostic facilities and poor communication between health givers. He abhored the practice of treating trivial infections or viral Infections with antibiotics. He pointed out that many use antibiotics without knowing the basic principles of antibiotic therapy and many medical practitioners are under pressure for short term solutions. The issues are further muddled by commercial interests of Pharmaceutical industry pushing the antibiotics. He warned that Frequent, irrational, and indiscriminate use increases antibiotic resistance and the real danger of health care associated infections spilling to society and increase of community associated infections. Unnecessary consumption of antibiotics kills the Normal flora, and reduces the immunity and makes them potential victims in future
of Antibiotics in Hospital Setup”. Antibiotics are essential treatments for serious infection. They are one of the most important and valuable discoveries of modern medicine. Administration of antibiotics can lead to the selection of Antibiotic-resistant organisms. These organisms can give rise to healthcare associated infections which are associated with increased morbidity and mortality. Responsibility of the person administering antibiotic drug includes appropriate knowledge and understanding of the medicine to be administered, appropriate training, must be competent, must have skills to complete aseptic insertion of venous access device, should have skills and confidence to manage any potential anaphylactic reaction which may occur and Must be satisfied with the prescription, ensuring it is signed, dated, clear and unambiguous. The main purpose of the above guidelines and policies are to ensure proper use of antibiotics and in particular to facilitate the introduction by health care organization of the most effective antibiotic strategies which will prevent the emergence of bacterial resistance.

The Indian Council of Medical Research (ICMR), New Delhi started the Short Term Studentship (STS) Program in the year 1979 with a view to foster scientific interest and aptitude for research among the medical undergraduates of our country. The primary objective of this program is to provide an appropriate platform for undergraduate students that would help enable to familiarize themselves with research methodology and techniques. Under this endeavour, the students selected by ICMR would associate themselves with their respective Research Supervisors / Guides for a short duration (two months) with ongoing research program or by undertaking independent projects. This would instill in them an incentive to take up research as a career in future. Our Institution has been providing the discerning students with all facilities for carrying out research, ever since its inception, and more particularly in recent times, quite a number of our undergraduate students have received the prestigious STS grant.

MEDICAL SIMULATION CENTRE
Sri Balaji Educational & Charitable Public Trust
THE MEDICAL SIMULATION CENTRE “I hear and I forgot, I see and I remember, I do and I understand” this was the idea behind the inception of The Medical Simulation Center (MSC). Since its inauguration on January 2011, the MSC lab has been providing hands-on simulation for both the medical and nursing students to develop preparedness and insight that is translated into improved real-world communication, collaboration, teamwork and crisis management. The simulation programs covers a range of healthcare areas, including anesthesia, emergency medicine, intensive care, labor and delivery, surgery, transport teams, nursing and crisis management response in many domains. The Centre also functions as an assessment tool for the Instructors to find out how successfully a medical student can practically apply the knowledge they had gain from their syllabus. Time table has been set on consultation with the Heads of various departments to include simulation program into the curriculum of medical and nursing students. Simulation program of varying difficulty and complexity have been assigned to the medical and nursing students based on their curriculum.
Simulation trainings by various departments Pediatric department have been training students in neonatal resuscitation, basic life support for infant and baby, umbilical vein catheterization and foreign body aspiration. The department of Anesthesia imparts training in BLS, ACLS using, SimMan scenarios, IV line insertion and pnemothorax training to the students.
Skills and Assessment to develop both individual and team skills. The simulator allows observation and recognition of most vital signs which are used in emergency medicine either directly on the manikin itself or on the included simulated Patient Monitor. The monitor may be configured to display the desired parameters and curves and it is possible to save and retrieve configurations. Colors and curve trace speeds can also be changed. Some of the trainings that have taken place in the SimMan simulator include: • Cardiac arrest team training. • Anesthesia crisis management. • Common emergencies in adult. • Ventricular fibrillation. Earlier there existed some kind of disconnection between the classroom and the clinical teaching and learning process. Students were inadequately trained in history taking, physical examination, diagnosis, and management. But now Medical Simulation has been proposed as a technique to bridge this educational gap. Belonging to the medical fraternity, we have a tremendous responsibility to our students, patients, and society as a whole. We must help our students to develop the ability to recognize their own limitations and knowledge gaps and provide them with the tools to fill these voids. Simulation, in all its incarnations, is a tremendous tool for healthcare educators, in that it allows students to achieve these goals without patient being put in risk. Indeed, it is very likely that the future will bring the widespread use of medical simulation center in all aspects of our hospital set up.
OBGY department provides practice session to students on female pelvic examination, birthing simulation and episiotomy repair training Ophthalmology & ENT department performs training in eye examination and ear examination. Surgery department is involved in catheterization training, rectal examination and NG tube insertion training to the students. Nursing students have been practicing central line infusion, injection training and catheterization training. SimMan is a full body, adult manikin that allows the simulation of Basic and Advanced Life Support

Maxillofacial defects may be a result of congenital malformations, trauma or surgical resection of tumors. The primary objective of rehabilitating these defects is to eliminate the disease and to improve the quality of life for these individuals. Among various maxillofacial defects, intra oral defects in the form of clefts and opening into the palate are very common. Several methods have been advocated for reconstructing these defects. The use of obturator prosthesis is one of them. According to the glossary of prosthodontic terms obturator is defined as a prosthesis used to close a congenital or an acquired tissue opening, primarily of hard palate and or contiguous alveolar structures. The name obturator is derived from the Latin verb “obturare” which means close or to shut off. Effective obturation of maxillary defects produces sufficient separation of the oral and nasal cavity to improve the quality and intelligibility of speech. It also enhances masticatory function, deglutition and esthetics. The weight of maxillary obturator prosthesis is often a factor to be considered with respect to retention and comfort of the patient. Hence it is desirable to design light weight prosthesis. Prosthetic intervention should occur at the time of surgical resection and will be necessary for the remainder of the patient’s life.

resection of squamous cell carcinoma. The chief complaint of the patient was inability to speak and presence of nasal regurgitation of food and liquids which made his life miserable. After getting consent from Department of Oral & maxillofacial surgery regarding post operative wound healing, the prosthetic management was commenced with the consent of the patient.
base was constructed in self cure acrylic resin material and a wax bite block was constructed. It was tried in the patient’s oral cavity and the wax block was recontoured to confirm to the patient’s esthetics and occlusion with the opposing mandibular arch dentition. Later the bite block was used for articulation of the maxillary and mandibular casts in the same intra oral relationship in the articulator. Artificial
Making impression of the defective area to get a positive replica for construction of the prosthesis was the initial step. The undercuts in the defective area were blocked out with moist sterile gauze prior making impression. This helps to prevent the impression material getting stuck in deeper areas which may lead to infections in future. Floss thread was attached to the gauze and extended out of the oral cavity to prevent accidental intrusion of the packed gauze into nasal /maxillary sinus cavity during intra oral impression making. The impression was made with elastomeric impression material and the patient’s tissues were molded functionally in the impression to prevent interference of muscular movements. At the same time the patient was asked to give head rotation to allow the impression material to flow into the defect completely.
acrylic teeth were arranged to match the occlusion and the labial contour and again tried in the patient. Final adjustments were made and the trial denture base was processed with heat cure acrylic resin to obtain the final obturator. The processing method included separate steps to make the meatal portion of the obturator as a hollow bulb in order to make the prosthesis light in weight for patient’s comfort as well as to improve the retention against gravity.
A cast was obtained from the impression and was duplicated for laboratory use. A trial denture
The retention of the prosthesis was obtained from the anatomic undercut areas that were found post operatively and also through stainless steel wrought wire clasps that were adapted on the dentition present on the other side of the maxillary arch. The prosthesis rehabilitated the patient in terms of function by providing better masticatory efficiency, phonetics by adding resonance to the voice hence improving the clarity of speech and also improved the esthetics of the patient. The use of a hollow bulb design improved the comfort of the patient by decreasing the weight of the prosthesis. It has greatly benefitted the morale of the patient.
