Guided theatre experience vital for nurses By Karen Keast
A
ustralia’s undergraduate nurses should have guided operating theatre experience as part of their preparation for surgical ward nursing, new research recommends. A study of 350 nurses across Australia showed undergraduate and graduate nurses with guided operating room experience had significantly more knowledge when it came to surgical ward nursing. Operating room nursing was removed from the undergraduate nursing core curriculum in 1994. In the aftermath of a major surgical incident more than 10 years ago, questions have been raised about the ability for nurses to care for pre- and post-operative patients on acute surgical wards without the knowledge and expertise gleaned from guided operating room experience. Dr Paula Foran, an honorary clinical senior lecturer at Deakin University’s School of Nursing and Midwifery and Deakin’s School of Medicine, examined whether nurses with non-guided or no operating room experience could care for patients before and after surgery with the same level of knowledge and expertise compared with nurses with a guided learning experience. Her PhD study, a quantitative analysis recently published in Nurse Education in Practice, knowledge-tested undergraduate nurses at the end of their training and again at the end of their graduate year on areas of post-operative care. The study compared participants’ knowledge to the model of operating room education to determine whether there was a correlation between the two. Dr Foran found undergraduate nurses with guided operating room experience had a 76

per cent pass rate compared to 56 per cent for non-guided learners, while graduate nurses with guided operating room experience as an undergraduate or graduate nurse had a 100 per cent pass rate compared to 53 per cent for nonguided learners. Dr Foran, the education officer at the Australian College of Operating Room Nurses (ACORN) and a perioperative nurse with 30 years’ experience, said the results showed guided operating room nursing experience supports best patient outcomes in surgical patients. “I can assure you that novice nurses learn more about post-operative surgical nursing working in an operating theatre and a surgical ward than if you’ve just worked in a surgical ward alone,” she said. Dr Foran said operating room experience provides transferable skills in pre and post-operative patient education and care, acute pain management, and asepsis. “The operating theatre is an area of massively rich learning - for every case when patients are in theatre and in PACU, we are always thinking about the possible complications of that case and looking out for them,” she said. “Many novice nurses do not go to theatre any more but it’s the middle part of a three part process and so if you’re looking after a patient post-operatively, to have actually seen and understand what they’ve had done in the operation is really valuable.” Dr Foran said a lack of operating room education and experience was also impacting on the specialty’s ability to recruit and retain nurses.

New standards for perioperative nurses The Australian College of Operating Room Nurses (ACORN) has unveiled five revised standards for perioperative nursing. The standards in infection prevention, perioperative attire, fatigue, environmentally-sustainable perioperative practice and asepsis have been updated in line with the latest evidence-based research. ACORN education officer Dr Paula Foran, a perioperative nurse with 30 years’ experience, says the revised standards keep perioperative nurses at the forefront of best practice. “New evidence is being developed all of the time,” she says. “Three of the five reviewed standards are specifically related to the prevention of infections for patients undergoing surgery. “Preventing infection is a huge issue. Once you cut into someone’s skin they have the potential to develop an infection and that is why perioperative nurses have very specific standards of practice that guide infection prevention in our patients. “And unfortunately when bacteria mutate, they don’t send us a text message to warn us,” she adds. “We have to be ahead of pathogenic changes all of the time to prevent infections in our patients, so when there’s new evidence we will update Standards.”

Dr Foran, who holds an honorary position at Deakin University’s School of Nursing and Midwifery and a position at Deakin’s School of Medicine, says reviewing the standards for perioperative attire is another chance to remind nurses about the importance of infection control standards. “Times change and the College will update Standards accordingly,” she says. “Years ago we used to wear boiler suits, they were one piece outfits and now we’ve gone for the pyjama style because we know that they shed less bacteria. “We know that different types of pathogens can be harboured by hair which is why we have hair covered in the operating theatre. “Staff like to wear their own coloured hats which is lovely and the patients love that too but we have to remind our staff then they should launder those hats in their own health care facility, not to take them home. “A lot of our policies are not just about preventing infections for the patients, but they’re about preventing infections for ourselves too.” ACORN is the largest specialty nursing college with more than 3500 members. The standards review is a rigorous process where teams of experts in the specialty field, including academics, clinicians, educators and industry representatives, oversee the development or review of several standards each year. Dr Foran says ACORN then holds live webinars and recordings of education sessions to educate perioperative nurses up to speed on the changes, in a bid to assist compliance in the workplace. She says other standards earmarked for upcoming review include the role of the anaesthetic nurse, handling of loan equipment, and surgical scrubbing, gowning and gloving.
“There’s new information in scrubbing, gowning and gloving - that ritualistic operating theatre behaviour to prevent infection,” she says. “There is now lots of evidence-based practice information on the scrub process. For example, we would normally scrub for five minutes for the first of the day and subsequently for three minutes thereafter. “Now, we’re actually looking at waterless scrub solutions that have been used overseas, so all of these sorts of things will be incorporated into the new standard. “We’re also writing a policy on loan equipment - how to deal with it, how soon should it arrive prior to surgery so it’s ready for the patient’s procedure, and how to make sure that it’s cleaned and sterilised appropriately. “It’s a big business organising the loan equipment.” In another major development this year, ACORN has partnered with the University of Tasmania to offer postgraduate nursing specialisation courses in perioperative nursing, anaesthetics or postanaesthesia nursing. The University of Tasmania is providing full scholarship for the online courses for ACORN members, covering all HECS fees. Dr Foran says perioperative nursing is a “fantastic” career path. “I really love it,” she says. “It’s an area where the technical nature of perioperative nursing sometimes leads people to believe that there isn’t as much nursing care by the nurses that work there, that it’s a more technical role than a nursing role.
“But in fact, I would argue really strongly that the more technical the operating theatre has become, the more perioperative nurses have risen to the occasion and provided more nursing care, and that perioperative nurses are caring for patients and nursing them all the way through their perioperative stay. “Unfortunately because of the drugs we use in anaethesia, patients often don’t remember the care that they’ve had with us - we’re a little bit invisible. “People will say - I was asleep the whole time, but when they’re there, we might be chatting to them at lots of different points. “Certainly there are nurses there holding patients’ hands, explaining what is going to happen and, relaxing them prior to surgery.” Throughout the operation, perioperative nurses are very heavily involved in preventing conditions such as deep vein thrombosis, hypothermia and pressure areas. That monitoring continues in the post anaesthetic recovery unit, where perioperative nurses are overseeing the condition of patients, before they move into the ward. “The more technical that theatres become, in my opinion, the more nurses’ nurse,” Dr Foran says. “We work as the patient advocates. We speak up for the patient at all sorts of levels. “Patients really need to know that when they’re having an operation, and feel at their most vulnerable, perioperative nurses will be there caring for them.”

idwives have more opportunities than ever before to access their education at home, at the click of a button. With rapid advances in technology, elearning for midwives has become user-friendly while also enabling midwives to access education right at their fingertips, whenever and wherever they choose. The Australian College of Midwives (ACM) has launched a new course on supporting women to breastfeed as part of its ever-changing and growing suite of elearning education, Midwives Learn. ACM education unit midwifery advisor Ruth King says the breastfeeding course links in with ACM’s role as the governing body for the Baby Friendly Health Initiative (BFHI). The initiative is designed to protect, promote and support breastfeeding through recognising and awarding accreditation to maternity facilities that demonstrate best practice in breastfeeding. Nationally, the program has accredited more than 70 BFHI maternity facilities. “As part of the initiative, everyone who undertakes the BFHI assessment needs their staff to undertake breastfeeding training,” Ms King says. “Breastfeeding is key to midwifery - it’s one of the core outcomes. We would really like all women to breastfeed their babies or have the opportunity to breastfeed - it’s obviously not appropriate for everyone.

“To facilitate that we thought it was beneficial to have an education course that provided information for midwives, mainly refreshing their knowledge. “So, it’s not at the level of training for lactation consultants but it is at a level of providing that refresher information for midwives on breastfeeding, best techniques, World Health Organisation and Australian guidelines from the National Health and Medical Research Council (NHMRC). “Providing consistent information in one course we hope will ensure that midwives have best practice and mothers have optimal choice and options when it comes to breastfeeding.” With midwives playing a pivotal role in the care, information and emotional support for new mums, the eight CPD recognition point course is designed to equip midwives with the necessary knowledge and skills to assist women to begin and continue to breastfeed their babies. The course covers current breastfeeding practices of Australian women and strategies designed to protect, promote and support breastfeeding at a national and international level, while it also identifies and reflects on factors that impact on breastfeeding decisions and practice.

New physiotherapy treatment for incontinence after prostate cancer University of Queensland researchers are working to change the way physiotherapists provide treatment for prostate cancer patients experiencing incontinence after prostatectomy. In the first international work of its kind, Professor Paul Hodges, a physiotherapist and National Health and Medical Research Council (NHMRC) Principal Research Fellow, is leading a team of researchers who have identified a new way to assess and train men’s pelvic floor muscles. “Several years ago, we realised not only do men have a very high prevalence of incontinence after prostate cancer, but in the research world the understanding about how men actually recover continence after they have surgery is far from complete,” he says. “The problem is - you’ve got cancer in the prostate, they remove the prostate and varying amounts of the structures that normally control continence. Basically you lose a lot of the mechanism that helps to automatically keep the urine in the bladder. “Normally your continence is controlled automatically, your nervous system just does it. These muscles automatically contract and stop the urine from leaving your bladder. “Surgery to remove the prostate interferes with much of that mechanism and all of a sudden - how are you going to keep continence?” Professor Hodges says their research shows men have a series of other muscles which can also control continence, but men have to train to use them in a new way. It’s also been challenging for physiotherapists to assess whether men are using the right muscles. About five years ago, a Cochrane Review publication argued physiotherapy treatments for men with incontinence after a prostatectomy were unsuccessful.

UQ researchers believe their studies reveal why the treatments were not working – older studies have been using assessments that measure the wrong muscles, those designed for fecal continence not urine continence. “Men and women have a series of other muscles that are available to help contribute to maintaining urine in the bladder,” he says. “Our research shows that these muscles normally help to control continence in challenging situations like coughing. You can also voluntarily contract these muscles, you can turn them on. “What we’ve identified is that these muscles are really important for controlling continence in demanding situations.

Psychology researchers investigate spirituality
N
ew Zealand psychology researchers are putting the effects of near-death experiences, out-of-body experiences and meditation experiences under the microscope. Massey University psychology lecturer Dr Natasha Tassell-Matamua and psychologist Dr Karen Frewin, a senior lecturer in counselling and guidance at Massey’s Institute of Education, are investigating the psychological impacts of spiritually transformative experiences in a way to better inform psychology practice. Dr Frewin said the study will provide a foundation for bridging the competence and training gap for health professionals, including psychologists, when it comes to addressing clients’ concerns around spiritual experiences and how they impact on their lives. “What we do know is that often these experiences precipitate a variety of life changes and pervasive psychological shifts in those who have them,” Dr Frewin said. “Positive implications can include increased quality of life and perceived wellbeing. Often a greater sense of spirituality is also a consequence.” Spiritually transformative experiences can also be confused with mental illness, she said. The researchers are calling for New Zealanders to participate in the study. They will collect and analyse data from participants about the types of spiritually transformative experiences in New Zealand and whether participants seek assistance in the wake of their experience.

“We are interested in finding out who has these experiences, how they are described, and the ways people who have them integrate them into their lives,” Dr Tassell-Matamua said. “The positive and negative implications are of equal importance to us. We want to gain a snapshot of how ordinary New Zealanders view spiritual experiences and how they are transformed by them.” Dr Tassell-Matamua, an expert on near-death experiences, said spiritually transformative experiences can range from near-death experiences, such as a sense of leaving the body or travelling down a tunnel to being absorbed in a bright light, while near death-like experiences focus on the same phenomena but without the physiological danger of dying. Other spiritually transformative experiences can include out-ofbody experiences, meditation experiences and kundalini, which is achieved through yogic meditation. Earlier Massey University research on neardeath experiences has studied the sociological and psychological perspectives surrounding the phenomenon and also the life-changing impact of near-death experiences. Research has also investigated how well end-of-life and palliative care models cater for Maori culture, spiritual beliefs and traditions.

Nurses Living and Working In Abu Dhabi - Rachel Rowson’s Story Where are you from? Manchester, England. When did you travel to Abu Dhabi? I came to Cleveland Clinic Abu Dhabi in February 2015. Why did you choose this side of the world? I have a friend that works in Saudi Arabia and ever since she went there almost 4 years earlier, I had been interested in going. I was keen to work abroad as this was something I had never done and the thought of having nice weather and the financial incentives were all very attractive to me. In August I got a call from CCM Recruitment International asking me if I was still interested in working in the Middle East. At the time, I was taken by surprise but I was told that there was a recruitment day being held in two days’ time. I thought I had nothing to lose so went along. When they showed the CCAD empathy video, I suddenly realised how much I wanted this. The film really touched me and I saw aspects of the nurse I’d always aspired to be in the video. Why did you choose CCAD? Cleveland Clinic has a brilliant reputation. I learnt more about the intention of CCAD, which is to bring world-class healthcare to the UAE and surrounding countries in the Middle East. Currently, the population usually has to travel abroad to receive healthcare when they are seriously unwell. I really wanted to be a part of the project as

I could relate to patients that had travelled long distances to receive care at the hospital where I was working. Would you encourage others to make the change? Why? People say that I am brave for making the change, but it’s one of the best things I’ve ever done for myself. The things that I have experienced so far are life changing, and has made me grow as a person. I can’t recommend it enough! Culturally how have you found it? I love working and learning about different cultures. The local people are so welcoming. You must be respectful to their culture and religion, but they are so open to expatriates living in their country. It’s surprising how quickly you pick up small words that help you get by.
