PROVINCIAL INCEPTION MEETING- KHYBER PAKHTUNKHWA

INTRODUCTION The provincial inception meeting is a part of project titled ‘Studying the knowledge, beliefs and practices of unmarried and married adolescent girls with respect to nutrition and on the sources of nutrition information the adolescents have access to and use’ , funded by the Maternal and Newborn Health Programme Research and Advocacy Fund (RAF) and is implemented by Fatima Memorial Hospital. The technical assistance for the project is being provided by the Principal Investigator, the four provincial coordinators, technical expert gender, M&E, research, statistics and epidemiology. The project further has support of an entire panel of nutritional experts on board of Fatima Memorial Hospital and Nur Center for Research and Policy. This provincial inception meeting is a joint collaborative project of FMH and the provincial partner in Khyber Pakthunkhwa i.e. Khyber Medical University.

OBJECTIVE OF MEETING i. ii. iii.

To develop an understanding of the project To understand the need of the study keeping MDG 4 and 5 in perspective To understand the need to study adolescent girls due to lack of evidence in this age group, particularly after the National Nutrition Survey 2011 To bring about a consensus on the purpose of the study and possible outcomes

SCOPE The Meeting focused on five areas:     

Theme 1:MDGs 4 and 5 Theme 2: Adolescents in Pakistan Theme 3: Role of early marriages Theme 4: Role of nutrition Theme 5: Outcome of research study

PARTICIPANT PROFILE Participants included Vice-Chancellor and Academicians from KMU, Senior Health Professionals from MNCH and PGHS, UNICEF Nutrition, GIZ, DGHS, other Provincial Program representatives, policymakers, activists, healthcare workers, educators, community leaders, UN Agencies representatives, senior level staff of local, national, regional and international NGOs and CSOs working for nutrition.

The discussion was moderated by Dr Mohammad Hafizullah, Vice Chancellor Khyber Medical University. Dr. Hafizullah is a senior health professional. He is Professor of Cardiology, Head of Department, Cardiology Department, Lady Reading Hospital and Hayat Abad Medical Complex and served as Chief Executive Lady Reading Hospital Peshawar and Vice Dean, Research, Postgraduate Medical Institute, Peshawar. He is Editor in Chief of Journal of Postgraduate Medical Institute official Journal of PGMI Peshawar since 1993. He is the project director of upcoming Peshawar Institute of Cardiology being established at Peshawar. He is Supervisor for trainees preparing for FCPS Cardiology and Moderator for MD Cardiology examination held under the auspices of University of Peshawar. He has actively contributed to national guidelines on thrombolysis, acute coronary syndrome, and ST elevation myocardial infarction developed by Pakistan cardiac Society. He was chairman of committee to develop guidelines for Research Ethical committee by PMDC. He chaired the committee of Pakistan Cardiac Society to develop guidelines on Stable angina. He is vice President of Pakistan Hypertension League and has contributed to develop guidelines on hypertension. He is chairman of Council on Preventive Cardiology by Pakistan Cardiac Society and has developed national guidelines on preventive cardiology. He has authored more than hundred scientific articles and abstracts which have been published internationally and locally.

MEETING METHODOLOGY 



Power Point Presentation on the project, the research problem and background of the grantee i.e. Fatima Memorial Hospital. Presentations lasted between 15-30 minutes and provided opportunity for questions and discussion. Discussion during which participants discussed the role of various stakeholders in the nutrition of adolescent girls, particularly that of LHWs, LHVs, TBAs and CMWs. The discussion also revolved around the steps that can be taken by institutions like KMU and FMH, in covering nutrition in the curricula at undergrad, grad and post-grad levels.

PROCEEDINGS After the welcome note by the esteemed Vice Chancellor at KMU, the background of the meeting was brief by Dr. Tasleem Akhter, Technical Advisor to both NCRP and KMU and Professor emeritus at KMU. Dr. Akhter elaborated that the data resulting from the study will be provided for policy making and will give us a direction to move towards. This research will enable us to make informed decisions. There have been numerous initiatives like population planning, family planning, LHWs, child health and TBA trainings but nowhere do we talk of achieving MDGs 4 and 5 in the region. This is a small project trying to draw attention to marginalized groups, in order for policy makers to have a holistic approach. By studying adolescent girls poverty-wise, gender-wise, at home and in community, the multisectora approach will enable policy makers in the social welfare sector and education section, will inform them and enable them to take a holistic approach.

After the presentations by Dr. Aziz and Dr. Zaman, the forum was declared open for discussion.

DISCUSSION CLOUDS

Understanding the poverty and affordability, we want to find out if this population is optimally utilizing what their pocket allows them to eat.

A desired impact could be revisions in curricula of LHWs that grossly lacks nutrition.

Understand the source of information for the adolescent girls and educate them i.e. in case of birthing, if the mother-in-law is delivering more than the TBA, perhaps the mother-in-law should be trained in midwifery rather than the TBAs.

We are interested in learning about both malnutrition and under-nutrition.

A similar study needs to be carried out on a) women in reproductive age group but above 35 years of age to find out the reasons of obesity in this age group and b) males in adolescent age group

We want to establish the extent of lack of knowledge and the attitudes towards nutrition

The reasons of improved ‘average national age’ need to be studied. What is the role of IMR and MMR in this improvement?

We cannot just import practices from abroad here. We need to generate local evidence before making these policies.

Ground realities need to be studied. Penetration of LHWs, LHVs and TBAs is not well documented. The ground realities are starkly different from the information available.

Our main issue is that strategies and policies are derived and developed in isolation. When initiatives CMWs are undertaken by the government, issues like deployment arise. Furthermore, there is no monitoring for such initiatives.

Our dilemma is lack of community participation and hence lack of sustainability of initiatives like these.

CLOSING COMMENTS BY THE VICE CHANCELLOR

    

This was an important and productive forum. A note of thanks was given to all the participants. The intended guest list included the Chief Minister and his presence would have contributed immensely towards the purpose of the meeting. This group of participants can together act as a think tank, towards generating more information and evidence for policy makers. The meeting ended with a vote of thanks to the Chair, Vice Chancellor Dr. Muhammad Hafizullah.

The meeting has reinforced the concept of networking and linkage development especially in a scenario of limited resources. A good networking forum! The discussion was effectively moderated a productive dialogue was generated.
