Explanation: Huddersfield has high asthma prevalence and hospital admission rates are higher than the national average. There is good evidence that asthma self-management improves health outcomes including for example reducing emergency contacts (and hospital admissions) and improving quality of life. Patients (adults and children) with asthma should receive a self-management plan to prevent hospital admissions. Primary care clinicians can improve compliance with self-management plans and self-management should be part of asthma reviews in primary care.
Specific intervention: 1) To identify from the practice asthma register and case review all registered patients who meet the following criteria:

All patients with asthma. Must include both Adults and Children.
•
Patient/carers have the capacity to understand and follow an asthma self-management plan
