Midwifery issues hit the bush By Nina Hendy The current nursing shortage hitting the Australian health system could ultimately force midwives to give up their quali¿cations, it has been revealed. Midwives are apparently ¿nding it dif¿cult to take time away from their job to undertake the required training, knowing the days away from the workplace would leave their patients and colleagues under huge strain given that replacement staff don’t exist. The situation is so bad that some have even considered letting their midwifery skills go. A clinical management nurse working in rural Western Australia says it is practically impossible for midwives to take time away from their job to travel into city areas to attend the required training to maintain their midwifery status. Theresa Beech, who is in charge of the nurses at the rural West Australian Kellerberrin Memorial Hospital, says while the training courses to keep the midwifery skills are necessary, it’s increasingly dif¿cult for staff in rural and regional areas to attend. “I just can’t spare staff to travel into the city to attend the necessary training because there’s absolutely no-one to back-¿ll their roles. That means that having to send staff to Perth for even just two days of training would be a huge strain on us. “It’s becoming a major issue for midwives, with some having to consider the possibility that they may need to let their quali¿cations go. Midwives aren’t the type to leave their colleagues in the lurch.” Beech says while the rural hospital is fully staffed by 14 nurses working in both full time and part-time positions, reserve staff positions to replace someone during training days don’t exist. The issue is no doubt replicated at hospitals across the country, but is likely to be a far bigger problem in rural and remote areas. The situation highlights the skills shortages that have

hit the midwifery sector. In fact, the Australian Institute of Health and Welfare (AIHW) has reported statewide skills shortages of registered midwives and nurses in all jurisdictions, and shortages of enrolled nurses in NSW, QLD, WA and ACT. Debra Cerasa, CEO of the Royal College of Nursing, Australia and The College of Nursing says the country’s nursing and midwifery workforce is also ageing, which means the situation could worsen. Statistics from the AIHW shows that between 1997 and 2007 the average age across both nursing and midwifery had increased from 40 to 44 years. It also showed that between 2003 and 2007, the proportion of nurses who were aged 50 years and older increased from 28.2 per cent to 33 per cent. “If this trend continues, which appears likely, then we are facing a situation fairly soon where our most experienced nurses will be leaving the workforce. This will exacerbate existing shortages and create additional skill mix problems. “We need a massive commitment to ensure nursing and midwifery professionals are able to anticipate and effectively bridge the gaps that will emerge as the workforce starts to retire in large numbers. It’s not just about numbers. It is also about how we shape the future workforce to meet the community’s future health needs,” Cerasa says. There doesn’t appear to be an easy answer to the issue. The Australian Nursing Federation (ANF) is no doubt hoping its calls to the Federal Government to provide incentives for nurses who move to regional and remote locations will go some way toward solving the issue, but even if its calls are successful, changes won’t occur overnight. The ANF says that new data from the Australian Institute of Health and Welfare reveals that nurses are providing the bulk of healthcare in regional and remote areas. Yet a gulf exists between the level of support they are given compared to doctors who take up employment in remote locations. It says that some incentives are paid to nurses and midwives on a state by state basis, but that these are grossly inadequate compared to doctors. However, perhaps the short-term answer lies in technology. More reliable broadband could make virtual classrooms commonplace for midwives practicing beyond the city limits. The ANF says patients could speak with a nurse, midwife, nurse practitioner or other health professional through a broadband-enabled network, which is only one step away from a face-to-face consultation. However, technology could also be used for greater education of midwives. Although whether the issue is going to make it onto the agenda of the decision-makers is yet to be seen. Copyright NCAH

IN HUMANITARIAN WORK Two Royal Australian Air Force (RAAF) nursing officers have returned to the familiar sounds of jet engines and planes taking off, having left behind the everpresent hum of a ship’s engine they have experienced for the past six weeks. Squadron Leader (SQNLDR) Toni Murry and Flight Lieutenant (FLTLT) Danny O’Neill have returned from the multi national military exercise Pacific Partnership 2010 (PP10), onboard United States Naval Ship (USNS) Mercy. The nursing officers, as part of the first Australian Defence Force (ADF) contingent to participate in the US-led assistance mission, were assimilated into a 900-strong team providing humanitarian assistance in Vietnam and Cambodia. Patients from both countries were brought on board the 69,000-tonne hospital ship, which is equipped with 12 full size operating theatres and 1,000 bed hospital. FLTLT O’Neill assessed and admitted the patients in the casualty receiving area while SQNLDR Murry nursed patients in the surgical ward before and after operations. Both found the experience to be challenging and rewarding, professionally and personally. FLTLT O’Neill said the experience
was daunting for most patients so a reassuring and comforting approach in the casualty reception area went a long way in easing the stress of the situation. “I had my team of United States Navy hospital corpsmen and together we welcomed, assessed and admitted the patients upon arrival,” he said. “Most of them had never been on a big ship before so they were nervous and also eagerly anticipating their treatment.” SQNLDR Murry said the usual demands of the surgical ward workload were enhanced by communication issues. “In order to ease the communication difficulties we had interpreters in each country and, while they worked really hard to help us, sometimes they had trouble understanding my Australian accent,” SQNLDR Murry said. “I was able to rely on some sign language, though, which always managed to get a laugh from my patients.“The patients were just beautiful and even though they were only in my ward very briefly I was always sad to see them get on the boat to go back ashore after their surgery. “At the same time, though, I was filled with pride, having helped in some small way to improve their lives.”
