The effects of childhood stroke on motor skills Stroke is one of the top 10 causes of death in children, yet there is very little research around childhood stroke. Researchers from the Murdoch Children’s Research institute (MCRI) have undertaken a first ever study to systematically evaluate motor function across a 12 month period, following the diagnosis of arterial ischaemic stroke (a stroke that is caused by a blood clot). The longitudinal study looked at three groups of children – neonates (before 30 days), preschool (30 days to five years) and school age (over five years to 18 years) and analysed their progress over four time points – acute, one month, six months and 12 months. Researchers reviewed the 64 children involved in the study across these four time periods, looking primarily at how each child’s motor skills had been

impacted by the stroke and their recovery. They also looked at the impact of motor impairments on their daily lives. Stroke has traditionally been linked to older people and not to children; however it is quite common in children, particularly newborns. In fact, in their first week of life a term newborn has a stroke risk that is three times that of a hypertensive, smoking diabetic adult. This study has clearly demonstrated that children who suffer a stroke require individualised care over a much longer period than adults who experience a stroke. From the moment she did her first rotation in theatre as a graduate, Alison Jenkins was hooked on becoming a perioperative nurse at Peninsula Health. “Working in theatre with high acuity patients is exciting. Every day is different,” says Alison, an Anaesthetic and Recovery Nurse at Frankston Hospital. “You never know what you’re going to get until you walk in and see your name on the list. Today I’m on the orthopaedic trauma list, but it could be a caesarean list, a kids list or recovery for the day.” One of Alison’s most memorable moments from her five years at Peninsula Health is from her time working on the caesarean list. “My first caesarean is the one I remember the most,” recalls Alison. “Seeing them pull out the baby, I didn’t cry but I was emotional. It was the first baby I’d seen born and it was just so overwhelming to watch them pull out a little human – it was special.”

Alison enjoys seeing so many different patients every day – whether it’s for life-saving surgery, or something more routine like removing tonsils. “Every day here is rewarding because the work is interesting and there are so many specialties.” Alison’s advice for others thinking of specialising in perioperative nursing at Peninsula Health: “ just do it, definitely do it!”

Transforming perioperative nursing education into webinars From infection prevention and workplace fatigue to asepsis, perioperative attire, chemicalfree cleaning and innovations in allocations, thousands of perioperative nurses are tapping into webinars right across Australia. In 2014, the Australian College of Operating Room Nurses (ACORN) launched a webinar initiative in a bid to deliver education to its 5,000 members across 4,000kms of the nation, spanning cities, regional, and rural and remote areas. The education program is the only perioperative nursing education in Australia delivered via webinars, and has been such a success, the webinars are often booked out within 24 hours. Dr Paula Foran, the education officer at ACORN and a perioperative nurse with 30 years’ experience, recently presented on the unique perioperative education initiative at the European Operating Room Nurses Association (EORNA) Congress in Greece. Dr Foran says the live, interactive online presentations with experts in the field are designed to provide accessible, regular and affordable online education for perioperative nurses, regardless of where they live and work. “It reaches our members from all over Australia,” she says. “Access to face to face education at conferences, run in the cities, is not only difficult but expensive when accommodation and travel are included.”

Research shows while the percentage of Canadian perioperative nurses with postgraduate qualifications stands at 77 per cent, only 29 per cent of Australian perioperative nurses have postgraduate qualifications. Other research shows only 21 per cent of perioperative nurses working at 10 metropolitan hospitals in Australia in 2014 had postgraduate perioperative nursing qualifications. With a growing body of research pointing to a link between the quality of patient care and nurses’ education and expertise with patient health outcomes, the webinars are working to increase the education of the nursing specialty. The national perioperative webinar education program features monthly webinars, and those members who are unable to tune in live can utilise recorded versions of the webinars to access education at their fingertips, whenever and wherever they choose - on any mobile device. The program features two streams of webinars - standards and education. The standards stream focuses on the release of new or updated standards, covering areas such as environmental sustainable perioperative practice to surgical hand antisepsis and loan equipment. “This allows explanation and discussion with members about changes to practice in an effort to maintain compliance to current standards,” Dr Foran says.

The education stream covers areas such as malignant hyperthermia, perioperative inadvertent hypothermia anaphylaxis, difficulties with intubating and ventilating as well as human factors in the perioperative environment, the role of the perianaesthesia nurse, an introduction to orthopaedic surgery, evidence to action, and introductions to both general surgery as well as ear, nose and throat surgery. Dr Foran says the webinars are aimed at all perioperative nurses, and are being developed on management, clinical and educational topics, in response to member feedback. “My first job is to find experts on the particular topics members have requested, and then I brief them on the level of education delivery, novice to expert,” she says. “The presenter and I have practice sessions prior to the webinar, mainly to test the internet connections and explain the system to the presenter. “During the webinar, I troubleshoot the system where possible, usually assisting members having problems at their end. “If I am not presenting myself, I manage the questions and answers at the end, allowing the presenter to just verbally respond.” While the webinars are at the mercy of internet access and speed, Dr Foran says the webinars have been a huge hit with perioperative nurses, receiving positive feedback. An Echuca nurse applauded the program, saying - “Been a perioperative nurse for 20 years and still learning. Love these webinars.” Another regional Victorian nurse also credited the program for overcoming barriers to education, such as distance. “Love, love, love this type of education straight to the point,” she says. “Not so easy for us country folk to get the education in the city. This is a perfect way to see that we are keeping up with current practices.”

Understanding traumatic brain injury

A

unique Australian study into Traumatic Brain Injury (TBI) is helping health professionals better understand the range of factors that improve rehabilitation outcomes for people with TBI. The Longitudinal Head Injury Outcome Project, which examines the psychological and cognitive consequences of brain injury, is a large, ongoing study which has tracked 3000 people with TBI since 1995. The project, a collaboration between the Monash Epworth Rehabilitation Research Centre and TAC, is led by Centre director and Monash University Professor of Neuropsychology Jennie Ponsford. Professor Ponsford says the study, which recently received renewed TAC funding, records and examines the experiences of TBI patients treated at Epworth, following their discharge from rehabilitation. “We set up this longitudinal study where we follow up people at one, two, three, five, 10, 20 and soon we will be starting 30 year follow ups of the patients,” she says. “It’s enabled us to identify what are the key long-term problems they experience, which tend to be more problems with their cognitive function and their behaviour and their emotional state, more than physical problems. “That then has enabled us to make changes to our rehabilitation program and we’ve also, based on the findings, set up specific treatment trials to address some of the key issues that have emerged from the study.”

What is TBI? TBI is a complex injury to the brain, often the result of a blow to the head, which leads to temporary or permanent disruption in brain function, that impacts a range of physical, sensory and cognitive systems. While mild TBI can cause short-term symptoms such as dizziness, blurred or double vision, headaches, insomnia and poor balance, moderate or severe TBI can result in longterm cognitive and behavioural changes, including a shortage of attention, difficulties with new learning and memory, and impaired planning and problemsolving. Professor Ponsford, who has coauthored a book on rehabilitation for TBI, says while the physical outcomes of achieving independence in activities of daily living is positive, more than two thirds of people with TBI experience continuing problems with memory, concentration, speed of thinking, and difficulties when it comes to planning and organising their daily activities. “They also unfortunately often have personality changes - so they’re more irritable or short tempered, more impulsive, and that makes them more difficult to get on with, and so they have a lot of social interaction problems and tend to lose their friends,” she says. “They may get back to study but they then have difficulty holding down a job, and then in the long-term they become anxious and depressed and socially isolated.”


How to care for TBI patients Professor Ponsford says it’s important for

clinicians to understand people with TBI will experience long-term difficulties, that impact much more than their physical capabilities. “A physiotherapist who is working with the patient needs to really understand how to manage their behaviour, how to interact with them appropriately to do that, and the team really has to be all working on the same goals, which are in the interests of the client,” she says. “And to also be thinking about what motivates the client from the beginning, not just about what the therapist wants to achieve. “The therapist might want to achieve better mobility, better range of movement, better upper limb function, better independence in preparing meals but that might not be what matters to the patient. “What matters to the patient is that they might want to get back to driving or get back to work or be with their mates, and so I think it’s very important to try and craft rehabilitation to be meeting the goals of the patient to maximise their motivation.” Professor Ponsford says in the early stages of recovery, people with TBI often have little awareness of the changes to their brain function, which can create challenges for rehabilitation. “They can’t see the reason why they are in rehabilitation - they just want to get out and go home. It can be very easy for therapists to set all their goals for them and then the person with a brain injury is not really kind of with them on the same page.” As a result, Professor Ponsford says it’s imperative clinicians engage the patient’s family from the beginning of recovery, and throughout the rehabilitation process. “Clinicians need to understand the perspective of this young person who has got all the sort of goals and aspirations in life that they had before they had their injury, and who really have no idea of what lies ahead of them in terms of how the effects of this injury is going to impact on them,” she says. “They need to get alongside that person, and go forward with them, in setting the goals of rehabilitation.”

Research and treatment The centre’s research is now pointing to a greater understanding of effective treatments for people with TBI. Professor Ponsford says psychologists are now being encouraged to use an adapted form of Cognitive Behaviour Therapy that taps into teaching people with TBI more repetition, using handouts and shorter sessions, to reduce anxiety and depressive symptoms. Some of the centre’s other treatment studies focus on tackling the significant problem of fatigue and sleep disturbance in people with TBI. Professor Ponsford says the centre is studying the impact of melatonin on sleep difficulties and light therapy for fatigue. “Light therapy is exposing the person to short wave length light in the morning,” she says. “It helps them be more aroused for the rest of the day and reduces their fatigue and day time sleepiness. “The other study is doing psychological therapy for people with fatigue and insomnia helping them to better pace their lifestyle and manage their sleep problems. “We’ve also just published some papers that are showing that that is also effective in helping their fatigue and sleep problems.” Another large study, funded by the Institute for Safety, Compensation and Recovery Research (ISCRR), provides positive behaviour support intervention to people with severe behaviour problems in the community. “I’ve got six clinicians who are psychologists or occupational therapists or speech pathologists who work with us to deliver the therapy, and it’s about going into people’s homes and creating a more positive environment around the person, helping make them more motivated and changing the triggers of their behaviour around them.”

How nurses are tapping into smart bedside solutions At Epworth Eastern, nurses are using an innovative Point Of Care (POC) bedside system to access patients’ pathology results, streamline discharge processes, and provide patient education. This Oneview Healthcare technological solution, introduced several years ago at Epworth Eastern, is optimising patient engagement, clinical outcomes and workflow efficiencies. It’s been such a success, the system has been rolled out to thousands of bedsides spanning all Epworth campuses of Victoria’s largest not-for-profit private health care group. The technology not only provides a range of entertainment, health care education and information services for patients, it also provides clinical staff with access to a range of applications, from electronic prescribing and administration to electronic nurse rounding, managing room cleaning services and electronic meal ordering. The system uses 22 inch Cybernet terminals at the patient bedside which are linked to nurse station locations. Clinical staff log on to the bedside terminals using thumb print recognition technology. Epworth Eastern Acute Surgical Ward Nurse Unit Manager Ian Kite says the digital technology is revolutionising the way nurses manage patient care at the bedside - all at the touch of a button. “You can log in to a nursing access screen which gives you an overview of where your

patient is currently sitting from a medical acute surgical prospective in terms of their condition,” he says. “It gives you access to the recent pathology results, access to their recent tests and investigations, that can be in the form of an X-Ray or a CT or an ultrasound, so you can physically see what tests patients have had and what their results may well be. “It gives you the ability to update patient requirements in terms of dietary intake, and certainly from my nursing perspective, my patients have had acute abdominal surgery. “They need to start off on a particular type of diet and they need to progress upwards, so from fluids to a different type of fluid, and then through to a meal. “It gives the nursing staff that ability to make that live, which then informs the catering department, as an example, that the menu access has changed and they need to then update what the patient can have for their meals.” Mr Kite says the system’s online electronic drug chart works to reduce the risk of medication errors and improve patient safety. “Obviously drug charts previously were written by doctors and anaesthetists with handwritten skills that could be interpreted as not very good,” he says. “Now they electronically type in what medication they want, the frequency, the dose, any variations or variables to the medication.


and refer to rehabilitation, confirm appointments and print out information for patients. It also assists with patient flow, enabling nurses to discharge the patient directly from POC. Mr Kite says the digital process reduces the amount of phone calls to other departments and accelerates and streamlines discharge process efficiencies. “That alerts the kitchen department so that they then don’t put together the next meal for the patient…and that limits waste,” he says. “You can log on and request that the cleaning department comes through and perform a discharge clean on the room - that seamlessly improves the progression of your next patient coming in from theatre or from home, because the room is ready and in an appropriate time and fashion.” The system caters to nursing documentation but Mr Kite says nurses and doctors are currently using handwritten documentation for their clinical and risk assessments. Mr Kite says the digital POC system has unlimited potential. “It gives nursing staff more time, I would say, to perform the clinical aspect of the job,” he says. “Previously you might not have had the option to look for your pathology results. From the POC terminal, nursing staff can review the pathology results and they can see what potassium levels are doing, and what haemoglobin levels are doing. “They can put together what is happening from a pathological perspective, and relate it to their patient, who is lying in their bed in front of the screen,” he says. “That can have an impact on the patient care and the patient condition because if you can see that the blood count is dropping, and the patient is physically looking as if they are deteriorating, that escalation process can follow.”

“In terms of dosing, it’s a more accurate way of establishing when a patient’s had a medication and when they are next due to have a medication, and it gives you an alert to say that something’s due.” POC also assists nurses to complete their hourly rounding, where nurses log on to the system to ask a series of questions and record the answers, which is designed to eliminate risk and improve patient safety. Nurses question patients about whether their possessions or the call bell are in reach, ask about pain levels, whether the patient has moved recently as part of their pressure area care, or whether patients have been to the toilet. “This has a big impact with my particular patient cohort, who are waiting a lot of the time to pass urine or to have their bowels opened, so it gives you a tracking device as to where they are with their acute care,” Mr Kite says. “If we are not asking these questions on a regular basis, patients may well just choose to get up and go to the toilet themselves, and that way if they’re of a particular age or if they’ve got particular conditions or comorbidities there’s a falls risk. “If you’ve had an operation, you’ve got an anaesthetic on board and that can limit your balance and your ability to move, so we need to make sure we’re keeping patients safe by reducing risk, and obviously we do that by assisting patients to and from the bathroom. “The impact it has on the statistics of falls… certainly in our organisation it has reduced the amount of falls in hospital, which obviously is improving patient outcomes.” POC features leader rounding, where the nurse in charge will visit each patient daily to discuss with them their stay and record any patient feedback, which can escalate any patient concerns or potential problems, and report positive feedback to staff. The system boasts discharge planning, where clinicians can plan ahead for patient discharge

In 2014, people aged 55 and over showed the largest increase in app downloads.
