Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential Living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. Continuing Care Retirement Communities/Life Plan Communities To portray the “vibrant, forward-looking nature of the contemporary senior living community,” some continuing care retirement communities have chosen to describe themselves as life plan communities. A life plan community follows the definition of a CCRC set by The National Investment Center: Age-restricted properties that include a combination of independent living, assisted living, and skilled nursing services (or independent living and skilled nursing) available to residents all on one

campus. Resident payment plans vary and include entrance fee, condo/co-op, and rental programs. Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) and choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function mainly on their own but may need help with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, 50plus Living 2022
personal hygiene, securing and managing healthcare, self-administering medication, and proper turning and positioning in a bed or chair. Assisted Living Residences provide an environment that combines housing and supportive services to allow residents to “age in place” without having to move to a licensed long-term care facility when their care needs increase. ALRs have living units with kitchen capacity, private bathrooms, and only one person per unit. An ALR may provide services that would be provided in a long-term care facility (personal care homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospitalbased care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. Acute Care Facilities are licensed as hospitals. They provide the highest level www.onlinepub.com
of care under a physician. Some services provided at an acute care facility include emergency services, critical care, medical/ surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services offer assistance and support to those living with memory issues. These units/communities enable the person living with memory issues to be as independent as possible while enhancing freedom of movement in a safe, secured environment. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care; assistance with eating, using the toilet, and taking medication; and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care, assisted living, or nursing care environments. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as nonmedical services, such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses and provides professional treatment of pain and symptom management with support and counseling for medical, psychological, and spiritual needs. Palliative Care also focuses on providing relief from the symptoms and stress of serious illness but, unlike hospice, can be provided along with curative treatment.

Susquehanna Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July, with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 20-23 degrees. Average annual precipitation is 41-47 inches.
Taxes Sales tax is 6%. Sales tax exemptions include groceries, health-related items, medical services, prescription drugs, and groceries. State income tax is 3.07%; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities.
State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging.
PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit.

PACE and PACENET eligibility is determined by your previous calendar year’s income. Effective in 2014, Social Security Medicare Part B premiums are excluded from income. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brandname prescription at your pharmacy. These co-payments are based on a 30-day supply.
PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. A single person’s total income must be between $14,500 and $27,500. A couple’s combined total income must be between $17,700 and $35,500. PACENET cardholders that do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost of their medications. If the deductible is not met each month, it will accumulate. In addition, the individual will pay no more than $8 for each generic prescription medication and no more than $15 for each brand name. 50plus Living 2022
Susquehanna Valley Profile PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET co-payments of $8 for each generic prescription medication and $15 for each brand name. PACE Plus Medicare Under PACE Plus Medicare, PACE/ PACENET coverage is supplemented by federal Medicare Part D prescription coverage and offers older Pennsylvanians the best benefits of both programs. Older adults continue to receive the same prescription benefits while, in many cases, saving more money.
Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route services at all regular operating times on weekdays, weekends, and holidays. You will need a senior citizen transit identification card to ride free. The identification card is available free of

charge from participating local transit operators. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing shared-ride transportation services. Reduced-fare transportation is provided to medical appointments, senior community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the National Safety Council. It is intended for drivers 55 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (lowvision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees’ field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $36 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

Questions to Ask When Evaluating a Memory Care Program By Laura Gifford If you’re seeking care for a loved one dealing with Alzheimer’s or another form of memory loss, you may be feeling overwhelmed with all of the choices and unsure where to start. Memory care is a complex area. Not every senior living facility offers this specialized service. However, a dedicated memory care program can help someone with Alzheimer’s or dementia thrive through specialty training and brain fitness. Since people living with memory loss have unique needs, you’ll need to advocate for your loved one to be sure they receive the appropriate level of care. Here is a list of questions to ask as you are evaluating different options.

What Security Measures Do You Have in Place? Your loved one’s safety is a top priority. In fact, it’s probably one of the main factors driving your decision to look for a senior living facility. A dedicated memory care unit should have entrances and exits that remain locked or open to a secure courtyard. This prevents dangerous wandering. Another critical safety feature is providing residents with emergency call pendants. These devices allow individuals to get help in the event of a fall or accident. Many senior living communities also have registered nurses on site or on call 24 hours a day. What Type of Training Has Your Staff Received? Staffing is another high-priority item. The people working with your loved one on a daily basis should be highly skilled, professional, and compassionate. Memory care requires additional training beyond a typical senior care role. Employees need to know how to communicate appropriately with individuals living with memory loss. They should also receive and maintain specialty training to cue daily tasks and assure the trust and comfort of your loved one. Also ask about staff-to-resident ratio. The community should have enough team members to give your loved one the individualized attention they need.

What Activities Are Offered? Socialization is just as important as your loved one’s care needs. Ask about the community’s activity programs and how individuals with memory loss are accommodated. Those living with memory loss should receive specialized programming for their needs. People living with dementia can get overwhelmed by sensory input or become agitated. Ask how these situations are handled. You want to be sure your loved one can participate in a variety of activities while also having their needs addressed. How Do You Develop a Resident’s Care Plan? Individual needs differ, so most senior living communities develop a customized care plan for your loved one. Inquire about that process. Who is involved in creating the plan? Can family members provide input? Are plans regularly evaluated and updated as needed? Can the facility accommodate additional care needs or medical needs? Depending on your loved one’s medical condition, you may need to seek care at a skilled nursing facility (nursing home). However, dedicated memory care facilities are often the best option for people with dementia. These communities are specifically designed to assist individuals with memory loss while maintaining independence and living in a homelike environment. What is Your Approach to Care? There are many different approaches to memory care. You’ll want to learn more about a community’s specific approach and the different types of interventions it uses. Some common ones include: • Brain fitness exercises • Physical fitness programs • Art therapy • Music therapy • Pet therapy • Aromatherapy • Cognitive stimulation appropriate to the individual’s abilities; this may include things like puzzles, trivia, and matching games 12

There are specialized training programs that are recommended, such as Positive Approach to Care, Validation Method, and more. What is Your Facility’s Inspection Record? Licensed assisted living senior living facilities in Pennsylvania are evaluated by the Department of Human Services each year. Inspectors are trained to evaluate over 400 areas, including: • The quality of care and amount of time given to each resident • Safety protocols • Staff training • How well the facility is maintained You can access these inspection reports for assisted living and personal care homes from the Pennsylvania Human Services Provider Directory (www. humanservices.state.pa.us/human_service_provider_directory). If you want to view a nursing home’s survey results, visit the Pennsylvania Department of Health website (www.health.state.pa.us/surveysposted). Look for any major violations or a consistent pattern of repeat violations. There are many other details you will consider as you search for memory care, from the dining program to onsite amenities. However, these questions are the most important ones to ask. You want your loved one to be healthy, safe, and living up to their full potential. A good senior living community helps individuals with memory loss thrive. This checklist can help identify areas that might need improvement. Understanding Financial Options to Pay for Long-Term Care
By Chris Orestis Long-term care is a reality that everyone will eventually confront — either for themselves, a loved one, or both — and it is an expensive proposition that too few people are prepared for or understand. Unfortunately, most people don’t think about how they will pay for care until they are confronted by a serious health situation for themselves or a loved one. Many people simply avoid the topic of long-term care and fail to understand it until, one day, a crisis situation throws them into a world for which most people are completely unprepared. The worst time to start planning is in the midst of a crisis because the options to pay for care can be complicated and take some time to access. The three primary ways to pay for care are with Medicare, Medicaid, or private pay. 1. Medicare is an “age-based” program that will cover the first 100 days of rehabilitation care in a licensed skilled-nursing facility upon direct discharge from a hospital. 36

2. Medicaid is a “means-based” program that covers skilled nursing care. To qualify, an applicant must meet both standards of medical necessity and be below set asset and income levels. Applying for Medicaid can be a challenging process that requires the applicant to submit detailed medical and financial records. Medicaid will “look back” five years at financial records to make sure that assets have not been hidden or transferred to family members. 3. Private pay primarily comes from an individual and/or a family’s savings, insurance, assets, and income. People who use private pay can choose any form and location of care that they want. Independent and personal care/assisted living, as well as most forms of home care, are primarily private pay. Nursing homes are primarily covered by Medicaid for people who can qualify. Skilled rehabilitation and hospice are primarily covered by Medicare. Private-Pay Options for Senior Living and Long-Term Care Families need to do all they can to prepare to fund long-term care and protect themselves from both the financial costs and the possibilities of legal liabilities. There are actions people can start taking today to prepare for future costs, and there are long-term care funding tools to address immediate need for care as well. What are some of today’s options for private-pay funding of long-term care? Long-term care insurance – These insurance policies will provide a fixed monthly payment to cover approved forms of qualifying long-term care. Longterm care insurance can be purchased as a standalone policy or as a life/LTC hybrid policy. The younger and healthier a person is when buying a policy, the more affordable their premium rates and the higher the likelihood they will qualify. LTC life settlements – A life settlement can be used to fund a long-term care benefit plan, which is similar to a health savings plan. This LTC benefit plan is an irrevocable bank account that is professionally administered, with payments made monthly to long-term care providers. Medically qualified policy owners who use an LTC life settlement are able to immediately direct tax-exempt payments to cover their senior housing and long-term care costs. www.onlinepub.com

Veterans Aid & Attendance benefit – Veterans of active combat duty and/ or their spouses are eligible to receive monthly benefits paid directly towards qualifying long-term care service. Like Medicaid, the applicant must meet both medical necessity and income/ asset level requirements to qualify. Reverse mortgage – Homeowners with little to no remaining mortgage balance who are age 62 or older can qualify to take a HUD-backed home equity conversion mortgage (HECM) loan against the home in the form of a lump sum, monthly income, or a line of credit. To qualify, the home must still be the primary residence, and the loan must be paid back with interest and fees after the homeowner dies (typically through the sale of the property). Senior living loans – These are loans that can be secured specifically to pay for long-term care services. These loans are unsecured by collateral and instead are guaranteed by family members who cosign (one or more). Interest rates are similar to a credit card, and the loans are typically between $50,000 and $500,000 for a term of one year or less. Medicaid-compliant annuity – This single-premium immediate annuity can be purchased to set up a guaranteed income stream for a spouse while the annuitant qualifies to go onto Medicaid and into a nursing home. This is an irrevocable annuity established for a period equal to or less than the remaining life expectancy of the annuitant. The state is named as the remainder beneficiary to receive any funds after the annuitant dies. The key to successfully navigating any long-term care situation is to understand your financial options and understand the differences between what will be covered by Medicare, Medicaid, or private pay. Planning and informing yourself as far in advance as possible is best, but there are also a number of funding options available that can help people address a sudden and immediate need for care.
