Working safe – ending violence against remote and rural nurses by Belinda Smart While working in remote and rural Australia is often immensely rewarding, the isolation that typically accompanies such positions, particularly in the remote outback, can have pitfalls. One of these is the violence occasionally experienced by health professionals working off the beaten track, which can stem from a number of factors including substance abuse, mental illness, or in some cases premeditated criminal motivations that take advantage of an isolated position, often occupied by a relative outsider. Whatever the reasons behind it, the issue is on the rise, to the extent that a new project, known as the ‘Working Safe in Rural and Remote Australia’ framework has recently been launched with a view to stemming violence against remote and rural health workers. Jenny May, chair of Rural Doctors’ Association of Australia’s Female Doctors’ Group, and one of the prime supporters of the initiative, confirms the framework is its initial stages. “Stage one of the project will seek to lay the foundations for building safer workplaces in rural and remote Australia by increasing our understanding of current initiatives and strategies and their effectiveness, and developing a national framework for action for whole-of-community responses to working safely,” Dr May explains. www.ncah.com.au

With the timeframe for completing stage one of the project set at December 2012, stage two will focus on monitoring the incidence of violence in rural and remote workplaces and the effectiveness of existing and future initiatives and strategies, while stage three will establish a multi-disciplinary professional working safely training program. Dr May believes the project is vital to safeguarding a future in which nurses and other health professionals feel incentivised to take up postings in rural and remote areas. “There is little doubt anecdotally these issues are key to recruitment and retention,” she says. The proposal document for the Working Safe in Rural and Remote Australia project echoes this view. While the extent to which poor management of workplace violence contributes to the turnover of staff in rural and remote areas is unknown, “we do know that there is a shortage of professionals and a high turnover of staff in rural and remote areas,” the document states. “We also know that staff in rural and remote areas require more support than their urban counterparts because of the isolation they experience.” “Staff are often exposed to potentially threatening and confronting circumstances and must be supported strategically and professionally. A high turnover of staff leads to poor evaluative reviews of critical incidences and poor continuity of knowledge of strategies for improvement relating to issues such as violence in workplaces. In turn, high turnover of staff is disadvantageous

Lenthall recently co-authored a paper ‘Trends in workplace violence in the remote area nursing workforce,’ whose findings show increases in all incidents of reported violence in the workplace between 1995 and 2008. “Verbal aggression, property damage and physical violence are the most frequently experienced forms of violence as perpetrated directly towards remote area nurses, with statistically significant positive correlations between all types of workplace violence and PTSD symptoms.” “Nurses working in very remote regions in Australia are fearful for their personal safety,” the paper confirms. According to Carole Taylor, CEO of The Council for Remote Area Nurses Australia, also known as CRANAplus, appropriate training is key to any measures designed to reduce violence. Taylor makes a distinction

“It’s ill advised to be sending out staff and workers who are not properly oriented to deal with the challenges of the remote communities,” she says, adding she is also “vehemently against” the practice, still common in some places, of single nurse or even two-person postings. “Aside from the fact that nurses benefit from the moral support of having someone else around, they also need to take time off if they fall ill and need back up if they are called away to tend to a health issue.” As well as running its own short orientation courses for remote nurses, CRANAPlus is also currently piloting a community connectedness program that assist nurses in forging ties with key members of the local community. “In a setting where spearings and snake bite are common issues, training for remote nurses should include localised, cultural awareness,” Taylor says. “You also have to have a personality that’s pretty much bomb-proof and be happy operating in an isolated place. It’s not for everyone.” Above all, Taylor says, one of the keys to dealing with the problem of violence against remote nurses and health workers lies in greater recognition of the contribution they make. “It’s about whether the service is considered valuable and whether or not the people working in it are valued.” careers@ncah.com.au

Outback Queensland Lures Dietitian Dietitian Neha Bhatia knew moving to Queensland’s outback would be a big step. But it’s one the accredited practising dietitian has taken in her stride. Originally from New Delhi, India, Neha took a position in the small town of Charleville, which has a population of just 3500 people set in the heart of rural and remote south west Queensland, after graduating from university more than a year ago. “As an international person I was always fascinated about working with the Indigenous Australians to help educate them about food and its relation with disease,” she said. “During my university years I learned that there is a great need for qualified health professionals in the remote parts of Australia as more and more people were diagnosed with chronic diseases in the outback. “This made me move…so that I can give my services to the ones in need and also learn as I experience.” As the manager of clinical services and as a dietitian for the south west region with RHealth’s GP Division, Neha’s role involves day to day dietetics counselling for patients and she also visits the outreach areas to provide clinical services, occasionally taking to the air with the Royal Flying Doctor Service. She also plans nutrition education events and activities for people living in the region and manages a team of allied health practitioners. Neha said she has no plans to move - she loves the outback and its lifestyle, while she believes working in rural and remote Australia has bolstered her skills. www.ncah.com.au

It is far from the city rat race and has a strong community bond. “In my opinion everyone should experience the rural-remote as it can be (a) major life changing experience. I have learned to be more tolerant with my practice.” Neha said there is still a lot of work to be done to help close the gap for Indigenous Australians. “The biggest challenge is to get clinicians and health professionals working in the remote areas,” she said. “We need more and more Indigenous Australian health workers to support the much needed target groups.”

Victorian rural nurse bank to expand A dynamic casual nurse bank specifically designed for rural health will expand across Victoria and could more than double its staffing numbers. CasConnect, based in the Grampians region, has been given the green light to expand across rural and regional Victoria after a successful pilot phase. Tasmanian nurses in limbo Tasmania’s public sector and graduate nurses are in limbo as the State Government works to trim $100 million from its health budget this financial year. The government has postponed its recruitment of 120 graduate nurses while there are union fears nursing positions will be axed under the budget cuts. Australian Nursing Federation Tasmanian branch secretary Neroli Ellis said the union was seeking an urgent meeting with the government in a bid to protect all 120 graduate positions. Ms Ellis said the government’s decision had come after the mainland hospital recruitment period had closed.

“I’m glad that they will no longer have to worry about having something to give to bereaved parents.” The memory boxes include items from sponsors at Russ Teddies, Australian Soy Candles, David Jones, Bubba Blue, Kora Organics By Miranda Kerr along with handmade gifts. “Our boxes are beautiful and we make sure every box is perfect,” Erin said.

That kind gesture prompted Erin to launch Mums Like Me, an organisation that now supplies free memory boxes to 23 hospitals in Australia.

“We also supply handmade scrapbook albums and tiny clothes for babies born after 20 weeks for those hospitals who don’t already have access to these.”

“I really had the most amazing midwives and nurses caring for me when my sons died,” Erin said.

Erin said the group has donated 90 memory boxes in the past nine months.

“They were so very kind and presented me with a gorgeous keepsake bag …so that parents had something. “I thought it was a beautiful gesture - all at their own time and expense.” A year later, Erin met another mum who lost her baby daughter. “It was such a positive meeting, that I didn’t want anyone else to have to wait a whole year before meeting someone like them,” she said. “It was then that the Mums Like Me concept evolved. Once a month ‘mums like me’ meet and create our memory boxes, which are donated to hospitals who have requested our program. “It is my hope that by creating these memory boxes and donating them to hospitals, I am also giving back to the wonderful professionals who cared for me. www.ncah.com.au
