ACN submission supports rural nurse workforce by Karen Keast The Australian College of Nursing (ACN) has called on the Federal Government to fund a rural scholarship program to support newly registered and enrolled nurses. The proposed $660,000 three-year trial of 50 transition scholarships would support early career nurses undertake three months’ structured clinical learning in a rural health service. In its pre-budget submission outlining seven recommendations to the Federal Government, the ACN states the scholarships, providing $12,000 each for health services employing first year RNs and ENs, would address the ageing workforce, and also work to attract and retain nurses in rural areas. “The Department of Education, Employment and Workplace Relations’ Labour Market Research Nurses 2012-13 states that employers in regional areas had less success recruiting nurses than those in metropolitan areas,” the submission states. “Some locations in New South Wales, the Western Australian wheatbelt, Tasmania and Northern Territory report particular difficulties. “Often first year nurses experience working in rural health services as very stressful because many services have few or no staff available to provide clinical decision support, particularly outside business hours. “Many rural health services do not offer special supports designed for inexperienced first year nurses to transition into clinical practice.” ACN has also recommended the government provide $500,000 annually to fund a rural education scholarship scheme to support postgraduate study and professional development for rural nurses. “Both of these initiatives offer important measures to address the current health care inequalities and high costs faced by people living in rural Australia,” ACN CEO Adjunct Professor Debra Thoms said. The pre-budget submission calls for $350,000 for the design, trial and evaluation of a National Transition Framework to support new nurses transition into clinical practice, and to also support practising nurses to move from one area of clinical practice or setting to another. The submission proposes $1 million to establish an independent, national system for collecting, analysing and comparing nurse sensitive indicators, and funding for a project to map the clinical roles of nurses in non-acute care. It also suggests the government consider providing $250,000 to fund the development of leadership programs for nurse managers, and calls for funding for more Medicare Benefits items and increased rebates for nurse practitioners.

Nurse practitioners still face barriers by Karen Keast Australia’s trained and highly skilled nurse practitioner (NP) workforce is still an untapped resource, according to the Australian College of Nurse Practitioners (ACNP). ACNP vice president Leonie Burdack said despite gaining access to the Medicare Benefits Schedule (MBS) and Pharmaceutical Benefits Scheme (PBS) in 2010, legislative, policy and local barriers in both the public and private sectors are restricting NPs from working to their full capacity. “Unfortunately approximately 30 per cent of this highly skilled and experienced workforce are not being utilised for the job that they have been trained to do,” she said. Ms Burdack, who has been involved in developing the NP role in Australia since 2004, said small changes could result in significant impact that would benefit patients. “For example, at the Commonwealth level providing ‘refer and request’ provider numbers for public sector NPs would streamline access to health care service and result in minimal, if any increase in aggregate cost for the health budget - it may even provide some modest cost savings,” she said. “Most importantly, this change would improve the system for patients. “In addition, full utilisation of the NP role could also be facilitated through improved access to MBS item numbers.” Ms Burdack said the College has lobbied the government to implement simple, cost-neutral solutions. “Unfortunately, however, federation and the current state-commonwealth health funding model creates an arbitrary divide that frequently results in politicking about cost-shifting,” she said. The comments come after Australia’s 1000th NP was recently endorsed, while the Northern Territory state government has also announced it will create an extra 25 full-time NP positions by 2016. The state’s health department currently has three full-time equivalent NP positions. The government has unveiled a strategic plan for NPs, with further work planned to increase opportunities for nurses to progress their careers. “More nurse practitioners in the bush will enhance health outcomes for children, chronic disease management, including diabetes and heart disease, and mental health - just to name a few,” Health Minister Robyn Lambley said in a statement. “There are many wonderful long-term nurses who have provided health care to countless Territorians, at our homes and in our clinics, hospitals and health centres. “Increasing the opportunity for nurses to advance their careers and provide even more services…will benefit not only our nurses but our health system and the community.” Ms Burdack said the announcement was an encouraging step with the potential to significantly improve access to health care in remote areas of central Australia. She said NPs are pushing traditional health care boundaries and successfully pioneering innovative models of care, including one NP who consults patients from the surrounding rural area in a shearing shed. “The NP has a good working relationship with the ‘local’ GP who is very supportive of this practice,” Ms Burdack said. “Without this innovative model these patients would not obtain adequate health care due to access barriers.”

Federation University Australia’s Mount Helen campus in Ballarat, Victoria, features an innovative Simulated Home Environment Learning Space (SHELS), enabling students to experience real-life conditions in a patient’s home.

Professor Wellard said the space has already forced students to contemplate a variety of scenarios, safety considerations and to develop responses.

The lab boasts a house layout - complete with kitchen, bathroom and toilet, bedroom and living room - while glass walls work to provide the audience in the 50-seat auditorium with a view into the simulated home for nursing, midwifery, psychology and exercise science rehabilitation students. The space also features an ambulance and sedan, enabling simulation for manual handling for paramedic and nursing students. Professor of Nursing Sally Wellard said she believed FedUni could be the only university in Australia to have such a unique lab. Professor Wellard said the lab, which is designed for live transmission from the house to the auditorium, shines the spotlight on patient care in the home. “Students in the audience can view demonstrations or they can view peers figuring out how to work with people and how to understand how people live, rather than just thinking of giving them care and support in hospital but what actually happens when people leave,” she said. “People have to learn how to navigate and negotiate with patients in the patients’ home and they also have to understand the context of where someone comes from. She said with an ageing population, more people living with chronic disease and shorter hospital stays, nurses and allied health professionals are delivering more health services in the home. “People need support and have to manage things at home in ways that they never used to,” she said. “Nurses should always think about the environment because the environment in which people live and the circumstances of that have a huge impact on their health and well-being and what sort of resources they may or may not have to support them. “The earlier you think and understand that, the better you are going to be able to work with someone.”
