Explanation: Respiratory infection is a frequent cause of exacerbation (and hospitalisation) in patients with COPD. Prompt treatment of infective exacerbations is accepted best practice and may prevent hospital admission. Access to a written self-management plan and rescue courses of antibiotics and (where appropriate) prednisolone may reduce the impact of an infective exacerbation and prevent hospital admission.
Specific intervention: 1) To identify from the practice COPD register and case review all registered patients who meet the following criteria:

All COPD patients

Patient/carers have the capacity to understand and follow a COPD self-management plan
2) For each of these patients ensure that appropriate patient held selfmanagement plan is in place and where appropriate that anticipatory courses 1
