How nurses can be more mindful in their practice

T

aking three sighs is a mindfulness technique nurses are using in their everyday practice to cope with the pressures of their high-stress work environments. Far from being the latest zen trend to counter our fast-paced and plugged-in lifestyles, Australian researchers are investigating the benefits of mindfulness for nurses to prevent burnout and compassion fatigue. Dr Mark Craigie, a privately practising clinical psychologist who specialises in mindfulnessbased cognitive therapy, says nurses choosing to sigh when they notice they’re getting tense or entering into a stressful situation can help them remain focused on the task at hand. “You take a normal in-breath and then a very long slow out-breath and then on the out-breath you sigh just gently, and focus on the feeling of the out-breath as you sigh,” he explains. “That’s just enough to switch you away from being overreactive to a more balanced level of reactivity, so it helps you relax just a little bit. “Just redirecting the attention to something very simple, the feeling of your breath as you sigh on the out-breath, that’s an extremely easy thing to do. “As you get better at doing that you might only need one sigh to relax your body.” Dr Craigie and Associate Professor Clare Rees, of the School of Psychology and Speech Pathology at Curtin University, are part of a team

who are trialling the benefits of mindfulnessbased interventions, specifically designed for nurses, as part of a study at Perth’s Sir Charles Gairdner Hospital. Their research team is equipping about 90 nurses at the hospital with a range of skills to practice efficient and effective mindfulnessbased interventions, including the three sighs technique, as part of the controlled trial. After the completion of the trial, researchers hope to roll-out mindfulness-based techniques to other health services nation-wide and will also trial an online program of mindfulness training. Assoc. Professor Rees, who leads the International Collaboration on Workforce Resilience which is focused on building nursing resilience, says mindfulness can be particularly beneficial to nurses who are often at risk of stress, anxiety, depression and burnout. “A lot of people get too caught up in things, they can’t distance themselves from what’s going on, they get overly absorbed and reactive but you actually can step back and be more of an observer, and in touch with your own values and intentions,” she says. The group-based intervention comes after an initial pilot study of about 20 nurses at the hospital returned significant outcomes, from reducing nurses’ levels of stress to improving their workplace satisfaction.

Research investigates the power of mental imagery

F

or years, sports psychologists have encouraged athletes to use mental imagery to improve their goals and performance. Now a team at the University of Queensland is grounding the principle of the power of mental imagery in research to drive people’s behaviour change, around smoking or alcohol addiction, to reduce their high-calorie food intake or increase their exercise. Professor David Kavanagh, a clinical psychologist who leads the ePsych research group at QUT’s Institute of Health and Biomedical Innovation, says Functional Imagery Training is being used to assist people participating in several trials to lead a healthier life. “We discovered that when people are wanting something or desiring something or they’re motivated to get a goal, it’s a very sensory experience,” he says. “The thought is embodied, in the sense that we feel it in our bodies, we have imagery about it. “If you’re wanting to have a cup of coffee, if you just think the words - a cup of coffee, it’s not really all that motivating. “But if you think about what it’s going to smell like, what it’s going to taste like…the sensation of swallowing it and the fact that you’re going to feel energised if you have the coffee, that is really very motivating. That is the core thing to the whole idea.” Professor Kavanagh says imagery can be pulled from memories and enhanced.

“If you’re walking past the coffee shop and you smell the coffee, suddenly you get a whole lot of memories, you don’t have to work at that,” he says. “But after you get those, given that it’s pleasurable, you start actually elaborating that image and you think about exactly what it’s going to look like and what it’s going to taste like and smell like and so on. “You work at that and that takes limited memory capacity and particularly it takes the kind of memory capacity that’s about visual spatial working memory, the area of working memory that you require to make visual images. “That’s an important insight because what we’ve discovered is that it’s possible to interfere with that - so if you want to be resisting having a cup of coffee, if you do another task which also needs that same limited working memory capacity, then it blunts the image of the coffee and it also reduces your desire for the coffee at that point. “It moderates it a little bit so maybe you can cope with it a bit better.” While some of the research has focused on imagery to interfere with problematic desires, the research also looks at how to increase desires using a similar approach.

Osteopathy fastest growing allied health profession By Karen Keast Osteopathy, podiatry and occupational therapy are the fastest growing allied health professions in Australia. Australian Health Practitioner Regulation Agency (AHPRA) statistics show in 2014-15 the osteopathy profession has grown 6.8 per cent, with podiatry increasing 5.9 per cent, followed by occupational therapy at 5.7 per cent. While still small in national numbers with a total of 2000 registered osteopathy practitioners, osteopathy is predominantly a young health profession with most osteopaths aged in the 2529, 30-34 and 35-39 age brackets. More than half of osteopaths practice in Victoria, at 52 per cent, with 28 per cent in New South Wales, and nine per cent in Queensland, while no osteopaths have their principal place of practice in the Northern Territory. It’s a profession almost evenly divided among the genders - almost 54 per cent of osteopaths are female and 46 per cent are male. There are now 4,386 podiatrists, with most, or 912, aged 25-29, while 61 per cent are female. In the occupational therapy profession, there are now 17,200 registered OTs, with most, or 3,808 aged 25-29 and 3,356 aged 30-34, while an overwhelming majority are female, at 91 per cent. AHPRA statistics also show the midwifery and Aboriginal and Torres Strait Islander health practice professions had the largest growth for the year, each standing at 12.3 per cent. There are now 29,903 practitioners registered as a nurse and midwife and 3,641 registered as a midwife while the overall number of nurses and midwives stands at 368,087. Statistics show there are 292 eligible midwives, and 169 eligible midwives able to

prescribe scheduled medicines, alongside one midwife practitioner. Most general registration nurses and midwives, or 13 per cent, are aged 55-59 and 50-54, at 12 per cent, while 10 per cent are aged 25-29 and 11 per cent are aged 30-34, while more than 89 per cent are female and over 10 per cent are male. There are now 391 Aboriginal and Torres Strait Islander health practitioners, with most aged 45-49, more than 75 per cent are female, and more than 55 per cent practice in the Northern Territory, 12 per cent in Western Australia and Queensland, and just one per cent in Tasmania and the ACT. In other developments, the National Boards have reduced practitioner registration fees for professions including occupational therapy, medical radiation practice, optometry, osteopathy, physiotherapy and podiatry, while fees have been frozen for Aboriginal and Torres Strait Islander practitioners, in Chinese medicine, and in nursing and midwifery. Registration fees have been increased in the chiropractic, dental, medical, pharmacy and psychology professions to meet national CPI. With the national scheme funded by practitioners’ registration fees, AHPRA CEO Martin Fletcher said the fees for 2015-16 reflect the National Boards’ and AHPRA’s commitment to prudent financial management. “The National Boards and AHPRA have set fees so that we can meet our regulatory responsibilities under the National Scheme, while also aiming to be effective and efficient regulators of registered health practitioners, and protect the public,” he said.

Medical concierge lends patients and older people a helping hand

H

elping patients is no new feat for Marg Sacre. But instead of nursing them back to good health, the retired nurse with more than 40 years’ experience is helping patients, and older people living in the community, to navigate the nation’s complex health care system. Ms Sacre has joined forces with former Wall Street lawyer Susan Turner-Kapsanis to launch an Australian-first medical concierge service. The Medical Concierge will select and organise specialist doctors and treatment programs, transport patients to appointments, attend specialist appointments, arrange flights, accommodation and meals, source home nursing, and assist patients with benefits from their private health insurers and Medicare. The Sydney-based business also has an arm called The Home Care Team that provides health assessments for older people living in the community and links them into essential services. The Home Care Team will work to assist older people to remain living in their homes through the provision of health assessments, early intervention and preventative care. “We just keep finding there are so many people who are neglecting their health or old people at home, diabetics for instance - who are losing their toes because they haven’t seen a podiatrist and no one has organised it for them,” Ms Sacre said.

“We are now planning to do aged care assessments for GPs as well in people’s homes because at 75 everybody can have a health assessment, and there’s lots of services out there to help them, it’s just accessing them is often complicated.” Ms Sacre is a former endoscopy nurse unit manager at St Vincent’s Clinic and has also responded to international emergencies as a volunteer with the Australian Red Cross, including Aceh in the aftermath of the 2004 Boxing Day tsunami. But even Ms Sacre, a former director of the education branch of the Gastroenterological Nurses College of Australia (GENCA), found the health care system challenging when her husband, Howard, was diagnosed and treated for cancer. “I took three months off work to look after him, and he was going to surgery and radiotherapy and chemotherapy, and the bills kept coming and the claiming was a nightmare,” she said. “Just getting to appointments and working out who I should go to - was this the best person to deal with that? “I remember thinking one night - if I wasn’t a nurse, how would you ever do this?” With her experience as both a care provider and a consumer in the health system, Ms Sacre found friends and family were constantly asking her for advice about doctors and health care.

Golden future for Australian psychologists Australia’s psychologists will reflect on the past and be inspired to develop plans for the future at the Australian Psychological Society’s (APS) gold-laden 50th annual conference this month. More than 1000 psychologists are expected to attend the Golden Jubilee Conference, to be held on the Gold Coast from September 29 to October 2. Conference chair Professor Paul Martin OAM, a health and clinical psychologist with 40 years’ ex-perience, said the milestone event will focus on the theme, ‘Celebrating the past, looking toward the future’. “I always think when you have these big anniversary events it’s always a time for reflecting on the past and what has been achieved, and what you would have liked to achieve but not succeeded in doing, and using that as a platform for developing plans for the future,” he said. “I think it’s a great opportunity for updating our members about the most recent developments in psychology but also stimulating their thinking and activating them.” Renowned American psychologist Professor Carlo DiClemente, the director of the MDQUIT To-bacco Resource Center and the Center for Community Collaboration at UMBC, is one of the key-note speakers. Professor Martin said Professor DiClemente stands in the top 100 of the most cited psychologists of all time. “His work has a huge influence - so sure he’s a psychologist influencing the work of psychologists but he influences the work of doctors, particularly GPs,” he said. “What his work is about is how ready you are to change and this has become a huge thing in the health field because when

people go and see the general practitioner, so often what the GP really wants to say to them is something like - you’ve got to give up cigarette smoking, you’ve got to lose weight, you’ve got to drink less alcohol. “What DiClemente’s work is about is saying, well - the way you go about that depends on how ready they are for change so, for example, at one extreme - have they never thought about giving up smoking or are they at the next stage, where they’ve thought about it but just never quite devel-oped the plan, or the next stage - they’ve tried but they just haven’t succeeded. “Having someone like him deliver a keynote, it’s very special because he’s just not a prominent psychologist; he’s one of the most prominent psychologists.”

University launches MOOC to bridge the Dementia divide

T

he University of Derby has recently launched a new free Massive Open Online Course (MOOC) titled Bridging the Dementia Divide to inform family members who want to care for someone living with Dementia and as a CPD tool for health and social care professionals. The MOOC will be introduced from July 13, 2015 and have been developed jointly by the University of Derby’s recently established Academic Innovation Hub and the University of Derby Online Learning (UDOL) in conjunction with the University’s College of Health and Social Care. MOOCs are short, free courses run by major learning providers to share their knowledge of a particular academic topic or to help students develop a specific skill. There are more than 342,800 Australians living with dementia, and the number is expected to increase to 400,000 in less than ten years. Each week, there are more than 1,800 new cases of dementia in Australia, approximately one person every 6 minutes. Course Instructor and Academic Lead for Health and Social Care at UDOL David Robertshaw, said: “Dementia is a syndrome, usually of a chronic or progressive nature, caused by a variety of brain illnesses that affect memory, thinking, behaviour and ability to perform everyday activities. “The number of people living with dementia worldwide is currently estimated at

47.5 million and is projected to increase to 75.6 million by 2030. Dementia is the third leading cause of death in Australia (the second leading cause in women) and there is no cure. If dementia were a country, it would be the world’s 18th largest economy. Dementia is overwhelming not only for the people who have it, but also for their caregivers and families. “There is a lack of awareness and understanding of dementia in many countries, resulting in stigmatisation and barriers to diagnosis and care, impacting caregivers, families and societies physically, psychologically and economically. This course provides a platform for people to come together to make the World a better place for people living with dementia.” David is a UK Nursing and Midwifery Council Registered Nurse with a background in adult acute care. He maintains strong links with practice and local hospitals, and is Programme Leader for three programmes at UDOL including: BSc (Hons) Nursing Studies top up, MSc Integrative Health and Social Care and a University Advanced Diploma in Dementia Care. The six-week free course will help you learn how to improve the lives of people with dementia, adopting a person-centred and integrated approach. The units include: • Introduction to dementia • Communication and compassion • Independence, control and quality of life


This course is suitable for anybody who is interested in dementia and its effects on society, culture and economics, including health and social care professionals, policy makers and families of people with dementia. It will enable learners to develop a better understanding of communication and compassion, and how you can help the person living with dementia to maintain independence, control and a good quality of life. Munib Hadi, Head of the Academic Innovation Hub at the University of Derby added: “One of the primary reasons we undertook the MOOCs project was to reduce the barriers to Higher Education and give a flavour of the high quality teaching on offer at UDOL. As part of our open education strategy our courses have no prerequisite requirements. “Even though they are free courses, University of Derby MOOCs have a gone through a similar quality review process as our paid for courses. This ensures our MOOC learners get a quality learning experience. “Typical retention for MOOCs is around 5%. At the University Derby we believe the traditional methods of calculating retention calculation are obsolete in the MOOCs context due to the free and drop in/drop out nature of some of these courses. A great deal of learners who log on to MOOCs do learn but don’t have the time to complete a typical six to eight weeks course. “With this in mind we are developing tools and mechanism around our proposed MOOCs to record these micro learning achievements. As part of this approach our learners will be able to gain digital badges even if they are completing part of the course. This would interest learners who enrol on MOOCs to learn about specific aspects of a course but are not interested or don’t have time to complete the whole course.”

The University has also launched a second MOOC titled Digital Me: Managing Your Digital Self, which was launched on July 6 and is aimed at anyone wanting to manage their digital presence and profile. All MOOCs will be delivered through Canvas Network, the second largest provider of free online courses in the world. Canvas Network specialise in delivering MOOCs to learners all over the world, helping to break barriers to learning, efficiently, safely and responsibly. Upon completion of the courses an E-Certificate will be issued as evidence. Enrolment is now open.
