The main facility suffers a major local disaster such as flooding, fire or prolonged electrical outage.
The main facility is on-line and operational, but staff cannot attend site or do not have control of the site.
This leaves the facility in a state where it can no longer originate channels.
This could be evacuation due to potential malicious or physical threat, a cyberattack or sudden unavailability of operational staff. In this case the facility could still be operational but not available for channel origination.
