Clinical Midwifery Consultant - High Risk Pregnancy Royal North Shore Hospital An exciting position exists for a highly motivated and experienced midwife to join the multidisciplinary team within the Division of Women’s, Children’s and Family Health at Royal North Shore Hospital. The Clinical Midwifery Consultant - High Risk Pregnancy provides expert clinical advice to patients, carers and other health professionals within their defined specialty. The role provides for expert midwifery advice to a range of health professionals in both inpatient and ambulatory settings as it pertains to pregnancy with additional risk factors. The role also has the scope to provide continuity of care and expert midwifery management for women presenting with high risk pregnancy in collaboration with all members of the Maternal Fetal Medicine team. Royal North Shore Hospital is a Level 6 tertiary referral hospital within the Northern Sydney Local Health District. Royal North Shore Hospital is one of seven referral hospitals in NSW providing tertiary maternity and neonatal services across the state. The MFMU is a high quality multidisciplinary service providing expert diagnosis and management for women with complex pregnancies. The Clinical Midwifery Consultant has a pivotal role in providing and co-ordinating midwifery care for women with complex health needs across the organisation. Tasmania’s graduate nurses migrate north by Karen Keast Tasmania’s graduate nurses are migrating to the mainland for work with only 41 of the state’s 275 graduate nurses securing full-time work in the public health sector this year. The bleak news on the job front for new nurses comes after NCAH recently revealed Tasmanian nurses with families are flying interstate for shift work as the state’s health woes continue to bite. Meanwhile, the State Government has announced recruitment for graduate nurse positions within the Department of Health and Human Services (DHHS) will open from September 5-14 for early 2013 positions. Health Minister Michelle O’Byrne said the government was working to help new graduate nurses find employment but was Page 6 | www.ncah.com.au

unable to reveal how many positions would be made available. “The exact number of positions is still being determined as the THOs (Tasmanian Health Organisations) and Statewide and Mental Health Services work to identify as many positions as possible,� she said. “The number of graduate nurses appointed must take into account the skill mix and support requirements of each clinical setting. “The process for offering new places is a complex one and the number of positions fluctuates from year to year.� For the full article visit NCAH.com.au

Nurses push for walk-in centres by Karen Keast South Australian nurses are calling for four walk-in centres to be established to ease the pressure on the state’s emergency departments.

“We are all well aware that the hospital system is under considerable pressure and the government has a need to reduce its expenditure in the area of health.

Australian Nursing and Midwifery Federation SA branch secretary Elizabeth Dabars said the union wanted walk-in centres staffed with nurse practitioners and advance practice nurses.

“This is a safe, effective way of providing better access to quality care.”

Ms Dabars said the centres could be located in metropolitan Adelaide, in central, north and south locations, and one initially in country South Australia, with more to be rolled out at a later stage. “We would really like to see some positive developments in the area sooner rather than later,” she said. Prostate cancer program for specialist nurses tracking well by Karen Keast A program established to ensure men diagnosed with prostate cancer receive coordinated, specialist nursing care throughout their entire journey has received strong uptake by the health and nursing sector. Launched in May by the Prostate Cancer Foundation of Australia (PCFA) with the support of the Movember foundation, the PCFA Prostate Cancer Specialist Nursing Service is a new program to trial nurses in both metropolitan and regional Australia. Designed to deliver 13 specialist nurses to metropolitan and regional hospitals in the Northern Territory, Queensland, New South Wales, Victoria, South Australia, Tasmania, Western Australia and the ACT, the service looks set for a strong future, says Julie Sykes, Director of Nursing at Prostate Cancer Foundation of Australia. “Since its launch, PCFA has received over 60 expressions of interest from hospitals across Australia who are interested in hosting a prostate cancer specialist nurse.” Sykes said the idea behind the service was to eliminate the disjointed nature of care for Australian men diagnosed with prostate cancer, adding Australia’s first national Prostate Cancer Specialist Nursing Program would also provide nurses with a new arena in which to expand their experience, she said. “The Prostate Cancer Specialist Nursing Program provides registered nurses who are interested in working at a specialist level Page 12 | www.ncah.com.au

Kissing sailor and nurse revealed by Karen Keast end of the war was announced on August 14, 1945. In an interview with CBS, George admits celebrating the news with a few drinks before he saw the nurse and complete stranger, Greta Zimmer Friedman. George left Rita’s side, and in a moment of appreciation and celebration, he caught Greta, who was actually a dental nurse, in a passionate embrace in New York City’s Times Square. “The excitement of the war being over, plus I had a few drinks…so when I saw the nurse, I grabbed her and I kissed her,” he told CBS. “I did not see him approaching, and before I know it, I was in this vice grip,” Greta recalled.

The sailor at the centre of the famous kiss with a nurse that symbolised the end of World War II has revealed the passionate embrace was a celebratory moment of gratitude for nurses. A new book, The Kissing Sailor, reveals the identities behind the famous kiss, captured by Life magazine photographer Alfred Eisenstaedt, and unravels the mystery behind the iconic image in US history almost 70 years on. The sailor has been identified as George Mendonsa, then aged 22 and now aged 89, who was on leave after surviving fighting in the Pacific, where he watched nurses care for wounded soldiers. Neither Rita, George or Greta saw Eisenstaedt take the photo which later appeared in the August 27 edition of Life magazine and generated widespread interest about the identity of the kissing couple. In 1980, Life launched an investigation in a bid to identify the kissing sailor and nurse, prompting an overwhelming response from war veterans and nurses who claimed to be the couple. That was when George, who now lives with Rita in Rhode Island, realised it was him in the photo. Greta, from Austria who fled to the United States with her sister, now lives in Maryland and is a widow with children.

Queensland nurses urge politicians to take heed of nursing campaign by Karen Keast Queensland nurses hope the Australian Nursing Federation’s new national advertising campaign will serve as a timely reminder to the State Government of the valuable contribution nurses and midwives make to the health system, in the lead up to next month’s budget. Queensland Nurses’ Union secretary Beth Mohle said the ANF’s positive nursing campaign comes as the state’s health sector faces uncertainty amid rumours up to 4000 Queensland Health jobs face the axe. Ms Mohle said there was widespread anxiety surrounding the job cuts, which are expected to target middle management, and their ramifications for nurses and midwives when it comes to service delivery. “Our members are so distressed right now about the uncertainty, about what the future holds,” she said. “If they have got their jobs under a cloud and they don’t think straight…that could have a flow on effect to patient care. “The campaign is about reminding everybody, including politicians, about the valuable contribution nurses and midwives make.” Ms Mohle said it was coincidental the campaign, which has been in the pipeline since last year, was released as the new government prepared to curve spending on health care, one of its biggest expenditure areas. She said the campaign was not aimed as a salvo against cost cuts but was instead a Page 16 | www.ncah.com.au

Congress provides dietitians with food for thought by Karen Keast Australia’s dietitians are counting down the days until Sydney takes to the world stage for the 16th International Congress of Dietetics (ICD2012). More than 2000 of the world’s leading dietitians, nutritionists and nutrition science researchers spanning 60 countries will converge on Sydney for the September 5-8 event. Held every four years, this is the first time Australia will host the event since 1977. Delegates will explore the theme ‘LEAP’: Leadership, Evidence and Advancing Practice in a dynamic program that will showcase the latest nutrition research on issues including eating for sun protection and the use of omega-3 fats in pregnancy. Dietitians Association of Australia CEO Claire Hewat said the event will feature local and international experts presenting research on the most pressing nutrition issues of our time. “In more than 200 presentations, they’ll look at topics such as nutritional genetics, food security and childhood obesity, along with the latest findings in nutrition research – including eating for sun protection, omega-3 fats in pregnancy and the benefits of cereal fibre,” she said. “It’s the place to be to hear the latest on nutrition research from across the globe.” Ms Hewat said the event was an amazing avenue to explore new directions and debate

Collaboration and insurance win for midwives by Karen Keast Privately practising midwives will be able to forge collaborative arrangements with hospitals and health services in a move being heralded as a major win for the midwifery profession. Health Ministers have also backed plans to extend the professional indemnity insurance exemption for midwives providing homebirthing services until June 2015. Australian College of Midwives spokesperson Hannah Dahlen said the Standing Council on Health’s recommendations came as a “relief” to midwives. “It’s one small step forward – one small step for midwives and one giant step for womankind, I hope,” she said. Dr Dahlen, a University of Western Sydney Associate Professor of Midwifery and privately practising midwife, said only “a handful” of midwives across Australia had managed to secure collaborative arrangements with obstetricians under the current system. Dr Dahlen said collaborative arrangements were only required under legislation where privately practising midwives want to provide patients with access to Medicare rebates. She hoped the changes, enabling agreements between midwives and hospitals and health services, would encourage more of Australia’s 17,000 midwives to become eligible to provide Medicare-funded care. “We have to now see hospitals opening their doors for that to work and for there to be a good take up of it,” she said. ACM president Professor Sue Kruske said midwives had struggled for two years to Page 24 | www.ncah.com.au

achieve collaborative arrangements with obstetricians. “Some midwives have written to every obstetrician in the state they live in seeking a collaborative arrangement, only to have either no response or negative responses,” she said. “The vast majority of midwives practising today collaborate without the requirement to have a formal arrangement with a ‘named doctor’. “It is women who have suffered due to the current arrangements.” Professor Kruske said the arrangements would have no impact on homebirthing as Medicare rebates are not available for home births. Dr Dahlen said the professional indemnity insurance exemption was due to expire in June 2013 and recommendations for it to be extended for two years would provide certainty to women falling pregnant from as soon as September.

New app for physiotherapists by Karen Keast A new application is helping physiotherapists educate their patients on neck manipulation.

The Australian Physiotherapy Association recently released the Manipulation Consent app, designed for iPhones and iPads, to end confusion for physiotherapists when it comes to obtaining informed consent for spinal manipulation.

“This app allows physiotherapists to communicate clearly and effectively, and ensure patients understand any risks associated with the procedure.”

The app, which costs $2.99 from the iTunes App Store, is the APA’s first app and already more than 100 of the apps have been sold. The app features an 80 second video for patients, available in seven languages, demonstrating the process of neck manipulation; designed to show patients what they can expect while also outlining the potential risks. The app also includes six other resources for physiotherapists, which can be emailed or printed, including a patient education fact sheet, informed consent labels, clinical guidelines and VBI assessment and cervical treatment flowcharts. It also features videos for physiotherapists on simulated manipulation position and sustained end-range cervical rotation testing. APA president Melissa Locke said the app was designed to ensure clear and comprehensive communication with patients. “Physiotherapists are committed to their ethical and legal obligations to disclose any risk to their patients prior to an examination and treatment,” she said. “The APA recognises that there can be some confusion as to how to obtain informed consent; and the process can be Page 26 | www.ncah.com.au

An APA spokesperson said the app had been well received from the physiotherapy community and the association would consider releasing more apps. “There are no definitive plans at this point but we’re considering many ways to take advantage of technology to better service the physiotherapy profession and promote wellness among Australians,” he said. “This may include developing apps for the APA’s physiotherapy conference, our publications, or some other purpose.”


ANF is now the largest health and aged care union by Karen Keast Twenty thousand people have joined Australia’s peak nursing union in the past year, helping it reach a milestone of 220,000 members. The Australian Nursing Federation is now the nation’s largest health and aged care union and is also one of the country’s fastest growing unions. The milestone comes as the union prepares to launch a new national campaign, titled Nurses and Midwives – You Couldn’t Be in Better Hands. The campaign will include television, radio and cinema advertisements featuring real nurses and midwives in a bid to show the dedicated and outstanding professional care that nurses deliver daily to their patients. ANF federal secretary Lee Thomas said the campaign, which included a new website, would enable the community to get behind the nursing profession. “The campaign showcases the incredible and invaluable work that nurses and midwives do day in and day out across Australia,” she said. “Nurses, midwives and the general public will also have the opportunity to share the ads on social media and go to the ANF web page to show their support for nurses and midwives.” Established in 1924, the ANF represents nurses in urban, rural and remote locations in

both the public and private sectors, including hospitals, health and community services, schools, universities, the armed forces, statutory

authorities,

offshore territories and industry. New guidelines to help nurses cope with moral distress by Karen Keast New Zealand researchers will develop new guidelines in a bid to reduce the risk of nurses suffering from moral distress, after a landmark study found nurses are struggling to cope in the face of ethical issues and workplace constraints. The Massey University study, the first of its kind in New Zealand, found 48 per cent of nurses are considering leaving their jobs due to moral issues beyond their control. Topping the list of moral dilemmas was nurses’ concerns with the delivery of “less than optimal care” due to management pressures to cut costs, while nurses were next most concerned with patient care declining due to a lack of provider continuity. Nurses also raised concerns of working with colleagues and other health care professionals who were “not as competent” in providing the care the patient required, and held concerns with carrying out physicians’ orders for what nurses believed were unnecessary tests and treatments. Nurses also flagged moral concerns over moves to perform extensive life-saving actions which they believed were only prolonging the dying process. The findings come from a survey of 412 hospital-based nurses across New Zealand and support the outcome of a pilot study of 15 nurses conducted last year, which found six out of 10 had resigned from nursing positions due to moral issues. Nursing ethics and education expert Dr Martin Woods, who lead the research, said the study was proof that nurses are struggling with moral distress, which arises when internal or external constraints impact upon Page 30 | www.ncah.com.au

a professional’s ability to deliver ethicallyappropriate actions. “The research findings showed that in nearly all cases it is the institutional constraints, and not personal factors, that cause distress for nurses when they confront moral issues in the workplace,” he said. In the survey, one nurse revealed she was considering quitting a job she mostly enjoyed due to “lack of leadership and pressures from management to accept more numbers of patients or patients with high acuity of care with no increase in resources”. Dr Woods said moral distress can lead to nurses suffering from burnout, stress and depression. He said researchers will use the findings of the survey to develop guidelines for nurses and health care agencies in a bid to help safeguard nurses from moral distress.

Falling cost of PBS medicines a double-edged sword by Belinda Smart

While the falling costs of Pharmaceutical Benefits Scheme medicines thanks to the introduction of price disclosure in 2008 are a plus for the sustainability of affordable medicines, the news may not all be good for pharmacists. A significant drop in the average cost to government of PBS prescriptions was recorded in the Medicines Partnership of Australia’s PBS Scorecard for August. The drop is confirmed in the June 2012 PBS data in the form of a $5 drop in the average cost of prescriptions – falling from just over $44 to just over $39. Price disclosure was introduced by the government to ensure price transparency as growing numbers of low cost generic medicines entered the market, following the patent expiration of their “blockbuster” branded counterparts. According to the Medicines Partnership of Australia, an alliance of peak industry associations representing key members in the supply chain that deliver medicines and pharmacy expertise, dropping prices reflected the efficacy of price disclosure in fostering significant government savings. “This is another example of why the Medicines Partnership of Australia argues that we need to have clear agreed, publicly available measures of volume and price growth in the PBS, so that government, industry and all stakeholders can make meaningful comparisons and draw reliable Page 32 | www.ncah.com.au

projections of this important public sector expenditure item.” Greg Turnbull of the Medicines Partnership of Australia said with an average of 75 percent of pharmacists’ business residing in PBS related sales, the sustainability of the PBS scheme resulting from keeping prices low was of significant benefit to maintaining a strong future for medicine delivery. However he did also submit that there might be challenges ahead for pharmacists. “I should stress that in principle we are very strong supporters of price disclosure. However while price disclosure creates savings for taxpayers it may have a negative impact on some pharmacists’ margins. If you’re a pharmacist and your rents go up, you don’t have the choice of raising prices on PBS medicines to boost your margins. Around 120 pharmacists have gone bankrupt in the last 12 months to June, which is more than the total for the previous 10 years. Not all of that is because of price disclosure, but it’s a contributing factor.”

Speech pathologists urge Aussies to speak out by Karen Keast Speech pathologists are urging Australians to share their stories. The profession’s peak national body, Speech Pathology Australia, is encouraging the more than 1.1 million Australians estimated to have difficulty communicating to get involved in a national story-telling campaign. The campaign, held to mark Speech Pathology Week from August 19-25, is designed to showcase the essential work of speech pathologists and how the profession changes lives. Titled ‘The Great Australian Communication Story’, the campaign encourages people to share their experiences of communication and communication impairment in speech bubbles.

The speech bubble submissions, which will be open online for several weeks, will be transformed into an e-book. An SPA spokesperson said the campaign showcased the invaluable work of speech pathologists. “The purpose of this is to take a snapshot of the stories and experiences of people living and working with communication impairment in Australia and will demonstrate the need and vital role of speech pathologists,” she said. “The speech bubbles will be turned into an e-book, which we hope to publish in September, and will provide an insight into all the different ways we communicate in Australia in 2012.” For the full article visit NCAH.com.au Nursing Careers Allied Health - Issue 17 | Page 33
