Health History Form Prior to starting classes at Northwest and moving into your residence hall room, students must: • Mail or bring completed Health History Form to SOAR. This was mailed with acceptance letter or can be found at nwmissouri.edu/wellness/ PDF/HealthForm.pdf. • Meningococcal Disease vaccinations will be offered at SOAR at an additional cost of $167 with an insurance card.
Transcript Submit final transcripts (high school, college credit, AP and IB scores) and official ACT scores to the Office of Admissions.
Money matters If you plan to apply for financial aid and you have not already done so, please complete the Free Application for Federal Student Aid (FAFSA) form online at www.fafsa.ed.gov. List Northwest’s school code (002496) to give us access to your record for processing.
Meet your SOAR leaders At SOAR, you will team up with a SOAR leader (an upperclassman who is active on campus) and a small group of new students who share similar interests. Freshman directory A freshman directory is available on CatPAWS through Sept. 15. This directory includes your name, hometown, high school, major, SOAR date and, later, your Northwest email. It is set up to help you connect with other students in your area and major at SOAR and is only accessible to other new freshmen.
