Who is Eligible? Any adult patient with a life-limiting illness who requires:

• Pain and symptom management • Assistance addressing frequent hospitalizations • 24-hour medical care not possible at home • Help stabilizing symptoms so they can return home safely

What does the MJHS Hospice Inpatient Unit Offer You and Your Family? Tailored to patients requiring acute symptom management, our 6-bed hospice inpatient unit delivers the best in end-oflife care. We provide a team-oriented approach to expert medical care, pain management as well as emotional and spiritual support, including: • Effective pain and symptom management for patients who can’t be managed at home • 24-hour care provided by our culturally-sensitive multilingual staff • Onsite staff of hospice professionals including: board-certified physicians, nurses, medical social workers, spiritual care coordinators, creative arts and music therapists, bereavement counselors and volunteers • Ventilator care for patients who require pulmonary support • Daily visits by hospice medical staff • Acceptance of patients without DNR/DNI • Ability to provide palliative care consults onsite to patients who are experiencing pain • 4 private and 1 double room, each with a private bathroom • Welcoming common areas including a family lounge and dining room • Conveniently located on the grounds of Silvercrest Center for Nursing and Rehabilitation in Queens

Patients may receive hospice care in the comfort of their own
