A FUTURE IN FOCUS 2015 ANNUAL REPORT

Northern Nevada HOPES is dedicated to building a healthier community by providing coordinated care and support for individual and family wellness. Our community health center combines primary care, medical specialties, behavioral health and prevention with a team of experienced professionals who are committed to high-quality care.

Dear Friend, Just one year ago, Northern Nevada HOPES broke ground on its new three-story community wellness center. Today, after twelve months of construction and providing patient care from a temporary clinic, The Stacie Mathewson Community Wellness Center has opened its doors. This facility marks a new chapter for HOPES as we continue to transform healthcare in our community. In northern Nevada, people face homelessness, poverty, addiction, chronic disease, and behavioral health issues at higher rates than the national average. At HOPES, we are committed to addressing these issues through team-based, patient-centered services that eliminate barriers and address the entire spectrum of a person’s wellbeing. The new building helps us take our mission of builidng a healthier community even further. Every patient at HOPES now has access to an interdisciplinary care team that fully integrates behavioral health and wellness into primary care. With dedicated clinic pods, 32 exam rooms, a pharmacy, and onsite radiology and diagnostics, the new wellness center will provide the most comprehensive and personal healthcare in northern Nevada. Thank you to all the visionary donors who helped fund our new community wellness center. And thank you to all the community members who have made HOPES their medical home. The future of Northern Nevada HOPES was made possible because of you. With gratitude,

Sharon Chamberlain CEO Northern Nevada HOPES
