The services, programs, and facilities in this guide are all threads in the fabric of your community, able to help you maintain or improve quality of life for you or a loved one. That may mean acquiring home care services to stay in your current home, downsizing to a more manageable house or apartment, or, if you’re a caregiver, using adult daycare services to help you balance the care of your loved one. When a transition in living arrangements becomes necessary for you or a loved one, this publication is a useful aid in determining which options make the most sense for your family. We are fortunate to have abundant housing/ living accommodations, healthcare, and resources in our region. Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential Living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. Continuing Care Retirement Communities/Life Plan Communities To portray the “vibrant, forward-looking nature of the contemporary senior living community,” some continuing care retirement communities have chosen to describe themselves as life plan communities. A life plan community follows the definition of a CCRC set by The National Investment Center: Age-restricted properties that include a combination of independent living, assisted living, and skilled nursing services (or independent living and skilled nursing) available to residents all on one campus. Resident payment plans vary and

include entrance fee, condo/co-op, and rental programs. Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) and choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function mainly on their own but may need help with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, securing and managing healthcare, self-administering medication, 50plus Living 2021


and proper turning and positioning in a bed or chair. Assisted Living Residences provide an environment that combines housing and supportive services to allow residents to “age in place” without having to move to a licensed long-term care facility when their care needs increase. ALRs have living units with kitchen capacity, private bathrooms, and only one person per unit. An ALR may provide services that would be provided in a long-term care facility (personal care homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospitalbased care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. Acute Care Facilities are licensed as hospitals. They provide the highest level of care under a physician. provided at an acute care facility include emergency services, critical care, medical/ surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services offer assistance and support to those living with memory issues. These units/communities enable the person living with memory issues to be as independent as possible while enhancing freedom of movement in a safe, secured environment. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care; assistance with eating, using the toilet, and taking medication; and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care, assisted living, or nursing care environments. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as nonmedical services, such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses and provides professional treatment of pain and symptom management with support and counseling for medical, psychological, and spiritual needs. Palliative Care also focuses on providing relief from the symptoms and stress of serious illness but, unlike hospice, can be provided along with curative treatment.

Susquehanna/Delaware Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July, with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 20-23 degrees. Average annual precipitation is 41-47 inches.

Taxes Sales tax is 6%. Sales tax exemptions include groceries, health-related items, medical services, prescription drugs, and groceries. State income tax is 3.07%; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities.

State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging.

PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit.

PACE and PACENET eligibility is determined by your previous calendar year’s income. Effective in 2014, Social Security Medicare Part B premiums are excluded from income. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brandname prescription at your pharmacy. These co-payments are based on a 30-day supply.

PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. A single person’s total income must be between $14,500 and $27,500. A couple’s combined total income must be between $17,700 and $35,500. PACENET cardholders that do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost of their medications. If the deductible is not met each month, it will accumulate. In addition, the individual will pay no more than $8 for each generic prescription medication and no more than $15 for each brand name. 50plus Living 2021


Susquehanna/Delaware Valley Profile PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET co-payments of $8 for each generic prescription medication and $15 for each brand name. PACE Plus Medicare Under PACE Plus Medicare, PACE/ PACENET coverage is supplemented by federal Medicare Part D prescription coverage and offers older Pennsylvanians the best benefits of both programs. Older adults continue to receive the same prescription benefits while, in many cases, saving more money.

Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route services at all regular operating times on weekdays, weekends, and holidays. You will need a senior citizen transit identification card to ride free. The identification card is available free of

charge from participating local transit operators. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing shared-ride transportation services. Reduced-fare transportation is provided to medical appointments, senior community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the National Safety Council. It is intended for drivers 55 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (lowvision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees’ field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $36 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

5 Tips for Remote Caregiving During COVID-19

If you’re one of the millions of caregivers in the United States, you know that COVID-19 — and the realities of social distancing and isolation — may complicate the day-to-day tasks of caregiving. But even now, it’s important to remember that you can continue to make a positive impact on your loved one’s life.

While you may be unable to physically be with your loved one, remind yourself that what you’re doing from a distance does matter. Careful, thoughtful planning and clear communication may help make remote caregiving easier on you and the person you’re caring for. 1. Stay connected. Communicate through video chats whenever possible to help them feel engaged. Encourage family and friends to call often or write letters and notes. You may also be able to set up a medical alert system that can help you track and monitor the person in your care. Look for a system that’s designed with caregivers in mind. 2. Get smart with prescriptions. If the person in your care regularly takes medication, talk to his or her pharmacist to see if they offer a mailorder option. You may even be able to order early refills or get 30- or 90day supplies. During the COVID-19 outbreak, some pharmacies may waive delivery fees. 3. Explore remote options for food. Visiting the grocery store may feel unsafe for those who are at a higher risk of serious infection. The good news is, there are many options for food delivery. Whether it’s groceries, meal kits, or Meals on Wheels, there are ways to help make sure your loved one is taken care of with nutritious meals. 4. Consider telehealth for medical needs. If a medical appointment is necessary, connecting virtually through a phone or video app may be a good option for non-emergency needs. Call your loved one’s doctor’s office to see what options are available.

5. Have a plan of action. With the current uncertainty surrounding COVID-19, it’s important to regain a sense of control and confidence. Some steps may include: • Collect contact information. Build a list of contacts, including family members, care providers, pharmacies, care managers, senior service agencies, and anyone else you may need to get a hold of. • Gather health documents. Because care recipients often have complicated medical situations, keeping all paperwork — including important financial and legal records —in one place can be invaluable. This could be accomplished using a physical or an online organizational tool, or both. UnitedHealthcare, for example, has a Care Organizer available, which can be a way to keep all your information together. • Become an authorized representative. As an authorized representative, you’re able to help with payments, the choice of doctor, and other care decisions. To add an authorized representative, your loved one can visit his or her health plan website or call the customer service number on the back of the ID card. Caring for loved ones at a distance may be unfamiliar territory, but knowing what tools and programs are available to help you can help ease uncertainty — both for you and those in your care. 5 Safety Tips for Medications in Your Home Did you know that 70,980 Americans died from drug overdoses in 2019? The Centers for Disease Control and Prevention reported that figure, citing over 194 drug-related deaths in the U.S. every day last year. And unfortunately, the COVID-19 pandemic has only increased the dangers posed by misuse or abuse of drugs in the home. Beyond the fact that the pandemic has temporarily closed resources such as drug drop-box sites and take-back day events, opioid and other drug misuse risks have also increased due to financial and emotional stressors such as isolation and unemployment. Any medications in the home carry the risk of someone misusing them, whether accidentally or on purpose. According to the National Institutes of Health, an estimated 92% of post-surgical prescriptions went unused, making them available for potential misuse. And according to a Journal of the American Medical Association study, over 60% of people with leftover prescription opioids kept pills for future use rather than disposing of them, with 1 in 5 admitting they shared medication with someone else. How can you help prevent drug misuse? Here are tips to keep your home safer for everyone. 20

1. Store medicines securely While many store medications in a medicine cabinet, humidity levels in a bathroom can actually break down medications, making them less effective. In addition, storing meds in a medicine cabinet makes them susceptible to theft by anyone entering your home looking for drugs. Storing prescription medications in a bedside table may also seem convenient, but again leaves them open for misuse or abuse. This is especially true if you have children living in or frequently visiting your home, it’s best to keep all medications — including vitamins — in a high cabinet and secured out of harm’s way. 2. Discard medications safely Go through prescription and over-the-counter medications regularly to identify leftover or expired medicines. This helps prevent accidental or purposeful misuse, which protects your entire family. To safely dispose of medicines, use a medication disposal pouch or container, such as the Deterra® Drug Deactivation System, that can be used at home. It is the safest, most effective way to irreversibly destroy and properly dispose of unused, unwanted, and expired medications in minutes, with the simple addition of tap water. 3. Keep poison control information handy Post the contact information for Poison Control where everyone can see it, like on your refrigerator door. Access Poison Control online at www.poison.org or by calling (800) 222-1222. 4. Don’t call medications “candy” If someone in your family takes medicine or vitamins, never refer to it as “candy” to convince the child to take it or to explain why you’re taking it. This could make the child more likely to try taking pills that they find. 5. Don’t share prescriptions You may think you’re doing a friend or relative a favor by sharing pills from a leftover pain prescription, but that medication was prescribed to you by a healthcare provider for a specific purpose, with knowledge of your health condition and other medications you take. You may not be aware of potentially harmful drug interactions or side effects your friend could experience by taking your medication. Fast food, snacks, and comfort meals are tempting but usually lack healthy amounts of protein, fiber, vitamins, and minerals. In addition to preventing low energy, muscle loss, illnesses, and unplanned weight change, a healthy diet may help reduce the stress and fatigue that can come with caring for a loved one. These tips from the Administration for Community Living can help caregivers take steps toward better nutrition without adding to already full to-do lists: 1. Drink healthy beverages at each meal and two to three times between meals. Water, milk, juice (to replace missing fruits and vegetables), or nonsweetened drinks are all better-for-you options.

Commit to Getting the Care You Need as You Age Older adults in America are living longer lives and will outnumber people under the age of 18 by nearly 500,000 by 2034, according to the U.S. Census Bureau. 36

However, living longer does not automatically mean living healthier or happier. In fact, nearly 61% of people in America aged 65 and older live with chronic conditions, according to the National Institutes of Health, and a report from the National Academies of Sciences, Engineering, and Medicine found nearly 1 in 4 older adults are considered to be socially isolated. Research published in Health Affairs indicates that people who advocate for their own care feel more empowered. The age-friendly health systems approach can help older adults take control of their well-being and demand care that helps them maintain independence by making sure their doctors know their needs and preferences, also called “what matters.”

Just as the name implies, age-friendly care prioritizes what matters most to each individual older adult and helps meet goals such as daily walks around the neighborhood or cooking favorite meals. To get started, consider these four essential elements of age-friendly care from The John A. Hartford Foundation to discuss with your doctor: What Matters. As people age, what matters most to them might change. It is important to share concerns, goals, wishes, needs, and experiences with your healthcare providers. What matters also includes end-of-life care goals and preferences. Medication. Many older adults take multiple medications, which can cause side effects and, in turn, impact daily living. Honest conversations with doctors about medications and their interactions and side effects can ensure treatment plans are optimizing quality of life. Mentation. Older adults are at higher risk for dementia, depression, and delirium, and conversations with medical providers about changes in memory or mood and annual screenings for dementia and depression can lower the risk. Mobility. Staying active can help older adults maintain health and independence. Talking to healthcare providers about mobility concerns can identify personalized tips to improve strength, flexibility, and balance.
