1.0 OBJECTIVES OF THE PAPER 1.1 To share the progress made for the Care Streams and Cross Cutting Themes. 2.0 BACKGROUND 2.1 All Care Streams are now fully operational and preparation has begun to establish Cross Cutting Theme (CCT) groups to support key work within the care streams to enable business cases to be developed in line with the critical path. 3.0 CARE STREAM PROGRESS 3.1 Childrens Care Stream – Chair: Sajid Azeb Vice-Chair: Dr. Hazel Carsley The Children’s Care Stream has just had its 5th meeting. The group has agreed and signed off Terms of Reference (ToRs) and have also agreed their 10 year Vision for Children’s Care: ‘Every child will have a healthy start and continue to lead a safe, happy life with every opportunity to achieve their potential within their families and communities’ The Care Stream has carried out extensive scoping work and are including preconception and maternity within their scope, as well as transition. The group has identified some broad areas, where they want to concentrate development of business cases (outlined below) but have still to agree specific priority areas. This will be finalised this week. • • • • • •
Emotional and Behavioural Wellbeing for Children and Young People Acute Services; Right person, Right place, Right time On-going Care for Children and Young People with Long Term Conditions/Vulnerabilities - Keeping them well and out of Hospital Improving Assessment and Management for Children with Special Educational Needs and/or Disabilities Improving the Planning and Management of Transition – Implementation of a Transition plan for all young people with Long-Term problems Improving inequalities and outcomes in pregnancy and neo-natal care
3.2 Long Term Care Stream – Chair: Jackie Ramsey vice-Chair: Mick Mellors The Long Term Care Stream has had its 2nd meeting. The group has agreed and signed off ToRs and has set a 10 year Vision for Long Term Care: ‘To improve the health, well-being and quality of life of all our communities. • We will deliver the best quality of long term care and treatment to maximise independence, choice and personalised care. • Our people will be innovative and focus services on the needs of those who use them, and their carers. 1
