Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. CCRCs (Continuing Care Retirement Communities) are facilities offering a variety of residential living options in addition to comprehensive medical and nursing services. Many CCRCs are selfcontained communities offering educational and recreational activities, dining accommodations, banking facilities, transportation services, etc. Residents move between independent living, assisted living, and nursing care based on changing needs.

Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) who choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function on their own most of the time but may require assistance with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services in or of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, securing and managing 50plus LIVING 2014
healthcare, self-administering medication, and proper turning and positioning in a bed or chair. Assisted Living Residences are designed to provide an environment that combines housing and supportive services to allow residents to “age in place� without having to move to a licensed long-term care facility when their care needs increase. ALRs will have living units with kitchen capacity and private bathrooms and only one person per unit. An ALR may provide services that otherwise would be provided in a long-term care facility (Personal Care Homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospitalbased care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. www.onlinepub.com
Acute Care Facilities are licensed as hospitals. They provide the highest level of care under a physician. Some of the services provided at an acute care facility include emergency services, critical care, medical/surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services may be offered in either a nursing or residential care environment. Generally, staff are specially trained in memory care. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care, assistance with eating or using the toilet, assistance with taking medication, and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care and assisted living or a nursing care environment. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as non-medical services such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses. Hospice provides professional treatment of pain and symptom management with support and counseling. It includes medical, psychological, and spiritual support.

Susquehanna/Delaware Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 2023 degrees. Average annual precipitation is 41-47 inches. Taxes Sales tax is 6 percent. Sales tax exemptions include groceries, healthrelated items, medical services, prescription drugs, and groceries. State income tax is 3.07 percent; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities. State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging. PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You 6
cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brand-name prescription at your pharmacy. These co-payments are based on a 30-day supply. PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. A single person’s total income must be between $14,501 and $23,500. A couple’s combined total income must be between $17,701 and $31,500. PACENET cardholders that do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost of their medications. If the deductible is not met each month, it will accumulate. In addition, the individual will pay no more than $8 for each generic prescription medication and no more 50plus LIVING 2014

PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET copayments of $8 for each generic prescription medication and $15 for each brand name. Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route services at all regular operating times on weekdays, weekends, and holidays. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing sharedride transportation services. Reducedfare transportation is provided to medical appointments, senior

community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the NSC. It is intended for drivers 50 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (low-vision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $36 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

Visiting a Hospitalized Relative or Friend Lisa M. Petsche
If you are planning to visit a relative or friend in the hospital, follow these suggestions to help ensure a positive experience. Before You Go Call ahead to the patient or their next of kin to find out if visits are welcomed, and if so, the best time of day to come. Also ask if there is anything you can bring. If you are not an immediate family member or close friend, reconsider visiting unless the patient has few local supports. Instead, send a card or email (some hospitals offer the latter option on their website) and plan a visit when the person returns home. Before arranging for any kind of get-well gift, find out the hospital’s policy around balloons and flowers and take into account space limitations in the patient’s room. It’s best to hold off on gifts until the person returns home. Before planning to bring in food or beverages, find out if any dietary restrictions have been implemented. Postpone your visit if you are not feeling well. Telephone instead.

What to Bring If you feel the need to bring something, keep it simple and practical. Ideas include reading material, a notebook and pen, a box of facial tissue, hand sanitizer for the bedside, and ear plugs, especially if the person is a light sleeper. Some ideas for cheering a patient: bring children’s artwork, decorative window clings, or a special food treat. What Not to Bring For security reasons, don’t bring in anything of value. Discourage the person from keeping identification, jewelry, electronics, or more than a few dollars on hand. Don’t bring in prescription drugs, over-the-counter medications, herbal remedies, or alcoholic beverages. Don’t wear perfume or cologne or bring the patient heavily scented toiletries. Many healthcare facilities are “fragrance-restricted” environments due to patient and staff allergies. Visiting Tips Find out what the visiting hours are and stick to them. 50plus LIVING 2014
} To help ensure a positive experience, follow these suggestions if you are planning to visit a relative or friend in the hospital.
} Clean your hands when you enter and leave the hospital and before and after visiting in the patient’s room. This helps to prevent the spread of disease-causing microorganisms. Watch for signs regarding the use of cell phones. In permissible locations, set your phone to vibrate mode and exercise discretion when conversing about sensitive matters. If the patient is in isolation— indicated by a sign on or beside their door—go to the nurses’ station to inquire whether they can have visitors and, if so, what precautions to take. You may need to don gloves, a gown, and a mask. It’s important to follow all instructions. Unless you are the patient’s next of kin, don’t request medical information from staff or get involved in care issues. If you are one of several immediate family members, keep in mind that patients are asked to appoint one contact person in order www.onlinepub.com
to streamline communication. This is whom you should speak to if you would like more information than the patient is able to provide. Limit visitors to a few at a time. Speak softly in the room and hallways so you don’t disturb patients who are resting. Exercise good judgment when it comes to bringing children, and ensure adult supervision at all times. If the patient is safely able to walk or transfer into a wheelchair, visit outside the room—perhaps in the visitors’ lounge on the ward or the hospital coffee shop. Be prepared that a healthcare professional may ask you to leave the room in order to provide care or conduct an assessment or test. Dear Savvy Senior, What tips or products can you recommend to help make a home safer and more convenient for aging in place? My husband and I are in our 70s and don’t have the money for any big renovations, but we want to do what we can to stay in our house as long as possible. – Homebodies Dear Homebodies, There are lots of small adjustments and simple modifications you can do to make your home safer and livable as you age that are very inexpensive or completely free. Here are several to consider. Fall-Proofing Since falls are the leading cause of home injury among seniors, a good place to start is by picking up any possible clutter that can cause you to trip, such as newspapers, books, shoes, clothes, and electrical or phone cords. If you have throw rugs, remove them or use double-sided tape to secure them. And if you have stairs, consider putting handrails on both sides. www.onlinepub.com
Good lighting is also very important, so add lamps or light fixtures where needed, and install brighter, full-spectrum bulbs in existing fixtures to improve visibility. Also purchase some inexpensive plug-in nightlights for the bedroom, bathroom, and hallways, and consider installing motion-sensor lights outside the front and back doors and in the driveway. In the bathroom, get some nonskid bath rugs for the floors, put a non-slip rubber mat or self-stick strips on the floor of the tub/shower, and have a carpenter install grab bars inside the tub/shower. And in the kitchen, organize your cabinets so the things you use most often are within easy reach without using a stepstool. Simple Modifications Growing old can also bring about various physical limitations that can make your home more difficult to use. Some simple solutions, for example, that can help weak or arthritic hands is to replace round Continued on next page 43
doorknobs with lever handles or get some inexpensive doorknob lever adapters. The same goes for twist-knob kitchen or bathroom faucets. You can easily replace them with lever faucet handles that you can purchase for a few dollars in most hardware stores, or get a single-lever handle faucet installed. In the kitchen, you can make your cabinets and pantry easier to access by installing pullout shelves or lazy susans. And D-shaped pull handles for the cabinets and drawers are also recommended because they’re more comfortable to grasp than knobs. In the bathroom, consider getting a handheld, adjustable showerhead installed and purchase a shower or bathtub seat, so you can shower from a safe, seated position if need be. And for easier toilet access, purchase a toilet-seat riser for a few dollars. This can make sitting down and standing up a little easier, especially if you’re 6 feet or taller. If you get to the point where you need to use a walker or wheelchair, you can adapt your house by installing ramps on entrance steps and mini-ramps to go over high-entrance thresholds. 44
