Safety Corner by Dawn Lulua-Claxton Unusual Occurrence Forms: What are they? They are Forms found: S:\_Shared Bulletin Board\Forms and Instructions used by Employees to report incidents. Employees are required to immediately report any unsafe condition or hazard that they discover in the workplace to their supervisor via the NAHC Unusual Occurrence/HIPAA Incident Report and the supervisor will then forward a copy of this report to the either your supervisor, a witness, or the Executive Director. No employee will be disciplined or discharged for reporting any workplace hazard or unsafe condition. Employees who wish to remain anonymous may report unsafe conditions or hazards by submitting Incident/Accident report to the Executive Director without identifying themselves. Why are they used? Documentation is a very key aspect to healthcare for liability purposes. Who stores & maintains the records? The Executive Director maintains an Unusual Occurrence log as well as corporate files in which all incidents are documented.
