M

y husband of 36 years, Mickey, lit up a room and my life. He was a man of great principle and ethics—a financial guru on one hand and a humble individual on the other. Simply put, he enjoyed life and found humor in everything. It was a gift he passed on to his three children and three grandchildren. Mickey did everything out of the box, including living for 18 precious months after being diagnosed with pancreatic cancer. It was a miracle he lasted so long. Originally a patient at Memorial Sloan Kettering, we eventually made the decision to stop actively fighting the cancer. Mickey transitioned to home hospice provided at our house in Queens. That’s when MJHS came into our lives.

I’m a physician’s assistant (PA) and familiar with hospice. When Mickey was first discharged from the hospital, his MJHS Hospice nurse and I were able to manage his symptoms. Continued on page 2

For over a week, the two of us were able to provide all the support he needed. His nurse was just wonderful. Unfortunately, Mickey’s symptoms became so distressing we could no longer manage his care at home. In fact, his issues were so acute that he couldn’t go to just any hospice residence, we needed one that could provide him with a hospital level of care, ideally in a home-like environment.

The private one-bedroom suites were warm and welcoming, and the location on the oceanfront grounds of Menorah Center in Manhattan Beach is extraordinary. Mickey and I are both beach lovers, so it couldn’t have been a better fit.

On Friday, New Year’s Day, an MJHS rabbi called and asked if we needed anything. We are an Orthodox Jewish family, so I told him what we needed for Shabbos. Delivering Joy

F

or families of pediatric hospice patients, the holidays can be an especially difficult time. In an effort to spread a bit of cheer, MJHS social workers, nurses and music therapists coordinated a holiday gift distribution on Christmas Eve. A month before, clinicians gave the MJHS Foundation a wish list of toys for patients and their siblings. Less than three weeks later, generous MJHS donors purchased all of the toys— over fifty gifts in total. Seven volunteers from UJA-Federation of New York’s Engage Service Corps, a group of retirees dedicated to volunteering throughout the city, gathered at MJHS Foundation office to hand-wrap and label each gift.

On the morning of Christmas Eve, MJHS Hospice staff, dressed as elves and reindeer, were joined by the AfterDark Ryders, a motorcycle social club in the Bronx, to hand out the gifts. Music therapists led the families in holiday songs and social workers helped boost spirits. MJHS staff traveled across the city to ensure every family received a visit. The fun day created lasting memories for the patients and their families.

HomeFirst Making a Difference I called Meals on Wheels for an emergency delivery, gave Arthur a list of nearby food pantries, and reached out to his sister-in-law so she could bring him food. Although it took a little convincing, I suggested that Arthur accept more personal care worker (PCW) hours to help him with cleaning, cooking, bathing and dressing, as well as to accompany him to medical appointments.

I

’m a licensed clinical social worker who has been employed by MJHS for nearly 30 years. Today, I work for MJHS Health Plans as a Care Manager for HomeFirst, a product of Elderplan, and one of the largest managed long-term care plans in New York. At HomeFirst, I get to offer help and protection to members who are desperately in need—people like Arthur. A vulnerable 70-year-old gentleman from Queens, he suffers from coronary artery disease and osteoarthritis. Arthur lives alone, has never been married and his brother and sister have both passed away. With the exception of a sister-in-law, he’s pretty much all alone. But, despite his many obstacles, Arthur is a fighter and a cancer survivor. I first met Arthur over the phone. During our lengthy conversation—which doubled as the initial care plan assessment—Arthur told me he had been mugged the day before of his entire monthly income. He had no food at home, no money and no food stamp card. Arthur sounded weary. I was determined to improve his quality of life. I worked with JASA (an organization committed to enriching the lives of the aging) to file a report with the Crime Victim’s Compensation Board.

A few short months later, Arthur was mugged again—this time at gunpoint in the elevator of his own high-rise building. The Crime Victim’s Compensation Board was able to reimburse Arthur for his financial loss, but we all feared what might happen next time. A number of advocates, including me, convinced Arthur to start using a debit card instead of walking around with cash. We also suggested that he avoid going to the bank by himself. This past spring, Arthur went to the emergency room because of a fall that injured his hip and broke his walker. Fortunately, he escaped without any fractures or broken bones. As his medical frailty increased, so did his trust in HomeFirst. He accepted our physical therapy referral, a new walker and orthopedic shoes. The man who wasn’t used to asking for help, now allowed us to help him. When I became a Care Manager, I feared that I would miss one-on-one interaction and problem solving. Then I started working with members like Arthur, who showed me I could still put my social work skills to use and make a difference. At the end of the day, Arthur and I both have HomeFirst—and by extension the Four Brooklyn Ladies —to thank. I must admit that it feels really good. — Mona P.

Center for Jewish End of Life Care Rabbi Richard Hidary, Ph.D. is Associate Professor of Judaic Studies at Yeshiva University. He is also the Distinguished Rabbinic Fellow at Congregation Shearith Israel. Amir Kishon, Ph.D. is the Chairman and CEO of Wellness Layers, Inc., a leading provider of strategic web and mobile patient engagement solutions. Dr. Kishon is also an active board member of several affiliated MJHS agencies and programs.

T

hree years ago, the Center for Jewish End of Life Care (Center), an important web-based resource for Jewish people across the continuum of observance, was formed as a collaboration between MJHS and the UJA-Federation of New York. The Center’s mission is to spur and facilitate conversations about the physical, emotional and spiritual needs of Jewish people affected by any advanced or terminal illness. It serves as a practical and professional interactive resource for families, patients, physicians, professional caregivers and spiritual leaders. Last year, the Center earned a great deal of visibility, including a presentation at the White House. Accolades and awards also poured in, such as from AJAS (Association of Jewish Aging Services) and LeadingAge New York. Earlier this year, an Advisory Board was formed comprised of prestigious leaders in the fields of medicine/health care, religion, technology, law and community outreach. “It will be our role to maximize the value and impact of the Center by shaping its strategic direction,” said Eli S. Feldman, Chairman of the Advisory Board. MJHS and UJA-Federation of New York welcome the following esteemed members to the Advisory Board and thank them in advance for their commitment to taking the Center to the next level: Craig Blinderman, M.D. is Director of the Adult Palliative Medicine Service at Columbia University Medical Center/ New York-Presbyterian Hospital and Co-Director of the Center for Supportive Care and Clinical Ethics in the Department of Medicine. Eli S. Feldman is CEO Emeritus of MJHS. He is also Executive Vice Chair of MJHS Foundation.

Christine Kovach, Ph.D., RN, FAAN is the Jewish Home and Care Center Research Professor in Aging, University of Wisconsin-Milwaukee, College of Nursing. Dr. Kovach is also Editor for the Journal Research in Gerontological Nursing. Rabbi Irwin Kula is the President of Clal (The National Jewish Center for Learning and Leadership), a leadershiptraining institute, think tank and resource center in New York City. Marc E. Narotsky, Esq. is a member of the Florida bar, Florida Court certified in Civil and Family Mediation, a Qualified Arbitrator and a Millennial with caregiving experience. Barbara Paris, M.D. is Director of the Division of Geriatric Services in the Department of Medicine, Maimonides Medical Center, Brooklyn, NY. She is also Clinical Professor of Medicine, Geriatrics & Palliative Medicine at the Mount Sinai School of Medicine in New York. John Ruskay, Ph.D. is former Executive Vice President and CEO of UJA-Federation of New York, a philanthropy that cares for New Yorkers of all backgrounds and the Jewish people world-wide. Rabbi Springer is also a certified supervisor in the Association for Clinical Pastoral Education and a certified Jewish chaplain. Helping Menorah Residents

Remember When

A

movie, flower or delicious baked treat can trigger powerful memories. For those with dementia, trying to recall and share experiences can be frustrating, embarrassing or lead to surprising behavior. Menorah Center for Rehabilitation and Nursing Care uses structured therapeutic recreation programs to help residents with dementia express themselves in supportive and engaging ways. Our goal is to use nonpharmacologic approaches to help each person maintain dignity. Families are particularly helpful because they know about favorite pastimes. Pet therapy is particularly popular with residents

of all abilities, as are those that incorporate exercise, sensory and relaxation techniques.

Caregivers often talk about seeing a resident go from being unresponsive to wiggling toes during chair Zumba or participating in a stretching class. Some even start talking for a short time. As the sun goes down, soothing sounds, personalized music therapy and sensations help ease the anxieties of residents who wander or get agitated at night. The sundowning initiative is an extension of Menorah Center’s award-winning and innovative music therapy program for residents with dementia. Those who participate are shown to have improved appetites, increased communication, and a happier affect, in addition to reduced depression and agitation.

Ultimate Goal Beyond the therapeutic recreation team, Menorah’s doctors, nurses, social workers and chaplains help family members cope with the emotional stress of having a loved one with dementia. The entire staff’s fundamental belief is that each person has a rich history and an abundance of feelings, as well as a sense of purpose and belonging. The ultimate goal is to provide each resident, regardless of ability, with a caring, compassionate and interactive environment.

Staff are trained to recognize that so-called behavioral problems are often a response to cognitive overload. And, while each activity is fun, it serves a much larger purpose—exercise the body, challenge the mind, soothe the spirit and warm the heart. Top: This past fall and winter saw temperatures yo-yo from very cold to unseasonably warm. This led to the creation of a more formal horticulture therapy program, which brings the outdoors in. Participants, especially those who don’t speak, enjoy feeling soil between their fingers and watching seeds grow. Leaves even get incorporated into arts and crafts projects. Bottom: The cooking club and kaffeeklatsch is a go-to place to share wisdom, secret ingredients and family recipes. For some residents, it’s soothing to be around divine aromas. Others enjoy chopping, stirring... even cleaning up!

Putting Patients First “ I’m a wound care nurse for MJHS Home Care, and I know just how essential it is for our patients to receive the daily wound care they require. Continuity can make all the difference,” says Josephine.

T

he day New York was hit with the second biggest snowstorm in its history was no exception for her. It didn’t matter that city transportation services had been shut down. She was scheduled to go see three patients—and that’s just what she did.

This time, a policeman stopped her and asked why she ignored the mayor’s encouragement for all non-essentials to stay home. She proudly showed her MJHS ID and said, “I am essential.”

After 20 minutes, Josephine finally arrived at her patient’s home. e are exceedingly proud of the communitybased MJHS Road to Recovery orthopedic program. It showcases our commitment to providing innovative healthcare solutions to address the needs of our patients in the comforts of their own homes. Last May, the MJHS Road to Recovery program expanded to include targeted rehabilitation services that could be offered to patients recovering from hip and knee replacements. Intensive PT services start within 24 hours of returning home—and are typically provided up to seven days a week. This unprecedented continuity of care coupled with a

high level of skilled services makes it possible for most hip and knee patients to skip a post-acute stay in a rehab facility. To date, MJHS has treated hundreds of joint replacements with this aggressive physical therapy program. The results demonstrate that we are making a difference. Rehospitalization rates are lower than the national average and clinical outcomes exceed national benchmarks. Perhaps even more importantly, our patients are happy they can receive the therapy they need while at home and are pleased with the results.

I

t is increasingly common to know someone with chronic pain, depressed mood, cognitive decline, sleep problems or mobility difficulties. Work being done by Helena Knotkova PhD, Director of Clinical Research and Analytics at the MJHS Institute for Innovation in Palliative Care, may help improve the lives of people who are living with these conditions. In collaboration with a research team at the City University of New York, Dr. Knotkova has created a new type of transcranial direct current stimulation (tDCS) unit that involves the delivery of a mild electrical current to the outside of the head.

As part of the evaluation process, Dr. Knotkova and her colleagues have initiated two innovative research studies. One will determine whether very ill patients who have distressing symptoms can use the new tDCS device effectively with the help of a family member. The other will collect data about the effect of the device on a broad array of symptoms, ranging from lack of sleep or concentration to pain, worrying and feeling sad. This research may introduce an important new tool in the nonpharmacologic management of many problems related to advanced illness. Prior to Dr. Knotkova’s work, tDCS was difficult to implement in chronically ill patients because treatment required repeated visits to a facility. Helped Bring My Mother-in-Law

in for a Soft Landing

M

y mother-in-law, Norma, was a warm, wonderful, sometimes stubborn but always proud woman. At 92, she was a loving mother and grandmother who couldn’t wait for her first great grandchild. Norma suffered from pulmonary hypertension, congestive heart failure and dementia. Yet even with these health issues she was still able to live on her own in an assisted living community, with the support of a home health aide, until her illnesses led to hospitalization.

Over time, the MJHS Hospice team (including nurses, a social worker, home health aides and a rabbi) cared for Norma—and our family. Every single person was warm, caring and treated her with respect. They spoke to us in laymen’s terms, making everything easy to understand. I was so impressed with the level of responsiveness and how the team knew just how to comfort her.

While Norma had multiple health issues, she was still active, so I felt confident she wasn’t ready for hospice. It turns out I was wrong. You don’t have to be close to death to receive hospice services. Not only was Norma eligible for hospice, but she was also eligible for another aide. By providing that second home health aide, MJHS Hospice made it possible for Norma to return home. That in itself was a gift.

Not unexpectedly, during the last two weeks of her life she went into a rapid decline. That’s when MJHS sent over a nurse so she could administer the morphine Norma required to effectively manage the pain. We couldn’t have gotten through without it. During this time, Charles was a huge comfort not just to Norma but to all of us. His words made a huge difference in how I looked at the process. MJHS Hospice is here to “bring her in for a soft landing,” Charles said. And that’s just what they did.

Rose Art Foundation Provides Seat at the Table

I

n February 2016, MJHS became the beneficiary of 20 geri chairs (geriatric chairs), maneuverable recliners on wheels, generously donated by the Rose Art Foundation. Much to the delight of our patients and their caregivers, these chairs now have a special home at MJHS Hospice Residences across New York City. Varda Yoran established the Rose Art Foundation in 2012, inspired by the experience her husband, Shalom, had at the end of his life. Shalom, a Holocaust survivor, suffered from dementia and other illnesses that caused him to become immobile and isolated from his family. Varda and their two daughters, Dafna and Yaelle, became desperate to find a solution that would make Shalom comfortable and allow him to live with dignity,

surrounded by friends and family. After extensive research, they found the geri chair. This chair transformed Shalom’s end-of-life experience for both he and his family. Shalom Yoran lived an incredible life and was just 14 years old when the Nazis invaded his hometown of Warsaw, Poland. At 17, his parents were murdered and Shalom fled with his brother to join the Jewish partisans living in the woods, fighting the Nazis. Shalom escaped to Israel and joined the Israeli Airforce and became a leader in the Israeli aircraft industry. In 1979, his family immigrated to the United States, but he never lost his calling to help others. His legacy lives on at the Rose Art Foundation and through the 520 geri chairs that have been distributed across the country!
