ince 2003, Natural Doctors International has been providing free naturopathic healthcare in Nicaragua, in addition to working in Mexico and Haiti, collaborating with the North American naturopathic community on various global heath issues and developing internships and scholarships . At least five times a year students, doctors, and other professionals join NDI on a 10-day Global Health Course, where they give their heart and soul to provide medical relief in our main site on the Island of Ometepe, Nicaragua. As of 2011, approximately 13,000 patients have been treated and more than $350,000 have been donated in supplies and medicines. Over 500 volunteers and doctors have worked with NDI, not including the volunteer doctors that serve up to 13 months. In addition to clinical services, NDI has also worked in collaboration with various community projects such as Calzado Ometepe, a marathon that donates hundreds of shoes to island kids, and the NDIHBI Water Project, that will eventually give hundreds of families clean water from a newly built purification plant, to mention a few. For 7 years, we have been committed to our mission of “promoting global health and social justice through natural medicine,” so that we can work together for a prosperous and healthy community here in Nicaragua and globally.

esde el año 2003, Natural Doctors International ha estado proporcionando asistencia sanitaria naturopática libre en Nicaragua, y desde entonces se ha expandido para trabajar en México y Haití. Legal Medicine have been asked to write about this topic for many years as more and more naturopathic doctors find themselves volunteering overseas. Working in a developing country can be very rewarding. Patients are grateful for the services you provide and the need is tremendous. Often, if you are setting up your own experience, you may find yourself teaming up with a

local doctor, working for a church mission in some capacity, or just setting up shop. Getting legal permission may seem like a hurdle you do not want to tackle, and depending on the country, the bureaucracy involved may indeed be overwhelming. But as difficult as it may be to obtain proper permission, it is imperative to do so. Not doing so may have personal ramifications, but may also negatively impact the naturopathic profession. If you are volunteering, ask the organization your working with to provide a copy of their contract or formal agreement from the government of the country where you will be working. Without this, you risk breaking international law as you are practicing medicine without a license. More importantly, you undermine the very people

Ethical Implications of Practicing Medicine Abroad

n March 25th of 2011, Bastyr student, Michelle Eustache, lost her life in a tragic drowning while attending a 10 day NDI Global Health course. On a morning excursion, 9 women visited the popluar swimming spot at the Jesus Mary point in Equipulas on the island of Ometepe. For those of you who have visited this sandbar, one might find it hard to imagine how this could happen. Photos taken minutes before Michelle drowned show the confident woman we all knew and loved walking in stride on the sandbar of that point. It was a shock to us all - the NDI team, the local community and her fellow classmates. In this issue we have included some photos of Michelle’s last days to remember the joy she brought to all of us. We remember a young woman full of laughter, smiles and brilliance. A committed doctor-to-be, a tremendous friend, a loving daughter. Michelle spent her last days in service to the people of Ometepe - and many patients remember her fondly. To our sister Michelle - we miss you dearly - but celebrate your vibrance everyday and continue to learn from you even with your passing.

iosa, meaning goddess or beautiful woman in Spanish was founded in 2008. The idea was to combine empowerment education and quality women's health services and treatments for Nicaraguan women. As the program developed it provided women with safe, clean, and respectful PAP exams which became the hallmark of the program. Today, over 260 PAP's have been performed which has increased the total amount in our municipality by 33% over 2 years. Patients also receive quality naturopathic and conventional care for women's health issues. In addition, NDI sponsors International Women's Day and other solidarity events throughout the year. Mama Cama! Beds for Mamas - A DIOSA sponsored Campaign.

eds for Mamas, is a new project that NDI is promoting with the sole purpose of seeking funds to purchase new beds for pregnant women in the hospital in Rivas, the southernmost state of Nicaragua. Many of the beds are in total disrepair, because the mattresses are worn out and deteriorating due to bleeding, afterbirth and other fluids. As each day passes it becomes increasingly difficult for beds to be allocated to every woman. After giving birth many of the women have to stay in these beds for days in very unsafe and unsanitary conditions. The cost for a bed for the hospital costs $80. We ask all those reading to buy a bed for the women and contribute to improving the quality of care here in Nicaragua.

International Women’s Day 2011

his year NDI celebrated the 100th year anniversary of International Women’s Day in the main port town of Moyogalpa on the Island of Ometepe, Nicaragua. Over 400 women were in attendance for the festivities which included a rally through the town, special guest speakers, theatre, food and drink. Thank to all those who participated!

Cada día que pasa se hace más difícil asignarles una cama a cada mujer. Después de dar a luz muchas de las mujeres tienen que permanecer en estas camas por días en condiciones muy inseguras e insalubres. El costo de una cama de hospital cuesta $ 80. Les pedimos a todos para ayudarnos a comprar una cama para una mujer y contribuir a mejorar la calidad de la atención aquí en Nicaragua.

h en Dr. Tabatha Parker first learned about the earthquake in Haiti from her colleague Dr. Sabine Thomas, she knew NDI needed to help. For NDI, one of the few international naturopathic organizations in world, this meant taking the lead in coordinating relief efforts was a top priority. Thomas, who had family in Haiti, told Dr. Parker her story. “We did not hear from my family until Jan. 15; three long, agonizing days not knowing.”Sadly, when she was able to speak to relatives in Haiti, they had unimaginable news.“I lost my cousin, Valencia Mongérard,” Thomas said. NDI immediately reached out to the larger ND community in North America and coordinated raising funds through their already established networking capabilities, forming the Haiti Disaster Relief Committee (HDRC) with broad representation from all major naturopathic organizations. Thanks to generous donations from organizations like NABNE who gave $5,000, and SCNM along with more nearly 100 individual donors, NDI raised over $13,000 for relief efforts. Companies like Heel and Hylands gave generous in-kind donations of medicine.

After an initial assessment trip, Dr. Thomas and the HDRC assembled a skilled team to return in June 2010. In accordance with NDI policy, the group obtained permission from the Ministry of Health to temporarily practice medicine in Haiti. In 5 days nearly 500 patients were seen. “We saw patients come in to be treated for malnutrition, malaria, typhoid fever, wound infection, musculoskeletal issues, post-traumatic stress disorder, depression, diabetes and hypertension,” recalled Thomas. Their methods of treatment were embraced.“What makes naturopathic medicine particularly fitting in Haiti is that our primary therapies are already utilized in Haiti,” Thomas said. “We were well-positioned to then provide services that are already in line with the health care that patients are receiving.”

Naturopathic medicine in Zambia

n the 1950s Dr. Foster, a Seventh-Day Adventist MD from Yuchi Pines Institute in the US, came to Zambia to establish the Riverside Farm Institute about 30 miles from Lusaka, the capital of Zambia. It operated for many years as a naturopathic school. Unfortunately, when the Zambian government changed, the new government was not well disposed to natural medicine and ordered the institute to change the clinic over to conventional medicine only. Soon after former students, including Dr. Lawrence Chanza were determined to start a new school. Dr. Lawrence Chanza was born in the Copperbelt Province of Zambia and at three years of age was sent to live with his grandfather his ancestral village. There he learned traditional ways of life, including traditional medicine. In 2004, Dr. Chanza began speaking to government officials about naturopathic medicine. The World Health Organization (WHO) and the Ministry of Health of Zambia used him

as a paid consultant in drafting new policy regarding traditional medicine. He developed a strong relationship with the WHO and his reports would be used by the WHO Regional Advisor on Traditional Medicine Africa Region in Congo –Brazzaville. Eventually, keeping with his promise he and several colleagues of naturopathic medicine got together and decided to start a school called ZINARE (Zambia Institute of Natural Medicine and Research.) They volunteered their time and donated money to get the school started. They contacted the Canadian College of Naturopathic Medicine President, Dr. Bob Bernhardt, who provided technical assistance and help

developing their curriculum. The WHO awarded them a grant of $3,000 along with technical assistance for their curiculum. In 2010 Natural Doctors International began a partnership with ZINARE. After an initial trip to Zambia in 2009, NDI granted ZINARE a $1,000 Paul C. Bragg’s Global Health Scholarship. Today, ZINARE is still growing, collaborating, and creating a presence for naturopathic medicine in Africa.

Natural Doctors International es una organización sin fines de lucro ubicada en la isla de Ometepe, Nicaragua. Se ha brindado atención gratis a los residentes locales durante los últimos 7 años. En segundo lugar, el programa consta de las discusiones formales en torno a la globalización, la historia y la política de Nicaragua, el papel de Estados Unidos en esta política y la economía global.

his brigade increased my appreciation for the role naturopathic medicine plays in third world countries. “My head hurts, I am sore all over, it burns when I pee, my back hurts, my legs hurt from the knees down and my heel hurts.” Over half of the 60 patients I saw in Nicaragua came into the clinics for these symptoms. The treatments we used in the clinics included homeopathy, herbal remedies, vitamins, nutritional advice and physical medicine therapies. The conventional medicine available to them was very limited with the exception of antibiotics. The large amount of antibiotics given to the Nicaraguans has led to an antibiotic resistant population. It was great to see our medicine provide an avenue for them to regain their health. Natural Doctors International is a nonprofit organization located on the island of Ometepe, Nicaragua. It has provided free naturopathic care to the local residents for the last 7 years. This organization gives students and doctors an amazing opportunity to work with patients who are looking for an alternative to the prescription drugs that are either not available to them or don’t improve their health. Within the clinics students are encouraged to ask questions, perform physical exams, and help with the development of a treatment plan for the patient. Before I came to the island I had little knowledge of the way Nicaraguans lived or the ways of which globalization has affected them. What does NDI mean to Ometepe?

DI has worked hard to set itself apart from other foreign health brigades by working with the local health ministries and doctors to become established and a permanent fixture of the community here in Nicaragua. As of August 2010, NDI has had it’s own independently operating clinic in the community of Los Angeles, as well as continuing to work in the main hospital in Moyogalpa doing both private consults and assisting the local doctors with their overnight shifts in the emergency room. Through my time here with NDI, I have seen various patients that have been coming to see the NDI doctors since NDI began and people still speak highly of doctors who have not been here for years. To me this indicates that NDI has become accepted as a part of the local community. Each month there are more people being referred to our care by current patients, and our patient base is expanding from neighbouring communities of the clinics to communities on the other side of the island. The current medical system here in Nicaragua, especially on the island, is challenged and stretched thin due to lack of resources and poverty. Often the local hospital and the health centers aren’t able to treat conditions as they have run out of medicine or equipment, especially in remote areas such as here on the island. People come to NDI hopeful that we can provide what they cannot access through the public system. Even the local health professionals refer to NDI in cases when they are unable to provide comprehensive treatment or relief for a patient. ast year I traveled to Ometepe on a brigade. It was my first time in Latin America, my first time speaking Spanish, and among my first clinical experiences as a student. This time around, I am working with NDI as clinical intern. I am here for a longer period of time, and my clinical skills are better, as is my Spanish. I am living with a host family again, but instead of just smiling at each other, we are now able to discuss their livelihoods, where their food comes from, and how the island has changed over time. In addition, a bit of the romanticism has been stripped away to reveal the realities of living in a developing country and practicing medicine in a rural community. This has made for a richer experience and taught me much about what makes a community thrive. As so often happens, my patients and families have been my greatest teachers and I am forever grateful to have been welcomed into their lives. l año pasado viajé a Ometepe en una brigada. Fue mi primera vez en América Latina, la primera vez que hablo español, y entre mis primeras experiencias clínicas como estudiante. Esta vez, estoy trabajando con el NDI como pasantía. Yo estoy aquí por un largo período de tiempo, y mis habilidades clínicas son mejores, como mi español. Estoy viviendo con una familia de nuevo, pero en lugar de sonreír el uno al otro, estamos ahora en condiciones de hablar de sus medios de subsistencia, donde vienen sus alimentos, y cómo la isla ha cambiado con el tiempo. “It’s very good because medicines are very expensive especially in these critical times. Personally, they have supported me and treat me well and they also help the rest of the population. We hope that over time they can improve the clinic and bring equipment for ultrasounds, CT scans, etc.

NDI Selects 2012 Doctor

DI is excited to announce that we have selected our 2012-2013 doctor. Dr. Patricia Beck will begin in early 2012 and serve her 13 month volunteer rotation as the principal NDI doctor. Dr. Beck recently attended a Global Heatlh Course in Nicaragua where she confirmed her desire to make the long term commitment with NDI. Dr. Beck is a 2005 SCNM graduate. She is looking forward to beginning her journey with NDI. Dr. Beck has a long history of working in healthcare, with a previous career as a physical therapist. She has traveled to Nigeria and Nicaragua previously in service work.

DI se complace en anunciar que hemos seleccionado nuestro médico 2012-2013 Dra. NDI Homestay Families A model of sustainability

he NDI holistic model of healthcare acknowledges the economic determinants of health of the individual and community. NDI preferentially uses local families in our community over hotels whenever possible to house students and doctors. This “homestay” model has been used by many social justice nonprofits. Why? Not only does it give volunteers a more genuine experience, it creates a significant positive economic impact that directly helps local families. It is empowering to families, who then have an income which they use to make positive choices for the health and well being of their family – whether that means buying medicines, putting food on the table, renovating their houses, buying farming supplies, or helping their kids go to school – it is in their hands. Families in the program receive classes on nutrition, cultural co petency, and meal preparation and each receive a water purifier. In a community where the average income is between $3 and $6/day, families in the program receive $15 per person on average of 8 days. In a typical brigade, a family makes over $200. NDI worked exclusively with a group of 19 women, known as Pueblo Hotel, for years. In 2011, NDI made a decision to incorporate new families into the homestay program. The world economic crisis has had a devastating impact everywhere, and Ometepe is no different. We had many requests from families in our community who wanted to participate. In these difficult economic times, we wanted to stand in solidarity with our community, and help as many families that had the desire to participate. The result has been phenomenal. All in all, 29 families now have benefitted from the program, which in the last 5 years has brought more than $40,000.00 USD to participating families. NDI now rotates between families who meet the requirements to participate and provide quality service. Here is what families have to say...

NDI utiliza preferentemente a las familias locales en nuestra comunidad respecto a los hoteles siempre que sea posible a los estudiantes de la casa y los médicos. When they come they are part of our family and bring much love to give. I have had about 13 pairs of brigaders in my house. At first I was nervous about cooking well for them because some were vegetarian but I have not had any problems and now I know a lot more about cooking.” “Life is hard here especially when you have not been educated and you depend on farming to get by like I do. In the past it was hard to farm the land because of our lack of resources and we did not eat three meals a day. Since we have had their help we are able to grow rice, beans, corn, and vegetables on our land. In our house we bought two beds, a fan, and dishes which before we would not have been able to. I thank God for opening a door through NDI and thank everyone who is involved with this great help in our lives.” “Ha sido una relación muy bonita ya que de ellos aprendemos un poco y ellos de nosotros. “Ya que la vida y situación aquí es un poco dura sobre todo cuando no somos personas estudiadas y se depende del campo. Desde que tenemos la ayuda de ellos hemos podido sembrar nuestras tierra con arroz, frijoles, maíz, y verduras. En nuestra casa pudimos comprar dos camas, abanico, y trastes. Antes no sembramos la tierra por la falta de recursos y no comíamos los tres tiempos de comida en cambio hoy en día si podemos hacerlo. Yo le doy gracias a dios por abrir esa puerta maravillosa a través de NDI y todas las personas que tiene que ver con esa gran ayuda a nuestras vidas.”
