Medicines for diabetes, HIV and cancer on PBS list by Karen Keast Medicines for the treatment of type two diabetes, brain tumour, HIV, colorectal cancer and juvenile idiopathic arthritis will be listed on the Pharmaceutical Benefits Scheme (PBS) from April 1. Health Minister Peter Dutton has announced 10 new medicines will be added to the PBS, ranging from greater access to several new diabetes medicines, known as gliptins, for patients with type two diabetes.

(tocilzumab) will be listed for children relying on treatment for juvenile idiopathic arthritis while Tobi Podhaler (tobramycin) has been approved for listing in powder form for the treatment of a bacterial lung infection that commonly affects cystic fibrosis patients. “It means patients will have subsidised access to the powder form of the medicine, which can be used with a transportable inhaler device,” Mr Dutton said.

“These medicines will have expanded access on the PBS allowing patients that require treatment with multiple medicines for diabetes to access innovative new treatments earlier,” Mr Dutton said in a statement.

“The listing of the powder form of tobramycin on the PBS will help reduce the treatment burden for patients, many of them children, who already have a time-consuming and complex treatment regime.”

Former midwife banned and fined A former midwife operating in Adelaide has been found guilty of professional misconduct following her involvement in high risk homebirths that saw the deaths of four infants.

As reported by abc.net.au, Lisa Barrett opted to save the Health Practitioners Tribunal time and money by not challenging disciplinary action for professional misconduct brought against her by the Nursing and Midwifery Board.

“The respondent is reprimanded in the strongest terms,” the judgment said.

The tribunal’s president, Magistrate Michael Ardlie reportedly commented that Barrett’s conduct fell below the standard expected of practitioners of her experience, adding that for the protection of the public, only health practitioners who were suitably trained and qualified to practice in a competent and ethical manner should be able to be registered. The tribunal concluded the protection of the public was its paramount consideration and Page 14 | www.ncah.com.au

Barrett was fined $20,000 and banned from providing midwifery-related services. While Barrett had surrendered her midwifery registration in January 2011 the tribunal heard there was evidence she kept working in the field. As reported late last year by abc.net.au, The Nursing and Midwifery Board of Australia alleged Barrett’s conduct in the South Australian births was inappropriate, unsafe and harmful to the reputation of the midwifery profession. Push for midwives’ medicine bag by Karen Keast Australia’s growing number of prescribing midwives should be equipped with a midwives’ bag of emergency medicines, according to a leading midwifery advocate. University of Western Sydney Professor Hannah Dahlen, a privately practising midwife and spokesperson for the Australian College of Midwives, said establishing a midwives’ bag is the next goal for prescribing midwives. Professor Dahlen, who recently gained her Medicare provider number and prescription pad, said prescribing midwives need access to emergency drugs such as Maxolon, which is often used to treat women vomiting in labour, and Syntocinon and Syntometrine, used to prevent and treat haemorrhaging. “There are a series of drugs that midwives should be able to have on them,” she said. “They are rarely used drugs but if you need them, you need them, and you can’t predict if you’ll need them. “For example, Maxolon is a drug that stops vomiting. “You can’t predict which women in labour are going to vomit and so you need to be able to have that with you, so that if you’re in a situation where a woman is getting dehydrated from vomiting you can give that drug and prevent either a transfer from a home birth or the need for drips and other intervention.” Professor Dahlen said a midwives’ bag would also reduce the cost of replacing medicines that expire. “To have a vile of one of the really important drugs to stop haemorrhaging it’s $70,” she said. “You may not use it, it goes out of date, you have to then spend another $70. “Whereas in the system of having a midwives’ bag, the pharmacist would then renew it and these midwives wouldn’t be out of pocket all of the time for these drugs. “We have to now go through the process in each state and territory of approaching the chief pharmacist and getting a midwives’ bag up, that we can have with us all of the time.” Professor Dahlen estimates about 100 eligible, privately practising midwives across Australia now have the power to prescribe after completing an approved prescribing course and applying for their Medicare provider number and script pad. “The numbers are expanding daily,” she said. “To be honest, we are dealing with well women – there’s not a lot of drugs that we need to prescribe to achieve or to fulfill our role.” Eligible, privately practising midwives are able to prescribe schedule 2, 3, 4 or 8 medicines appropriate for midwifery practice across pregnancy, labour, birth and post natal care.

Auckland nursing and midwifery jobs at risk by Karen Keast The New Zealand Nurses Organisation (NZNO) holds fears nursing and midwifery jobs could be targeted after Auckland District Health Board (DHB) revealed plans to introduce costcutting measures. Auckland City Hospital, the country’s largest public hospital and clinical research facility, has been told to reduce costs in a bid to control over-spending, amid concerns about a potential $12 million budget shortfall. NZNO DHB sector industrial advisor Lesley Harry said the organisation planned to meet with the director of nursing and senior management at the hospital to discuss its plans. “We are very concerned that cuts to staffing and a delay in recruitment will be seen as the solution to the deficit,” she said. “But there is no justification for nurses and midwives’ staffing cuts as DHBs are understaffed now.” According to Radio New Zealand, the DHB will focus on costs for operating theatres, children and older people’s services. It will also look to implement change in other areas where there are significant losses, such as cardiothoracic surgery, vascular surgery, general surgery, gynaecology, neurosurgery, orthopaedic surgery, respiratory medicine and intensive care. Dietitians and nurses support better food labelling by Karen Keast Australia’s dietitians, nurses and midwives have thrown their support behind the introduction of a front-of-pack labelling (FoPL) system for packaged, manufactured or processed foods. The Dietitians Association of Australia (DAA) has long lobbied for a single FoPL model aimed at helping shoppers choose healthy foods. The Australian Nursing and Midwifery Federation (ANMF) also wants a simpler, more transparent system, like the federal government’s proposed Star Rating system, to help curve the rising rates of obesity and chronic health illnesses. The support comes after the government’s Star Rating food health website went live last month but the website was removed just hours later, amid claims its launch was premature. DAA CEO Claire Hewat said while the organisation supported a better food labelling system, she was not in a position to make detailed comment on the Star Rating system. “The organisation has not been involved in the development as such, only in consultations at several points during the process, and has not seen the version at the time of the Star Rating website being pulled down,” she said. “Whatever system is agreed, it should have a strong evidence base and be adequately tested before implementation, to ensure that the front-of-pack labelling/Star Rating system achieves its goal of assisting Australians to make healthy food choices.” Ms Hewat said while FoPL has the potential to assist Australians make healthy food choices, along with other government initiatives such as the Australian Dietary Guidelines, it also raised some concerns. “DAA is concerned about the potential for untoward effects on the intake of ‘positive’ nutrients, such as calcium and folate,” she said. “A labelling system should assist consumers to make informed choices about mixed or processed foods rather than making unnecessary comparisons between foods within the basic five food groups.” Ms Hewat said any rating system should be launched and maintained as part of an overall strategy to support Australians to make healthy choices. “This could include educating health professionals, teachers, health promotion professionals, workers in the fitness industry and others,” she said. “Consumers with low levels of health literacy stand to gain most from public health initiatives, however support must be offered to interpret FoPL/Star Rating system. “Practical advice must be provided to Australians to guide food choices while aiming for consistency in public food policy.” ANMF federal secretary Lee Thomas said nurses and midwives see the devastating consequences of obesity, from children to older Australians and in particular pockets of the population, such as Indigenous communities. “Regulators must work with the government and consumers to make it easier for people to make informed decisions about the foods and drinks they are choosing for themselves and their families,” she said. “We believe there’s now a real need for a frontof-pack labelling which will be an important way of fighting obesity across the community - starting at the supermarket shelf.”

Paramedics ramp up industrial campaign by Karen Keast Victorian paramedics will consider stop-work meetings in a bid to bring their 19-month dispute over pay and conditions to an end. Ninety-eight per cent of the state’s 1700 paramedics recently voted in favour of adding 22 new work bans, ranging from internal communication bans to taking strike action for the first time since 1973. The last enterprise agreement expired in November 2012, and paramedics have already implemented a range of bans, which include wearing ‘Code Red’ campaign tees and displaying campaign messages on ambulances. Under the move to increase bans, Ambulance Employees Association Victoria must provide Ambulance Victoria with three working days’ notice of its intention to implement the Fair Work Commission-approved work bans. AEAV general secretary Steve McGhie said strike action would be at the “extreme” end of the range of work bans. “If we ever hold a stop work meeting we will make sure there are still some ambulance resources available to respond to emergency cases,” he said. Mr McGhie said paramedics decided to increase bans in a bid to resolve the dispute. “It’s a hard decision for paramedics to make…but they believe they have got no choice,” he said.

Nursing student sings for mental health by Karen Keast A nursing student has told how the skills he learnt in his studies and part-time orderly work helped him approach a semi-naked homeless man passed out on the wintry streets of Brighton in the United Kingdom. Third year Griffith University Bachelor of Nursing student Liam Maddigan has since written a song about the experience, which he has been asked to perform at the Homelessness Australia conference on the Gold Coast in September. Liam, who works weekends as an orderly in intensive care at the Wesley Hospital, said his knowledge and experience in effective communication and building trust with patients helped him approach the man, who was lying on the street with his pants around his ankles, in freezing conditions. “It’s all stuff that I have seen - it’s the way that nurses talk to their patients or if we are in a code situation and how we deal with that and talk to family members,” he said. Liam, 30, said he knelt down next to the man and gently roused him from his sleep. “He was drunk and confused,” he recalled. “I told him who I was and that I just wanted to help him up and get his pants back on. “Because of his confusion, he started to raise a fist towards me but I held his hand and told him that I only wanted to help him. “He started to calm down and although still very confused, he began to cooperate with me.” Liam, who was with his fiancee Fiona, who is also studying nursing and has a passion for mental health nursing, was then approached by a few men who knew the man, named Roger. One of the men called for an ambulance and thanked Liam and Fiona for their “act of kindness”. “He said - most people walk past us and ignore us. It’s not often that you see this type of humanity.” Page 26 | www.ncah.com.au

Liam said they saw quite a few people living on the streets during their UK holiday and constantly stopped to help, alarmed that others were simply walking past. “Fiona was constantly buying them food and coffees. We tried to do what we could for those people, especially on those cold nights.” Liam, who was part-owner in a Brisbane restaurant before deciding to follow in his parents’ footsteps and pursue a career in nursing, said he later felt compelled to write a song around homelessness, mental health issues and other people’s attitudes towards it. On a whim, Liam sent a recording of himself playing the song to Homelessness Australia and was amazed at their overwhelming response. “It is such an honour,” he said. “Being able to create a piece of music inspired from my nursing experience is the icing on the cake.” Liam said he hopes the song inspires others to stop and assist homeless people. “It could be just buying them some food or, if you see someone lying on the street, make sure they are alright.”
