Midwifery Scholarships> Have you always dreamed of working for Australia’s famous aeromedical health service but do not meet the Flight Nurse position requirements to be endorsed as a Midwife? The Royal Flying Doctor Service is now providing an exciting opportunity for suitably qualified Registered Nurses to access a limited number of Midwifery Scholarships. The Scholarships, available across Australia, will provide financial support to those Nurses so they may gain this important qualification. These scholarships are to be used for costs associated with your study towards a Midwifery Qualification in 2013/14 such as course fees, text books, etc. Selection Criteria:  Registered General Nurse with AHPRA Registration;  Emergency Nursing or Critical Care Certificate/experience;  Significant relevant post graduate nursing experience;  Australian citizenship or permanent residency in Australia.

3CHOLARSHIPS AND &UNDING !VAILABLE FOR %LIGIBLE 3TUDENTS Demand for capable and experienced registered nursing staff is forecast to continue growing beyond 2025 influenced by an ageing population, increased demand for health services and an increasing expectation for service delivery in the home and in registered facilities. Workforce planning authorities’ estimate that by 2025 Australia will have a shortfall of 100,000 nurses. A number of these nurses may be drawn from the community of registered nurses who have already completed their training and are currently working in non-health roles. However for many nurses it is simply not possible to undertake full-time study while balancing the demands of family and other work. Online education has been embraced by the healthcare sector and now nurses seeking to re-enter the nursing workforce can complete their re-entry training through a blended online learning program. The Institute of Health and Nursing Australia now offers a reentry program for nurses allowing them to complete online up to 120 hours of required 160-hours of practical training, with a further one-day per week of practical training at one of the Institute’s local Education Centres around Australia. The availability online of the Return to Nursing program will

enable trained nurses living in regional and rural areas to complete the theoretical component of their training online while undertaking practical training and their Clinical Placement at a local hospital. Bijo Kunnumpurath, CEO at the Institute of Health and Nursing Australia noted that feedback from nurses around Australia showed they are not always able to leave their current nonhealth jobs and support full-time study, and that classroom based study required attendance at classes and draws heavily on extensive childcare and alternative family arrangements during the hours of study. “Our aim is to realise lifelong careers in healthcare,” says Mr Kunnumpurath. “The investment in training and education throughout a nursing career is considerable, and this education must be accessible and engaging if we are to retain our valuable nursing resources in the healthcare system.” The Return to Nursing online program costs $8,250 (incl GST) which includes all materials and access to IHNA’s online library and student support services. Hospital to trial nurse-led discharge program by Karen Keast Nurses and allied health professionals will be able to discharge patients under a new pilot program at South Australia’s Flinders Medical Centre. The state government has announced its Criteria Led Discharge initiative, now being developed by doctors, nurses and allied health professionals, has been designed to reduce patients’ length of stay in hospital and free up hospital beds. Expected to be introduced early next year, the program will enable nurses and other authorised health professionals to discharge patients who have met specific criteria established by the patient’s doctor or consultant. Australian Nursing and Midwifery Federation SA branch secretary Elizabeth Dabars said the union had been calling for more government focus on advanced practice nurse roles, including nurse-led discharge. “We are very pleased that the nurse-led discharge was picked up,” she said. “It’s a very important initiative; we are very pleased to see it. “We believe it will have the capacity to reduce the number of people spending unnecessary time in a hospital bed, which is of benefit to freeing up the emergency department.” For the full article visit NCAH.com.au

Go flying with midwifery scholarships by Karen Keast Interested in taking your nursing skills to new heights? The Royal Flying Doctor Service has a limited number of midwifery scholarships across Australia for registered nurses wanting to become flight nurses with the nation’s renowned aeromedical health service. RFDS WA flight nurse coordinator Paul Ingram said it was vital flight nurses have midwifery qualifications as about 10 per cent of the organisation’s work is obstetric-related. “These scholarships are important in attracting high quality nursing staff with a critical care background to flight nurse positions with the RFDS,” he said. “Royal Flying Doctor Service flight nurses have to be highly skilled and capable of responding to a wide range of situations. “On any given day they may have to respond to many different injuries, illnesses and conditions.” In Western Australia alone, RFDS has 50 flight nurses located across five bases at Jandakot, Kalgoorlie, Meekataharra, Derby and Port Hedland and each year awards two midwifery scholarships. Shane Miller was the first person to take up a midwifery scholarship in WA. Seven years ago, Shane was working as a clinical nurse specialist in a hospital emergency department when he inquired about becoming a flight nurse. Australia’s nursing and midwifery stats revealed by Karen Keast There are almost 34,000 male nurses and midwives in Australia and six nurses who are aged 86 and over, according to new statistics from the Nursing and Midwifery Board of Australia. The national board has released its latest registration statistics which provide a snapshot of the nation’s nursing and midwifery workforce. The data reveals a state by state breakdown of nursing and midwifery numbers and registration types and also the age of the workforce by registration type.

with 64,335, Western Australia with 33,830, South Australia with 30,695, Tasmania with 8,091, the ACT with 5,246 and the Northern Territory with 3,856. There are 765 nurse practitioners, which includes prescribing scheduled medicines, registered in Australia, there are 777 registered nurses who can supply scheduled medicines in rural and isolated practice and one midwife practitioner who is located in NSW.

There are six nurses registered who are aged 86 and above, 35 who are aged 81 to 85, 254 who are aged 76 to 80, and 1,417 who are aged 71 to 75.

There is also one eligible midwife, who can prescribe scheduled medicines, in the nation who is located in Victoria, and there are 130 eligible midwives with a notation on their registration who are endorsed to prescribe scheduled medicines when they complete a board-approved program of study for prescribing midwifery medicines.

Most nurses and midwives, at a figure of 51,055, are aged 51 to 55, there are 44,782 nurses aged 46 to 50, 42,783 aged 41 to 45, and 39,947 aged 56 to 60.

There are 38,621 aged 36 to 40, 35,499 are aged 26 to 30, 32,937 are aged 31 to 35, 22,537 are aged 61 to 65, 20,352 are aged 21 to 25, and 6,969 are aged 66 to 70. When it comes to gender, the figures show there are 33,891 male nurses and midwives with 27,307 male registered nurses, 5,348 male enrolled nurses, nine male registered midwives and 633 male registered nurses working in midwifery and nursing. It also shows Australia has a total of 337,807 nurses and midwives, with 210,320 female registered nurses, 53,559 female enrolled nurses, 2,270 female registered midwives and 32,089 female registered nurses working in midwifery and nursing. Nurses’ union takes its aged care campaign to parliament by Karen Keast “Our members spoke passionately and articulately about the valuable work they do for our country’s most vulnerable citizens and it was heartening to see how interested our members of parliament were in the problems and solutions,” she said in a statement. “There was clearly cross-party agreement that the residential aged care sector needs attention. “We hope they can continue to work together to make the changes that will ensure quality care for older New Zealanders and decent pay and working conditions for those who care for them.”

New Zealand’s embattled aged care sector is once again in the spotlight after the Health Select Committee heard calls for fair pay and safe staffing levels. In its submission to the committee, the New Zealand Nurses Organisation, which represents more than 45,000 nurses and health workers, outlined the problems facing the sector and also a series of solutions to tackle the issues. The union has repeatedly called for an overhaul of the sector, including funding transparency in a bid to show how government funding is used in the sector, along with better pay, training recognition and the development of standard safe staffing levels. NZNO spokesperson Susan Stewart said the solutions came from the nurses and health care assistants who work in aged care. Page 18 | www.ncah.com.au

More than 11,000 people last year signed a charter backing the union’s calls for safe staffing levels, better pay and conditions for aged care workers, training, and a guarantee that government funding is used solely for the nursing and personal care of residents. The hearing comes after a Human Rights Commission report found discrimination is rife against New Zealand’s 48,000 aged care workers, with many workers found to be underpaid and undervalued, despite the sector evolving into one of the country’s fastest growing industries. The inquiry found a serious inequity in pay rates with care workers in the community, funded by District Health Boards through providers, often paid $3 to $5 an hour less than caring staff directly employed by the DHB. The report recommended a series of solutions including pay parity, a fair travel policy, the voluntary safety standard to become compulsory and a stakeholder summit.

Fears more aged care nursing jobs to go by Karen Keast The Queensland Nurses’ Union fears up to 100 nurses face redundancy and hundreds more jobs are at risk if government-owned aged care services continue to be privatised.

“The skills mix within Queensland Health facilities is significantly better with the proportion of registered nurses and enrolled nurses to unlicensed workers.

The concerns come after Metro North Hospital and Health Service announced plans to close two properties at its Eventide Aged Care Campus at Brighton.

“In the private sector, there is much evidence to support there are limited numbers of registered nurses in private aged care facilities and significantly lower and sometimes no registered nurses on shift in private facilities.”

The closure affects an estimated 120 jobs, including about 100 nurses, with residents to be moved before June next year. In announcing the decision, board chair Dr Paul Alexander said the private and nongovernment sector had the capacity to manage residential aged care in Brisbane’s northern suburbs. “We have looked at the market and there is the capacity and capability to look after these residents,” he said in a statement. “We feel they are better placed to provide this type of care as we are primarily in the business of acute hospital care.” QNU assistant secretary Des Elder said the union holds concerns more State Government aged care services could follow suit. “The government has made comments to the affect that they believe their core business is around acute care and aged care certainly isn’t their core business,” he said. “With aged care residents we are very concerned because we are looking at the comparison between staffing numbers in Queensland Health aged care facilities and those in the private sector facilities and staffing levels in Queensland Health are significantly higher. Mr Elder said the union was also alarmed at a growing trend in the private sector to use unlicensed workers to handle medication through Webster-paks. “It’s a real concern for us because there’s a lot more to the management of medication than giving it to the patient – it’s understanding the side effects and also the reasons for the medication,” he said. “We believe closing these facilities is the wrong approach – the public sector does have a very important role to play in aged care services.”

New resource welcomed by aged care experts by Belinda Smart A new information resource to support endof-life care looks set to benefit staff in aged care facilities. The RAC (Residential Aged Care) Hub was launched as part of the international activities celebrating World Hospice and Palliative Care Day on 13 October and is described as an online resource providing information about palliative care, provided to those whose illness cannot be cured. Designed for those working and living in residential aged care, the hub contains over 50 pages of information about end-of-life needs and the appropriate care of clients experiencing the challenges presented by this life phase. A spokesperson for Leading Age Services Australia (LASA), the national peak organisation for all providers of care, services and accommodation for older Australians, told NCAH the site offered a number of pluses for nurses and other aged care professionals. “The RAC Hub importantly is written in plain English accessible to a range of aged care workers. It is also developed so it is easy to navigate which is another positive. It helps raise awareness of Continuing Professional Development (CPD) Points, and directs the reader to further information and resources. “I just love being challenged” by Belinda Smart 65 year-old Maggie McKenna has worked as a Registered Nurse for the past 30 years at The Canberra Hospital. In that time she worked for 15 years as a paediatric nurse, took a four year break to look after her own children, and then found her passion providing renal care. For 30 years Maggie said she always glanced at the pages in medical journals and magazines about other places where she could nurse, but one day she noticed an advertisement for RAHC in the Australian Nursing Journal and decided to act. “I spent 11 years in renal care in Canberra, and when I left The Canberra Hospital in December I knew it was my time to finally practice remote nursing. “It was just a matter of the timing being right and me actually making a phone call. “My dad was a teacher and as a child we moved around outback South Australia. So I have always had a fondness for remote

areas, and more importantly for Indigenous people.” While Maggie wasn’t sure initially if she had the skill set to complete the work in Tennant Creek, she has since found she is suited to her role and believes the support available to remote area nurses during their placement is in invaluable. “Tennant Creek has been an amazing place to come to work in because there’s a good support team here. Even though it’s a busy clinic there’s always somebody available to ask for help.” “I feel really proud of myself and the work that I’m carrying out here.” Maggie says a typical day at the Anyinginyi Clinic in Tennant Creek is hard to describe. “It’s just so completely different to being in a Canberra hospital. “We start work at 8 o’clock in the morning and there’s always people lined up for the


clinic, with a line for the men and a line for the women. “In an ordinary shift you can see babies and children right through to 80 or 90 year olds and people coming in for ordinary GP-type consultations, bad backs, sore throats, grazes and sometimes for more serious issues. During her time at Canberra Hospital, one of the things Maggie loved about her job was carrying out overall health assessments. “The thing that strikes me about the working remote is that the nursing is so thorough. Every single person you see, you do a full set of observations on. “My impression of remote nursing is that the health professionals are just outstanding. They will not let anyone slip through the system. I’ve been hugely impressed by that.

“The other thing is that all your allied services are just a phone call away. Every day I could phone the nutritionist, the diabetes educator, the podiatrist and the dentist.” In between her RAHC placements Maggie works casually at a dialysis centre in Canberra, but she knows she has more to do as a remote area nurse. “It’s so stimulating being a nurse in a remote setting. I know that next time I go remote, if I’m lucky enough to do another placement, my skills will continue to improve because there is so much to learn. “It would probably take ten years to get your head around everything because every day is just full of huge surprises. New app for allied health professionals and nurses by Karen Keast A new iPad app has been designed to help physiotherapists, occupational therapists and nurses better assess older people’s fear of falling. Neuroscience Research Australia has developed the iconFES app (available for $4.99 through iTunes), which is the first app of its kind to use images to describe a range of activities and situations, and it has also been proven to work with people who have dementia. NeuRA research fellow Dr Kim Delbaere, an emerging leading international researcher in the area of falls in older people, said her research showed having a fear of falling is an independent risk factor for older people falling. “One in three older people fall at least once a year. About two in three people have a fear of falling,” she said. “It is more common to be fearful of falling than to fall. Many, many older people would say they have some fear of falling.” Dr Delbaere said older people with a fear of falling often adopt a “cautious gait”, which can increase their risk of falling. “It’s not a stable way of walking so they actually make it harder for themselves,” she said. Dr Delbaere said the app aims to help allied health professionals and nurses better assess fear of falling, enabling them to create targeted fall prevention strategies for older people. Nursing fears as SA health jobs to be axed by Karen Keast Nursing jobs could be in the firing line in South Australia, after the State Government announced plans to axe almost 350 public health jobs and 114 beds.

The announcement comes after independent reports into the state’s local health networks recommended job cuts and bed closures in a bid to shave costs from the health budget.

Health Minister John Hill has revealed 197 hospital and health service positions will go and 52 health department jobs will be culled by 2015-2016, in a move designed to save $270 million over four years.

Ms Dabars said the union’s response to the hospital budget performance and remediation review outlined a series of solutions designed to improve efficiencies.

Another 100 jobs could be cut through enterprise bargaining negotiations mid-next year. Australian Nursing and Midwifery Federation SA branch secretary Elizabeth Dabars said the union holds concerns for nursing jobs and patient care. “We do have concerns and we do have some reservations,” she said. “The only EBA we are aware of that it would impact upon would be the nurses’ and midwives’ EBA. “We know we are going to be entering into an EBA situation early next year. “People are very concerned about what’s being proposed. I have spoken to nurses and midwives…and they have expressed serious concerns and they are nervous about the consequences for patients and patient care.” Ms Dabars said the union is now waiting on SA Health to provide more detail about the proposed cost-saving measures. The union’s submission includes recommendations such as opening nurse practitioner primary care clinics as well as two 16-bed pilot nursing convalescent units. It also recommends a review of leadership arrangements for hospital sites and nursing midwifery practice out of hours in a bid to ensure sufficient numbers of senior staff are on hand, along with a change in the scope of nursing/midwifery management facilitator roles. Ms Dabars said the submission also included the introduction of the nurse-led discharge program, which the government recently announced will be trialled at Flinders Medical Centre from early next year. “That was part of our solutions package,” she said. “Our view is you cannot pick one piece of a jigsaw puzzle and expect an outcome. You need to have all of the pieces to be operating effectively to get change. “We want to understand how SA Health and the state government are proposing to achieve their efficiencies in the absence of introducing such strategies.”

More paramedics connected to alleged drug theft by Belinda Smart Seven paramedics in the past four years have now been linked to the alleged theft of drugs from Ambulance Victoria. The revelation comes after Victoria Police arrested a second paramedic in relation to the alleged theft of a large quantity of the pain killer drug Fentanyl. Hundreds of patients were affected when vials of Fentanyl were allegedly replaced with tap water. The ambulance service is remaining tightlipped on the recent alleged thefts as police continue their investigation into the incident. “As there is a formal police investigation underway, unfortunately we are unable to provide any further details of the investigation

at this time,” Ambulance Victoria general manager regional services Tony Walker said. Mr Walker said in addition to the current investigation, three paramedics have been dismissed following an investigation into the theft of drugs while two paramedics have resigned during investigations since Ambulance Victoria was formed in 2008. “Only one of the previous cases involved patients and we advised the local community and spoke directly with all seven patients involved,” he said. “Victoria Police was notified in all of the cases in line with our processes. “Ambulance Victoria immediately acts on any suspected cases of drug misappropriation and reports any thefts to the Victoria Police.”

Mr Walker said while the service had “very good surveillance and controls�, paramedics require access to drugs. “No matter how many security measures we have in place, paramedics must have access to drugs in the course of their duties, and if an individual is determined to misappropriate the pharmacology available, they will unfortunately do whatever they can to circumvent those security measures,� he said. “The use of substances in the community exists and therefore it will exist in probably any workforce of any description and ambulance is not immune to this.� Mr Walker said paramedics found to have misappropriated drugs are offered support regardless of whether or not they remain an employee.

“Ambulance Victoria has a psychological counselling unit to refer paramedics to for help which also conducts research to track the wellbeing of paramedics,� he said. “This is also supported by our chaplaincy service. “Paramedics are trusted with patients’ lives, treatment and privacy, and they are also entrusted with pharmacology. “The issue of us maintaining public trust in paramedics is very important. Paramedics fulfil that level of trust almost perfectly.� Following an earlier request from Ambulance Victoria, Victoria has become the first state in Australia to now use Fentanyl vials with a more tamper-proof steel cap, replacing the former rubber cap which could be penetrated with a needle. While about 1700 students have received a State Governmentfunded graduate position at Victorian public hospitals, 1142 students - including Victorians and those who studied interstate - are still looking for a Victorian nursing job. “This year the rules have changed, with hospitals saying, ‘If you don’t do a grad year, you can’t get a job as a registered nurse’,� Gilbert said. “This isn’t necessarily the case in other states, but in the Victorian system hospitals won’t receive graduate nurses unless they have completed a graduate program.� The ANF had written to the Victorian health minister and had not received a response at the time of writing, he said. It had also contacted the Federal Health Minister, leading to a meeting with COAG-backed initiative Health Workforce Australia (HWA) in September with a view to solving the problem.
