Remote health professionals wanted for trauma study
A
new study aims to improve the management of remote health professionals who experience traumatic events. CRANAplus, the peak body for remote health practitioners, and The Centre for Remote Health have joined forces to review and enhance how management oversees remote nurses, midwives, allied health professionals, doctors and Aboriginal and Torres Strait Islander health workers in the wake of traumatic events. CRANAplus Bush Support Services (BSS) senior clinical psychologist Dr Annmaree Wilson said the idea for the study came about several years ago after callers to BSS’s 24-hour, seven day a week telephone counselling service reported their recovery was hindered due to management. “Callers have felt that the management and the other people that they work with inadvertently exacerbated the problem in a way because of the way they’ve dealt with the individual who’s been traumatised,” she said “We thought it would be really interesting to more formally look at the stories that people have around those sorts of traumatic experiences and how they’ve been dealt with by management and by the people around them.” Dr Wilson said remote health professionals are more likely than their regional and urban counterparts to repeatedly experience traumatic events due to the nature of their work in remote and isolated areas.

“There’s a whole gamut of traumatic events that remote area health workers repeatedly experience and potentially can be quite traumatic,” she said. “You might have a remote area nurse who is the first on the scene of a motor vehicle accident and so the nurse is attending very injured people for an extended period of time while waiting for more help to come in. “For whatever reason, their normal coping strategies don’t work - it could be that they’re going through a difficult time in their personal life, they might have money worries, they might have relationship issues. “For whatever reason, this particular event impacts on them significantly and they’re feeling like they can’t cope, they’re feeling very teary, they’re feeling unable to attend properly to their patients. “You might have a situation where you’ve got their direct line manager who is struggling to understand what they’re going through, is very much of maybe a goal directed, logical attitude as has been fostered in their career and they’re not really reflecting on the feeling-based issues that have arisen for the person that has witnessed this accident and so they put undue pressure on that worker just to pull their socks up and return to work.”

Midwifery practice scheme safeguards midwives By Karen Keast Australia’s privately practising midwives providing birthing services in the home have been unable to access professional indemnity insurance (PII) since the collapse of HIH in 2001. Commonwealth, state and territory health ministers have agreed to exempt self-employed midwives from the insurance requirement, which is mandated under the Health Practitioner Regulation National Law, with the exemption now extended until December 31, 2016. While Medicareeligible midwives have been able to access the Commonwealth-subsidised Midwifery Practice Insurance Scheme for antenatal and postnatal care and for care to women birthing in hospital as private clients, midwives who are not Medicareeligible no longer have access to insurance for antenatal and postnatal care after insurance provider VERO ceased offering PII to midwives in April. The ongoing insurance crisis for privately practising midwives has prompted the Australian College of Midwives (ACM) and Queensland Health to find a solution. Under a contract with Queensland Health, ACM is developing a Midwifery Practice Scheme, designed to not only increase the confidence of governments, midwives and midwifery clients but to also make PII a viable commercial opportunity for insurance companies.

ACM chief executive officer Ann Kinnear says the scheme is a safety and quality framework of processes, policies and guidelines that’s now being developed to support midwives. Ms Kinnear says the scheme will provide a model that other states or territories may also embrace, creating the potential for the development of a national scheme for privately practising midwives. “The current situation with insurance is a poor situation for midwives and mothers to be in,” she says. “It exposes midwives, if there is any complaint against them that takes them to court or takes them to the Australian Health Practitioner Regulation Agency (AHPRA). “Midwives could be up for a lot of legal expense, which they are not covered for, and if they are found to be at fault then they may have to compensate their client and also pay their clients’ legal fees, so they are quite exposed to complaints if an error is made. “Insurers need to know what the risk is that they’re insuring and they need to know that the risk is being mitigated to the extent that it can be. “Our job is to create the Midwifery Practice Scheme which hopefully will attract a private firm to bring forward an insurance product.” Under the scheme, evidence-based national guidelines will be developed for clinical practice
for births at home and for transfer from birth at home to health services. The scheme will also provide a statement of midwifery practice requirements, and develop risk management, audit and compliance processes along with support systems for midwives in private practice. Ms Kinnear says it’s important the guidelines focus on evidence-based maternity care. “Some current clinical guidelines say the position in which a woman should give birth is the one that is most convenient for the practitioner,” she says. “Obviously, you would get a better outcome if the woman was able to choose her position of birth that most suited her at that time and she was supported to make that decision. “That’s an example of where things can be non-evidence-based and they are not actually standards supported by midwives. “So we want to make sure that when we make statements that they are based on evidence and obviously, in this example, the position that a woman chooses in birth is the one that’s most convenient to her and the one that is most appropriate for her stage of giving birth.” Work on the Midwifery Practice Scheme began in April and is due to be completed before the end of the year. A steering committee, chaired by ACM president Professor Caroline Homer, has been created and includes representatives from the Nursing and Midwifery Board of Australia, AHPRA, the Australian Commission on Safety and Quality in Health Care, the ANZ Council of Chief Nurses and Midwifery Officers, the Australian Nursing and Midwifery Federation and Maternity Choices Australia to name a few, and also includes privately practising midwives. While the scheme will not duplicate the requirements for midwifery registration, it will put in place some additional checks and balances to minimise risk, such as a requirement for open disclosure.
“Open disclosure is simply if a health practitioner feels that something has gone wrong during the provision of a service to a client then they should explain what’s happened and explain if it’s going to have adverse outcomes or not,” Ms Kinnear says. “It’s very important that health practitioners provide information to their clients. This is one of the things we would be strongly recommending to midwives, if they don’t already do it, is to have open communication.” Ms Kinnear says the scheme will also work to assist midwives when it comes to addressing complaints. “We would like to give them the skills and knowledge to be able to deal with that, and if they have difficulties then we would suggest a mediation process and we would explain how that would work,” she says. “We want to be able to help consumers work with midwives, get the information that they wish but also have midwives provide good information to their clients. “This is not saying that midwives don’t already do this but this is giving them the skills and knowledge to make sure they can do it to the best of their ability.” Under the scheme, privately practising midwives will also be required to complete a program similar to the ACM’s peer-supported Midwifery Practice Review. “The Midwifery Practice Review allows midwives to think quite deeply about what they do and the skills and knowledge that they require, and to move forward so that they have continuous improvement. A program like this would be a requirement.” Ms Kinnear says while many privately practising midwives are already implementing many of the proposed measures, the scheme will cement the initiatives as best practice and work to elevate the midwifery profession.

Mental health nurses excluded from expert group First it was nurses, midwives and nurse practitioners being excluded from the federal government’s two key health reform groups, now mental health nurses have also been overlooked. Mental health nurses, the largest part of the health care workforce, have been left off the Mental Health Expert Reference Group (ERG) advising the government on its response to the National Mental Health Commission’s recent review of mental health programs and services. Australian College of Mental Health Nurses (ACMHN) chief executive officer Adjunct Associate Professor Kim Ryan said the 13-member group is top heavy with psychiatrists despite the review not including any specific psychiatric program or service. “I don’t have any problem with the people that are on the mental health ERG but I cannot understand why there’s six psychiatrists on it,” she said. “It’s a review of services and programs none of which are psychiatry-led so why is the review then predominantly looked at through the lens of psychiatry? “Nurses are there 24 hours a day, 365 days of the year, across all areas, across all of the country in all sorts of service provision. “I’m not saying there needs to be a member of my college on there,” she added. “I’m saying there are nurses out there that can contribute to this and I don’t understand why, when there’s about 300,000 nurses that they couldn’t find one and they found six psychiatrists.” Ms Ryan, who recently raised concerns that the voice of nursing is being left out of the crucial conversation around the nation’s health reforms, said nurses are continuing to be overlooked.

“I don’t think it’s a deliberate tactic to not include nurses,” she said. “I’m just worried that they don’t see nurses as being people who can contribute to things at that level. “They just see them as being the health care providers that are out there doing the work and not think about the fact that they actually are the people that run the system. “Nurses are the people that manage the crisis, they are the people that work across state and territories, Commonwealth, private and public, rural and remote regions, they’re everywhere.” The review called for an end to the freeze of the unique Mental Health Nurse Incentive Program (MHNIP), an increase in funding from $41.7 million to $72 million a year, and an extension of MHNIP eligibility to residential aged care facilities and multipurpose services. It also recommended examining the cost effectiveness of extending the Better Access to mental health nurses with a postgraduate qualification in mental health nursing. “The Mental Health Nurse Incentive Program is the only program that’s funded and structured the way that it is so I’m not sure how they will review that through the lens of the committee that’s there,” Ms Ryan said. “There’s nurses out there that work in policy, there’s nurses out there that work in administration, management, in research - I’m not sure why they don’t look at a better mix of people that work in the health profession on the ERGs.” Ms Ryan will write to Health Minister Sussan Ley to express her concerns at the ERG.

Telehealth to connect rural New Zealand An integrated telehealth advice service will work to enhance rural health care in New Zealand, linking pharmacists, nurses and GPs to people right across the country. Tele-triage organisation Homecare Medical was recently announced as the preferred provider for New Zealand’s free 24/7 telehealth service, which is set to go live on November 1 this year. Homecare Medical chairman Dr Martin Seers said the company will work to deliver a service that’s accessible through a range of channels from phone, websites, email, text messages and chat with plans to also add video calling and mobile applications in the future. “People expect to get help and advice through a variety of channels, they expect to be

World-first physiotherapy trial targets hip pain By Karen Keast A Melbourne physiotherapist is conducting a world-first study into a crippling condition that causes pain on the outside of the hip, particularly common in post-menopausal women. The Gluteal La Trobe (GLoBE) trial is investigating whether a hormone supplement and targeted exercise programs can improve the symptoms of greater trochanteric pain syndrome (GTPS). The syndrome causes pain on the outside of the upper thigh or side of the hip, which can lead to difficulty exercising or completing general activity, from walking up and down stairs and ramps to lying in bed and sitting for long periods at a time. Risk factors for GTPS include the female gender, age, weight, lifestyle changes, back pain and a reduced angle of hip joint to thighbone in women. Women can experience changes in their tendon structure due to reduced oestrogen levels and collagen production during menopause, resulting in poorer tendon structure. A change in activity, from taking up a new exercise program, starting new stretches and increasing exercise frequency, can overload and compress an already vulnerable tendon and lead to GTPS. La Trobe University PhD candidate Charlotte Ganderton, a physiotherapist at Mill Park Physiotherapy and Eastern Health, said the study in-

volves two randomised controlled trials - one examining the effect of a targeted exercise program while the other investigates the use of a hormone therapy cream coupled with targeted exercise. Ms Ganderton, a member of the Australian Physiotherapy Association, said most research into lateral hip pain has focused on medical interventions such as cortisone injections and surgery. “We know exercise helps other tendon conditions such as in the Achilles, patellar and hamstring yet very little has been explored into similar management of GTPS,” she said. “Studies into supplemental oestrogen have found that it is beneficial for increasing skeletal muscle strength, reducing fractures, preserving bone mass and preventing a decline in the collagen content of the skin. “Studies have also suggested that tendon cells contain hormone-specific receptors, suggesting the tendon is responsive to oestrogen. “Investigating the role of hormone supplementation as a treatment for GTPS is a worldfirst, and is also a very important step towards helping patients manage this debilitating condition.” The study provides participants with free physiotherapy sessions, education about managing GTPS and a home exercise program that
includes three simple exercises. The interventions span a three-month timeframe and the study monitors participants’ progress over a year. Ms Ganderton said the study, which began in January, is already producing encouraging results with participants reporting reduced pain and improved hip movement. “I had a participant ring me the other day and say - ‘the next time I see you, I’m going to give you a big hug, you’ve changed my life’,” she said. “It seemed a little bit unbelievable that just a small amount of education and a few exercises has completely changed her outlook on life. “Impacting upon people’s day to day life is probably the most rewarding thing to come out of the study.” Ms Ganderton said while 24 women are enrolled for the hormone study and another 56 are enrolled in the exercise study, more participants are needed.
“Our aim is to get about 80 for the exercise study and 120 for the hormone study,” she said “We are administering the hormones via a cream and we’re also making sure that these participants have had a consultation with a sports physician, they’ve had a full medical, blood test and an ECG before they commence the study.” Ms Ganderton said lateral hip pain is often misdiagnosed as back pain, bursitis or hip arthritis, leading to poor management of the condition and ongoing issues for those affected. “These women bounce from health professional to health professional, from injections to sports doctors to imaging, and they never really get on top of what’s going on,” she said. “We hope we can change that and help women get back on with living a much better quality of life with a lot less pain.

Aussie nurse fights Ebola battle in Liberia
A
n Australian nurse will soon return home after working in Liberia to combat the deadly Ebola virus. Brisbane registered nurse Jebet Kipsaina, who was born and raised in rural Kenya, has been working for Aspen Medical in the African nation now experiencing a new flare up of the virus. Ms Kipsaina, who completed pre-deployment training in January this year, will remain in Liberia, the nation worst hit by Ebola, until the end of July before returning to Australian shores as the longest serving RN on the Liberian project. “There have been a lot of good things coming out this experience in Liberia,” she said. “The best has been the people I have worked with and for - Liberians are very accepting people and eager to learn and teach at the same time. “It has been professionally uplifting to see other nurses blossom in their careers and take on roles they were so reluctant to take up in the beginning.” Ms Kipsaina worked as a nurse for Western Australia’s Department of Health in Perth and regional areas before completing her Master’s in Public Health while working in the emergency department of The Royal Brisbane and Women’s Hospital. She worked in rural and remote practice in Australia and at asylum-seeker processing centres in Papua New Guinea, Nauru and Christmas

Island and also practiced as a rescue nurse and injury management officer with a coal mining company in Queensland before being deployed to Liberia. Ms Kipsaina said her nursing experience laid the groundwork for her role in the Ebola-embattled nation. “It has been an excellent opportunity to share what I have learnt in an industrialised world while being able to recognise and appreciate the difficulties faced by a developing nation.” Aspen Medical, which led Australia’s response to the Ebola outbreak in Sierra Leone until April this year, continues to provide clinical management services to four 25-bed capacity ETUs at Barclayville, Bopolu, Gbediah Town and Zorzor in Liberia. The organisation, which was subcontracted by PAE on behalf of USAID, began managing the ETUs late last year and has now had its contract extended until October. World Health Organisation (WHO) figures show there have been 10,670 confirmed cases and 4807 deaths from Ebola in Liberia. An Aspen Medical spokesperson said with neighbouring countries continuing to grapple with the virus and with new Ebola cases confirmed in Liberia, it was important to continue to apply effective infection prevention and control measures.
