Ratio Win for NSW Nurses as Negotiations Continue Published NCAH.com.au 19th January, 2011 Nurse to patient ratios will be delivered to the majority of nurses in NSW hospitals, under a State Government offer still being considered by the NSW Nurses Association. The Association yesterday revealed it would continue to seek further improvements this week to the government’s initial offer on nurse staffing levels and nurse-to-patient ratios but it says the offer, in many areas, is “better” than working conditions in Victoria. The offer proposes that ratios be recognised as equivalents to Nursing Hours Per Patient Day (NHPPD) in the award. The pay offer, which remains the same, is a three year offer back paid to last July, with a 3% increase from this July and a 2.5% increase from July 2012. Association general secretary Brett Holmes said the offer was a major improvement thanks to the ongoing industrial action, which resulted in more than 570 beds across 67 facilities being closed.

“The current offer involves improved staffing and transparency for most wards in all acute hospitals, large and small,” he said. “This is a good start. It is an acceptance by the government that mandate, minimum nurse-to-patient ratios are important if we are to improve the quality of the NSW hospital system.” Mr Holmes said while the government acknowledged more discussion was needed in the areas of emergency departments and community health services, the union also believed there was more room for negotiation on mental health and operating theatres. More than 220 workplace delegates from public and private hospitals, community health services, aged care facilities and other health care facilities from across the state attended Tuesday night’s meeting. The council will meet on Tuesday, January 25, to consider whether to take further industrial action or put the offer to a membership ballot.

Specialist Physios Must be Recognised: APA The Australian Physiotherapy Association is calling for the registration of specialist physiotherapists.

Ms Locke said it was also vital that physiotherapists were involved in the governance of the new Medicare Locals.

APA president Melissa Locke said the move was important for the profession in terms of recognition and career pathway but also valuable in ensuring patients had access to the right physiotherapist at the right time.

“The creation of Medicare Locals and their proposed structure and function is an opportunity to get more recognition of the vital services that physiotherapists provide as primary contact professionals,” she said.

“We are advocating to State and Federal Governments for the permanent recognition of Fellows of the Australian College of Physiotherapists in a similar way that medical specialists like paediatricians and surgeons are recognised,” she said.

“They have a lot to give to the community in terms of getting people active to help address the underlying causes of chronic conditions such as cardiovascular health and diabetes.”

Ms Locke said the Physiotherapy Board of Australia had made the recommendation, which was now being reviewed by State and Federal Governments. “We expect a consultation process to begin in March next year, and will continue to tell politicians why specialisation is so important to the public and the profession,” she said. www.ncah.com.au

Ms Locke said the sector also needed workforce reform in a bid to reduce funding, legal and cultural barriers to foster more innovative practices. She said physiotherapists were forced to send their patients to GPs to order ultrasounds or to get referral letters for medical specialists such as orthopaedic surgeons.

The tragic death of Bob Fenwick, a 63-yearold mental health nurse killed at work in New South Wales last month has again raised the issue of occupational violence, which nurses and other health professionals face daily in their work.

When the patient attacked Emily Pritchard, a 20-year-old nurse on duty with Mr Fenwick, he went to her aid and was fatally stabbed. At his private funeral service, Ms Pritchard paid an emotional tribute to his bravery and selflessness in saving her life. At the end of the service, his nursing colleagues formed a guard of honour as his coffin was carried out.

With the introduction of national registration, the ANF is lobbying hard for a federal response to workplace violence, which will hopefully see uniform regulations set in place to counteract it.

While there have been no details released on how the patient obtained the weapon or even what sort of blade it was, there will be at least three separate investigations into Mr Fenwick’s death by the NSW police, the coroner and by the Western Local Health Network.

The New South Wales Nursing Association has (strictly limited places) received reports of other nurses being attacked in the workplace. In October 2010, a nurse working in the psychiatric emergency care centre at Blacktown Hospital was brutally assaulted while trapped in the nurses’ station by a mental health patient. Two years earlier, a male doctor had been cornered in the same small room by a patient who stabbed his face, causing severe injuries requiring extensive facial surgery.

Seventy per cent of organisations had a written policy that addressed occupational violence, which ranged from one paragraph to 15 pages of detailed explanation of policies and procedures, but it was not possible to determine whether they were successful or if the number of incidents of violence and bullying incidents had decreased. All health services had an incident form of some type, although these varied significantly. Some were simple one-page forms with time, date and incident headings. Others were pages long, with extensive tick boxes and follow-up forms, as well as detailed instructions on how to fill out the form and the procedures that would follow its completion. Although not all incident forms covered near misses, verbal abuse or bullying, they all covered physical violence. Metropolitan and rural health services showed significant differences in policies to address violence and bullying, although these could often be linked to scale and resources. number of programs and procedures in place to deal with occupational violence and bullying, and a greater promotion of these policies than anticipated. Rural health agencies appear to rely heavily on the organisational orientation program to inform employees of current policies. Mental health facilities showed the greatest degree of evaluation of the effectiveness of local policies for the prevention and management of occupational violence and bullying. They also demonstrated the greatest amount of promotion of occupational violence strategies, using several media to make sure that all staff members were fully informed. Responses from aged care facilities showed a very different picture, with the smallest number of mechanisms used for the prevention and management of incidents of occupational violence and bullying.

ANF Slams Productivity Commission Report The Productivity Commission has ignored calls for urgent workforce reform in the aged care sector, according to the Australian Nursing Federation. The ANF has slammed the commission’s Caring for Older Australians draft report, which it said ignored key workforce issues and failed to deliver on quality care. ANF federal secretary Lee Thomas said Australians who rely on the aged care sector should be “very concerned”. “A failure to recommend any substantial changes to the care received by elderly residents’ means the Productivity Commission has not upheld the key objectives of the report,” she said. In its submission to the commission, the ANF called for the appropriate number of staff with the right skills to ensure adequate care for residents, and closure of the $300 a week wages gap between aged care nurses and nurses in the public system. It also demanded transparency of funding to guarantee government money goes directly to the care of older Australians. The report, which will make recommendations to the

Federal Government on appropriate courses of action, warned the number of people relying on the system would more than treble to 3.6 million people by 2050. Pensioner groups attacked its flagging of compulsory nursing home bonds while the government welcomed moves to overhaul the sector. Ms Thomas said the report failed to highlight the importance of minimum care hours, staffing levels and skills mix, and made no recommendation to license all workers in the sector, which was vital to protecting the public. She said the report also failed to address the wages gap. “Nurses and assistants in nursing are not just the back bone of the sector; they are keeping the system going day in and day out,” she said. “The Productivity Commission has ignored their call for urgent workforce reform. They have also let down the residents and their families because the key issue of care barely features in the lengthy document.”

Specialist physios must be recognised: APA The Australian Physiotherapy Association is calling for the registration of specialist physiotherapists. APA president Melissa Locke said the move was important for the profession in terms of recognition and career pathway but also valuable in ensuring patients had access to the right physiotherapist at the right time. “We are advocating to State and Federal Governments for the permanent recognition of Fellows of the Australian College of Physiotherapists in a similar way that medical specialists like paediatricians and surgeons are recognised,” she said. Ms Locke said the Physiotherapy Board of Australia had made the recommendation, which was now being reviewed by State and Federal Governments. “We expect a consultation process to begin in March next year, and will continue to tell politicians why specialisation is so important to the public and the profession,” she said.

Ms Locke said it was also vital that physiotherapists were involved in the governance of the new Medicare Locals. “The creation of Medicare Locals and their proposed structure and function is an opportunity to get more recognition of the vital services that physiotherapists provide as primary contact professionals,” she said. “They have a lot to give to the community in terms of getting people active to help address the underlying causes of chronic conditions such as cardiovascular health and diabetes.” Ms Locke said the sector also needed workforce reform in a bid to reduce funding, legal and cultural barriers to foster more innovative practices. She said physiotherapists were forced to send their patients to GPs to order ultrasounds or to get referral letters for medical specialists such as orthopaedic surgeons.

Thousands of nurses face missing registration deadline Published ncah.com.au 25th January. Around 5000 of Victoria’s 84,000 nurses and midwives will not be able to work next week if they miss Monday’s deadline to register for a new national accreditation scheme. The new Australian Health Practitioner Regulation Agency has warned nurses and midwives who failed to register before last year’s December 31 deadline risk losing their registration and the ability to practise in Australia. Under the Health Practitioner Regulation National Law Act, the AHPRA says it must remove practitioners from the national register once the late period ends. About 5000 nurses are yet to have their applications processed. The Australian Nursing Federation has received hundreds of calls from members reporting difficulties registering with the new agency, from late registration letters to lengthy hotline delays. ANF acting secretary Paul Gilbert told media about a third of the state’s nurses had not registered before the December deadline, including many of the 2000 new graduate nurses due to start between January and May. Mr Gilbert said there could be more registration delays in May as part of the staged transition from state-based registration to the national system. “I’m troubled about the prospect of 80,000 nurses doing this again in four months’ time along with another 150,000 nurses from other states and territories,” he said. AHPRA chief executive officer Martin Fletcher said the agency was striving to ensure all practitioners were registered. “While it is up to practitioners to make sure they are registered, AHPRA is doing everything it can to support the registration renewal process,” he said.
