In this chapter you will learn:

1not a thing, but don't worry. There is plenty to come. Here is your chance to turn a few pages, take a deep breath or two, and get yourself settled and ready to roll. Relax. Pour sometea.Begin.

Onthe opposite page is a normal electrocardiogram, or EKG. Bythe time youhave finished this book and it won't take verymuchtime at all youwill be able to recognize a normal EKG almost instantly. Perhaps even more importantly, you will have learned to spot all of the common abnormalitiesthatcanoccur onanEKG,andyouwill begoodatit!

Some people have compared learning to read EKGs with learning to read music. In both instances, one is faced witha completelynew notational systemnot rooted inconventional language andfull ofunfamiliar shapesandsymbols.

But there really is no comparison. The simple lub–dub of the heart cannot approach the subtle complexity of a Beethoven string quartet (especially the late ones!), the multiplying tonalities and polyrhythms of Stravinsky's Rite of Spring, or the extraordinary jazz interplay of Keith Jarrett's StandardsTrio.

Electricity and the Heart

Electricity,aninnate biologic electricity,is whatmakes the heartgo.The EKGis nothingmore thana recording of the heart's electrical activity, and it is through perturbations in the normal electrical patternsthatweareabletodiagnosemanydifferentcardiacdisorders.

All You Need to Know About Cellular Electrophysiology in Two Pages

Cardiac cells, in their resting state, are electrically polarized; that is, their insides are negatively charged withrespectto their outsides. This electrical polarityis maintained bymembrane pumps that ensure the appropriate distribution of ions (primarily potassium, sodium, chloride, and calcium) necessaryto keep the insides ofthese cells relativelyelectronegative. These ions pass into and outof thecell throughspecial ionchannelsinthecell membrane.

The most common natural cause of sudden death in young persons is a disturbance in the electrical flow through the heart, called an arrhythmia (we will talk about this in detail in Chapter 3). Sometimes lethal electrical disturbances happen because of an inherited disorder ofthese ionchannels. Fortunately, these so-called channelopathies are quite rare. Many different genetic mutations affecting the cardiac ion channels have been identified, andmorearebeingdiscoveredeveryyear.

The restingcardiac cellmaintains its electricalpolaritybymeans of a membrane pump. This pumprequires a constantsupplyof energy,andthe gentlemanabove,were he realrather thana visualmetaphor,wouldsoonbe flatonhis back.

Cardiac cells can lose their internal negativity in a process called depolarization. Depolarization is the fundamental electrical event of the heart. In some cells, known as pacemaker cells, it occurs spontaneously. In others, it is initiated by the arrival of an electrical impulsethatcausespositivelychargedionstocrossthecell membrane.

Depolarization is propagated from cell to cell, producing a wave of depolarization that can be transmitted across the entire heart. This wave of depolarization represents a flow of electricity, an electrical current,thatcanbedetectedbyelectrodesplacedonthesurfaceofthebody.

After depolarizationis complete, the cardiac cells restore their restingpolaritythrougha process called repolarization. Repolarization is accomplished by the membrane pumps, which reverse the flow ofions.Thisprocesscanalsobedetectedbyrecordingelectrodes.

All of the different waves that we see on an EKG are manifestations of these two processes: depolarizationandrepolarization.

In A,a single cellhas depolarized. A wave of depolarizationthenpropagates fromcelltocell(B) untilallare depolarized(C). Repolarization(D) thenrestores eachcell's restingpolarity.

The Cells of the Heart

Fromthestandpointoftheelectrocardiographer,theheartconsistsofthreetypesofcells:

Pacemaker cells under normal circumstances,theelectrical power sourceoftheheart

Electrical conducting cells thehardwiringoftheheart

Myocardial cells thecontractilemachineryoftheheart

Pacemaker Cells

Pacemaker cells are small cells approximately 5 to 10 μmlong. These cells are able to depolarize spontaneously over and over again. The rate of depolarization is determined by the innate electrical characteristics of the cell and by external neurohormonal input. Each spontaneous depolarization serves as the source of a wave of depolarization that initiates one complete cycle of cardiac contractionandrelaxation.

A pacemaker celldepolarizingspontaneously.

If we record one electrical cycle of depolarization and repolarization froma single cell, we get anelectrical tracingcalled anactionpotential. Witheachspontaneous depolarization, a new action potential is generated,whichinturnstimulates neighboringcells todepolarize andgenerate their own actionpotential,andsoonandon,until theentirehearthasbeendepolarized.

The action potential of a cardiac pacemaker cell looks a little different from the generic action potential shown here. A pacemaker cell does not have a true resting potential. Its electrical charge drops to a minimal negative potential, which it maintains for just a moment (it does not rest there), and then rises gradually until it reaches the threshold for the sudden depolarization that is an action potential.Theseeventsareillustratedonthefollowingtracing.

The electricaldepolarization–repolarizationcycle of a cardiac pacemaker cell. Point A is the minimalnegative potential. The gentle risingslope betweenpoints A and B represents a slow,gradualdepolarization. Atpoint B,the thresholdis crossedandthe cell dramaticallydepolarizes (as seenbetweenpoints B and C);thatis,anactionpotentialis produced. The downslope betweenpoints C and D represents repolarization. This cycle willrepeatover andover for,letus hope,many,manyyears.

The dominant pacemaker cells in the heart are located high up in the right atrium. This group of cells is called thesinoatrial(SA) node,or sinusnode for short. These cells typicallyfire ata rate of 60 to 100 times per minute, but the rate can vary tremendously depending upon the activity of the autonomic nervous system (e.g., sympathetic stimulation from adrenalin accelerates the sinus node, whereas vagal stimulation slows it) and the demands of the body for increased cardiac output (exerciseraisestheheartrate,whereasarestful afternoonnaplowersit).

Pacemaker cells are reallygood atwhattheydo. Theywill continue firingina donor heart evenafter it has beenharvested for transplant and before it has beenconnected to the new recipient.

Ina restingindividual,the sinus node typicallyfires 60to100times per minute,producinga regular series of actionpotentials,eachof whichinitiates a wave of depolarizationthatwillspreadthroughthe heart.

Actually, every cell in the heart has the ability to behave like a pacemaker cell. This socalled automatic ability is normallysuppressed unless the dominantcells ofthe sinus node fail or if something in the internal or external environment of a cell (sympathetic

stimulation, cardiac disease, etc ) stimulates its automatic behavior. This topic assumes greater importancelater onandisdiscussedunder Ectopic Rhythms inChapter 3.

Electrical Conducting Cells

Electrical conducting cells are long, thin cells. Like the wires of an electrical circuit, these cells carrycurrent rapidlyand efficientlyto distant regions of the heart. The electrical conductingcells of the ventricles form distinct electrical pathways. The ventricular conducting fibers constitute the Purkinje system.

The conductingpathways inthe atria have more anatomic variability; prominent amongthese are fibers at the top of the intra-atrial septumin a region called Bachmann's bundle that allow for rapid activationoftheleftatriumfromtheright.

Myocardial Cells

The myocardial cells constitute byfar the largestpartofthe hearttissue. Theyare responsible for the heavy labor of repeatedly contracting and relaxing, thereby delivering blood to the rest of the body. These cells are about 50 to 100 μm in length and contain an abundance of the contractile proteins actinandmyosin.

When a wave of depolarization reaches a myocardial cell, calcium is released within the cell, causingthe cell to contract. This process, inwhichcalciumplays the keyintermediaryrole, is called excitation–contraction coupling.

Depolarizationcauses calciumtobe releasedwithina myocardialcell. This influxof calciumallows actinandmyosin,the contractile proteins,tointeract,causingthe celltocontract. (A)A restingmyocardialcell. (B)A depolarized,contractedmyocardialcell.

Myocardial cells can transmit an electrical current just like electrical conducting cells, but they do so far less efficiently. Thus, a wave of depolarization, upon reaching the myocardial cells, will spreadslowlyacrosstheentiremyocardium.

Time and Voltage

The waves that appear on an EKG primarily reflect the electrical activity of the myocardial cells, which make up the vast bulk of the heart. Pacemaker activity and transmission by the conducting systemaregenerallynotseenontheEKG;theseevents simplydonotgeneratesufficientvoltagetobe recordedbysurfaceelectrodes.

The waves produced by myocardial depolarization and repolarization are recorded on EKG paper and,likeanysimplewave,havethreechiefcharacteristics:

1. Duration, measuredinfractionsofasecond

2. Amplitude, measuredinmillivolts(mV)

3 . Configuration, a more subjective criterion referring to the shape and appearance of a wave

A typicalwave thatmightbe seenonanyEKG. Itis twolarge squares (or 10smallsquares) inamplitude,three large squares (or 15 smallsquares) induration,andslightlyasymmetric inconfiguration.

EKG Paper

EKG paper is a long, continuous roll of graphpaper, usuallypink(but anycolor will do), withlight anddarklines runningverticallyandhorizontally.The lightlines circumscribe small squares of1×1 mm;thedarklinesdelineatelargesquaresof5×5mm.

Thehorizontal axis measures time.Thedistanceacross onesmall squarerepresents 0.04seconds. Thedistanceacrossonelargesquareisfivetimesgreater,or 0.2seconds.

The vertical axis measures voltage. The distance alongone small square represents 0.1 mV, and alongonelargesquare,0.5mV.

Youwill needtomemorizethesenumbersatsomepoint,soyoumightaswell doitnow.

Bothwaves are one large square induration(0.2seconds),butthe secondwave is twice the voltage of the first(1mV comparedwith 0.5mV). The flatsegmentconnectingthe twowaves is five large squares (5×0.2seconds =1second) induration.

P Waves, QRS Complexes, T Waves, and Some Straight Lines

Let's follow one cycle of cardiac contraction (systole) and relaxation (diastole), focusing on the electrical eventsthatproducethebasicwavesandlinesofthestandardEKG.

Atrial Depolarization

The sinus node fires spontaneously(anevent not visible onthe EKG), and a wave of depolarization begins to spread outward into the atrial myocardium, muchas if a pebble were dropped into a calm

Eachcycle of normalcardiac contractionandrelaxationbegins whenthe sinus node depolarizes spontaneously. The wave of depolarizationthenpropagates throughbothatria,causingthemtocontract.

Duringatrial depolarizationand contraction, electrodes placed onthe surface of the bodyrecord a small burstofelectrical activitylastinga fractionofa second. This is the P wave. It is a recording ofthespreadofdepolarizationthroughtheatrial myocardiumfromstarttofinish.

The EKGrecords a smalldeflection,the P wave

Because the sinus node is located inthe right atrium, the right atriumbegins to depolarize before the left atrium and finishes earlier as well. Therefore, the first part of the P wave predominantly representsrightatrial depolarization,andthesecondpartleftatrial depolarization. Onceatrial depolarizationiscomplete,theEKGagainbecomeselectricallysilent.

The components of the P wave

A Pause Separates Conduction From the Atria to the Ventricles

Inhealthyhearts, there is anelectrical gate atthe junctionofthe atria and the ventricles. The wave of depolarization, havingcompleted its journeythroughthe atria, is prevented fromcommunicatingwith the ventricles bythe heartvalves thatseparate the atria and ventricles. Electrical conductionmustbe funneledalongtheinterventricular septum,thewall thatseparates therightandleftventricles.Here,a structure called the atrioventricular (AV) node slows conductionto a crawl. This pause lasts onlya fractionofasecond.

Ventricular myocardial depolarization causes ventricular contraction. It is marked by a large deflection on the EKG called the QRS complex. The amplitude of the QRS complex is much greater thanthatoftheatrial Pwavebecausetheventricles havesomuchmoremusclemass thandotheatria. The QRS complexis also more complicated and variable inshape, reflectingthe greater intricacyof thepathwayofventricular depolarization.

The Parts of the QRS Complex

The QRS complexconsists ofseveral distinctwaves, eachofwhichhas a name. Because the precise configurationofthe QRS complexcanvaryso greatly, a standard format for namingeachcomponent hasbeendevised.Itmayseemabitarbitrarytoyourightnow,butitactuallymakesgoodsense.

1. Ifthefirstdeflectionisdownward,itiscalleda Q wave.

2. Thefirstupwarddeflectioniscalledan R wave.

3. Ifthereisasecondupwarddeflection,itiscalled R′(“R-prime”).

4. The first downward deflection following an upward deflection is called an S wave. Therefore,ifthe firstwave ofthe complexis anRwave,the ensuingdownwarddeflection iscalledanSwave,notaQwave.AdownwarddeflectioncanonlybecalledaQwaveif itisthefirstwaveofthecomplex.Anyother downwarddeflectioniscalledanSwave.

The earliest part of the QRS complexrepresents depolarizationof the interventricular septumby the septal fascicle ofthe left bundle branch. The right and left ventricles thendepolarize at about the same time, but most of what we see on the EKG represents left ventricular activation because the musclemassoftheleftventricleisaboutthreetimesthatoftherightventricle.

Repolarization

After myocardial cells depolarize, they pass through a brief refractory period during which they are resistantto further stimulation. Theythen repolarize;thatis, theyrestore the electronegativityoftheir interiorssothattheycanberestimulated.

Justas there is a wave ofdepolarization, there is also a wave ofrepolarization. This, too, canbe seenontheEKG.Ventricular repolarizationinscribesathirdwaveontheEKG,the T wave.

Note: There is a wave of atrial repolarization as well, but it coincides with ventricular depolarizationandishiddenbythemuchmoreprominentQRScomplex.

Ventricular repolarization is a much slower process than ventricular depolarization. Therefore, theTwaveisbroader thantheQRScomplex.Its configurationis alsosimpler andmorerounded,like the silhouette of a gentle hill compared to the sharp, jagged, and often intricate contour of the QRS complex.

Naming the Straight Lines

Thedifferentstraightlines connectingthevarious waves havealsobeengivennames.Thus,wespeak ofthe PR interval,the ST segment,the QT interval,andsoon.

What differentiates a segment from an interval? A segment is a straight line connecting two waves,whereasaninterval encompassesatleastonewaveplustheconnectingstraightline.

The PR interval includes the P wave and the straight line connecting it to the QRS complex. It therefore measures the time from the start of atrial depolarization to the start of ventricular depolarization.

The PR segment is the straight line running from the end of the P wave to the start of the QRS complex.Ittherefore measures the time fromthe endofatrial depolarizationtothe startofventricular depolarization.

The ST segment is the straight line connectingthe end of the QRS complexwiththe beginningof the T wave. It measures the time fromthe end ofventricular depolarizationto the start ofventricular repolarization.

The QT interval includes the QRS complex, the ST segment, and the T wave. It therefore measures the time from the beginning of ventricular depolarization to the end of ventricular repolarization.

The term QRS interval is used to describe the duration of the QRS complex alone without any connectingsegments.Obviously,itmeasuresthedurationofventricular depolarization.
