New Zealand public hospital errors revealed

A patient waited 14 months for an urgent appointment at an eye clinic due to administration errors and is now legally blind while another patient was given a 10 times overdose of two different types of insulin.

The Health Quality and Safety Commission’s latest report found there were 377 serious and sentinel events that occurred in New Zealand public hospitals in the 2010-2011 year, up from 318 events on the previous year, excluding outpatient suicides.

Commission chair Professor Alan Merry said the patients involved in the 377 incidents were let down by the system that exists to protect them.

The commission found 86 people died during admission or shortly afterwards, although not necessarily as a result of the event, and falls accounted for 52 per cent or 195 of all serious and sentinel events. Clinical management events, such as errors of diagnosis and treatment, accounted for 29 per cent or 108 incidents, and medication events were the third largest category at seven per cent or 25 incidents. There were 11 incidents of the wrong patient, site or procedure taking place, seven incidents of retained instruments or swabs, four incidents with a missing or absent patient without leave, three incidents of a physical Page 10 | www.ncah.com.au

There were also five inpatient suicides and 15 diverse incidents listed under ‘other’.

“We should view these events through the eyes of patients and their families, and acknowledge that many of them should never have happened,” he said. Professor Merry said the country had an excellent health and disability system, with more than 2.7 million people treated in public hospitals or as outpatients each year, and there were very few occasions of serious harm. But he urged health professionals to review the report and consider ways they could make their services safer for patients. “It’s not acceptable to keep making preventable errors and all of us who work in health need to redouble our efforts to ensure patients receive the best and safest care.”

More healthcare organisations and nursing agencies are tapping into the benefits of social media to communicate with nurses. With social media statistics for January showing Facebook hit 10,703,340 users in Australia, YouTube had 11 million unique visitors, Twitter and LinkedIn hit 1,800,000 and WordPress at 1,500,000, social media has become the key tool for companies to keep in contact with shift workers in the healthcare profession. And nurses fit the social media profile. Statistics show 86 per cent of females aged between 18 and 50 are active in social media – which is the nursing profession’s main demographic. Leading healthcare recruitment provider Healthcare Australia looks after a total of 11 Facebook pages for its brands, including Nurse Jobs Australia, Nursing Agency Australia, Malvern Nurses Agency and WA Nursing Agency. Healthcare Australia brand and online marketer Bekim Ahmedi said social media enabled the organisation to open up a two-way conversation with healthcare professionals. “Platforms such as Facebook and Twitter receive more visits than search engines and Page 12 | www.ncah.com.au

enable you to communicate directly with a targeted audience whether they are existing or potential employees, as well as create viral content,” he said. Mr Ahmedi said the organisation had more than 3000 fans across its pages. “We have chosen to communicate via Facebook as it is the number one social media network with the greatest level of engagement,” he said. “It is also the platform that our employees are already using on a daily basis, so the choice was easy. “As there is a large amount of shift work that takes place within this industry, our employees have free time at different periods throughout the day. “Social media allows you to logon to Facebook and see a post at any time. It is also accessible across multiple locations.” Healthcare Australia also utilises Twitter @ HealthcareAus and LinkedIn and has plans to explore sharing relevant video content with a YouTube channel later this year.

Super glue could change the way intravascular catheters are secured Studies show a third of intravascular catheters fail while troubleshooting and replacing catheters costs the nation about $1 billion each year. Australian researchers now believe they are on the threshold to providing nurses with a simple solution. The answer could lie in a common product already found in our hospitals and in countless homes across the country, writes Karen Keast. Around 15 million intravenous drips are used in Australian hospitals each year, with most placed in the arms or hand veins of patients. One in three fail while in use. Of these, 70 per cent become blocked, 15 per cent become so painful they need to be removed and another 15 per cent simply fall

out or are accidentally pulled out, while about one in 1000 patients develops a serious infection in the bloodstream as a result of bacteria entering through the IV drip. The most common cause of IV drips failing is inadequate securement, causing micromotion of the drip and irritation of the vein, which can then swell and become occluded or can push the drip into the surrounding tissue. Inadequate securement can also lead to bacteria entering or for the drip to become partially or fully dislodged. These alarming statistics and outcomes for a common nursing procedure used aroundthe-clock in wards, theatres and in critical care in hospitals across Australia and around the world could soon be a thing of the past. Australian researchers believe tissue adhesive or ‘super’ glue could hold the solution. Griffith University Professor Claire Rickard, a Chief Investigator with the NHMRC Centre of Research Excellence in Nursing and a member of the Critical Care Research Group, said the results of laboratory trials showed medical grade superglues Histoacryl® and Dermabond® provide a reliable method of peripheral IVC fixation in vitro. “This glue has been around for a while. It’s not one you can buy down at Bunnings although it’s actually not much different…chemically it’s the same formula,” she said. “These glues are commonly available within the hospital at the moment. They are just not used for this purpose.

“They are mostly used for children in the emergency department. If you have a child


with a small wound, instead of suturing it up, which is very painful and then the stitches have to be removed, we just glue it up. “That would be the most common use but it is used for internal surgery and external wounds and even eye surgery. “The results of our studies are a surprise but often the common things are right under our noses.” Professor Rickard said the solution was discovered after her earlier study of 6000 IV catheters found a large number failed while still needed.

“People were having additional needles and missed out on the medication treatment,” she said.

The study, which was completely independent of manufacturers, was funded by the NHMRC Centre for Research Excellence in Nursing Interventions for Hospital Patients along with the Prince Charles Hospital Foundation.

“We thought – that’s absolutely unacceptable and we started looking at the reasons why this was happening and how we could improve it.

The series of three studies compared the glues to current fixation methods for compatibility with IVC material and the ability to prevent both pull out and microbial growth.

“We just really sat around talking. We looked at how catheters were being held in place and we looked at all of the new products… many of which are high cost but virtually untested.

The two glues, along with two standard dressings (Tegaderm™ 1624 and 1633) and an external stabilisation device (Statlock®) were tested to secure 16G peripheral IVCs in new born fresh porcine skin. Control IVCs with no securement were also studied.

“Our scientists were using the glue to secure lines in sheep in the animal lab – their greasy skin meant that usual dressings wouldn’t stick. “One of the research nurses breeds dogs and she reported that the vets were also gluing in the IVs, for similar reasons. Then we tracked down a couple of papers in the U.K. where an anaesthetist had been gluing in IVs and epidural lines – (he) wasn’t researching it but he had published his report – he reckoned it was 100 per cent successful. “We thought, of course we have to research this a bit more scientifically and that’s what happened.”

The results found that neither super glue weakened the catheters, and they were up to 400 per cent stronger than current dressings at preventing IV drips from pulling out. The super glues were also quick and easy to apply, there was no irritation or skin damage, no visible bacterial growth and the glues can be safely removed with paraffin, causing no tissue damage. A clinical trial, thanks to further support from the Prince Charles Hospital Foundation and the Griffith Health Institute, will begin as soon as March – studying the use of the Continued overleaf Nursing Careers Allied Health - Issue 04 | Page 21


glues in peripheral IVC fixation on about 400 patients at the Prince Charles Hospital, led by Professor John Fraser, and at the Royal Brisbane and Women’s Hospital, led by Professor Joan Webster. The trial will compare the glues and standard care dressings against other recently released commercial dressings and securement devices, including Statlock®, with results expected to be released late this year. Professor Rickard said there were also plans for the introductory study to be followed up with a larger scale study of about 2000 patients next year, if funding could be secured. “It’s a world first and very innovative and ground breaking,” she said. “We imagine that if successful, it would really change the game and all the tissue adhesives would be marketed and changed for this indication.” Professor Rickard said clear plastic polyurethane dressings have been used for around three decades to secure intravascular drips and now research to glue in IV drips could change the way nurses, including theatre and critical care nurses, work. “We find with patient transfer, say between the emergency department and ward, or between the operating theatre and ICU, there’s a lot of catheter migration because the lines get pulled or damaged, the tubing can catch on the bed clothes or the bed rail in the lift,” she said. “If the central lines migrate that can be lifethreatening. If suddenly the patient is not receiving their medication that can be an emergency situation. “We know where the problems are occurring. We hope we can minimise or avoid that all together. “And if nurses are not having to waste their time troubleshooting catheters that are faulty and removing them and replacing them, that will make a big difference,” she said. Professor Rickard said if the clinical trials prove successful gluing in catheters will be a new technique for nurses. “It’s just going to be a change in people’s mindset in hospital, to have these glued in,” she said. “We have certainly got the data to show that the dressings aren’t effective at the moment – it’s a Band-Aid solution.” For nurses, the monitor the glue slowly degrades to easily remove is removed.

Nurses’ ratio dispute continues More than 7000 people have signed a petition calling on Victorian Premier Ted Baillieu to save and improve nurse and midwife ratios. The Australian Nursing Federation’s Victorian branch launched the online petition – which sends an email to Mr Baillieu, Health Minister David Davis and Bentleigh MP Elizabeth Miller every time someone adds their signature. The petition calls on the government to resolve the nurses’ dispute and support and fund nurse and midwife patient ratios in the replacement enterprise agreement. It also demands Mr Baillieu legislate for mandated, safe nurse and midwife patient ratios and put an end to plans to replace qualified nurses with unqualified and Page 24 | www.ncah.com.au

unregistered health assistants, four-hour shifts and split shifts. The petition comes as Victorian nurses prepare to hold a state-wide public sector stop work meeting on February 22 at 2pm. Nurses will discuss a variety of options at the campaign meeting, including a new round of protected industrial action, which could go ahead when Fair Work Australia’s suspension order expires. In November, nurses began closing one in three hospital beds in their bid for an 18.5 per cent pay rise over three years and eight months. Nurses’ union demands rebate savings to be injected back into health The Australian Nursing Federation has called for the Federal Government to inject the $2.4 million saved from means-testing private health insurance back into the nation’s health system. ANF federal secretary Lee Thomas said the funds must be directed back into areas such as health promotion, primary health care, reducing elective surgery waiting lists, emergency waiting times and both the private and public aged care systems. “All Australians must have access to the very best healthcare, whether they have private health insurance or are patients seeking treatment in the public hospital system – there must be equity for all,� Ms Thomas said.

Legislation to means-test the private health insurance rebate was passed through the House of Representatives on February 15. Under the Federal Government’s legislation, singles earning more than $129,000 a year and families earning $258,000 or more in the next financial year will lose the rebate entirely. Individuals earning $83,000 or less in the next financial year, or families earning $166,000 or less, will not be affected. Federal Health Minister Tanya Plibersek said the legislation will save taxpayers $2.4 billion over the next three years or $100 billion over the next 40 years. “The legislation is a win for low and middle income earners, who for too long have been forced to subsidise the private health insurance of higher income earners through their taxes,� she said. Fertility nursing lures Guianna The company has 13 clinics across New South Wales and employs about 65 nurses. Guianna’s role includes spending time in day surgery, helping patients during their procedures as well as supporting couples throughout their treatments. “I also sit with couples when they first visit Genea, I explain what they can expect throughout their journey, I discuss the procedures, medications and our success rates,” she said.

Working as a fertility nurse is a dream come true for Guianna Marcelo. The Peru-born Sydneysider has spent the past five and a half years working at Genea, formerly known as Sydney IVF. The Registered Nurse first began work at Genea as an Enrolled Nurse, working in day surgery, and has completed her registered nursing degree with the support of the Sydney city clinic. Electronic drug records system for pharmacists A Tasmanian initiative to prevent the abuse of controlled drugs such as painkilling prescription drugs will be rolled out nationally from July. The Federal Government has announced plans to establish a $5 million national electronic records system that will enable pharmacists, doctors and other health authorities to monitor the real time prescribing and dispensing of addictive drugs. The Electronic Recording and Reporting of Controlled Drugs system, which has been used in Tasmania for more than a year, will enable pharmacists to access a centralised database over a secure computer network containing prescription history records. Federal Health Minister Tanya Plibersek said the system would help monitor the drug misuse and diversion of controlled drugs including oxycodone, morphine and codeine.

“Abuse of these drugs can cause enormous harm and is a growing problem in the community,” she said. Ms Plibersek said health professionals will be able to immediately detect people suspected of trafficking in painkillers, forging prescriptions and ‘doctor-shopping’. “The new records system will be able to flag patients in real time who have repeatedly sought controlled drugs, helping to prevent people from inappropriately using the drugs or selling them to others. “If a pharmacist determines it is not clinically appropriate to dispense a medicine to a patient, it is their duty of care to restrict access to that patient.” The system will replace paper-based prescription records still operating in some Australian states and enable pharmacists to check on prescription records from across the country. The national scheme comes as figures show the amount of prescription opioids used in Australia is on the rise. The total value of Pharmaceutical Benefits Scheme opioid prescriptions increased from $2 million in 1992 to $7 million in 2007, according to the Internal Medicine Journal.

Ambulance ramping affects paramedics and hospital staff Ambulance ramping has a significant impact on paramedics and hospital staff, patients and the community, according to the findings of a new Australian study. A three year Griffith University study found ambulance ramping was a manifestation of emergency department overcrowding and its consequences were labelled “unique and concerning”, with ramped patients experiencing access block in higher proportions. “…there is confusion about the level of responsibility taken by individual staff members, stress is experienced by both paramedics and hospital staff, and there is potential impact on ambulance community response times,” the study states. The exploratory study was the first official review of ambulance ramping with researchers examining its impact on the delivery of emergency health services across south east Queensland. The study found definitions of ambulance ramping varied depending on the hospital, the service and the individual healthcare workers, which affected how it was reported. Hold-ups in ambulance off-stretcher times were blamed on overwhelmed resources at triage coupled with delays as emergency department patients were moved to accommodate incoming patients.

Deborah worked in paediatrics for about 20 years and then in midwifery for 15 years and in November began work with the Women’s Health and Mother Baby Hub in Strathpine, Queensland.

“You can see their growth and they don’t often realise how strong they are.”

“It’s a wonderful concept and it has something for everybody – I really enjoy it,” she said.
