Editor’s Note Pokémon GO is making children of us all. While this may be true, we also need to address what the now-popular smartphone game has made out of our children. Concerns over children’s privacy have been making primetime slots and the world seems to be divided over what effects such a phenomenon could possibly have on child development. The game may be based on catching cartoon-based creatures for fun and competition, but what are the players really competing against? In the UAE, numerous “sightings” of Pokémon have prompted parents to ask: did we really need one more reason to worry about our children’s safety? The UAE Telecommunications Regulatory Authority (TRA) has issued a security alert to warn residents about sharing their geographical locations while playing online games. Experts have made plenty of suggestions on the matter, including ensuring that online identity is always concealed, or disabling location services altogether – an option which would come in handy for parents who are completely against the new trend. It may be tricky to prevent your children from accessing Pokémon GO altogether; it is crucial – however – to ensure you have guidelines in place for every online or smartphone interaction your children are engaging in. Meanwhile, it is worth pondering over this: Pokémon may eventually go away, but will we ever get our Pokémon GO-free days back?

It might sound odd, but just like with any other domain in our lives, there are trends in even the most instinctive of notions – parenting. While 2016 may have proven to be a rather exciting year so far, halfway through we realised there were some phenomena we just HAD to address. Due to the rise of more dual-income households, greater access to technology and shifts in parental roles, family dynamics are changing, hence resulting in fundamental shifts in parents’ approaches to raising children. Here is the latest in the world of parenting:
WERE YOU RAISED BY “Helicopter Parents”? Have you ever heard of this term? Neither did we, until we have reached the conclusion that this type of parenting was no longer common. A method whereby parents micromanage every aspect of their children’s lives, this type of parenting is now being replaced with a style that is much more relaxed, with less focus on scheduling and more on independence.
BOARD GAMES AND BORED GAMES Parenting experts are constantly making discoveries on the importance of sensory play. It has been found that activities which engaged kids’ senses helped in early childhood development. However, despite an increased focus on sensory play, it seems like parents are also going back to basics and using traditional board games to interact with their children. This also means a downsize in parenting roles; it is becoming more common for parents to ask relatives and caregivers to help out with previously parentonly tasks including car pooling and house cleaning. Hybrid BABY NAMES “Evily, would you come here please?” Do not be surprised, this is actually a hybrid between the names Emily and Ava. Parents are increasingly mixing and matching names for entirely new results which will ensure your child is the one and only in their class. There is also the Lorelei Gilmore Effect, where girls are being named after their mothers, and not only sons after their fathers as has always been common. While this may be the opposite of hybrid baby-naming, it seems like the mummies got a bit jealous and figured: why not? However, fathers have a trend of their own: “old-man” names for younger boys, although we are not sure naming a child Warren is always daddy’s idea.

1. Not understanding what sleep training is Sleep training is not about bedtime routines and lavender massages. It is about your kiddo learning how to go from being awake to being asleep all on their own, meaning, being able to fall asleep without the help of a caregiver, a car, swing or pacifier. In that way, nearly all sleep training methods are similar. As parents, you want to develop a plan where you can put your child down awake, and find a way that you feel most comfortable dealing with the inevitable protest crying that will initially occur (like doing timed checks, for example). Then, most importantly, making the commitment from that bedtime and on, that you will not assist your child in falling asleep. After several days, babies and toddlers will learn to fall asleep unassisted, and the sleep training will be complete.
2. Not sleep training for naps and nighttime simultaneously One of the biggest mistakes I see families make before working with me is their attempt to sleep train only for nights without training for naps at the same time, or vice versa. In an effort to not let their children become, “overtired,� parents often help their babies fall asleep for naps during the day, while only sleep training at night. Because there is so much melatonin in the body at bedtime, kids fall asleep relatively easily for bedtime, and throughout the night, yet struggle to nap unassisted during the day. In this case, after a few days, the child might sleep well at night. However, when MOTHER, BABY & CHILD | AUGUST 2016 | 15

parents try to tackle nap sleep, the child uses all their stamina from solid night sleep to then fight naps – making nap training extremely difficult. Worse so, if a child continues to receive help to sleep during the day, it is usually only a matter of time until a sleep disruption (such as illness or travel), causes nighttime sleep issues, which would then require a parent to start from square one. The parent now has to help their child fall back to sleep for bedtime, and in the middle of the night – all over again. In this case, it would only be a short period of time before sleep training progress completely falls apart. 3. Keeping the pacifier Unless a child is old, and dexterous enough to reach out in the middle of the night and quickly put a pacifier back in their mouth, sleep training while using a pacifier is not really sleep training. Truth is, whatever your child relies on to fall asleep at the onset of bedtime or nap time will be the same thing they will want to have to fall back to sleep with throughout the night. Even older babies who can easily re-insert the pacifier in their mouths often experience significant nighttime wake-ups just looking for their pacifier. Ensuring a child can fall asleep without any help whatsoever is always the best route to pursue for long-term results, and a well-rested baby. 4. Not making an ideal sleep environment a priority I always require that four things be present when sleep training a baby: Loud white noise, a very dark room, a video monitor, and a safe crib (with NOTHING in it other than a mattress, a well-fitted sheet, and baby –with possible exceptions being a mesh bumper and a very small lovey).

a recipe for disaster. Sleep training requires time, consistency, and patience. Without a solid plan in place, it can be difficult to stay consistent over the several days or weeks it may take to properly sleep train a child. 6. Not sticking with it
Babies do not need much to sleep well. Studies have found that loud white noise helps humans fall into deeper levels of sleep faster, and stay there longer. Darkness helps the body produce a constant level of melatonin to further aid in easily falling asleep. A safe crib is a no-brainer – never allowing your child access to an unsafe crib is very important, especially as they will be in it all night on their own. Finally, a video monitor helps parents know when their babies are actually asleep and awake, which provides peace of mind in knowing their babies are safe. 5. Starting sleep training without having a plan in place Families who want to start sleep training will be a lot more successful if they have a plan in place first. All sleep training plans should contain four parts: What to do at bedtime, how to deal with middle of the night waking, how to make sure kiddos start each day at the same time, and how to train for naps. Obviously, reading many books and working with a sleep training professional can take a lot of the guesswork out of how to deal with all of these different aspects of sleep training. However, not having a plan is
When done properly, sleep training usually takes about three-four nights for night time sleep, and one-two weeks for naps. This might seem like a small amount of time, but similar to caffeine or sugar withdrawals, shifts like this can actually feel pretty agonizing at first. One of the biggest mistakes that many families make is shifting gears or quitting after only a few days due to not getting immediate results. The commitment must be to stick with sleep training for one-two weeks, which usually results in huge success, regardless of the training method parents use. 7. Letting illness get in the way of healthy sleep habits Once a child knows how to fall asleep unassisted for naps and bedtime, they do not suddenly “unlearn” what they have learned...unless a parent reverts back to helping the child fall asleep. I am adamant with the families I work with in informing them that – once your child masters falling asleep on their own, do NOT go back to helping them fall asleep. Even when they are ill, love them, comfort them, medicate them under doctor’s orders, but when the yawn of sleepiness comes, always put them down awake and let them fall asleep on their own. I even advise my clients to share a room with their children overnight just in case their child needs them, but to simply stay out of the room when the child is actually falling asleep.

Being prepared to look for the right things and ask the right questions can make all the difference when it comes to feeling comfortable and satisfied with the decision you ultimately make. With plenty of options in town, what would the right one be? Mother, Baby & Child brings you the top tips, straight from the experts!

Have a look to see whether there is a number lock or an intercom at the gate? The harder for you as an adult to get in, the better it is. Monica Valrani CEO, Dubai’s Ladybird Early Learning Centre • The nursery should provide a warm and welcoming setting in which children can develop socially and academically within a safe, healthy and nurturing environment. Do you get that “warm and fuzzy” vibe when you visit the nursery? • Ensure the nursery provides a rich program of activities, suitable for the child’s age, enabling physical, social, language and creative development. • All nursery teaching and support staff should be qualified, have appropriate experience and have an obvious passion for working with young children. • Health and safety should be a visible priority.
• Check if all staff have their First Aid certificates. Is there a full-time nurse? • Look for the cleaning checklists in the lavatories and ask for the toys’ cleaning schedule. 2. Check out the premises and learning environment • Is it safe, warm, inviting and stimulating? • A properly-structured learning environment aids development. There should be well-organised areas around the room (science, mark-making, reading and construction corners, etc.) • Facilities should adapt to independent and adult-led work. PARENTING • Look for evidence of the constant re-invention of the learning environment. 3. Research into curriculums • Research in advance the Early Years Foundation Stage (EYFS), Montessori, High Scope or other curriculums to ensure that you have a basic understanding of what you are looking for. Understanding which curriculum you prefer will immediately narrow down your choices. • When visiting a nursery, look for evidence of curriculum application. Timetable, planning and goals for each child should be displayed where parents can see them. Make sure that you see “science” behind play. It takes skilful adult guidance to make the most of what children can learn from the tools provided. 4. Take a look at the ratio, qualification and turnover rate of the staff • Observe: How many staff are present in each classroom? Are they talking to each other or the children? • Ask the nursery manager:

What are the teachers’ qualifications? For the EYFS curriculum, look for a minimum of CACHE Level 3 Diploma in Pre-School Practice / NVQ Level 3 (UK) or equivalent relevant to the position. • Ask about staff turnover; stability is crucial. It helps your child settle if one adult is their “key worker”. 5. Check that there are established policies in place • A well-run nursery should have established and defined policies for everything from opening and closing times to dealing with emergencies. • Ask what the pre-school’s “first-day policy” is. Can a parent stay for a while, or will you need to leave immediately? • Ask about the illness policy. Find out which illnesses will keep your child at home and for how long. 6. Make sure you can be involved
• The nursery should be encouraging parental involvement. You want a pre-school that will work with you and respect your knowledge as a parent. Also, check the schedule of the pre-school’s events where parents are invited to add them on your calendar. 7. Check tuition fee structure and the nursery’s operating hours • Opening hours vary from nursery to nursery. Choose what works for your schedule. If you are a working parent, you would prefer a nursery with long operating hours. • The tuition fee often depends on the hours you choose. Generally, there are the morning, afternoon and early morning programs. All of them are charged separately. 8. Enquire about additional services such as food and transport • Provision of transport and food can be crucial for you, especially if you are working. • Is the food cooked on the premises or is it delivered? What are the menus like? What are the bus routes and hours of operation?

THE Health CHECKLIST Keeping our children healthy and in top shape for back-to-school season may get overwhelming at times. With hundreds of health flyers from school or nursery and doctors warning us about the weather, immunisations and vision tests, many mummies say: “But we are not doctors!” Here are some ways to ensure the children stay healthy and happy throughout school season:

PARENTING • Make sure your child gets sufficient sleep, ideally eight hours every night, and consume a healthy, nutritious breakfast every morning. Check on what the school will be feeding the children as nutrition is an essential part of your children’s mental and physical development; if you are not happy with the options, plan ahead with healthy, balanced lunchboxes.
• Get their vision tested. It is often very difficult for parents to make the observation that their child is suffering from vision problems, even though it is very common for children to not see properly out of one of their eyes. • Check on the necessary shots with your paediatrician. As this is a controversial topic, it is best to consult them on the matter so that they can give you a clear picture which is in line with Ministry of Health guidelines for that particular time of year.
• Ensure you have the right school supplies. In terms of backpacks, in which they tend to carry plenty of books and stationary, they should be rolling/wheeled rather than carried on the back. For when they need to carry them, backpacks need to have wide straps and a padded back. • The school nurse needs to be familiar with any medication your child is taking, even if they only take it at home. Any health problems, including allergies, should be made known to the school. Also inform the school of any physical restrictions, including asthma or a scoliosis brace.
• Update emergency information. The school needs to have all of the required numbers. Apart from a designated relative’s and yours, they often need your doctor and dentist’s numbers as well. If you ever change your number, which we know happens a lot, remember to call the school and update it immediately.

Mother, Baby & Child interviews Dr Deepti Chaturvedi, Specialist – Paediatrics at Burjeel Hospital Abu Dhabi, to answer the most common motherhood questions from a mother, who is also a doctor’s, perspective.

yyHow do I know if my newborn is healthy? Newborn babies may cry for several hours a day. This is usually their way of telling you that they need something such as when they are hungry, tired, feel cold or hot, their diaper needs changing or need to be comforted. Generally, a healthy baby means an active and alert baby who is feeding well, sleeping well, and passing urine five-six times a day without any issues. It is advised to monitor your baby's growth and development with a paediatrician every fortnight monthly for the first six months and quite regularly thereafter too. Any warning signs like lethargy, refusal to feed, a continuous high-pitched cry, fever, vomiting, abnormal behaviour or movements should be a reason to visit a paediatrician in between the regular visits.

It is not unusual for newborns to experience irregular breathing. Sometimes, newborns stop breathing for five to ten seconds and then instantly begin breathing again on their own. At times, their eyes might be crossed because it is hard for them to focus but by two to three months, babies should have more control of their eye muscles and be able to focus better. Newborns can differentiate between sounds including voices. You should talk to your baby often to find out if your they turn towards the sound of your voice. yyAre vaccines safe?

Every toddler is different and the ways to deal with their behavioural issues will be different too. However, there are some strategic points which could help in turning the “terrible twos” into “awesome twos”.

toddler elicits different responses from parents at different points in time, it can be confusing for them to understand the difference between right and wrong. Keep it short and simple: Toddlers do not understand long instructions, so try to keep them short and simple enough for them to comprehend and follow. Know your child's triggers: It could be hunger, lack of sleep, overactivity or simple boredom. Every temper tantrum has a pattern to its trigger; find it and try to avoid it as much as humanely possible. If all else fails, give them a “time out”. A time out should be given in accordance with the child’s age; do not forget to tell the child why he has been given one. Stay positive: Raising children is not an easy task and toddlerhood is as tricky as the teenage years. So, remember to stay positive throughout and avoid showing your anger. Remember, this too shall pass. yyHow can I get my child to sleep
through the night? There are many reasons as to why your baby might be awake during the night. They may need to be fed, comforted or might be teething which could also upset their sleep patterns. Babies require a consistent bedtime routine to help them settle easily. You could try making daytime feeding more social and fun compared to nighttime feeding where the environment is more relaxed and calm. This will help your baby learn the difference between day and night. Also, try to give your baby the chance to fall asleep on their own (natural circadian rhythms) often known as “sleep-wake cycle”, which helps regulate sleep. Babies up to four or five months old are less likely to be fed at night and are able to sleep for longer. yyWhen should I start potty training? Children are not usually ready to start using a potty until they are between 18 months to three years old. Boys tend to be ready a few months later than girls. Parents should start the training at the age of two to get them into the habit of going to the bathroom by themselves by the age of three.

BODY & SOUL yyWhy do you think it is often difficult for mummies to “love the skin they are in”? After pregnancy, and even more so after giving birth, mothers find themselves focusing more on their family. They have no time to take care of themselves like they used to and – at the same time – their bodies are not what they used to be. Accompanied with the lack of an active social life, many mummies find themselves spiralling down into depression, not knowing what to do with this new life they have been given. It is miraculous, but any mother can tell you, it is not easy. yyWhat is the main issue you face at the clinic with mothers seeking treatment? I face plenty of issues, but the most salient would be extra fat, loose skin, cellulite and stretch marks. Needless to say, these are always worsening due to an unhealthy lifestyle which patients tend to be following unless directed otherwise. yyWhat do mothers complain about the most and how do you deal with it? They complain about their bodies! This is a very sensitive job, but I do my best to use all of the technology we have been blessed with to make them happy and confident. I support them; what people may not know is that there is a psychological side to all of this, so we try to be there for them – they can come to me for all of their problems, be these physical, or psychological as a result of the former. yyTell us about the most moving story you have ever encountered. This mother had come to me with so much “mummy guilt”, it was terrible. She had no self-confidence because she had gained so much weight after giving birth. The breaking point was when her husband had called her a “whale” and she had to
seek an answer. After working with her, first and foremost, towards a healthier lifestyle, incorporating sports and in parallel with slimming technology she lost 20 cm off her waist and became so happy you could see that the burden had been lifted. yyWhat are your views on plastic surgery? You often need plastic surgery, but before considering it make sure you have thought of the non-invasive alternatives. I think advanced plastic surgery in specific should be a last resort rather than first option. yyWhat is your number one tip for mothers so they would feel confident and happy at all times? Mothers must adopt a healthy lifestyle, always care about their bodies and love their bodies no matter what.

We have all experienced them at some point in our lives and, admit it, nothing frustrates us more than looking in the mirror, only to see those unwanted shadows under our peepers. Mother, Baby & Child interviews Dr. Nic Isse, plastic surgeon at The American Surgecenter in Abu Dhabi, for the final answers to our dark circle dilemmas.

ark under eye circles can be caused by a variety of things. At times, they can be an after-effect of too little sleep or fatigue; they can be caused by allergies or poor lifestyle choices, symptoms of facial ageing or – worse – a combination of heredity and genetics! Whatever the cause may be, dark circles make us look tired and years older too. Treatments on how to banish dark circles abound. They work in quite different ways and with quite varying (and lasting) effects. “It is good to understand that the skin around our eyes is the thinnest and most fragile skin on our entire body. As it is the most delicate, it is also the most susceptible to any concern occurring in your body,” says Dr Isse. If you are feeling ill, nursing a cold, sleep deprived or dehydrated, the thinnest skin in the body will show it first as the blood vessels that supply oxygen and nutrients are smaller in there. In cases like these, concealers and topical creams are among the quick fixes available. Although they rarely provide a long-term solution with significant results, they may help reduce or temporarily mask dark circles. There are instances, however, that even after layers of makeup and creams, dark circles will just not go away. “Dark patches under the eyes can also be brought about by causes beyond our control such as inherited pigmentation irregularities and ageing. Pigmentation irregularities are of particular concern among Asians. On the other hand, thinning skin around the eyes and loss of fat and collagen, which are common as we age, can make the reddishblue blood vessels which lie below become more visible,” explains Dr Isse. However, do not lose hope! There are plenty of cosmetic procedures available for patients suffering from these conditions who are looking to erase their dark circles altogether. According to Dr Isse, recommended procedures include skin resurfacing, hyaluronic acid facial fillers and
fat injections. He explains that skin resurfacing involves chemical peels which help reduce dark circles by increasing collagen production, decreasing pigmentation problems and improving overall skin quality. This procedure yields results after one session only and it could be done up to twice a year if additional lightening is required. Dr Nic also said that chemical peels are most effective when used as part of a combined approach in treating dark circles. Injectable fillers containing hyaluronic acid are often used to correct dark circles created by thinning under eye skin or fat and tissue loss in the under eyes or upper cheeks. Hyaluronic acid fillers are absorbed by the body within a few months to one year, so results are temporary. The only permanent filler typically recommended for the under eye area is a patient's own fat. These fats may be grafted from another body area of the patient and injected into the tear troughs to treat dark circles. Fat injections – or a fat transfer – are typically recommended for the correction of dark circles caused by thinning under eye skin or fat and tissue loss in the under eyes or upper cheeks. Unlike hyaluronic acid fillers, fat injection results may be permanent, but results are not guaranteed, as fat cells do not always survive the fat transfer process. Also, fat transfer could need up to three sessions depending on how the body absorbs the fat. Maintaining an ideal body weight helps with results lasting longer and individual cases vary. Fillers need to be redone after four to six months depending on how quickly the body reabsorbs them. For extreme cases however, the surgeon recommends either surgery for under eye circles or a cheek lift. “Eyelid surgery, or blepharoplasty, is generally the most effective way to permanently get rid of dark circles. On the other hand, a cheek lift, or mid-facelift, can effectively and permanently correct dark circles caused by cheek descent, or drooping, a common result of facial aging.” MOTHER, BABY & CHILD | AUGUST 2016 | 45

et over two floors, Bertin is pretty huge. Facing the door was a mouthwatering assortment of desserts – chocolate overload – which seemed to attract visitors miles away. At any given time, there were at least three people marvelling at the display, which overlooks the dining area, a cosy set-up with an open kitchen. The unassuming, friendly staff handed us an extensive menu with dishes we had never heard of before, but descriptions which made it hard to choose as everything sounded just perfect. Although essentially Spanish, Bertin’s selection of food is far from paella-andpatatas-bravas; it is as international as can get and avoids the cliché gourmet references us consumers are accustomed to. Now for the most important part, and this because it is really exceptional, the food. I went for their meat platter, a new specialty which basically includes every type of meat you would possibly want to eat, along with four types of sauces. A feast of mouth-watering steak options, slow-cooked ribs, succulent lamb chops, oven-roasted baby chicken and more, it is cooked for three, but I doubt you will want to share. The sausages were my favourite, followed by the thinly-sliced steak perfectly cut into rare and medium-
well slices. My companion reached for the lamb chops – because this is how amazing they looked – and we both agreed they were the best we had ever tasted. It almost made her regret her choice, but not quite. A fan of healthy eating, she had gone for the seabass, which was a story in itself, cooked to perfection and laying on a bed of vegetables and potatoes. Speaking of potatoes, Bertin potatoes are unlike any other. They do wedges justice, using absolutely no oil and baking them to perfection. They also serve mashed potatoes with their meat of which I had to take two spoons to-go with my meats – despite the embarrassment. Now for the dessert! We had to share this one because the portions are massive. Pain Perdu is the name, and it is a beyond-heavenly cake-ish dessert served oozing in a cup with a heart of orangette, then covered with fleur de sel (a handharvested sea salt) caramel. Apart from smelling like angels’ feet, the sweet temptation was something our taste buds had never experienced before. With a special menu for children, Bertin is the perfect place to head out either with your friends (the crowd is largely female; we would assume they come for a chat over Pain Perdu). Spacious, unique and affordable, this is a venue which ticks all the boxes. Bertinbistro

As human beings, we witness miracles in the form of little blessings in our daily lives. As doctors, we are fortunate enough we get to witness miracles of magnificent scale – right in front of our eyes – and such was the case of baby Shifae.

hifae was born at 23 weeks of pregnancy, 17 weeks earlier than the due date. The delivery was conducted by one of our senior obstetricians, with the neonatal team on standby including senior neonatal doctors and nurses. The team had prepared the area to welcome her including equipment to put her on breathing support as well as measures to keep her warm and safely move her to the Neonatal Intensive Care Unit (NICU). When she came into the world, she weighed a mere 700 grams and was in critical condition with no breathing efforts. Shifae was managed by the team using the early care pathway followed in the neonatal unit. This is a structured, organised way of quickly, but safely stabilising her and giving her the necessary medications for the lungs to start working. She was admitted to the NICU on breathing support and had a stormy first month of life being on an off the mechanical ventilator and needing lots of medication. However, after a month, her breathing condition improved, luckily along with some muchneeded weight gain. Initially, she received artificial feeds into her veins and then slowly moved on to tube feeding,
eventually reaching the stage of feeding by herself. She also received treatment for her eyes to manage the immature retina. During her stay in the NICU, her parents were closely involved with the daily aspects and engaged in providing skinto-skin care while staff were busy providing her with the life support treatments she needed. She stayed there for 125 days after which she was sent home with her parents. Prior to discharge, her parents learnt the care patterns with the support of the specialist nurses working in the neonatal team in order to ensure they could later provide the necessary support. Shifae is now over six months old and doing well at home. Her parents are overjoyed at the fact that, against all odds, the baby has managed to pull through. She still has a long path ahead of her; her brain development and growth are closely monitored in the neurodevelopmental clinic. While there will still be challenges ahead of her, Shifae has shown that, however sick you may be, there is always a ray of hope, a little light which drives us to work harder every day to give these babies a chance at life, a hope for the families to nurture and a prayer that more will be helped in the future.

SURVIVE Sandstorm SEASON Asthma awareness is crucial when living in the GCC, especially when we live in a climate which has significant environmental triggers such as sandstorms. Several studies conducted in Arabian Peninsula countries with a high rate of sandstorms have confirmed the increased prevalence of asthma.

was also reported that sandstorms frequently cause adverse health effects on the respiratory function and just 15 minutes of exposure to smallersize sandstorm dust particles can increase the risk of asthma. Asthma requires long-term treatment. However, it can be managed if precaution is taken to reduce the severity of the symptoms. Sugar-sweetened beverage sales in the UAE place the GCC at the top of the global chart. In the GCC, residents are drinking more SSBs than five years ago. According to the International Chair on Cardiometabolic Risk (ICCR), the burden of diabetes is expected to surge over the next decades, as the Middle East will face the greatest relative increase in the prevalence of Type 2 diabetes worldwide, with 60 million diabetics in 2030. Despite better acute care and chronic pharmaceutical management, the prevalence of cardiovascular disease
(CVD) in the GGC remains high due to poor diet habits and low levels of physical activity, contributing to the region’s obesity epidemic – especially abdominal obesity, metabolic syndrome and Type 2 diabetes.

yyWhat is the percentage of patients suffering from fertility problems in the UAE and what is usually the cause of such problems? Similar to international statistics, infertility problems are diagnosed in one in six couples as per World Health Organization (WHO) figures. The reasons behind infertility can involve one or both partners and include tubal blockage, low sperm count or motility, ovulation disorders and endometriosis. In some cases, it may remain unexplained. yyIs IVF legal in the UAE? Who regulates it?
yyIn simple terms, what is IVF? In Vitro Fertilization (IVF) is a well-known method of assisted conception. The process involves hormonal stimulation and the monitoring of ovulation; eggs are collected with a simple needle-guided procedure and fertilised at the laboratory with a sperm sample collected from the husband. Once the embryos are ready, one or two of them will be transferred to the uterus. yyWhat are the different reproductive surgeries and infertility treatments? Currently, the available reproductive technologies include In Vitro Fertilization (IVF), Intracytoplasmic Sperm Injection (ICSI) and Intrauterine Insemination (IUI). The differences in simple words are: In IVF, sperms are mixed with the eggs to allow them to fertilise the eggs, while in ICSI the embryologist will
select the best sperm and inject it directly into the egg – under the microscope. An IUI treatment involves the perpetration of the sperm sample and inseminating it in the uterus at the time of ovulation for the fertilisation to occur inside the body. Some patients may benefit from reproductive surgeries such as hysteroscopic surgery for intrauterine pathology (polyp, fibroid or septum) and laparoscopic surgery for pelvic diseases such as endometriosis or ovarian cysts. yyWhat is PGD? How important is it? Preimplantation Genetic Diagnosis (PGD) testing is a procedure used prior to embryo implantation to help identify genetic defects within embryos fertilised through in vitro fertilisation to prevent certain diseases or disorders from being passed on to the child.
IVF is legal in the UAE and strictly regulated by the Ministry of Health, Abu Dhabi Health Authority and Dubai Health Authority. In the UAE, there are clear policies and procedures governing the practice of IVF and gamete storage; these regulations are also in line with almost all the known Islamic authorities. yyHow successful is IVF? The success rate of IVF depends on the age of the female partner and cause of infertility; generally, with those below the age of 35, the success rate is around 45%, but this varies between centres. We have successfully achieved these rates and higher at the HealthPlus Fertility Center over the past five years, starting with 600 treatment cycles during 2011 and expecting to perform over 2000 treatment cycles this year. yyWhat are the risks and side effects? Another risk is Ovarian Hyper Stimulation (OHSS) which is rare, the percentage is only around 2%. yyWho benefits from egg and sperm freezing? When should a person consider it? Egg and sperm freezing is available and permitted in the UAE; it is very useful for those who are delaying their pregnancy for career or educational reasons. It is also very useful prior to chemotherapy or cancer treatments. yyPlease brief us on a successful, complex case that underwent IVF in HealthPlus. Recently, one of our patients achieved a pregnancy after nine failed IVF cycles. The couple were diagnosed with poor ovarian reserve, severe male factor and recurrent implantation failure. The couple underwent detailed assessment and treatment prior to IVF including screening for natural killer cells, autoimmune antibodies and genetic testing. The husband’s treatment started prior to IVF to improve the outcome. As she was usually producing only one or two eggs, we adopted the new approach of egg accumulation where we collected the eggs consecutively and froze them until we achieved a number of eight eggs, then fertilization and embryo transfer was conducted. Abu Dhabi-based group of physicians successfully treated an Emirati family applying an advanced IVF treatment. The family, who could not have children due to some medical issues, conceived a baby who was HLA-matched to his sister – a Thalassemia patient requiring frequent blood transfusions. She is currently under treatment using stem cells and bone marrow from her younger brother. “This advanced treatment unlocks new doors to the treatment of many genetic diseases. We now have technologies for conducting accurate tests on embryos during the early days of prenatal development and before placing them back to the mother’s uterus, which makes choosing healthy embryos possible, and thus reducing chances of genetic disorders. It also helps with identifying the embryos which are HLA-matched to their siblings in order to treat them for different diseases using stem cells.” Mohammed Ali Al Shorafa Al Hammadi, CEO & Managing Director, United Eastern Medical Services (mother company of the HealthPlus network of specialty centres). Dr Walid Sayed, the consultant and medical director at HealthPlus Fertility Center who successfully performed the treatment along with his team, explains that the couple had come to the centre to seek help with their infertility problem. My work day starts with the review of upcoming patient appointments and making sure that I have all the information at the time of visit so I can make informed recommendations to families and patients. ow important do you think vitamin D is? Well, vitamin D deficiency is among some of the most diagnosed health problems in Dubai. Studies are attributing the drop in vitamin D levels to the lack of exposure to the sun, especially since sun rays are the source of 80 to 90% of the body’s needs. For the last twenty years, medical research departments worldwide have grown fully convinced that the danger of vitamin D deficiency lied in its general effects on health as well as its possible role in causing diseases including breast, colon and prostate cancer, with incidence rates rising by 30 to 50%, in addition to other diseases like the osteoporosis, diabetes and blood pressure among others. The deficiency can also cause psychiatric problems, especially depression and schizophrenia. On the other hand, normal vitamin D levels help in forming blood cells and raising body’s immunity levels. The vitamin also plays an important role in reducing the risk of cancer, in and plays a vital role in maintaining insulin levels in the body. Doctors see vitamin D as a regulator of the immune system, with a crucial role in the treatment of rheumatic bone and muscle disorders.

However, the percentage has been increasing in the Middle East, and raising many concerns, as 73% of adults suffer from a deficiency of less than 15 ng/m – which is extremely low. MOTHER & BABY yyHow does breastfeeding benefit the mother? We frequently receive requests for medical advice on breastfeeding, as infants make up our largest patient group. Breastfeeding has significant benefits to both mother and child, and statistics show that the longer babies are breastfed, the greater the health effects for both mum and baby. So, we generally recommend to mothers who are able to breastfeed their babies to keep it up for 12 months, if possible. The first few months after birth are a wonderful period where the child’s rapid pace of growth and development continues outside the womb. Nursing the child promotes emotional health and increases the bonding between the mother and her newborn thanks to the hormone oxytocin, which is released during breastfeeding. Studies have shown that breastfeeding also lowers the risk of breast and ovarian cancer, as well as osteoporosis.

It is also financially beneficial as breastfeeding mothers save around AED 4,380 per year on average compared to those who use formula. yyHow does breastfeeding benefit the baby? Breastfeeding protects babies from common illnesses such as stomach viruses, lower respiratory illnesses and ear infections. Meningitis occurs less in breastfed babies and they are less likely to suffer from fevers after immunizations.
In addition to reducing the risk of childhood obesity, breastfeeding protects babies from developing allergies. Immune factors, such as Secretory Immunoglobulin (IgA) which is found in breast milk, help prevent allergic reactions to food. Research conducted on more than 17,000 infants from birth to six years has shown higher IQ scores in babies which were exclusively breastfed and for prolonged periods of time. yyWhat are some common misconceptions about breastfeeding?

If babies feed a lot or seem hungry often, that means they are not getting enough milk. Because breast milk is much easier to digest, breastfed babies generally get hungrier sooner than formula-fed ones. Timing of feeds should be every two hours for breastfed infants. Never wake a sleeping baby up to breastfeed. Babies will rouse when it is time to feed, but you must make sure that they are fed every two hours in the first two months of life, even if they are asleep. There is no milk in the first few days. During the first few days after delivery, small amounts of concentrated milk called “colostrum” are excreted by the breasts. Colostrum is very important for the baby as it contains essential nutrients, antibodies and immunoglobulins. Once the baby masters suckling, more

milk will come in its usual form. It is okay for breastfeeding to hurt. When breastfeeding hurts, that means it is being done wrong, and both baby and mum will suffer. Mums will experience injuries and ulcers around the nipple and babies will not latch well to suckle enough milk.
yyWhat nutritional advice would you offer breastfeeding mums? New mums usually like to start a diet right away to lose those pregnancy pounds. However, limiting calorie intake or following a strict diet while breastfeeding is not recommended. We advise new mothers to make healthier choices, and they will need an additional 200 calories per day in comparison to pregnancy in order to cover their new nutritional needs. Nutrients in breast milk stay the same regardless of what you eat. However, the body will start using the mum’s stores of nutrients and vitamins if they are not made readily available in her diet. yyA model postpartum diet should contain the following:

Protein: Two to three servings per day. Good sources of protein include meat, poultry, eggs, cheese, milk, yoghurt and cottage cheese.

Seafood is also a good source of protein, but mothers should avoid shark, swordfish and king mackerel due to their high mercury content. Calcium: 1,300 milligrams/day. One cup of milk or yoghurt has 300 mg of calcium. Good sources of calcium include milk, yoghurt and hard cheeses. Iron: 9 mg/day. Good sources of iron include meat, poultry, seafood and egg yolks. Vitamin C is found in broccoli, cantaloupe, potato, tomato and bell pepper. Vitamins and minerals: Despite a well-balanced diet, a daily multivitamin is highly recommended for the first six months at least. Smoking and alcohol consumption
should be avoided. Caffeinated foods and beverages should be limited. We also recommend avoiding spicy and citrus foods, which can upset the baby’s sensitive stomach.

The benefits of milk are many, ranging from the tiniest tooth until the biggest bone. Known for its high levels of calcium and contribution to keeping bones and teeth healthy, milk is essential for children to include in their daily diets as it provides them with the needed energy as well as essential vitamins and minerals for healthy growth and development. As children grow, they naturally drink less milk, either because they feel more grown up and independent or start to substitute it with other less nutritious beverages. Here are a few tips to get your kids to ask for more milk!

THE YUM! FACTOR 1. Involve your children in the preparation of their milk: When your child chooses and helps out in preparing their milk, they are most likely to enjoy having it. Make sure you use clean utensils when doing so. 2. Dip in the biscuit: Serve some cold milk with your child’s favourite whole grain cereals or biscuits; show them how to dip them in and enjoy them even more with milk. 3. Make it a family choice: Try to have milk as the drink of choice at family meals. Milk, whole grain cereals and fruits for a family breakfast are a great way to do so. 4. How many times have we thought “I will just keep it in the fridge” and ended up regretting our choice? The fridge is often our saviour when it comes to keeping food items and beverages fit for consumption, especially in Dubai’s scorching heat, but maintaining freshness and quality inside the fridge can be a difficult task. Here are some tips and tricks to ensure you can store items for as long as you possibly can: • Wrap leafy vegetables in newspapers to maintain their crispness. • Use a cloth to rub white vinegar onto your cold cuts (and other salt-cured meat) to prevent dryness and unpleasant smells. • Store milk and other perishable items in the middle section of the fridge. • Keep your berries fresh by washing them with one part vinegar to ten parts water. • Separate onions from potatoes; onions make potatoes sprout. • Keep cucumber and tomato on your kitchen counter, they fare better without refrigeration. • Wrap your cheese in absorbent material such as parchment paper so it maintains freshness. • An opened bottle of maple syrup should be kept in the freezer – high sugar content will prevent the syrup from solidifying. • Store ginger in the freezer to make it easier to grate. • Use kitchen thermometers to ensure perishable items are stored at proper temperatures to avoid bacterial growth. • Measure and maintain the right pH balance to keep the fridge healthy and food fresher. pH meters are available in leading supermarkets.
