CME ON BONE TUMORS The Pondicherry Orthopaedic Association (POA) conducted its First Annual Conference on “Bone Tumors” on 19th November 2011 at Mahatma Gandhi Medical College and Research Institute. The Conference and the associated CME program were inaugurated by His Excellency Dr.IQBAL SINGH, Lieutenant Governor of Pondicherry. Prof.K.R.Thyagarajan, the President of Pondicherry Orthopaedic Association and Organizing Chairman gave the welcome address. The Vice Chancellor of SBV University Prof.D.R.Gunasekaran felicitated the gathering, Prof.G,Muthurangan,Dean,MGMC&RI presided over the inaugural ceremony and Prof.V.Veerappan proposed the Vote of thanks. The CME program on “Bone Tumors”- POACON 2011 included eminent speakers like Prof.Mayilvahanan Natarajan, Prof Rajagopalan, Prof Rajamani, Prof Vijayaragavan & Prof S.J.Bose. The Presidential Oration was delivered by Prof J.James Gnanadoss. The President of Orthopaedic Association of South Indian States Dr.Jayaram Pingle attended the inaugural function. Prof. S. N. Mothilal, Head of Department of Orthopaedics, MGMC&RI gave a lecture on “Giant cell Tumour”. The President of POA – Prof. K.R. Thyagarajan along with Dr. Ravichandran, POA – Secretary, Dr. V.Veerappan, Organizing Secretary, and Dr. Krishnagopal (Organizing

Secretary) presided over the program in a smooth manner. The conference was well attended and included poster and paper presentation, following which the general body meeting was held. Dr.Pandian was elected as President, Prof Dr. Rajagopalan as Vice President and Dr.Veerappan as Secretary. The one day conference was attended by all the faculties of various departments and a total of 200 delegates from South India participated in the events, making it a grand success.Secretary) presided over the program in a smooth manner.

GUEST TALKS - 2011

The Scientific & Academic Forum (SAF), the scientific wing of MGMC & RI, was established in the year 2004 by Prof. M. Danabalan, then Head, Dept. of PSM. This Forum conducts regular meetings every month where the Members of the Faculty and Residents present Rare Case Series, Case Reports and Innovations and at the end of the academic year the forum after external and internal evaluation, gives the best paper awards to each of the category. The Forum also invites important personalities in the field of Medicine for guest talks. SAF also commemorates the important days by organizing patient awareness programs. Academic Activities SAF Guest lecture on “Minimal Invasive Surgery (MIS) in children”

urologic and chest surgery. The advantages of MIS include less pain, early recovery, smaller scars, decrease hospitalization & early return to school. A lower incidence of postoperative adhesions and postoperative small bowel intussusceptions is another advantage following laparoscopy. He discussed in detail about the evolution of MIS at his Department of Pediatric Surgery at Apollo Children’s Hospital Chennai, where they began with basic laparoscopic procedures such as diagnostic laparoscopy and appendicectomy and now have progressed to advanced procedures in Laparoscopy and VATS. He added that they have done about 200 procedures and it would include the age group from neonatal age to 16 yrs. The commonly done procedures are: Appendicectomy, Cholecystectomy (Removal of Gall Bladder), Surgery for Impalpable Undescended Testes, Ovarian Pathology, Diagnostic Laparoscopy, Mesenteric Node Biopsy, Intussusception Reduction when failed by Saline Reduction under US guidance, Meckels Diverticulectomy and Varicocoele ligation

The speaker was Dr. Raj Kishore Bagdi, Senior Consultant Paediatric Surgery of Apollo Hospitals, Chennai. He presented a guest lecture on “Minimal Invasive Surgery (MIS) in children” The following are the gist of his excellent hour long presentation which was greatly appreciated by one and all. A wider vision through the tunnel: A roadmap to interventional radiological procedures in clinical perspective. Interventional radiology is a fast growing clinical genre, plays an important role in patient management. The basic concept of interventional radiology is to diagnose and treat patients using least invasive techniques available in order to minimize the risk to the patient and improve health outcomes. The procedures like FNAC and biopsies guides the clinician in the diagnosis and other treatment procedures are done in a palliative or curative intent. Interventional radiological procedures can be broadly classified into non-vascular and vascular. The image guidance used for the procedures can be fluoroscopy, sonography, computed tomography or in combination.

He finally concluded saying that MIS is possible because of a TEAM approach, backed by excellent anesthetic services and well trained OT staff. The results of MIS are thus very gratifying as the parents as well as the children go home extremely happy.

The speaker was Dr. Radhakrishnan Ravikumar MBBS, DMRD, DMRE, PhD and Diplomate in Cardiac CT (CBCCT, USA), Advanced Cardiovascular imaging fellowship (Univ Of Toronto, Canada), Director, Thambiran Heart and Vascular Institute, Chennai, Senior Interventional Radiologist, Apollo Hospitals and Global Hospital , Chennai . He

Non-vascular procedures may be either diagnostic or therapeutic procedures. head and neck, lungs, chest wall, abdominal organs and bones, are performed as day care procedures and needs a short duration (2 to 4 hrs) of post procedural observation. MR arthrograms of shoulder / knee joint are performed after injecting contrast in the joint under fluoroscopy guidance.Pain management is another major field which has its significance in oncological patients with severe pain. Block can be given in celiac plexus, lumbar plexus, and sacral plexus or anywhere in the desired site.Other non vascular interventions include – biliary drainages (internal or external), biliary stenting, percutaneous nephrostomy and ureteric stenting, vertebroplasty (injecting bone cement to stabilize collapsed vertebra), drainage of liver abscess and other abscesses/ post operative collections.

Vascular interventions: Hepatobiliary interventions include hepatic venous pressure gradient measurement, embolisation in liver lacerations (in trauma), portal vein embolisation, transarterial chemo/ radio embolisation, radiofrequency ablation of liver tumors, and IVC angioplasty. Interventional procedures play a major role in GI bleeding (hemetemesis, melena or hematochezia) and mesenteric ischemia. Peripheral arterial disease with chronic total occlusions can be managed by angioplasty with or without stenting. Uterine artery embolisation for post partum hemorrhage and fibroids control bleeding and preserve the uterus in women in reproductive age group. Preoperative embolisation in hypervascular tumors is a very useful technique to minimize major blood loss during surgeries for tumors like nasopharyngeal angiofibroma, bone tumors (GCT, metastatic lesions from RCC), angiomyolipomas, hemangiomas & vascular malformations. Recently varicose veins are being increasingly treated by endovenous laser ablation technique instead of surgery which gives wonderful results.

Every year, August first week is being celebrated as “Breast feeding week” (BFW) throughout the world. The same was observed this year in our college by the Department of Pediatrics in a grand manner. Puppet show, Role play, Essay competition for Undergraduates, Nursing students and Postgraduate students were

intelligent. Yes it is possible. A study published 7 years ago, in “Pediatrics”, the official journal of the American Academy of Pediatrics, which compared the cognitive development among babies who were on formula milk and breast milk, concluded that “breastfed infants had significantly higher Mental Developmental Index (MDI) and Psychomotor Developmental Index (PDI)”. So the kids who were fed on Breast milk were found to be more intelligent and psychologically more mature than their fellow counterparts who were fed on animal/ formula milk.

An Overview of the Talk

Over the last ten years, various organizations and world bodies have taken up a major issue being faced by the World`s children and that is deprivation of their own mother`s milk. Breast feeding in the first 6 months of life is considered a survival advantage in many underdeveloped countries at no additional cost. Breast milk (BM) has many inherent advantages which make it a perfect baby food. It is qualitatively and quantitatively superior to other milk sources by virtue of possessing all major nutrients (carbohydrate, protein and fat), vitamins and minerals at the appropriate concentration in a naturally blended composition. As it contains many anti-infective properties, infections are less in any breast-fed infant. Establishment of bonding between the mother and emotional maturity in child are possible only with breast feeding. Recent research shows that BM is advantageous to both the children and mother in many other ways too.

Breast milk has anti infective and anti inflammatory and anti allergic properties that help us fight infection, inflammation and allergic diseases. Anastasia Rigas analyzed the medical records of children with Crohn’s disease and Ulcerative colitis (two diseases that affect the gut causing poor absorption and digestion, bleeding and poor weight gain) attending the Pediatric Gastroenterology and Nutrition clinic of Mount Sinai Medical Center, New York, and found that Breast feeding protects significantly against Crohn’s and Ulcerative Colitis. Also, he has observed the duration dependent trends, i.e. , more the duration of Breast feeding, more was the protection. Also, “Children who had not been breastfed were 1.5 times more likely to develop asthma” is the observation made in a study which included 5,182 children with no family history of asthma (J Asthma 2003;37:575-58). When big sized protein molecules breach the intestinal wall and enter our blood, they sensitize our immune system and on re exposure to the same molecule later, they develop allergy. BM has many growth promoters like Somatomedin C which help in the maturation of the infant’s intestines (gut), preventing the absorption of the macro-sized protein molecules thereby avoiding the development of asthma.

BM and Life style diseases: (Childhood Hypertension, Obesity and Type II Diabetes Mellitus)

So, it is understood that BM has many advantages for the baby as well as for the mother. Some are short term benefits; some are long term benefits. Some are understood; some are not yet understood. Some are evident; some are not yet evident. The Mother feeding her baby is a natural instinct and unfortunately, we humans need so much education, effort and enthusiasm to adopt it when several lower mammals do it routinely and intuitively. The theme for this year’s breastfeeding week is Talk to me – Breastfeeding – A 3D experience. Apart from time and place, Communication is the third dimension of breastfeeding. Communication is an essential part of protecting, promoting and supporting breastfeeding.

Apart from having anti-infective and antiinflammatory properties, few studies have shown that BM can possibly prevent cancer. In a meta-analysis (a re-analysis of several studies published on the same subject) “Review of Case-Control Studies Related to Breastfeeding and Reduced Risk of Childhood Leukemia” Jeanne-Marie Guise et al., opined that “50% of the studies claim that BM protects against Leukemia (a form of blood cancer) and that more studies are warranted about this potential preventive measure”. A collaborative reanalysis of individual data from 47 epidemiological studies in 30 countries, including 50302 women with breast cancer and 96973 women without the disease (Lancet 360.187-195, 2002), claims that the incidence of both ovarian and breast cancers are less in mothers who have breastfed.

India is slowly emerging as the new capital of life style diseases. Many theories have been proposed which analyze why we are at increased risk for such non communicable diseases. Majority of individuals adapt the life style of western countries when their body systems are not yet ready for the transition. Breast feeding helps in lessening the risk of acquiring life style diseases. A study published in Archives diseases of childhood in the year 2005, says “BM may lower older children’s blood pressure”. This retrospective study, which analyzed 2192 children randomly selected between the ages 9 years to 15 years, observed a marginal increase in blood pressure among nonbreast fed children. Owen et al, observed in their study “Effect of infant feeding on the risk of obesity across the life course: a quantitative review of published evidence” ( Pediatrics 115:1367-1377, 2005), that the chance of becoming overweight during childhood and adolescence is at least 20-50% less if an infant is breastfed, even after accounting for maternal obesity and family life style behaviors. The study “Breastfeeding and incidence of noninsulin-dependent diabetes mellitus in Pima Indians” published in “Lancet” in the year 1997, concluded that people who were exclusively breastfed had significantly lower rates of type 2 DM than those exclusively bottle-fed in all age groups. Erica P. Gunderson in his multicenter, population based 20 year prospective observational study, conducted in USA, (DIABETES, VOL. 59, FEBRUARY 2010) found that lactating women lose about 2 kg of fat if they breast feed for 6 months; they MGMCRI u SSSMCRI u IGIDS u KGNC

It was a day to remember, a day when age doesn’t matter, when life is just about having fun, because no matter who you are, there lives a child in all of us. November 14th, 2011 was that day. The Children day’s program was held by the Department of Pediatrics in the Pediatric ward at 10 AM. The ward was covered with colorful posters and decorations. Thanks to the Post graduates and the Interns.

Blood Donation Day

The celebration was officially inaugurated by the beloved HOD of Pediatrics, Dr. Soundarajan which was followed by a small talk by the Senior Resident of Dept. of Paediatrics, Dr. Devi on the importance of Children’s Day.The highlight of the event was the collaboration of the Dept of Pediatrics with the Dept of Pedodontics of IGIDS who arranged a Dental health awareness Program and Check-ups for all children and stressed on its importance in the early years of a child’s life. As the day progressed, plenty of games were conducted for the Children and their parents.

On the event of Voluntary Blood Donation Day on October 1st 2011, SAF members, Students Council and Department of Pathology conducted a program for public to motivate them towards voluntary blood donation at the MRD complex. The program was chaired by Medical superintendant, Dr. Varadharajan, President of SAF Prof. Tirou Aroul, Dr. Dhananjay S.Kotasthane, Prof and HOD, Dept. of Pathology and team members of Scientific and Academic Forum graced the occasion. Faculties, PG students of Pathology as well as other departments witnessed the function. “Importance and need for voluntary blood donation” was delivered by Dr. G. Kanimozhi, post graduate student, Department of Pathology. Awareness about eligibility criteria for blood donation, need for voluntary blood donors, need for replacement of blood, risks caused by getting blood from professional donors, pre transfusion tests were highlighted to public. Following which, students and public donated blood in our blood bank making the program a successful one. Dr.G.Koteeswaran, Associate Professor was deputed to participate in the digital literacy programme for the farmers in the village resource centre at M.S.S.Swaminathan Research Foundation at Puducherry on 31.10.2011. It was a question and answer session over the phone, in which farmers asked many questions on the topic “BLOOD” covering the aspects of anaemia, types of blood grouping , Rh negative factor , blood donation etc. Dr.G.Koteeswaran shared his expertise knowledge in answering the farmer’s questions in simple terms understandable to them. Some of the farmers were willing to donate blood and wanted a routine medical check up with a visit to MGMC&RI.

World Diabetes Day

The Scientific and Academic Forum, the Scientific Society of Mahatma Gandhi Medical College, along with the Department of Medicine and Surgery of MGMC & RI, Puducherry commemorated the World Diabetes Day on 14th November 2011 at the MRD premises by organizing a patient awareness program. Faculties from the Department of Medicine and Surgery spoke to the gathered audience. Dr. Kalirattiname, Asst. Prof, Dept. of Medicine spoke in exhaustive way about the definition of diabetes and the necessity for the early treatment of diabetes. Diabetes can decrease the body’s ability to fight infection. When diabetes is not well controlled, damage to the organs and impairment of the immune system is likely. He explained in detail about the symptoms of diabetes like fatigue, unexplained weight MGMCRI u SSSMCRI u IGIDS u KGNC

loss, excessive thirst (polydipsia), excessive urination (polyuria), excessive eating (polyphagia), poor wound healing, infections, altered mental status and blurry vision. He discussed in detail about the prevention of diabetes. Diabetes, can be prevented by controlling weight to normal or near-normal levels by eating a healthy low-fat, high-fiber diet. Regular exercise is crucial to the prevention of type 2 diabetes. Alcohol consumption should be kept low and smoking should be quitted. Lifestyle modification is very important in prevention. If a person has diabetes, focus should be on preventing the complications, which can cause serious disabilities such as blindness, kidney failure requiring dialysis, amputation, or even death. Tight glucose control: The single best thing the patient can do is to keep their blood sugar level within the suggested range every day. The only way to do this is through a combination of regular blood sugar checks, a balanced diet low in simple sugars and fat and high in complex carbohydrates and fiber, and appropriate medical treatment. The audience was asked to increase physical activity levels. Aim for moderately vigorous physical activity for at least 30 minutes every day. To drink an adequate amount of water and avoid taking too much salt. The skin should be taken care of; keep it supple and hydrated to avoid sores and cracks that can become severely infected. Next to talk was Dr. Ganesh, Post Graduate from the Department of Surgery who spoke in detail about Diabetic Foot Care. He said there are several risk factors that increase a diabetic person’s chances of developing infections in the legs and feet. Footwear: Poorly fitting shoes are a common cause of diabetic foot problems. Nerve damage: People with longstanding or poorly controlled diabetes are at risk for having damage to the nerves in their feet. The medical term for this is peripheral neuropathy. Because of the nerve damage, the patient may be unable to feel their feet normally. Also, they may be unable to sense the position of their feet and toes while walking and balancing. Poor circulation: Especially when poorly controlled, diabetes can lead to accelerated hardening of the arteries or atherosclerosis. When blood flow to injured tissues is poor, healing does not occur properly. Trauma to the foot: Any trauma to the foot can increase the risk for a more serious problem to develop. About Infections Dr. Ganesh said Athlete’s foot, a fungal infection of the skin or toenails, can lead to more serious bacterial infections and should be treated promptly. Ingrown toenails should be handled right away by a foot specialist. Toenail fungus should also be treated. Smoking any form of tobacco causes damage to the small blood vessels in the feet and legs. People with diabetes should learn how to examine

their own feet and how to recognize the early signs and symptoms of diabetic foot problems. He also suggested that the patients with diabetic foot should be cleaning their feet regularly in warm water and toe nails should be kept clean. The speakers emphasized more on treatment and prevention of the disease and prevention of diabetic foot .The awareness program was inaugurated by the Dean Prof. Muthurangan and by the Medical Superintendent Prof. M. Varadarajan.

Aids Awareness Week The Dept of DVL organized the HIV/AIDS awareness week from 25th Nov to 1st Dec to educate the public and the health care personnel. This is in brief on the week’s events organized by the Department of DVL for the seventh consecutive year. On 25th November, the programme was inaugurated at the MRD premises by Prof. K.R. Thiyagrajan, Senior Medical Officer, MGMC & RI, & felicitated by the Dean Prof. Muthurangan, Medical Superintendent and Prof. M. Varadharajan, Prof.

Neenu, PG, DVL. On 30th November , there was an interactive session with the public on HIV / AIDS, which was carried out by Dr. Kandasamy, Asst. Prof, department of DVL, followed by a talk on how to suspect HIV / AIDS from benign skin lesions, by Dr. Asritha, PG from department of DVL. On 1st December - WORLD AIDS DAY, the valedictory function of the week’s programme was conducted. The key event was a panel discussion. The panelists were Prof. N. Ethirajan Prof. & HOD. Department of Community Medicine, Rajah Muthaiah Medical College, Dr. Gilbert Fernandez, HOD of DVL, Indra Gandhi Government College & Post Graduate Institute & Mrs. Rajeswari, President of Cuddalore, PNP (Positive Network of People). Dr. Shivasakthy, Senior Lecturer in Prosthodontics, IGIDS, and Secretary of RRC was the moderator. It was a lively programme with many of the myths & Realities of HIV / AIDS being touched.

Kamalam, Principal, KGNC, Prof. Renuga from KGNC also attended the program. The programme was graced by many senior professors of MGMC&RI. On 26th November, there was a capsule talk on cutaneous manifestations of HIV / AIDS by Dr. Neenu Sadanandan, PG, department of DVL. This was followed by a skit enacted by the members of RRC at the MRD premises. On 28th November, the Dept organized an intercollegiate extempore competition. It was an enjoyable event. On 29th November – the members of SFDRT an organization working for the welfare of women in prostitution sang a beautiful song to start with & later an interactive session with the public on HIV / AIDS at the MRD premises. On the same day, there was an awareness programme for the public in UHC, Ariyankuppam in the afternoon. The members of Red Ribbon Club (RRC) performed a skit, followed by a talk on HIV / AIDS by Dr.

DEPARTMENT OF PEDIATRICS The Pediatric Department of Mahatma Gandhi Medical College and Research Institute is one of the busiest, robust and efficient departments catering to the medical needs of several thousand neonates, infants and children in and around Puducherry with highly qualified and dedicated academic faculty with a rich mix of experience and compassion.

Organization and administrative structure

The Department is headed by an experienced and Senior Professor Dr. Srinivasan Sadagopan with extensive clinical and academic wisdom. His special areas of interest in Pediatrics are in Pediatric Nutrition, Breastfeeding promotion and Infant and Young Child Feeding Programmes, Pediatric Cardiology, Pediatric Respiratory Medicine and Infectious Diseases. The Head of the Department is ably assisted by Senior Professors, Associate Professors, Assistant Professors and Senior residents. The Department is divided into four general Pediatric Units, a Pediatric Intensive Care Unit (PICU) and a Neonatal Intensive Care Unit (NICU). Each Pediatric Unit has a Professor/Associate Professor as its head with two Assistant Professors / Senior Residents, who supervise and train the pediatric residents’ in-patient management. The PICU and NICU are manned by well trained staff nurses and supervised by senior faculty members. The PICU and NICU create an excellent opportunity for the Postgraduate Residents and Junior Faculty Members to increase their competencies in the management of sick neonates and children.

level III care at an extremely affordable cost which no other private hospital or private medical college can even dream of, thus fulfilling the commitment of serving the needy and downtrodden. The NICU is capable of providing care to 15 sick babies at a time with adequate number of warmer cots, phototherapy units, multichannel monitors, infusion pumps, ventilators and bubble CPAP equipments. The PICU is adequately equipped to handle up to six emergency cases at a time and is likely to be further improved once it is shifted to the new Pediatric block. In the PICU, residents routinely manage sick cases like bronchopneumonia, meningitis, hepatic encephalopathy, dengue shock syndrome, acute Glomerulonephritis, status epilepticus, status asthmaticus, diabetic ketoacidosis, congestive cardiac failure and various other emergencies. The Pediatric department case load in outpatient is quite high; undertaking immunization services, growth monitoring, following up high risk new borns and also following up children with chronic diseases like tuberculosis, nephrotic syndrome, bronchial asthma, epilepsy, cerebral palsy and congenital heart diseases. The department provides outpatient services between 8.30 am to 1 pm on all days. Emergency services are provided round the clock on all days.

There is no dearth of cases for academic learning as the Pediatric department always has a busy outpatient department. The cases vary widely ranging from protein energy malnutrition, infectious conditions like respiratory tract illnesses, diarrhoeal illnesses, tuberculosis, typhoid fever, malaria, urinary tract infections, hepatitis to complex conditions like neurodegenerative diseases, complex congenital heart diseases, immunological diseases like systemic

lupus erythematosus, auto immune hemolytic anemia and so on. The department has a dedicated academic program which takes care of academic interests of the post graduates and undergraduate students. The department trains four MD postgraduates per year and two diploma candidates. The academic program consists of case presentations, PG seminars and symposiums, journal clubs and theory classes. Emphasis is given on bed side case work-up and presentation. There are two dedicated computers with internet connections for facilitating internet search to retrieve clinical information from the World Wide Web. In addition to computer facilities, the department has a departmental library with latest books covering all sub-specialities, including national and international journals. The Pediatric department encourages postgraduates to publish case reports, clinical briefs and other scientific letters in various journals. Apart from publications, the postgraduates are also encouraged to publish case studies in Scientific Forums and Conferences. The Undergraduate Students are posted in the department of pediatrics from their fourth semester on wards. Each clinical batch attends 12 weeks of posting spread over the fourth / fifth, sixth, eighth and ninth semesters. The clinical classes are taken by experienced faculty members and also post graduate students. The undergraduates are also posted in a sophisticated state of the art Skills-Lab where they practice various life saving skills like neonatal resuscitation, basic life support, pediatric advance life support, intravenous cannulation and other minor operative procedures like suturing, lumbar puncture and so on.

RESEARCH ACTIVITIES ‘Foundation Workshop for Students in Clinical and Laboratory Medicine Research’

The recipients of ICMR – STS 2011 had a great opportunity of attending the “Foundation Workshop In Clinical And Laboratory Medicine Research’ organized by Moving Academy of Medicine and Biomedicine, Pune in association with National Institution of Epidemiology (NIE), Chennai. This programme was sponsored by ICMR. It was a three day workshop held at NIE, Chennai from

‘Epi-info’ which makes the data management and statistical analyses easier. On the last day all the participants had an opportunity to visit National Tuberculosis Research Centre, Chennai where they were exposed to the clinical laboratory techniques like Flow cytometry, Real time PCR, ELISA, Gel electrophoresis, TB bacteriology. It was a privilege for them to be with Dr. Sowmya Swaminathan, a leading researcher in Tuberculosis research in India who gave the students an overview of the work being carried out in Tuberculosis research centre. The Director of the institute, Dr. Thomas distributed the certificates to all participants. It was a great exposure for undergraduate students which made them aware of the Research methodology, the formulation of a proposal to publication in Journals. This workshop created a profound interest among the young students minds to uptake research as a career in future.

October 11.10.11 to 13.10.11, where ten recipients of ICMR – STS 2011 MBBS students namely Monica Kumbhat, Keerthi Jain, Vijayanthi, Santhoshi, S. Arun Mitra, Prem Vignesh. K. Gowtham, S. Arun Kumar, G. Somasekhar and A. Nasrin Nisha attended the workshop. Out of 52 participants of workshop from various colleges around Tamilnadu, Pondicherry, Kerala; the maximum number was from MGMC &RI. Lecture handouts were given on the first day of workshop which was highly informative to the students. It was an immense pleasure for the students to be with Padma Shri Dr. M.G.Deo, the founder of Moving Academy of Medicine and Biomedicine, Pune who inaugurated the workshop and also delivered guest talk on “Formulation of Research Proposal and the General Principles of Basic Technology used in Laboratory Research”. Many eminent speakers gave them an overview on topics such as Epidemiological Study Designs, Biostatistics, Research Protocol, Ethics In Medical Research, Drug Trials, Scientific Writing which were all very informative and created interest amongst the students towards taking up research. A very interesting lecture on Data Management made the participants to be aware of a software named

Public health is the science and art of promoting health. the understanding that health is a process engaging social, mental, spiritual and physical well-being. Public health acts on the knowledge that health is a fundamental resource to the individual, to the community and to society as a whole and must be supported by soundly investing in living conditions that create, maintain and protect health.” -Kickbusch, Ilona (1989) For various reasons, mostly of political economy, public funds for health services in India have focused largely on medical services. This is reflected in a virtual absence of modern public health regulations and of systematic planning and delivery of public health services. In this program, participants will witness the challenges of health care delivery in developing economies, with special emphasis on the spread of AIDS in India, tuberculosis, and management of infectious diseases. Students are exposed to key public health challenges facing India such as immunizations, environmental sanitation, waste management, safe drinking water practices, and social reforms through education, training and awareness campaigns. In addition, participants will be exposed to the role of voluntary organizations in the delivery of health care and the importance and impact of culture in public health. The Public Health and Community Medicine program is unique from other CFHI (Child Family Health International) programs: it combines servicelearning opportunities in public health and clinical settings. Unlike CFHI’s traditional programs, this will focus more on learning about public health issues facing the region and participating, in a limited way in the provision of healthcare at clinical facilities. This unique program combines interactions and discussions with the senior public health professionals with clinical work at government and charitable hospitals, rotations in nonprofit organizations, and lectures with senior policy makers. These experiences will complement participants’ clinical immersion in hospitals in and around Delhi. The program is spread across rural and urban areas so participants get an overview of challenges and strategies in both settings. They will have an opportunity to see healthcare efforts in the rural area of Sawai Madhopur, and will rotate in the historic walled city of Jaipur. And sooner or later the program will be implemented throughout the country. The Department of Community Medicine, MGMC&RI not only carries out these learning, training and teaching process but also extends its health care services in the society in collaboration with various agencies and civil society group in and around Pondicherry District with all serious and hearty

endeavours to alleviate the sufferings and pain of the most unreached and rural populations by conducting medical and health camps. Besides these also provides constant health promotive, preventive and curative services through its two health care center namely Rural Health Centre (Seliamedu) and Urban Health Centre (Ariayan kuppam).

Health Camps From the month of October to December, 2011, Six Medical Camps were conducted in and around Pondicherry district. On 04.10.2011, a multispecialty health camp was conducted in Lawspet, Pondicherry in co-ordination with the Department of Medicine, Surgery, OBG, Paediatric, Orthopaedics, ENT and Ophthalmology. 289 patients were the beneficiaries and 53 patients were referred to the various departments of MGMC&RI for further evaluation and treatment. On 14.10.2011, another multispecialty medical camp was organized in coordination with various other specialist departments of MGMC&RI at Nallavadu, Pondicherry, in Collaboration with HOPE NGO. 148 patients were the beneficiaries and around 26 patients were referred to various specialist departments for further investigation and treatment. Another similar health camp was organized on 16.11.2011, at Muthaliarpet Community Hall, Pondicherry, where around 171 patients were the beneficiaries and 42 patients were referred to various departments of MGMC&RI for further required investigation and treatment as in-patients. On 23.11.2011 again, a camp was organized in coordination with various departments to call it as a multispecialty camp at J.J. Nagar, Moolakulam, Pondicherry, where 241 patients were beneficiaries and around 71 patients were referred to various department of MGMC&RI for further required treatment. Yet another Multispecialty camp was organized on 13.12.2011 at Duppurai pet, Pondicherry. 151 patients were beneficiaries and 29 patients were referred to MGMC&RI for further evaluation and treatment. The last health camp for the month of December and the year 2011 was organized on 29th Dec, in coordination with the Department of Medicine, Surgery, OBG, Pediatrics, Orthopedics, ENT MGMCRI u SSSMCRI u IGIDS u KGNC

and Ophthalmology at Muthialpet Community Hall, Pondicherry. 287 people were the beneficiaries and 63 patients among them were referred to various departments for further evaluations and treatments.

Activities in the Health Centre:

The two health centres, namely the Rural Health & Training Centre (RHTC), Seliamedu, and Urban Health & Training Centre (UHTC), Ariayankuppam, are attached with the Department of Community Medicine of MGMC&RI since the inception of the Institution not only for providing the preventive, promotive and curative health care in the community but also to provide clinical, managerial and administrative training to the CRRIs’ and Post-Graduate students of the department. These centres also host various training format for the undergraduate students through the Department of Community Medicine as necessary curriculum for undergraduate training in India as per MCI norms. In accordance with the World AIDS day on December 2011, on behalf of Mahatma Gandhi Medical College , the Department conducted an awareness program for women from Self help groups at the urban health center, Ariyankuppam on 29.12.11. The function was organized by the Head of the Department, Dr.Sharada M. P. Questionnaires were distributed to about thirty women from SHG & fifteen attendants of patients who participated in the program to assess their knowledge. Dr. Sharada M. P. gave an introductory talk and Post graduate students, Dr. Vaithiyanathan, Dr.Raghunath and Dr.Vijayalakshmi gave talks on HIV/AIDS, its preventive aspects, services available and social discrimination. This was followed by an interactive session. Dr. Shiv Datta, the medical officer in charge of the health center, gave away prizes to the participants who answered the questions correctly. A similar programme was conducted for anganwadi workers working around the field practice area of rural health center, Seliamedu on 15.12.11. The function was organized by the Head of the Department, Dr.Sharada. MP & presided by Dr. Partha Nandi. Thirty Anganwadi workers and 4 women from Self Help groups participated in the program. Dr. Prabakaran, Dr.Raghunath and Dr.Vijayalakshmi gave talks on different aspects of HIV/AIDS, its preventive aspects and services available. This was followed by an interactive session. Dr Abhijeeth, the medical officer in charge of the health center gave away prizes to those who answered correctly. The centre functions every week-day from 8.30 am to 4.00 pm except on Saturday, with activities up to 2.00 pm. The center provides O.P. services to the local and rural people with the basic and emergency MGMCRI u SSSMCRI u IGIDS u KGNC

required treatment, laboratory diagnosis and act as referral centre for MGMC&RI for those patients who need further evaluation, investigations and treatment. The O.P. Census of RHTC for the month of October 2011 was 1034, among which 485 (265 male & 220 female) cases were new cases and 549 (288 male & 261 female) were old cases. For the month of November 2011 the census increased to 1158 with 659 (345 male & 314 female) new and 499 (273 male & 226 female) old cases. Almost similar O.P. census was maintained for the month of December 2011, where 1153 patients were the beneficiaries among which 561 (267 male & 294 female) were new and 592 (273 male & 319 female) were old cases. The O.P. Census of UHTC for the month of October 2011 was 1232 among which 456 (195 Male & 261 female) were new cases and 776 (260 male & 516 female) were the old cases as the beneficiaries. There was a clear margin of increase in the O.P. Census for the month of November 2011 to 1327 among which 512 (226 male & 286 female) were new cases and 815 (245 male & 570 female) were the old cases among the beneficiaries. For the month of December the O.P. Census increased to 1468 in total among which 565 (233 male & 332 female) were the new cases and 903 (273 male & 630 female) were the old cases who attended the OPD for getting benefitted. Apart from the O.P. services to the patients attending the health center, health education and health awareness session were done on every Thursday at UHTC, Ariayankuppam, basically for all the patients attending and the accompanying persons with them. The various topic covered were general hygiene and sanitation, common communicable diseases, Diabetes Mellitus, Balanced diet and nutrition, Hypertension and prevention, family planning, cancer awareness etc. Similarly on every Wednesday, health educational and awareness session were organized at the RHTC, Seliamedu. The health education and awareness program conducted in the centre are well appreciated and were attended by the large number of local people as a beneficiaries.

TERTIARY CARE SERVICES Master Health Check-up Unit

Master Health Check-Up Unit is an exclusive unit that deals with the preventive as well as curative aspects of Medicine. Various private and public sector companies have the health check-up done for their employees which has become mandatory nowadays, through our Master Health Checkup unit. During the quarter October to December 2011, a total of 332 cases were examined in the Master Heath checkup unit. Among the private sector companies, Foseco India Ltd, Muthirapalayam, Puducherry had the health checkup of their employees during the last quarter of the year. Among the total of 332 cases 52 were company cases. Apart from this regular annual health checkup by the companies, STCMS, Virudhachalam, Chempalst Sanmar Ltd, Cuddalore and TANFAC Ltd Cuddalore had the pre-employment checkup for their new recruits.

“Tamil Nadu - Chief Minister Comprehensive Health Insurance Scheme (TN –CMCHIS) interim period

This TN –CMCHIS interim scheme was started in our Institution during the month of September 2011, and sucessfully got the approval for 18 patients. They have applied for the New Tamil Nadu Chief Minister’s Comprehensive Health Insurance Scheme which is

like Patau Syndrome (Trisomy 13) Edward Syndrome (Trisomy 18), Down Syndrome (Trisomy 21) and Turners syndrome (45XO), have been karryotyped. the results help in further the management of the patients. The Department of Pathology and Genetic unit of Anatomy at the Central Research Lab have been doing a series of cytogenetic studies on patients suffering from premalignant lesions and malignant squamous cell carcinoma of the oral cavity. Histopathological types and grades and Cytogenetic alterations in such patients show a high incidence of Satellite Associations, Micronucleus and Chromosomal Aberrations. The main purpose of the study is to utilize the findings to create awareness about oral cancer among the public, to facilitate early diagnosis and subsequent disease management tailored to the individual patient. The ultimate goals are to reduce both mortality and morbidity, and to improve patients’ quality of life. Chromosomal test for recurrent Aborters and Mentally retarded children aid the clinicians as a better diagnostic tool in the treatment and counseling of the patients. An unusual case with Extra Metacentric Chromosome was an interesting finding of the year. A 12-year-old female with abnormal features like mental retardation, retardation in growth and development and poor dentition was found to have a karyotype with 47 chromosomes with an Extra Metacentric Chromosome (EMC). This non satellited chromosome was smaller than a G chromosome and replicated early. Her parents and her two brothers had a normal karyotype. So far in literature no conclusion is reached about the origin of the extra chromosome material. A comparison of the physical features of this patient with other reported patients with an apparently similar extra small metacentric chromosome does not permit the identification of this extra genetic material but the karyotype abnormality is likely the aetiology of this child’s global developmental delay. Perhaps reports of further cases and a comparison of individuals with unusual karyotypes could aid in better classification, diagnosis, and prognostic information in those children with previously undiagnosed multiple malformation syndromes.

A 63 yr old female patient presented with complaints of pain and inability to use her left upper limb for last seven days. There was black discoloration of left forearm with multiple blisters over dorsum of hand and forearm since 4 days. Nails were blue and cyanotic. Limb was cold up to the middle of forearm, radial pulse and brachial pulse were absent, no active finger movements and sensation present. As the pulse were not detectable by Doppler study done on 15 .9.11, an angiogram was done on 16.9.11 which showed complete occlusion of axillary artery left. In view of gangrene of the upper limb, absence of pulse, capillary circulation, sensation and total functional loss, it was decided to do left shoulder disarticulation. Amputation is the meanest operation if it is done in unindicated case. Amputation is the noblest operation when it is done as a life saving procedure. This was a case of life saving operation. Hence left shoulder disarticulation was done on 17.09.2011

we wiped it with a white gauze piece soaked in ethyl alcohol. Surprisingly the lesions cleared, to leave normal skin. Thus we concluded that the ‘ugly’ lesions on face were nothing but accumulated dirt. So it was an instantaneous “cure” for the patient. The term Dermatosis Neglecta was first coined by Poskitt et al in 1995. Dermatosis Neglecta is an often misdiagnosed and under diagnosed condition. In Dermatosis Neglecta a progressive accumulation of sebum, sweat, Keratin and other dirt and debris occurs resulting in a localized hyperpigmented verrucous plaque. Failure to adequately clean/ scrub the skin, often in an area of hyperesthesia/ prior trauma can produce this condition. This dermatitis is associated with the act of omission. Early and prompt clinical recognition of this condition and its cause eliminates the need for aggressive diagnostic and therapeutic procedure. Dermatosis Neglecta is an under reported, asymptomatic but cosmetically bothersome dermatosis.

Face has various glands that secrete sebum, sweat, keratin and other substances. Accumulation of these along with dirt results in dermatosis. Dermatosis Neglecta is such a condition. A young 6 footer came to our OPD on a forenoon, with anxious looking face. As we enquired, he felt concerned about the raised black skin lesions over the both cheeks which was asymptomatic, since 2 years. Clinical examination revealed dark, multiple closely studded papules, nodules, which appeared verrucous, extending from one end of the side burns across the face to the ala nasi bilaterally. We considered a diagnosis of Verruca Vulgaris and Linear Epidermal Verrucous Nevi. When we stretched the skin, the lesions appeared to be superficial. So, MGMCRI u SSSMCRI u IGIDS u KGNC

The 2nd Triennial Conference for Physicians, Surgeons, Dentists and Associated Professionals, Glasgow.

The Chairman, Mr. M. K. Rajagopalan and Mr. K. Ravi Shankar, CEO, Sri Balaji Vidyapeeth University were invited as “SPECIAL INVITEES” to attend the 2nd Triennial Conference for Physicians, Surgeons, Dentists and Associated Professionals between 10.11.11 and 11.11.11 at SECC Glasgow. The 2nd Triennial Conference is one of the highlights of the Royal College of Physicians and Surgeons, Glasgow Calendar. This event was supported generously by the Williams & Elizabeth Davies Foundation Trust, who support a wide range of activities in the areas of Research, Education and Training in Cancer Care and Treatment. The two day conference was well attended by more than 400 delegates from all around the world. The theme of the conference was “Setting Standards, Achieving Excellence”. There were a lot of very high profile speakers who were the eminent members of the community. An interesting part of the session included some key note lectures from Sir Harry Burns, Chief Medical Officer for Scotland, Professor Sir Mike Richards CBE, National Clinical Director for Cancer, Captain Ken Kelleher US Navy and an inspirational lecture from Professor Mark Harrison, Oxford, entitled “ Medicine Victorious: Northwest Europe 1944-45”. The Plenary sessions programme was classified into: Medical Forum, Surgical Forum, Dental Forum, History of Medicine Forum, Autumn Lung Conference and Travel Medicine Symposium. The conference included an impressive, four or five parallel sessions, which ran simultaneously giving delegates the opportunity to focus on their own particular specialty. The two days conference and the interaction with the dignitaries was a rewarding educational experience. The purpose of the visit to Glasgow, Royal College was to build on some of the initiatives in the MOU like: potential examiner training for building on examination center for MRCS / FRCS Ophthal / MRCP at MGMCRI; conduction of some of the educational courses & e-learning, student exchange program; faculty exchange program with emphasis on teaching methods, better delivery of curriculum content improvising on teaching & learning aids and colloboration of the Research Activities with the Glasgow University. Out of the Blue was the meeting that was organized for eight Delegates to meet “Her Royal Highness” Anne Elizabeth Alice Louise, who also represents the Royal College. The Chairman and Mr.K.Ravi Shankar had the rare opportunity for a hand shake and a small conversation with “Her Royal Highness” and the Mayor of Glasgow, Scotland. The Chairman places his humble thanks to the Royal College and in particular, Ian Anderson, President, John Cooper, Chief Operating Officer, Royal College of Physicians and Surgeons, Glasgow for their usual professional approach in making the vision and mission of Sri Balaji Vidyapeeth University come true.
