eel the fear and do it anyway. That self-help book wasn’t titled in honour of all the gay men and women who get on with their lives regardless of the disapproval they know is out there, somewhere, but it makes a handy slogan. One lunchtime last week, a matter of days after the Orlando massacre at the Pulse nightclub, I saw two young guys – nineteen at most – walking hand in hand down the main road in a not too salubrious part of this provincial town. As I drove past they briefly stopped to kiss before carrying on, hand in hand. It was a joyous moment in a week dominated by the horrors of Orlando and the murder of Jo Cox MP. If these young guys were afraid of possible disapproval or hate, they reacted in the best possible way. Rather than hide their love away, they put it out there for everyone to see. Growing up gay can be isolating, so our visibility in society and the media – which has increased exponentially in my lifetime – is so important to the gay boys growing up today. It’s a far cry from the UK of 1967, when our basic human right to love each other started to be recognised in law. We’ve seen things steadily improve; even in the face of setbacks, the gay community has been resilient and, importantly, found support in an increasingly accepting society. The bomb attack on the Admiral Duncan and the shootings in Orlando are not signs that things are sliding backwards: they are reminders, these extreme actions by disturbed individuals, that our freedoms must not be undervalued. Be safe, of course, but be who you are. We have all felt the fear, but we won’t let the haters win. We’ve come too far.

o why did you decide being an escort was for you? Well, becoming an escort happened in a very natural way … First of all, I’m very open-minded. is the absolute top priority. Regular check ups and also safe sex are essential. I’m here to share a good time, not looking for trouble in the future for me or even for my clients. That mutual respect is always appreciated by good customers. If the client requests anything that you’re not able to offer, don’t be shy to say NO! Your body, your rules!!! game - do you have plans to change career in the future? The most important thing about this job is, plan your next step. Once you have experienced working for yourself, probably you will never be able to work for anyone else in the future. Have your business plan ready before you start this business would be my advice. It’s not about “young man’s game”, especially because when you are not mature enough, all money come and goes… The clients looking for all types of guys. As a young guy I could be working for many more years but it’s not just a physical job, we need to be psychologically connected and this can bring you trouble if you don’t see your life in constant progress. DON’T BE IGNORANT: 8 THINGS NOT TO SAY TO HIV-POSITIVE MEN I
don’t mean to cause alarm, but there are more people living with HIV now than at any other time in history. Even in the terrible early years, the newly diagnosed outnumbered the dying, and so the numbers of us who were positive grew. Since treatment became effective, the numbers of people dying from HIV (rather than of other causes, but with HIV) plummeted further and so the annual increase in our numbers gets larger. So by now, with so many of us living with the virus on the gay scene, or on Grindr, Scruff or Recon, on Facebook, Twitter, Instagram and Snapchat, everyone knows what HIV is all about, right? Sadly, no. Pretty much every person who has disclosed their HIV status will have some nightmare stories about encountering rejection, ignorance or abuse. There’s no need for it. There are enough of us who are living openly with HIV, and who are happy to chat about it, that it shouldn’t be hard for someone who’s uninfected or undiagnosed to get the information they need without causing needless offence. If you’re nervous that you might say the wrong thing, here are some ground rules: Don’t ask someone how they ‘got it’. Sure, you may be curious, but that’s really personal information. If they want to tell you they probably will. And anyway, in most cases the answer is, “I had sex.” Don’t assume that they’re promiscuous, stupid or out of control. It’s likely that the majority of new infections take place between people in relationships rather than as the result of wild sex with multiple partners. Don’t assume that they’re a power-bottom. Yes, you’re more likely to acquire HIV from receptive sex, but gay men with HIV, like most other gay men, are most likely sexually versatile. Don’t assume that you’re morally superior. HIV is a virus. It’s not Santa Claus. It doesn’t care if you’ve been bad or good. Don’t assume that the person with HIV will feel inferior. Most of us would prefer not to have HIV, but weeping and wailing and tearing of hair gets old pretty quickly.

THE TRUTH IS, YOU ARE NOT GOING TO FLOURISH AS A GAY MAN IN THE 21ST CENTURY UNLESS YOU KNOW ABOUT HIV Don’t assume that he wants to talk about it. Even if he’s got a handle like ‘Poz Guy’, if he’s on Grindr he may be looking for sex. It happens, I understand. Don’t be a dick about it. It doesn’t take a genius to work out that a euphemistic expression like ‘clean’ (when what you mean is ‘not carrying a particular virus’) is a shabby thing to say. Don’t say things like ‘You deserve to die.’ Just don’t.
Now that may sound like a lot of rules, but none of them are beyond the capabilities of anyone who knows how to switch on a computer or install an app. If you follow those simple rules you’re probably good to have the conversation without pissing the other guy off (and potentially exposing yourself to a social media shaming). And the benefits of having a conversation, either on line or in real life, if you feel that you don’t know much about HIV, are potentially huge. Someone living with HIV is likely to have a good idea of what’s risky and what’s not. They’re going to have a good understanding of the impact of treatment, both in terms of health and life expectancy and also in terms of transmission. People with HIV are not to be feared. Most of us who are diagnosed in the UK are undetectable on treatment, which means that the risk of us passing the virus on during sex is very low (and may be zero). If you’re not comfortable with that assurance, that’s your right. It’s up to you to decide what’s safe enough for you. Gonorrhoea, syphilis, chlamydia and others still get passed on during unprotected sex, so however confident you are about the impact of treatment on HIV transmission there’s no reason to chuck out the condoms. In London one in eleven gay men have HIV; across all of England and Wales it’s about one in 20. So odds are, unless you’re living a life of splendid isolation, you already know someone with the virus. The truth is, you are not going to flourish as a gay man in the 21st century unless you know about HIV. Considering the odds are high that you’ll be speaking with people with HIV, taking the time to learn about it, thinking about how to ensure your own safety and how to support your friends living with HIV is time well spent.

DROP THE PILOT:
WHY WE SHOULD ALL GET THE HPV VACCINE

IF THERE WERE AN ECONOMIC AND HUMANE ARGUMENT FOR VACCINATING BOYS IN THIS COUNTRY AGAINST VIRUSES THAT CAN CAUSE SOME OF THE MOST DEADLY AND DEBILITATING CANCERS, WOULDN’T YOU EXPECT THE GOVERNMENT TO START VACCINATING AS SOON AS POSSIBLE? BUT WHAT IF THAT RISK IS PRIMARILY AMONG BOYS WHO GROW UP TO HAVE GAY SEX, WOULD THAT MAKE A DIFFERENCE TO OUR GOVERNMENT’S DECISION?
T
he Human Papilloma Virus – or HPV – has numerous strains, and not all of them are bad for you or even require treatment to go away. A few of them cause genital warts – which are not dangerous, but are embarrassing and can get out of hand if left untreated – and a few HPV strains cause cancer. The rest, fairly benign, are unfortunately so common that most of us will have been infected with HPV not long after we become sexually active. So, don’t panic. This is a serious health threat, but just because you’ve had warts or HPV in the past does not mean you are likely to get an HPV-related cancer one day. Enough of us will, however, to make vaccination a cost-effective response: the Joint Committee on Vaccination and Immunisation (JCVI) has recommended it for all men-who-have-sex-with-men under 45 and the Royal Society for Public Health has called for it to be offered to all 12 and 13-year-old boys. Girls in the UK have been vaccinated since 2008, in recognition that strains 16 and 18 of HPV cause 70% of cervical cancer. If you are a straight man – or rather, one who does not have sex with men – this vaccination of girls will eventually benefit you too, as infections with dangerous strains of HPV become rarer and rarer in the straight population. But vaccinating girls has no such effect on men who do have sex with men. The only way to replicate the huge impact already seen on HPV in the straight population is to vaccinate us before we become sexually active – hence the argument that all boys should be vaccinated alongside girls, aged 12 or 13. What would this protect us from? HPV is linked to 95% of anal cancers, 35% of penile cancers, and 60% of oral cancers. Treatment is distressing and can leave lifelong effects or disfigurement. And it is very expensive. Gay men are at higher risk of these cancers, due to the sexual transmission of HPV. And cases have been on the rise over the last fifteen to twenty years. The vaccine also targets another two strains of HPV, 6 and 11, which cause 90% of genital warts, so it could save us the discomfort and embarrassment of these as well.

IF I WERE OFFERED THE HPV VACCINE, I WOULD TAKE IT. AND COMPLETE THE COURSE So when the government announced in May that it would be funding an HPV vaccination pilot targeting gay men, it sounded like a step in the right direction. 40,000 of us will be offered the vaccine, but only if we attend STI clinics and only if we’re under 45. Unfortunately, these low numbers won’t have the effect that vaccinating all girls has had, gradually driving out dangerous HPV infections in the target population. And many of the guys vaccinated, sexually active for years, will have already been infected with the very strains they are supposedly being protected from.
Dr Shaun Griffin at THT called the pilot ‘a cynical stalling tactic’. He points out that, ‘back in November 2015, the government said that all MSM up to the age of 45 would be able to access the HPV vaccine across the country – now, six months later, we are disappointed to see this has been scaled down to a small-scale and unnecessary pilot.’ It sounds familiar – just like PrEP, proven in studies and adopted in numerous developed countries (USA, Canada, Australia ...) for its benefits for gay men, the HPV vaccine is deemed unproven for us in the UK. This is something to complain about, to get angry about. Even if it is only a delaying tactic, it will leave thousands of boys and men needlessly unprotected. The JCVI won’t make its recommendation on HPV vaccinations for boys until next year. But what about you? Well, if I were offered the HPV vaccine, I would take it. And complete the course of three injections. If you don’t attend STI clinics, even if you are over 45, you can ask your GP for the vaccine. And you could sign HPVAction’s petition for vaccination of both genders in schools. As gay men, we already experience too many barriers to the services vital to our health. We are 20 times more likely to develop anal cancer than straight men – leaders of the medical profession recognise this, but not every doctor knows their patient is gay or that we face this risk. We need the HPV vaccine to be given to all boys, and we need an awareness campaign like the Australian one: we should be able to feel comfortable about getting checked out for anal and other HPV-related cancers, by doctors who know what they’re looking for.
