OTMH CANDLELIGHT BALL
1
LETTER FROM OUR CHAIRS Dear Gala Patrons, Welcome to the 25th OTMH Candlelight Ball in support of the Oakville Hospital Foundation. During these 25 past years, the Candlelight Ball has been the most important fundraising event of the year for Oakville Trafalgar Memorial Hospital as well as an evening to recognize the caring medical professionals at Oakville Trafalgar Memorial Hospital. We celebrate their dedication, talent, and compassion and are proud to do our part to enable quality patient care in Oakville. With an event of this magnitude, there are so many people to thank starting with our generous sponsors and auction donors. A very special thank you as well to our hardworking gala committee volunteers who are highlighted in this program. The success of this event is due entirely to your dedicated support! Thank you to Frank Rea and the Forget Me Not Flowers team for their beautifully designed dÊcor. And, finally thank you and all our gala patrons for your support. With your participation in tonight’s contests, auctions, and Cause Within a Cause we are certain to achieve our fundraising goal. We personally challenge each of you tonight to do your part to support our Hospital. Your support of this event has been at the heart of OTMH for 25 years but the beat goes on! Through your generosity tonight the gala will continue this legacy of excellence in supporting the ongoing evolution of OTMH. Sincerely,
Cassandra Callahan, Co-Chair OTMH Candlelight Ball Committee

VALUES ORIENTED INVESTMENT SOLUTIONS Celernus Investments Partners Inc. was founded by investment industry veterans whose goal was not merely to succeed against conventional industry standards, but to deliver best in class solutions that meet the needs of investors.
OUR FUNDS Costs are managed aggressively to keep the fees paid by clients to a minimum. This includes a low management fee of just 0.85%. Our funds are 100% focused on performance: growing long term capital and managing volatility over a market life cycle. Our performance bonus always has a high water mark and a compound annual return hurdle. Unlike many alternative funds, all our funds are valued weekly that provides liquidity to better meet the needs of our clients.
Celernus Absolute Growth Fund (CAGF) The Celernus Absolute Growth Fund is a long/short equity fund. The Fund aims to provide investors absolute returns over time using extensive quantitative, fundamental and technical research combined with our proprietary macro market model to identify opportunities and manage risk. Celernus Equity Neutral Fund (CENF) The Celernus Equity Neutral Fund is a market neutral, long/short equity fund. Security selection is guided by an empirical model that identifies and measures key quantitative relationships. The fund seeks to maintain a concentrated portfolio of long positions in stocks that exhibit the highest levels of these key quantitative relationships. Similarly, the fund will maintain short positions in stocks exhibiting the lowest quantitative readings. Celernus Realty Income Properties LP (CRIP LP) The Celernus Realty Income Properties LP is a real estate focused fund. Its objective is to provide long term growth through investments in quality income producing properties with the opportunity for capital appreciation. Celernus Mortgage and Income Trust Fund (CMIT) The Fund aims to provide investors consistent monthly income while simultaneously protecting capital, primarily through investments in mortgages with a loan to value measure of less than eighty percent that provides a desirable degree of security of capital. Due to the short term nature of the mortgages, the principle invested is not subject to the swings in value due to changes in interest rates that medium and longer term fixed income investments are subject to.
Celernus Mortgage and Income Trust Fund (CMIT) The Celernus Mortgage and Income Trust Fund was created in May 2013 to create a unique alternative to incomebased products, with the aim to provide a higher than average return on comparable cash or bond alternatives.

Advance Surgical Care at OTMH
W
hen a medical emergency happens, every minute counts. This is why surgical care provided close to home at OTMH is so important. With advances in the OR, OTMH physicians and caregivers are committed to returning people to their normal lives sooner than ever. This year alone, OTMH physicians have performed 20,750 surgeries in one of our seven operating room theatres in our new facility. With the development of non-invasive surgical procedures and other technological advances, approximately 80 per cent of all our surgeries are now being done as day surgery. This means that patients arrive in the morning for surgery and can return home later the same day to recuperate in the comfort of their own home. You can support OTMH’s surgical department by helping fund these urgently needed pieces of equipment: • An anesthesia gas machine to support procedures in the Ambulatory Procedure Unit GI Suite
• Medical instruments for vascular surgery, a new specialty at OTMH that enables minimallyinvasive procedures and surgical reconstruction • Surgical laser for less invasive and more effective treatment of kidney stones To thank you for your support, we are proud to recognize our donors with naming opportunities including: • Naming a patient room for $25,0000 • Naming a treatment station for $15,000 • Naming a bench on the OTMH grounds for $5,000 • Naming a tree on the OTMH grounds for $2,500 Please help us meet the ever changing healthcare needs of your much larger and more advanced Hospital. New advanced equipment and less invasive procedures make patient care at OTMH better than it has ever been. That is what innovation has done, and will continue to do, at OTMH with your continued support during this year’s Cause Within a Cause. OTMH CANDLELIGHT BALL
