Occupational therapist follows her heart to Africa by Karen Keast A Brisbane occupational therapist has returned to her African homeland to assist burns patients. Jody Mills is serving four months as a volunteer occupational therapist with the world’s largest private hospital ship, Africa Mercy, off the coast of one of the world’s poorest countries, the West African nation of Togo. Jody said her role on the ship as hand therapist involved working with plastic surgery patients, mainly those with severe or functioning burns contractures, and assisting with rehabilitation, from splinting and exercise to scar management and compression garments. “In Togo, health care and especially burns specialist centres are very limited,” she said. “Without proper care initially after a burn injury, people can develop terrible contractures and disfigurement that could have been avoided.” Jody was working at a hospital in Brisbane when she met a physiotherapist who had volunteered with the global charity. She said her African adventure was a unique experience that was both challenging and rewarding. “The best thing about this whole experience has been to work with the patients,” she said.

Agency nursing: The key to work-life balance? Imagine if you could dictate the days and the hours you wanted to work and achieve the ever-elusive work-life balance? Well, as an agency nurse, you can, writes Karen Keast. According to an Australian study, 60 per cent of people are not working the hours they want. University of South Australia Centre for Work and Life director Professor Barbara Pocock, in her 2008 presentation of Work-Life Balance in Australia, revealed most Australians are either working more or less hours than they want to, with only 40 per cent achieving their ideal fit with their actual and preferred working hours. Not surprisingly, an overwhelming majority, almost 85 per cent, of people content with their preferred working hours were also satisfied with their work-life balance. In comparison, only 66.9 per cent of those wanting fewer hours were satisfied with their work-life balance and 74.9 per cent of those wanting more hours were satisfied with their work-life balance. Professor Pocock’s presentation stated: “Significantly better work-life outcomes occur for those workers who can get a better fit between the hours they work and their preferences.” As Australians continue to walk the tight-rope in search of the ideal balancing act between work and lifestyle, many nurses are turning to agency nursing in a bid to control the number of hours they work, and at the times they want to work. And it’s not just working mums and dads, striving to juggle family commitments, who are searching for flexibility in their shifts.

A wide range of nurses are opting for agency work to suit their lifestyle, including nursing students balancing study with shifts, travelling nurses combining work with sight-seeing and nurses looking to balance work with caring for a sick family member. Brisbane-based Registered Nurse Alison Kristensen chose agency work over that of a full-time employer to suit her lifestyle. For Alison, who has 25 years’ experience in nursing, it was a decision that enabled her to take ownership of her work-life balance. Alison has spent the past six years working for Healthcare Australia brand Nursing Agency Australia while, at times, also pursuing her other passion – a career in sales. “I personally love the autonomy NAA provides me with, as I can decide when and where I work and feel like I am my own boss,” she said. “I also enjoy the flexibility of being able to participate in week-day business and social events and the freedom to decide when I take a day off or holiday.” With more than 20 years’ experience in the healthcare industry, WorkPac HealthCare CEO Stephen Thomas, who is also a Registered Nurse, said nurses often turn to agency work as the solution to their search for work-life balance. Mr Thomas said agency nursing provided the flexibility that full-time employers could not, particularly appealing to overseas nurses wanting to work and travel in Australia, students combining study and work, Reservists wishing to balance their service commitments with defence force commitments and, of course, busy working parents. “Many people find that the agency is a good balance for family commitments or if you have a partner that’s also a shift worker and the primary bread winner,” he said. “If you are in the stay-at-home role, being able to work around your partner’s commitments, you have a higher flexibility working through an agency than working for an employer.” Mr Thomas said while agency nursing had a range of benefits, such as professional development opportunities, nurses also had a say when it came to which facilities they wanted to work at, enabling them to control their travel distances. Most importantly, he said nurses could determine what days and shifts they wanted to work and how often.

Healthcare Australia recruitment consultant Christine Lingard said nurses often turned to the healthcare recruitment solutions provider in search of roles that offer flexibility with stability.

“It’s entirely up to you,” he said.

“Nurses from every age group join Healthcare Australia in order to work more often because they want to supplement their full-time income,” she said.

“On a regular basis you are requested to submit your availability and we factor that into our system.

“We then find many nurses, especially working mums, want to work less so they can have more time at home.

“If we have a job order for a week or a day, only those people who have said they are available come up in our search. “And the more that you say you are available the more work that will come your way.” One WorkPac HealthCare nurse, a mum, has designed her schedule so she only works at one particular hospital and only for night duty, to suit her family commitments.

“(Other nurses)..are often stressed, their hours are pushed and they crave flexibility. “Many nurses are also unhappy because their full-time roster does not suit their current lifestyle, so they may find it difficult to work night shifts but continue to be rostered on.” Ms Lingard said it was not only vital for nurses to achieve their ideal work-life balance, she said it was also possible.

Another nurse has a child with a disability and works around her child, depending on how her child is feeling on that particular day.

Fears new nursing ratios are under attack by Karen Keast The New South Wales’ Nurses’ Association has launched a campaign to defend nurse to patient ratios in the public sector amid fears the new ratios are already at risk. Nursing ratios were won in last year’s industrial relations dispute but now the NSWNA fears a recommendation to the State Government, outlined in the NSW Commission of Audit Interim Report, could wind back the clock with nurses returning to conditions before ratios were won. Recommendation 22 in the report states: ‘The Department of Premier and Cabinet should amend the NSW Public Sector Wages Policy 2011 to include a provision that workforce management policies (such as staff ratios) should not be included in industrial instruments.’

NSWNA general secretary Brett Holmes said the recommendation had “dangerous” implications for not only nurses but other emergency services, including police and ambulance. “This report simply says you should remove those out of anything that is enforceable and give it back to management - they will decide how they will staff workplaces,” Mr Holmes said. “Clearly you can’t trust important safety issues like staffing to be at the whim of management who are ultimately subject to the pressures of treasury.” Mr Holmes said the government was refusing to reveal its stance on the recommendation. The Milk Run

Matthew Bell and Jamie Smith are raising funds for breast pumps in the Women’s Newborn Intensive and Special Care (NISC) unit. Often described as ‘liquid gold’, breastmilk is a vital ingredient in the care of premature and unwell babies. Matt, who is the Nurse Unit Manager in the Women’s operating suites, and Jamie, a personal fitness instructor, know the importance of giving every child the best possible start in life. By running in the North Face 100, a gruelling 100km run through the NSW Blue Mountains, they hope to make a difference to the health of premature and unwell babies. Breastmilk has many unique properties; properties that support growth and protect against infection. Many of the babies admitted to the unit cannot be breastfed to start with, and can only receive breastmilk via a tube. For mothers with babies in intensive or special care, this means expressing milk with an electric breast pump, up to eight times every day. By running 100km, Matt and Jamie hope to raise enough money so that every mother with a baby in NISC has access to their own breast pump. Matt began his career at the Women’s after the birth of his daughter in 2010, and he is as passionate about his work as he is about competing in marathons and ultra-marathons. Jamie, who owns Next Phase Personal Training, trains with Matt every week and is excited about being a part of The Milk Run. “Matt suggested we raise funds for breast pumps, I was keen on this idea as I have seen what a great job the Women’s does; close family and friends of mine have had babies there and have been looked after so well by the hospital,” said Jamie. “Also some time in the near future my wife and I plan on having children and it is comforting to know a facility like the Women’s is able to deal with any situation associated with pregnancy and child birth.” The North Face 100 is considered the hardest 100km endurance race in the world, which Matt and Jamie plan to complete in under 20 hours to qualify for an ultra-marathon medal. New app helps parents in NICU by Karen Keast A smart phone application has been developed to help parents of premature and sick babies decipher the medical terminology they hear in Neonatal Intensive Care Units and Special Care Nurseries. Australian charity Life’s Little Treasures Foundation developed the free NICU Words app to assist the parents of the estimated 21,000 babies who are born prematurely each year. The app features a glossary designed to provide a clear explanation of the medical terms and medical machinery, and describe the roles of medical personnel in NICU and SCN. Foundation founding director Parool Shah, and mum to premature baby Luke, said the app enabled parents to decode the jargon and help them follow their baby’s progress while giving them the confidence to ask medical questions. “Having a child that is born either prematurely or unwell exposes you very quickly to the overwhelming world of NICU, where it can often seem like medical staff are speaking a foreign language,” she said. Australian College of Neonatal Nurses executive member Melissah Burnett, an NICU nurse, lecturer and course coordinator of Neonatal Nursing at La Trobe University, said the app was an important resource for parents and families, familiarising them with the terminology used around the care of their baby. “The app gives them the confidence to ask questions and make vital decisions for their babies,” she said. “Such a knowledge resource promotes family centred care in the NICU environment.” Established in 2005 by mothers of premature babies to provide information and support, the foundation also recently launched its Premature Baby Journal application. The app helps parents keep track of their baby’s milestones and the many activities they need to undertake on a daily basis, both in hospital and when returning home. NICU Words is a free app and the Premature Baby Journal app costs $2.99. Proceeds from the app go to support the work of the foundation. Nurses and midwives urged to renew online, on time by Karen Keast More than 332,000 Australian nurses and midwives are expected to renew their registration before the end of May. The Nursing and Midwifery Board of Australia has kicked off its 2012 renewal registration campaign and announced this year’s fee has been set at $160; an increase greater than CPI on last year’s fee. The board is reminding nurses to renew ‘online, on time’ before May 31, with online registration renewals opening in early April. There are 332,185 practitioners registered with the Nursing and Midwifery Board of Australia, including 1789 midwifery practitioners, 290,072 nursing practitioners and 40,324 nursing and midwifery practitioners. The national board also registers nursing and midwifery students enrolled in approved courses. Statistics reveal most registered midwives nominate Victoria as their principal place of practice, while New South Wales is the principal place of practice for most nurses and dual registrants in nursing and midwifery. In announcing the new payment, the board stated this year’s $160 fee was the lowest registration renewal fee for health practitioners under the national scheme. Board chair Anne Copeland said the fee was “fair and reasonable” and would enable the board to meet its range of responsibilities. “The national scheme is funded solely by registrants’ fees and there is no crosssubsidisation between professions, so we need to make sure the board can properly fund its work in protecting the public by regulating the nursing and midwifery professions in the public interest,” she said. The national scheme was first introduced across most of Australia on July 1, 2010, with Western Australia joining the scheme on October 18, 2010. The scheme now covers 10 health professions, including chiropractic, dental, medical, nursing and midwifery, optometry, osteopathy, pharmacy, physiotherapy, podiatry and psychology, which are all regulated under the Australian Health Practitioner Regulation Agency. Four new professions will join the scheme this year, including occupational therapists, Chinese medicine practitioners, medical radiation practitioners and Aboriginal and Torres Strait Islander health practitioners.

Maternity changes spark fears by Karen Keast Plans to move specialist maternity services from Whanganui to Palmerston North in New Zealand will affect about 90 maternity staff and about 800 women a year, according to the nurses’ union. The New Zealand Nurses Organisation has criticised the Whanganui and MidCentral DHBs boards’ regional women’s health service proposal, which also plans to move elective gynaecology services from Palmerston North to Whanganui. NZNO organiser Donna Ryan said she held grave concerns about the plan’s impact on nursing and midwifery staff. “One issue is that Palmerston North Hospital has no extra birthing rooms and only three extra ante-natal beds planned if the changes go ahead,” she said. “Staff will be overworked and underresourced. It is hard to see how the proposed changes will benefit the people of either area.” Ms Ryan said the proposal includes plans for the MidCentral Community Midwives to be disestablished, affecting about four full-time midwives who care for very high risk women with multiple issues. But she said the proposal lacked detail when it came to other staffing arrangements and she said it was difficult to estimate the number of jobs that would be lost in Whanganui with the reduction in births at the site. “The only mention of moving of staff is those who will need to accompany women while in labour,” she said. “There is no mention of staff being redeployed from one hospital to another if their job is disestablished.” Page 30 | www.ncah.com.au

The union also holds fears the plan will place the lives of newborn babies and mothers at risk. “The time it takes to transport a patient to Palmerston North will mean the difference between life and death in some situations,” NZNO professional nursing advisor Cathy Gilmore said. In a statement online, the Whanganui District Health Board said problems with recruiting and retaining obstetrics and gynaecology specialists at both Whanganui and MidCentral DHBs had reached a level “where current arrangements cannot continue without risking the safety and quality of care provided”. The proposal is open for public submission until April 11.

A top to tail service for older patients by Karen Keast A special service is helping pet-loving patients get the care they need in hospital. Pets of Older Persons, affectionately known as POOPS, is helping to care for the pets of socially isolated older people. POOPS was first established in New South Wales in 2003 when the aged care assessment team at St Joseph’s Hospital Auburn found older people were reluctant to leave their pets to go to hospital. The RSPCA now runs the program, providing assistance with veterinary care and emergency boarding throughout the state. Western Australia has now followed suit, with a group of 80 volunteers providing a free service ranging from feeding pets to walking them, and driving pets to vet appointments or kennels. POOPS WA president Tangea Tansley, who has always owned dogs and now has two rescue dogs, was inspired to introduce the charity to Perth in a bid to assist both the elderly and their animals. Dr Tansley, who has a PhD in comparative literature and is now a recently published author, said pets often provide a vital relationship for older people.

“Did she have a premonition that she herself would be in palliative care one day and could rest that little more easily knowing that her own beloved pets were being well looked after? I don’t know.

“If we have a volunteer available, we can help, and we have a lot of police-cleared volunteers stretching from top to tail of the Perth metro (area) waiting to do just that.

“Being Kath, I’m sure she was thinking only of others’ distress and being Kath, I’m equally sure she has a million hands to help her - but not all of us will be in the same boat.

“We’ve only been going in WA for a short time but almost all of our referrals so far have

Union defends Victorian paramedics by Karen Keast A car crash victim had no pulse and no obvious signs of life when two Victorian paramedics assessed him as deceased, according to the paramedics’ union. It was after the ambulance had left the scene that an SES volunteer, who was removing the 30-year-old Hawthorn driver from the vehicle, discovered the man was alive. The driver suffered life-threatening injuries when his sports car struck another vehicle and flipped onto a wire barrier, landing on its roof, on the Western Highway near Bacchus Marsh on April 1. Ambulance Employees Association Victoria general secretary Steve McGhie said the experienced male MICA ambulance officers, from the Laverton North station, did all they could in the face of challenging circumstances. Mr McGhie said the car was precariously perched on the safety wire, the victim was trapped upside down with major head trauma, and with limited access paramedics were unable to use a heart monitor. “With what they were confronted with and what they could get access to, they have ascertained no life signs for this gentleman at that stage and no pulse,” he said. “Certainly there was no palpable heart rate. “After a period of time they declared that he was deceased. At the time their decision was a reasonable decision.” Mr McGhie said some media had suggested the paramedics made an error but he said that was yet to be determined. Page 34 | www.ncah.com.au

“It hasn’t been decided it was an error,” he said. “It doesn’t mean to say that he didn’t have a pulse at the time he was assessed. The paramedics did what they would normally do.” Mr McGhie said while everyone involved in the accident was thankful the man was alive and hoped he would make a full recovery, the two paramedics involved in the case were “devastated” at how the events had unfolded. “They are coping but they are quite devastated with what’s happened,” he said. “They are adamant at the time in their checking of the patient in his car he had no pulse… and no obvious life signs. “There’s a hell of a lot of pressure on them now.” Ambulance Victoria has announced Professor Russell Gruen, from The Alfred Hospital’s trauma centre, will conduct a review of the incident.
