MJHS was founded in 1907 by The Four Brooklyn Ladies, who looked into the eyes of the old and sick — people in desperate need and deserving of loving kindness — and did something. They founded the Brooklyn Ladies Hebrew Home for the Aged. With the help of charitable support, the home provided poor, elderly members of the community with quality health care and a place to live in their time of greatest need. Their core values, rooted in compassion, dignity and respect for every individual, regardless of who they were or where they came from, have guided us every step of the way since then. In a very real sense, we recreate our founding moment every day. We comfort, we counsel, we heal, we put our values to work. With support from annual grants from the MJHS Foundation, we’ve grown from that small building among the tenements to one of the most expansive health systems in the region, providing exceptional care in our community. 2

MJHS Home Care Our home care programs are all about restoring the highest possible function and quality of life. MJHS Home Care might be right for someone recuperating from an illness, injury or surgery and needs advanced medical care. We work closely with patients, their personal physicians and you or other family members to design an individualized plan of care.
Putting Your Friends and Family on the MJHS Road to Recovery MJHS has proven expertise in helping patients recover from hip or knee replacement surgery, stroke, spinal cord, or any other orthopedic or neurological conditions that affect mobility. We can help your loved one regain the skills and confidence to live safely at home, avoid rehospitalizations and remain engaged in their communities. Our home care services are available in Manhattan, Brooklyn, Queens, the Bronx and Staten Island, as well as Nassau County and include: nursing, rehabilitation, social work and home health aides.

MJHS Palliative Care Palliative Care is the comprehensive treatment of the pain, symptoms and stress associated with a serious or life-limiting disease. It can be provided in conjunction with curative and aggressive treatment during any stage of an illness. Palliative Care is intended to reduce the burden of illness, maintain quality of life, minimize suffering and ensure that care is guided by goals consistent with the values and preferences of the patient. We offer palliative care to children as well as adults. Our Palliative Care program is offered in Manhattan, Brooklyn, Queens, the Bronx as well as Nassau County. MJHS is the only program that provides palliative care services at home.

MJHS Hospice MJHS Hospice provides comprehensive, compassionate end-of-life care to patients in their own homes, assisted living communities, nursing homes, hospitals and our own hospice residences. Our program helps patients and their families live life to the fullest by easing suffering, upholding dignity, affirming values and improving quality of life. It is also one of the very few programs in New York State to receive a DOH Waiver to provide hospice services to pediatric patients who are actively receiving curative treatment. MJHS Hospice serves residents of Manhattan, Brooklyn, Queens, the Bronx and Nassau County. When a patient can’t remain at home, MJHS Hospice has our own: • Mollie and Jack Zicklin Residence in Riverdale • Muriel and Harold Block Residence in the Bronx • Inpatient Unit at Beth Israel Medical Center in Manhattan • Inpatient Unit at Boro Park Center for Rehabilitation and Healthcare in Brooklyn Other specialty programs include Ang Lin, the MJHS Hospice program for the Asian community. In addition as part of our Jewish hospice program we offer Halachic Pathway and specialized end-of-life care for Holocaust Survivors, as well as Art & Soul™ creative arts therapy, pastoral care and bereavement services.

Centers for Rehabilitation and Nursing Care If your loved one needs short-term/sub-acute rehabilitation or long-term nursing care, we have two extraordinary centers. Both provide exceptional services and highly talented staff that, above all, are committed to treating your family member with compassionate, respectful and quality care. Our two centers include: •M enorah Center for Rehabilitation and Nursing Care on the ocean in Manhattan Beach • S horefront Center for Rehabilitation and Nursing Care, located on the historic Coney Island Boardwalk Many of the programs offered at Menorah and Shorefront exceed the services typically offered at for-profit facilities, and are made possible by MJHS Foundation.
Culturally Sensitive Care Your co-workers are as diverse as New York City. They receive cultural sensitivity training, preparing them to better meet the needs of the Jewish, Asian, Hispanic and LGBT communities. And many are fluent in Russian, Chinese, Spanish, Yiddish, Creole and Korean.

Health Plans We offer a variety of affordable, high-quality plans specially designed to help keep your loved ones independent and living life on their terms. • E lderplan was as one of the first Medicare Social HMO demonstrations in the United States, with a focus on frailer-than-average beneficiaries living in the community. Today, Elderplan offers Medicare Advantage Prescription Drug (MAPD) plans to Medicare beneficiaries in New York City, as well as MAPD Special Needs Plans for dually eligible beneficiaries (Medicare and Medicaid) and residents of nursing homes. Elderplan also offers a Medicaid Advantage Plus (MAP) plan that integrates Medicare and Medicaid benefits for dual eligible, nursing home-certifiable beneficiaries living in their homes under contracts with the Centers for Medicare & Medicaid Services and DOH. The Elderplan MAP is currently eligible to enroll residents of New York City, Monroe, Nassau and Westchester counties. •H omeFirst, a product of Elderplan, was founded as a New York Medicaid Managed Long Term Care Plan (MLTCP), one of the first in New York State. As an MLTCP, HomeFirst coordinates the medical and long-term care of chronically ill Medicaid and dual eligible beneficiaries age 18 and older who require more than 120 days of home and community based services, across the five boroughs of New York City, Monroe, Nassau, Suffolk and Westchester counties. Services include personal care, regular nursing visits, transportation to and from medical appointments and care management.
